RESUMO

Sixteen patients with recurrent high-grade glioma who were treated by domestic magnetic resonance-guided laser interstitial thermotherapy (MRgLITT) in the Neurosurgery Department of Xuanwu Hospital of Capital Medical University from 1st January 2021 to 31st December 2021 were prospectively included, with 11 males and 5 females, and aged 27-74 (50±16) years. The duration of surgery, the rate of ablation after surgery, and perioperative complications were assessed. The patients were followed up every 3 months to assess survival and progression. A total of 5 WHO grade Ⅲ patients and 11 WHO grade Ⅳ patients were included. The operation time was 144 (109, 176) min, 28 targeted lesions were detected, and the ablation rate [M (Q1, Q3)] was 91.0% (87.4%, 93.3%). After surgery, 2 patients (2/16) had decreased limb muscle strength, and no perioperative death or other serious complications occurred. The median time to a complete response was 12 (5, 14) months in WHO Grade Ⅲ patients, and one died 12 months after surgery, while the median time to a complete response was 3 (1, 8) months in 11 WHO Grade Ⅳ patients, with a total of 8 deaths at the last follow-up. Therefore, domestic MRgLITT has certain efficacy and safety in the treatment of recurrent high-grade glioma, providing a new option for patients with recurrent glioma.

Objective: To investigate the safety and efficacy of stereotactic-EEG (SEEG)-guided radiofrequency thermocoagulation (RF-TC) with three-dimensional lesioning in the treatment of drug-resistant insular epilepsy. Methods: Seven patients with drug-resistant insular epilepsy who underwent SEEG-guided RF-TC with three-dimensional ablation at the Department of Neurosurgery of Xuanwu Hospital, Capital Medical University from February 2017 to June 2019 were retrospectively enrolled. Presurgical evaluation including semiology, EEG and imaging results suggested that the epileptogenic zone was located in the insular lobe. All patients underwent SEEG recording and three-dimensional RF-TC. Postoperative follow-up was conducted by outpatient visits or telephone, the clinical efficacy was evaluated based on Engel classification and the relevant complications were documented. Results: Seven patients were followed up at 18 months after surgery. Among them, 4 were seizure free (Engel ⅠA), 2 had nondisabling simple partial seizures (Engel ⅠB) and 1 achieved 75% decrease (Engel ⅢA) in seizure frequency. Postoperative complications occurred in 3 patients which included decreased sensation in the right lower extremity, decreased strength in the right upper extremity, glossolalia, decline in memory and comprehension. No permanent neurological dysfunction was detected. Conclusion: SEEG-guided RF-TC with three-dimensional lesioning is minimally invasive, safe and effective in the treatment of drug-resistant insular epilepsy and can serve as a complementary method for resection surgery.

Objective: To investigate the safety and short-term efficacy of domestic magnetic resonance-guided laser interstitial thermotherapy (MRgLITT) in the treatment of drug-resistant epilepsy. Methods: Patients with drug-resistant epilepsy treated with a domestic MRgLITT system in the Department of Neurosurgery, Xuanwu Hospital, Capital Medical University from October 2020 to April 2021 were prospectively enrolled. The damage volume ratio was assessed immediately after surgery, and perioperative complications were recorded and followed up. The clinical safety and short-term efficacy were evaluated using the Engel classification. Results: A total of 22 patients were included, including 12 males and 10 females, aged from 3 to 45 years old [(24±13) years]. There were 5 cases of medial temporal lobe epilepsy (MTLE), 3 cases of hypothalamic hamartoma (HH), 7 cases of focal cortical dysplasia (FCD), and 7 cases of other types, respectively. The mean operation time and blood loss was (173±49) min and (3.7±1.6) ml. The postoperative length of hospital stay was (5.5±1.8) days, and the average damage volume ratio was 92.6%. Among them, only 2 patients (FCD of the parietal lobe) showed transient contralateral limb weakness, without any serious complications such as symptomatic intracranial hemorrhage and cerebral infarction. The follow-up time was 14 to 168 days. There were 13 Engel class Ⅰ cases (59.1%), 2 Engel class Ⅱ cases (9.1%), 2 Engel class Ⅲ cases (9.1%) and 5 Engel class Ⅳ cases (22.7%), respectively. Short-term incident-free rates were MTLE 5/5and FCD4/7, respectively. Conclusion: Domestic MRgLITT system is stable, reliable and safe in the treatment of drug-refractory epilepsy, and has better short-term efficacy in MTLE and FCD patients.

Objective: To explore the characteristics and significance of corneal biomechanics in patients with suspicious total deviation value in Belin/Ambrosio Enhanced Ectasia Display of Pentacam corneal topography. Methods: In this cross-sectional study, 206 patients (325 eyes) with myopia or myopic astigmatism were collected from refractive surgery center of Tianjin Eye Hospital. Among them, 110 males(213 eyes) and 96 females (153 eyes) were aged (22.68±5.20) years. 57 patients (71 eyes) with a total deviation value of 1.6-3.0 in the Pentacam corneal topography were included in the suspicious group, and 149 patients (254 eyes) of<1.6 were included in the control group. The values of the first applanation time (A1-Time), first applanation length (A1-Length), first applanation velocity (A1-Velocity), second applanation time (A2-Time), second applanation length (A2-Length), second applanation velocity(A2-Velocity), highest concavity time (HC-time), highest concavity deformation amplitude (DA), highest concavity peak distance (PD), highest concavity radius (HC-Radius, RoC), intraocular pressure were measured using the corneal visualization Scheimpflug technology. The total deviation value in Belin/Ambrosio Enhanced Ectasia Display were measured with a Pentacam. The biomechanical parameters of the suspicious group were compared with the normal group by the two independent samples t test and the identity regression analyses. The data correlation was performed using Pearson linear correlation analysis. Results: Compared with the D normal group, the K1, K2 and Km of D suspicious group were significantly increased (P<0.001). A1-Velocity [(0.152±0.015) m/s] and DA[(1.050±0.090) mm] were significantly increased (t=4.348, 2.708; P<0.001, 0.007), while the CCT, A1-Length, A2-Length and RoC were significantly decreased (P<0.01) in suspicious group. In suspicious group, there was a significant correlation between A1-Time, A1-Length, A2-Length, A2-Velocity, IOP and CCT(r=0.305, 0.324, 0.238, 0.346, 0.316; P<0.05). There was a significant correlation between the anterior surface refractive power K1 and the A1-Length (r=-0.361, P=0.002), and there was a significant correlation between the A1-Length, RoC and anterior surface refractive power K2 (r=-0.369, -0.242; P=0.002, 0.043). There was a significant correlation between anterior surface refractive power Km and A1-Length (r=-0.373, P=0.001). After adjusting the confounding factors between two groups by the identity regression analyses, the A1-Length (P=0.003), A1-Velocity (P<0.001) and DA (P=0.002) were still significant differences. Conclusions: In total deviation value suspicious group, the corneal thickness was thinner, corneal curvature became steeper and the corneal biomechanical properties were weaker. It is important to screen keratoconus before corneal refractive surgery and choose a reasonable surgical procedure to reduce corneal dilatation after refractive surgery. Objective: To investigate the effect of optical zone diameter on corneal biomechanical properties after small incision lenticule extraction (SMILE). Methods: In this prospective case-control study, 62 eyes of 42 patients with myopia and myopic astigmatism that had undergone SMILE procedure were divided into 2 groups according to the optical zone (OZ) diameter: group A, 31 eyes of 19 patients, OZ= 6.5 mm; group B, 31eyes of 23 patients, OZ=6.0 mm. These patients were examined for corneal hysteresis (CH) and corneal resistance factor (CRF) with the Ocular Response Analyzer (ORA) preoperatively and at 1 week, 1 month and 3 months postoperatively; at the same time, the central corneal thickness-corrected indices of DifCH and DifCRF were calculated. Pearson correlation was applied to analyze the correlation between CH and CRF and central corneal thickness. Independent-samples t test was utilized to compare CH and CRF between the two groups. Repeated ANOVA was used to compare the difference of corneal biomechanical properties at different follow-up timepoints. Results: The preoperative mean CRF and CH values were (10.13±1.19) mmHg and (9.65±1.12) mmHg in group A and (10.34±0.89) mmHg and (10.04± 1.05) mmHg in group B, respectively, and no significant differences were found in CRF and CH between the two groups (CRF: t=-0.807, P=0.423. CH: t=-1.405, P=0.165). The mean CRF values in group A at postoperative 1 week and 3 months were (6.62 ± 1.09) mmHg and (6.83 ± 1.07) mmHg, respectively, which were significantly lower than the relevant mean CRF values in group B (t=-2.703, P=0.009. t=-3.733, P= 0.001). Meanwhile, the mean CH values at 1 week, 1 month and 3 months postoperatively in group A were (7.31±1.06) mmHg, (7.37±0.96) mmHg and (7.82±0.97) mmHg, respectively, which were significantly lower than those in group B (t=-2.415, P=0.019. t=-2.113, P=0.039. t=-2.67, P=0.01). Moreover, the DifCRF and DifCH values in group A were significantly lower than those in group B at 3 months postoperatively (t=-3.409, P=0.001. t=-2.064, P=0.044). The CRF and CH values in both groups showed a significant reduction at 1 week, 1 month and 3 months postoperatively (P<0.05). Conclusions: The size of optical zone has some effects on corneal biomechanical properties after SMILE. The smaller optical zone diameter has a relatively smaller effect. Objective: To investigate biomechanical effects of central corneal thickness (CCT), and to explore the possible safe range for the tissue removal and percentage of tissue ablation (PTA) of small incision lenticule extraction (SMILE). Methods: Clinical observational study. One hundred and eighty-seven cases (365 eyes) undergoing SMILE surgery were enrolled. Corneal hysteresis (CH) and corneal resistance factor (CRF) were measured before and after surgery by the ocular response analyzer (ORA). Linear regression of CH, CRF and CCT, and percentage of tissue ablation were calculated. The fifth order polynomial functions provided the closest fit to the data of ΔCH, ΔCRF and the thickness of tissue removal. Results: CH and CRF decreased by 20.7% and 33.0%, respectively, after SMILE, and statistically significant differences were found between preoperative and postoperative values (P<0.05). CH and CRF showed a positive correlation to CCT (r=0.483, P<0.05. r=0.507, P<0.05). The fifth order polynomial functions showed that ΔCH and ΔCRF increased with the increasing tissue removal. The variety rate of ΔCH and ΔCRF was lower after 50 µm to 140 µm of tissue removal, but was higher when the tissue removal was less than 50 µm and more than 140 µm. The variation trend was similar for PTA. Conclusions: Corneal thickness has an effect on the corneal biomechanical properties, which decrease with the increase of tissue removal. It seems that the tissue removal of less than 140 µm and the PTA of less than 25% are safer in SMILE, and further investigations are needed. (Chin J Ophthalmol, 2017, 53:11-17).

The porcine somatoropin gene was inserted into the Pichia pastoris expression vector of pPICZ alpha A which contains AOX I promoter and alpha-factor signal sequence. The recombinant plasmid of pPICZ alpha A-pST was linearnized by Sac I and transformed into X-33 by electroporation. The multi-copy insert transformants were selected and cultivated in flasks. SDS-PAGE and Western blot analysis showed that PST gene products were observed in the supernants with a little larger molecular weight size than the natural PST's, however, the molecular weight size of the PST gene products in the soluble cellular proteins were identical to the natural PST's. Retransformation of the linearnized pPICZ alpha-pST showed the expression level was improved greatly and rPST has the same antigenicity as natural one. The expressed rPST accumulated up to about 956 mg/L. The N-glycosylation analysis showed rPST had no N-glycosylation.

Four experiments were conducted to study 1) factors affecting porcine oocyte maturation in culture medium and 2) a new method for oocyte maturation outside the porcine body. In Experiment 1, five groups of oocytes were cultured in m-TCM199 or m-KRB medium for 24 to 28, 32 to 36 or 40 to 42 hours and then were fertilized in vitro. The cleavage rate (two to four-cell stage) of oocytes cultured for 32 to 36 hours was significantly higher than those of the other oocytes. The results indicate that a suitable culture period for the in vitro maturation of porcine oocytes is 32 to 36 hours. In Experiment 2, four groups of oocytes were cultured in m-KRB or m-KRB supplemented with PFF, PMSG or FSH for in vitro maturation, and the cleavage rates of oocytes were 7.94, 22.56, 30.23 and 23.26%, respectively, after in vitro fertilization. The results show that porcine follicular fluid (PFF) and gonadotrophins added to the culture medium promote porcine oocyte maturation in vitro. In Experiment 3, oocytes were cultured in m-KRB or m-TCM199, supplemented with both gonadotrophin and pocine folliclar fluid for maturation in vitro. After fertilization in vitro, the cleavage rates of oocytes were 26.32 and 27.93% for the two media. The results indicate that the difference between m-KRB and m-TCM199 was insignificant when the media were used to culture porcine oocytes. But there was a significant difference when PFF and gonadotrophins were added to the basic media. In Experiment 4, porcine oocytes were transferred into the reproductive tracts of other animals for maturation. After 34 to 36 hours, the oocytes were collected and fertilized in vitro. The cleavage rates of oocytes were 10.42, 28.45, 3.33 and 36.36%, respectively, for the oocytes matured in mouse uterine horns, rat uterine horns, rat oviducts or rabbit oviducts. The results show that porcine oocytes can be matured in the reproductive tracts of other animals.

RESUMO

A number of tricyclic and bicyclic fused thiazole-2-acetic acid derivatives were prepared and the chemistry and biological properties of these compounds are discussed. Many of the esters exhibited antitubercular activity. The bicyclic thiazole-2-acetic acids had antidepressant activity. Interesting antimetastatic activity against Lewis lung tumor in mice was found with several compounds, in particular, the thiazolo[3,2-a]benzimidazole-2-acetic acid derivative XI.
