RESUMO

Pain is the most frequent complaint of patients visiting all types of physicians. With pain perception, sensation is described and perceived as causing actual tissue damage. In contrast, nocioception is defined as a response specific to a potentially tissue-damaging stimuli. This articles defines the various nocioceptive pathways and connections.

RESUMO

The authors used magnetic resonance imaging (MRI) to evaluate the formation rate of avascular necrosis following performance of 20 modified Austin bunionectomies. Five modified McBride bunionectomies without first metatarsal osteotomy were also performed as an MRI control. Results showed an avascular necrosis formation rate of 50%. The majority of the avascular necrosis areas were found dorsally within the cancellous bone substance of the first metatarsal head. These lesions, in all cases, did not cause any patient disability or result in any decline in the degree of patient satisfaction. The MRI positive avascular necrosis evaluations do suggest potential problem areas with the surgical technique that may be eliminated through further modification of the classic Austin bunionectomy procedure.

RESUMO

A retrospective analysis was performed to determine the long-term results of hallux abducto valgus surgery done under the auspices of the Texas Health Science Center at San Antonio and its teaching hospitals. Two hundred eighty-five patients with a postoperative follow-up ranging from 3 to 10 years were studied. Attempts were made to study many of the patient demographic factors and perioperative procedures that acted to affect the development of a long-term complication. It was found that type of procedure, past medical history, age, medical control, and location of the surgical suite were all important variables to consider in this regard.
