RESUMO

The biosafety of rare earth oxide nanomaterials has attracted more and more attention, and the autophagy response induced by such nanomaterials is also of great significance for cancer treatment. Autophagy plays a dual role in cell survival and death. Quercetin can promote autophagy, and rare earth oxides have been shown to induce different types of autophagy. Dextran-coated cerium oxide nanoparticles-loaded quercetin composite DCQ was synthesized, characterized, and studied for its effects on cell viability, oxidative damage, autophagy and apoptosis in human hepatoma cells HepG2. The results showed that the composite material was more toxic to HepG2 cells, and had no obvious toxic effect on normal cells, human umbilical vein endothelial cells (HUVEC). The composite material was found to induce production of reactive oxygen, block autophagy and promote apoptosis of HepG2 cells. This result shows that this nanocomposite can effectively kill human liver cancer cells, which may provide a new strategy for liver cancer treatment.

Objective At present, there is little report of clinical results of metal-on-metal HRA in China.This study was aimed at retrospectively analyzing the factors of survival rate of HRA in China. Methods A retrospective study was conducted in 113 patients who accepted HRA in our hospital from September 2005 to May 2010.All patients were followed up by telephone or outpatient visit,and the average follow-up time was 10.2 years.The clinical function was evaluated by Harris score,and the prosthesis was evalu-ated by pelvic film.Inductively coupled plasma mass spectrometry(ICP-MS)was used to detect the concentration of blood metal ions, ultrasound and MRI were used to screen and diagnose inflammatory pseudotumor.Cox proportional hazards models were used to analyze the factors affecting the survival rate of the prosthesis. Results A total of 91 patients(120 hips)were followed up while 22 patients (26 hips)were lost,the follow-up rate was 80.5%.17 patients underwent revision surgeries,including 8 cases of femoral neck fracture and 8 cases of aseptic loosening,the other one was infection.Revision in 13 cases was within 3 years, the proportion was as high as 76.5%.The 10-year survival rate of prosthesis was 87.1%,the abduction angle of acetabular prosthesis is the only risk factor affecting the survival. Conclusion Postoperative survival rate of HRA is relatively high, and the survival rate of prosthesis is relevant to socket position.We should notice that the risk increased when the acetabular abduction angle of the prosthesis was over 55 degree.O-verall,HRA is still a successful surgical method.

BACKGROUND: It is well-documented that both chemokine (C-C motif) ligand 19 (CCL19) and 21 (CCL21) mediate cell migration and angiogenesis in many diseases. However, these ligands' precise pathological role in ankylosing spondylitis (AS) has not been elucidated. The objective of this study was to examine the expression of CCL19 and CCL21 (CCL19/CCL21) in AS hip ligament tissue (LT) and determine their pathological functions. METHODS: The expression levels of CCL19, CCL21 and their receptor CCR7 in AS (n = 31) and osteoarthritis (OA, n = 21) LT were analyzed via real-time polymerase chain reaction (RT-PCR) and immunohistochemistry (IHC). The expression of CCL19, CCL21 and CCR7 in AS ligament fibroblasts was also detected. The proliferation of ligament fibroblasts was measured via a cell counting kit-8 (CCK8) assay after exogenous CCL19/CCL21 treatment. Additionally, the role of CCL19/CCL21 in osteogenesis was evaluated via RT-PCR and enzyme-linked immunosorbent assay (ELISA) in individual AS fibroblast cultures. Furthermore, the expression of the bone markers alkaline phosphatase (ALP), osteocalcin (OCN), collagenase I (COL1), integrin-binding sialoprotein (IBSP) and the key regulators runt-related transcription factor-2 (Runx-2) and osterix were investigated. Moreover, the CCL19/CCL21 levels in serum and LT were measured via ELISA. RESULTS: The mRNA levels of CCL19/CCL21 in AS hip LT were significantly higher than that in OA LT, and IHC analysis revealed a similar result. Exogenous CCL19/CCL21 treatment did not affect the proliferation of ligament fibroblasts but significantly up-regulated the expression of bone markers, including ALP and OCN, and the key regulators Runx-2 and osterix. In addition, the serum levels of CCL19/CCL21 were apparently elevated in AS patients compared to healthy controls (HC), and the expression of the two chemokines correlated significantly in AS patients. CONCLUSIONS: CCL19 and CCL21, two chemokines displaying significantly associated expression in serum, indicating a synergistic effect on AS pathogenesis, may function as promoters of ligament ossification in AS patients.

<p><b>OBJECTIVE</b>To study the differences in clinical outcome of double knee osteoarthritis patients undergoing unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA).</p><p><b>METHODS</b>From May 2009 to May 2012, 30 patients (60 knees) with isolated compartmental osteoarthritis of knees were enrolled. Each patient accepted UKA on one knee, TKA on the other. There were 9 male and 21 female patients, aged from 60 to 79 years, average 69 years. Patients evaluation focused on the hospital for special surgery(HSS) knee score, blood loss, hemoglobin 48 h after the operation, the time of knee being able to flex to 90° and patients' sensation after operation. Collection the UKA side and TKA side data and compare two groups of data.</p><p><b>RESULTS</b>All patients were followed up for 13 to 35 months, average 20.5 months. There were no component loosening and revision. HSS knee score improved significantly in both two groups: UKA group was promoted from 61 ± 3 to 87 ± 3 (t = 11.21, P < 0.001) and TKA group from 59 ± 5 to 86 ± 3 (t = 17.64, P < 0.001). Compared with the TKA group, the UKA group had less blood loss (t = 11.56, P < 0.001), and a decrease of hemoglobin 48 h after the operation (t = 12.38, P < 0.001). The dates of knees being able to flex ≥ 90° after operation were less (t = 4.03, P < 0.05) in the UKA group. As to therapeutic effects, 70% patients found that UKA was better than TKA; 16.7% patients had opposite opinion; and 13.3% patients found no differences between their two knees.</p><p><b>CONCLUSIONS</b>UKA for the treatment of isolated compartmental osteoarthritis of knee shows as well as TKA, and it has less trauma, less blood loss, more rapid postoperative recovery than TKA.</p>

<p><b>OBJECTIVE</b>Erectile dysfunction (ED) is now recognized as a comorbid condition, especially in men with cardiovascular disease or diabetes mellitus. This randomized controlled trial was to examine the effect of long-term small-dose tadalafil in the treatment of ED.</p><p><b>METHODS</b>A total of 98 men older than 18 years with at least a 6-month ED history were enlisted and divided into two groups to receive once-daily treatment with tadalafil at 5 mg (n = 60) and 20 mg (n = 38), respectively, for 12 months. The effects of medication were analyzed and compared using IIEF, Global Assessment Questionnaire (GAQ) and Sexual Encounter Profile (SEP), and so were the safety and tolerance of the two doses.</p><p><b>RESULTS</b>There were no statistically significant differences in the therapeutical results between the 5 mg and 20 mg groups (P < 0.05). The IIEF-5 score was raised by 8.1 points in the former and 7.9 points in the latter; the YES answers to SEP2 in the two groups were 51.3% and 49.2% before the treatment and 82.6% and 84.9% after it. No serious adverse events were observed, except some common ones, such as rubeosis (11.9% vs 8.7%) and headache (5.3% vs 4.9%) in the 5 mg and 20 mg groups.</p><p><b>CONCLUSION</b>Oral tadalafil at 5 mg once daily is efficacious with good tolerance in the treatment of ED, and it can be an alternative to on-demand medication for some men to eliminate the inconvenience of planned intercourse within a limited timeframe.</p>

Objective: To detect the expression of γ-synuclein in primary osteosarcoma, so as to assess the diagnostic value of γ-synuclein in diagnosis of primary osteosarcoma. Methods: The expression of γ-synuclein in 31 primary osteosarcoma specimens(from 31 patients in our department from Jan. 2004 to Dec. 2005) and 10 benign bone tumor specimens were investigated by tissue array and immunohistochemical method. Results: The expression or overexpression of γ-synuclein was found in 30 of the 31 osteosarcoma specimens; only one female patient's specinmen was negative of it, with a positive rate of 96.8%. All the benign bone tumor specimens had negative expression of γ-synuclein. The positive rate of γ-synuclein was not correlated with age or gender of patients. Conclusion: The γ-synuclein may serve as a marker for osteosarcoma and may be valuable in diagnosis, typing, and prognosis of primary osteosarcoma.

<p><b>BACKGROUND</b>Recently, the new generation of metal-on-metal total hip resurfacing arthroplasty is well known for preserving the proximal femoral bone stock, minimizing the risk of postoperative dislocation using large femoral heads, and expecting low wear of metal-on-metal articulation for longer prosthesis survival. It also has the advantage in biomechanical loading in the proximal femur. The osteoarthritis secondary to developmental dysplasia of the hip (DDH) has been the most common reason for total hip arthroplasty. Most of the patients are young and active, who require improved range of motion of the hip besides relief of the pain, even expect to resume the ability to run and jump after the joint arthroplasty, thus to be allowed an active lifestyle. The objective of the current study was to evaluate the early outcome of resurfacing arthroplasty for the mild DDH cases (Crowe type I and II).</p><p><b>METHODS</b>Between September 2005 and May 2007, twenty-one consecutive patients (twenty-six hips) with the diagnosis of osteoarthritis secondary to DDH underwent metal-on-metal resurfacing arthroplasty. The average age at the time of surgery was 46.5 years (range, 37-59 years). Six patients (28.6%) were male and fifteen (71.4%) were female. Clinical and radiographic results were observed. The follow-up was performed at 6 weeks, 3, 6, 9 months and then yearly.</p><p><b>RESULTS</b>All patients were followed for a mean of 18 months (9-29 months). During the follow-up period no complications, such as dislocation of hip joints, infection or femoral neck fracture occurred. The clinical outcomes, as rated with the Harris hip score, improved significantly compared with the preoperative ratings. The mean postoperative Harris hip score was 90.7, compared to 35.5 preoperatively. The radiographic analysis showed that all prostheses were fixed with no radiolucencies. All of the patients who had equal limb lengths preoperatively had equal lengths postoperatively. Of the nine patients with preoperative limb-length discrepancy of 0.8 to 1.2 cm, all regained equal limb length postoperatively. In addition the pain was nearly completely relieved, the range of motion was remarkably improved and no restriction was needed after operation regarding early exercise.</p><p><b>CONCLUSIONS</b>The new generation of metal-on-metal resurfacing arthroplasty may be a reasonable option for DDH of the Crowe types I and II.</p>

The synthesized full-length hGLP-1 gene was cloned into pET-32a(+) to get the recombinant plasmid pET32-GLP-1, which could express a fusion protein containing thioredox, hexahistidine, and rhGLP-1 consecutively. The recombinant plasmid containing hGLP-1 was transformed into E.coli BL21 (DE3) and expressed by IPTG induction. The fusion protein was purified from lysates with Ni?IDA His?Bind affinity chromatography. rhGLP-1 with the purity of 90% was achieved after enterokinase digestion, Ni?IDA His?Bind affinity chromatography again, then was concentrated by ultrafiltration. The purified rhGLP-1 showed a single band on IEF gel with an isoelectric point between pH5.2 and pH5.85. ESI mass spectrometry showed that the molecular weight was 3355.0kDa as expected. rhGLP-1 was digested with trypsin followed by mass analysis and the peptide mapping produced two expected fragments with the molecular weights of 2097.7kDa and 1005.5kDa, respectively. The purified rhGLP-1 also showed obvious biological activity for both lowering plasma glucose and stimulating insulin secretion in mice.

<p><b>OBJECTIVE</b>To identify the value of color Doppler flow imaging (CDFI) in the diagnosis of sterility caused by subclinical varicocele (SVC).</p><p><b>METHODS</b>The spermatic veins of 56 sterile patients with seminal abnormality were examined with CDFI and the internal diameters and the time of blood reflux of pampiniform plexus of the veins were observed. In addition, selective X-ray examination of internal spermatic veins was performed for contrast analysis.</p><p><b>RESULTS</b>The diameter of the pampiniform plexus was (2.24 +/- 0.16) mm under the static condition and (2.67 +/- 0. 26) mm during the Valsalva test. The time of blood reflux was (1 487 +/- 203.66) ms. The accuracy of CDFI for diagnosing SVC was 92.8%.</p><p><b>CONCLUSION</b>CDFI has been proved of more value in the diagnosis of SVC than that of clinical varicocele in the etiological screening of male sterility.</p>

<p><b>OBJECTIVE</b>To develop a high performance liquid chromatographic method for the determination of DNJ in Morus alba leaves with fluorimetric detection after precolumn derivatization with 9-fluorenylmnethyl chlorformate (FMOC-Cl).</p><p><b>METHOD</b>DNJ in Morus alba leaves was extracted with 0.05 mol x L(-1) HCl, reacted with FMOC-Cl, and separated on a HiQSiL C18 column at 250 degrees C. Mobile phase consisted of acetonitrile-0.1% aqueous acetic acid (55:45) with a flow rate of 1.0 mL x min. The fluorescence detector was operated at lambdaEX = 254 nm, lambdaEM = 322 nm.</p><p><b>RESULT</b>A satisfactory separation between DNJ and impurity was obtained. The calibration curve was linear over the concentration range from 0.567 microg x mL(-1) to 34 microg mL(-1), r = 0.9998. The average recovery was 97.2%. The contents of DNJ in M. alba collected in different seasons and grown in different environment were determined.</p><p><b>CONCLUSION</b>The quantity of DNJ in leaves of M. alba is related to the environment factor, temperature and growing period. The method can be used for qualioy control of the medicinal material.</p>

Purpose:To investigate the efficacy of infusion gemcitabine via artery port and catheter system (PCS) for advanced pancreatic cancer.Methods:57 cases advanced pancreatic cancer proved by pathology were implanted Implantofix PCS.The number of ports implanted subcutaneously in inferior right abdomen and subcutanceusly in inferior chest was 48 and 9 cases respectively.The first total does in experience group was gemcitabine 3.0,5-FU 3.0,IL-2 6 million u via three times infusion.Results:The overall implanted successful rates was 100% and indwelling catheter tip dislocation rate was 0. Two PCS were pulled out in 2 fat old female patients because of bad wound healing after two months.In the experimental and control group according to the standard of clinical benefit-responces were 37 and 20 respectively.The obvious improve- ment in pain in experimental and control group was 27.2% and 4.8%,respectively.The duration of clinical benefit-re- sponces were 18 weeks in experimental groups,13 weeks in control group.The midian survival in experimental group was 6.25 months,4.41 months in control group.The 6-,9-,12-months survival rates in experimental group were 46%,24%, 18% more thans 31%,6% and 2% in control group.Conclusions:Infusion gemcitabine via artery PCS can not only relieve pain,prolong survival rate but also provides a long-term intra-arterial chemotherapy and effctive access for advanced pancre- atic cancer.

Objective To preliminarily investigate curative effect of hepatic arterial perfusion of p53 gene in the therapy of advanced hepatocellular carcinoma.Methods Tirty cases of advanced hepatocellular carcinoma were divided into experimental group and control group,p53 gene was perfused into target artery confirmed by angiography.In experimental group,10~(12)Vp of p53 gene and 20 mg OPT were employed every week as a course for 21 days and 1 to 4 courses time the cases of the study.In control group,only 20 mg OPT was used.Results The survival period was 14 to 405 days in experimental group and 18 to 167 days in control group.There was a significant difference between the two groups(P<0.05). Conclusion p53 gene is efficacious in the therapy of advanced hepatocellular carcinoma.(J Intervent Radiol,2007,16:127-129)
