RESUMO

The purpose of the present study was to examine the relationship between five algorithm-derived functional movement screening scores (i.e., readiness, explosiveness, functionality, dysfunction, and vulnerability) obtained from an innovative three-dimensional markerless motion capture system (3D-MCS) and some of the key health-related physical fitness parameters such as maximal aerobic capacity (VO2max), body mass index (BMI), body fat percentage (BF%), waist and hip circumferences (WC and HC), and high-density lipoprotein cholesterol (HDL-C). BF% showed a weak positive correlation with vulnerability and moderate-to-strong negative correlations with readiness, explosiveness, and functionality scores. Similarly, but opposite to BF%, VO2max showed a weak negative correlation with vulnerability and moderate-to-strong positive correlations with readiness, explosiveness, and functionality scores. BMI, WC, and HC showed moderate negative correlations with vulnerability, readiness, and functionality scores, while HDL-C showed a weak positive correlation with vulnerability and a weak negative correlation with explosiveness scores. Therefore, it appears that 3D-MCS may be used a as a non-invasive testing alternative or in conjunction with currently implemented traditional testing modalities to provide health practitioners with additional information regarding some of the key health-related physical fitness parameters, especially within non-academic environments such as wellness and clinical settings.

RESUMO

In-office needle arthroscopy offers the potential advantage of reduced injury to intervention time, without the need for advanced imaging. It is particularly appropriate for those with contraindications to advanced imaging and also may reduce the risk of incorrect diagnoses in those situations in which imaging is associated with low sensitivity/specificity. The purpose of this article is to provide a standardized diagnostic approach to needle arthroscopy of the shoulder.

OBJECTIVES: The presentation of synchronous bilateral renal lesions is rare. We report our experience with the surgical management of these lesions in both a single and staged procedure. METHODS: We retrospectively reviewed the records of all patients with bilateral synchronous renal lesions who underwent surgical management by one surgeon between 2000-2007. We compared characteristics including pre and postoperative renal function, complication rates, and oncological outcomes between the single and staged cohorts. Data were analyzed using descriptive statistics, Student's t-test, and Fisher's exact test. RESULTS: A total of 26 patients (73% male, mean age 65.5 +/- 12.2 years) with bilateral synchronous lesions were identified with a mean follow-up of 25.9 +/- 19.7 months. Of these, 18 (69%) were performed as a single procedure, 5 (19%) were done as a staged procedure, and 3 (12%) had only the first part of the staged procedure performed. The single and staged cohorts were comparable in regards to preoperative creatinine (Cr) (1.1 +/- 0.4 mg/dl versus 1.1 +/- 0.2 mg/dl, p = 0.70), postoperative Cr (1.5 +/- 1.0 mg/dl versus 1.4 +/- 0.5 mg/dl, p = 0.73), and median hospital length of stay (HLOS) (5 days versus 4 days). The complication rate was 22% and 20% for the single and staged cohorts, respectively. One patient had a local recurrence and one patient developed metastatic disease in the single cohort versus no local recurrence or metastatic disease in the staged cohort. CONCLUSION: In the appropriate setting, surgical management of synchronous bilateral renal lesions can be done safely in a single procedure with comparable outcomes to those done in a staged manner.

RESUMO

Encrustation is a well-established complication of retained biomaterials in the urinary tract. Severe stent encrustation is a potentially serious complication of prolonged indwelling ureteral stenting often managed with open surgery when endoscopic techniques are unsuccessful. We present a case of a 2800-mm2 stent encrustation managed with serial nephroscopy and laser lithotripsy.

OBJECTIVE: The aims of this study were to determine how often the distal thoracic duct can be identified on neck CT and to characterize the CT appearance of the duct. MATERIALS AND METHODS: In a review of electronic medical records from January 2001 to January 2003 we identified the cases of 500 patients who had undergone CT of the neck. Because they had confounding factors such as cancer or cervical lymphadenopathy, 199 of these patients were excluded, leaving 301 patients in the study: 131 (44%) male patients and 170 (56%) female patients. The age range was 11-92 years (average age, 46 years). Two head and neck radiologists used strict diagnostic criteria and consensus to identify the distal thoracic duct on both sides of the neck. One half of the images selected at random were flipped left to right. The purpose of randomization was to avoid interpretation bias, because the thoracic duct is known to typically course within the left side of the neck. The configuration of the distal duct was tabulated, and effects of age and sex were statistically evaluated. RESULTS: The left side of the neck was unevaluable in 26 (9%) of 301 patients because of streak artifact. In the other 275 patients, the distal thoracic duct was identified in the left side of the necks of 150 (55%) of the patients. Eleven of these patients (4%) also had a visible duct in the right side of the neck, but a right-sided duct was never identified without a left-sided counterpart. The distal thoracic duct had a tubular configuration in 70 (43%), a flared configuration in 72 (45%), and a long segmental fusiform dilation in 19 (12%) of 161 patients. Patient sex had no significant effect on the appearance of the distal thoracic duct. Older patient age had a slight positive effect on the size of the duct. CONCLUSION: Familiarity with the normal CT appearance of the distal thoracic duct can be helpful in differentiating a normal duct from pathologic lesions of the lower neck, such as lymphadenopathy.

RESUMO

The reductive lithiation of alkyl and vinyl phenyl thioethers by aromatic radical anions is shown to be the most general method yet known for preparing organolithiums capable of intramolecular carbometalation of unactivated alkenes to produce five-membered rings and in one case a four-membered ring (in a far higher yield than known cases). The relative rates of cyclization for alkyllithiums are secondary > tertiary > primary, and the yields are very high. In the secondary case, the stereoselectivity is extremely high, producing a cyclopentylmethyllithium with a trans-2-alkyl substituent. A remarkable finding is that for all of the organolithiums a lithium oxyanionic group in the proximal allylic position to the alkene greatly accelerates the cyclization and leads almost exclusively to a trans relationship between the CH(2)Li group and the OLi group, the opposite relationship from that observed in intramolecular carbolithiations by allyllithiums. A mechanistic rationale for this divergence is discussed. One of the two types of proximal homoallylic lithium oxyanions exerts an analogous effect. An intriguing limitation, even occurring with the highly reactive secondary organolithium and in the presence of an allylic oxyanionic group, is the failure of intramolecular carbolithiation when a methyl group is at the terminus of the alkene.
