CONTEXT: Numerous chronic conditions including obesity exhibit autophagic dysfunction. Association of immune cell autophagic marker regulation by body fat percentage (%BF) is unknown. OBJECTIVE: Investigate autophagy activity in peripheral blood mononuclear cells (PBMCs) of adults with distinct %BFs and obesity-related circulating inflammatory markers. MATERIALS AND METHODS: Sixteen individuals (eight males) with %BF above (n = 8, 36.9 ± 3.6 years, 27.1 ± 8.1%BF) and below (n = 8, 37.1 ± 3.7 years, 13.3 ± 3.7%BF) their age- and sex-specific 50th percentile value based on the American College Sports Medicine guidelines participated. Body fat percentage was calculated from hydrostatic weighing. PBMCs were isolated from venous blood, and PBMC autophagic flux markers (LC3-I, LC3-II, and p62) were measured via Western blot. CRP, resistin, leptin, and adiponectin were measured via ELISA. RESULTS: LC3-II/LC3-I ratio correlated with %BF (r=-0.56, p=.023). Insulin (p=.05) and CRP (p=.018) were higher in high %BF participants. DISCUSSION AND CONCLUSIONS: Autophagic activity markers in PBMCs correlate with %BF, but are not different between %BF groups.

Severe COVID-19 disease is a hyperinflammatory, pro-thrombotic state. We undertook plasma exchange (PEX) to determine its effects on organ function and thrombo-inflammatory markers. Seven critically ill adults with severe COVID-19 respiratory failure (PaO2:FiO2 ratio < 200 mm Hg) requiring invasive or noninvasive ventilatory support and elevated thrombo-inflammatory markers (LDH >800 IU/L and D-dimer >1000 µg/L (or doubling from baseline) received PEX, daily, for a minimum of 5 days. No other immunomodulatory medications were initiated during this period. Seven patients matched for age and baseline biochemistry were a comparator group. Coagulation screening revealed no evidence of coagulopathy. However, von Willebrand Factor (VWF) activity, antigen and VWF antigen: ADAMTS13 ratio, Factor VIII and D-dimers were all elevated. Following 5 days of PEX, plasma levels of all the above, and ferritin levels, were significantly reduced (P < .05) while lymphocyte counts normalized (P < .05). The PaO2:FiO2 ratio increased from a median interquartile range (IQR) of 11.6 (10.8-19.7) kPa to 18.1 (16.0-25.9) kPa (P < .05). Similar improvements were not observed in controls. Acute kidney injury (AKI) occurred among five patients in the control arm but not in patients receiving PEX. PEX improved oxygenation, decreased the incidence of AKI, normalized lymphocyte counts and reduced circulating thrombo-inflammatory markers including D-Dimer and VWF Ag:ADAMTS13 ratio.

RESUMO

A previously unstudied trilingual medieval medical manuscript, ca. 1400, RARES 091 M31, has been in the State Library Victoria, Melbourne, since 1862. The texts in this codex reveal the pedagogical and personal interests of a compiler from the world of Oxford colleges, halls, and libraries in the late fourteenth century. It contains academic medical texts as well as writings of a personal nature-charms, verses, prayers-in Latin, French, and Middle English. It appears to have been associated with Henry Beaumond (d. 1415), whose name appears in the codex. Beaumond was a physician with a problematic association with Exeter College, Oxford University. A good deal of information survives about Beaumond and his books, as well as his association with the influential cleric at New College, Oxford, Walter Awde (d. after 1404), who is also named in the manuscript. This study provides images and a full physical description of the manuscript.

OBJECTIVE: To examine the effect of oral glutamine supplementation on inflammation and fatigue during and after simulated wildland firefighting (WLFF) tasks in hot conditions over 2 consecutive days. METHODS: Eleven men and women ingested a glutamine supplement or a placebo before and after simulated wildland firefighting in an environmental chamber (38 °C, 35% relative humidity). Subjective fatigue, markers of inflammation, and cellular stress were measured pre, post and 4 hours post-exercise on both days. RESULTS: Gastrointestinal damage, subjective fatigue, and ratings of perceived exertion were lower after glutamine supplementation compared with placebo. Heat shock protein 70 (HSP70) and nuclear factor kappa-inhibitor alpha (IκBα) levels were higher on both days of the glutamine trial compared with placebo. CONCLUSIONS: Glutamine supplementation may improve recovery after fire suppression in WLFFs. This may result from the upregulation of HSP70 which inhibits inflammation and protects against gastrointestinal (GI) barrier damage.

RESUMO

Winter moth, Operophtera brumata, originally from Europe, has invaded eastern Massachusetts causing widespread defoliation and damage to many deciduous tree species and a variety of crop plants in the infested area. We identified O. brumata nucleopolyhedrovirus (OpbuNPV) in winter moth larvae collected from field sites in Massachusetts by using PCR to amplify a 482 bp region of the baculovirus polyhedrin gene. Viral sequences were also detected in winter moth pupae that failed to emerge, suggesting that these insects may have died as a result of viral infection. This represents the first report of OpbuNPV in winter moth populations in the US.

RESUMO

Migratory and local bees in Massachusetts were analyzed for seven viruses. Three were detected: black queen cell virus (BQCV), deformed wing virus (DWV), and sacbrood virus (SBV). DWV was most common, followed closely by BQCV and then by SBV. BQCV and SBV were present at significantly higher rates in the migratory bees assayed, bringing into question the impact that these bees have on the health of local bee populations.

BACKGROUND: The Dystrophic Epidermolysis Bullosa (EB) Research Association (DebRA) of New Zealand has run 3 adventure camps specifically geared to the unique and specific needs of teenagers and young adults with EB. OBJECTIVES: We sought to describe how the 2007 winter camp was organized, funded, and run for teenagers and young adults with a range of EB severities. METHODS: Planning and fundraising by DebRA of New Zealand began 1 year before the camp. Nurses and international medical personnel volunteered as camp staff. Instructors qualified to assist persons with disabilities were hired to provide camp activities. The 5-day adventure camp was held at a national park on the North Island of New Zealand. RESULTS: The 2007 camp included 5 campers (aged 21-35 years) with recessive dystrophic EB, 3 of whom used wheelchairs, and two teenagers with EB simplex. All campers were male. Twelve international volunteers assisted with daily dressing changes and camp activities, which included skiing, whitewater rafting, and fly-fishing. LIMITATIONS: Challenges included difficulty in recruiting new campers each year, particularly female campers. CONCLUSION: The camp allowed campers to challenge themselves both physically and mentally, while developing lifelong friendships. It was immensely rewarding for all the volunteers. This camp demonstrated that it is possible to provide such activities safely to severely affected patients.

RESUMO

A single case of carcinoma in situ (actinic keratosis) was treated topically with a patch consisting of an aqueous paste of a commercially available mixture (50:1) of green tea concentrate plus Capsicum (Capsol-T®) for approximately 14 days. The carcinoma responded by the formation of apoptotic blisters whereas surrounding normal tissue showed no response. A second untreated carcinoma 17 cm distant from the treated area also responded indicative of a systemic action of the substance.
