RESUMO

This study describes the prevalence of nursing interventions across six nursing diagnoses and their related factors using the framework of the Nursing Minimum Data Set (NMDS). Six nursing diagnoses (pain, potential for injury, anxiety, decreased cardiac output, potential for infection, and knowledge deficit) were among the most prevalent in an acute care setting studied in 1991. The NMDS and a nursing information system using standardized classification systems for nursing diagnoses and interventions provided an opportunity to describe nursing practice. Multiple related factors were identified across all six nursing diagnoses; three or four were selected frequently within each diagnostic category. The related factors also influenced the selection of interventions. Implications for the development of nursing classification systems and recommendations for further research are described.

The purpose of this study was to obtain information about usage of the Nursing Minimum Data Set (NMDS). Forty-six individuals (44%) who requested the NMDS Data Collection Manual responded to a mailed questionnaire. Most respondents reported actually using the Manual to educate other nurses or to structure nursing documentation. More than one third of the respondents used all of the elements within the NMDS, but the majority used, or intended to use, the elements in the Nursing Care category. The majority of NMDS elements that were being used, or intended to be used, were available from handwritten patient records. The NMDS elements had been recorded on an ongoing basis by 86% of the respondents, but only 31% could retrieve readily all of the elements. Implications for access to comparable, minimum nursing care and resources data are discussed.

RESUMO

The Nursing Minimum Data Set (NMDS) represents the first attempt to standardize the collection of essential nursing data. These minimum core data, used on a regular basis by the majority of nurses in the delivery of care across settings, can provide an accurate description of nursing diagnoses, nursing care, and nursing resources used. Collected on an ongoing basis, a standardized nursing data base will enable nurses to compare data across populations, settings, geographic areas, and time. Public health nurses will be able to evaluate and compare services. The purpose of this article is to discuss briefly the following aspects of the NMDS: background including definition, purposes, and elements; availability and reliability of the data; benefits; implications of the NMDS with emphasis on nursing research; and health policy decision making.

RESUMO

Inconsistent, incomplete reporting of computer applications in nursing is a barrier to building a knowledge base in the field. Progress in building this knowledge base might be accelerated if system developers could use the available literature to build upon work previously done and to profit from lessons already learned. This article presents guidelines for developing articles and reports on computer applications in nursing. Important topics include statement of the problem, proposed solution, description of the system, system impact, relationship to previous work, problems encountered, lessons learned, professional issues, transferability, and future plans. A checklist is provided. The authors welcome comments on these guidelines.

An initial study of the availability of the elements in the Nursing Minimum Data Set (NMDS) and intercoder reliability across four types of clinical settings is reported. These clinical settings included an acute care hospital, a nursing home, a home health care agency, and two ambulatory care clinics. The health records of 116 randomly selected subjects were reviewed to determine the availability of the NMDS elements. A randomly selected subset of 23 of these records provided data on intercoder agreement. All but four of the NMDS elements were available for 85% or more of the subjects. The average intercoder agreement across all NMDS elements was a satisfactory 91%. However, the intercoder agreement on some NMDS elements was much lower, suggesting a need to refine the definitions and procedures for collecting some of the NMDS elements. Where appropriate, coefficient Kappa and Pearson product moment correlation statistics for reliability are reported on individual NMDS elements.

Selected education and employment characteristics of nurses with doctorates were examined. Data were obtained in late 1979 and 1980 about three time periods in the 1,964 respondents' careers: (a) doctoral study, (b) first position, and (c) current position if different from first position. Of the nurses studied, 76% completed doctoral programs in the 1970s. Education was the major field of study most frequently reported, followed by social/behavioral sciences and nursing; federal monies were the major source of financial support. Nurses with doctorates were hired in their first position primarily for teaching in baccalaureate and higher degree programs in nursing with a sizable shift to administrative roles in academia among those reporting current positions different from first positions, Only 6% to 8% of doctorally prepared nurses were hired primarily for the conduct of research in either first or current position.

RESUMO

A national survey of 1,693 persons (primarily nurses) from eight target groups was done to assess views on publication credit assignment. Data were collected using scenarios in a questionnaire based on the instrument developed by Spiegel and Keith-Spiegel (1970). Scenario items and the percentages of 897 respondents selecting each response alternative are presented in an appendix. Response rates indicated that authorship credit assignment is an important issue for nurses functioning primarily in academic settings. Among items selected by 85% or more of 620 nurses in four subsample groups (American Academy of Nursing, Council of Nurse Researchers, authors, and nursing doctoral students) with response rates greater than 50%, 10 points of agreement were found. These points are summarized as possible guidelines for publication credit assignment. Few differences were found among scenario item responses of: (a) subjects classified as high and low publishers, (b) those with research as their major role responsibility and others, and (c) doctoral students and others.
