RESUMO

Acute cholecystitis accounts for up to 9% of hospital admissions for acute abdominal pain, and best practice entails early surgical management. Ultrasound is the standard modality used to confirm diagnosis. Our objective was to perform a systematic review and meta-analysis to determine the diagnostic accuracy of emergency physician-performed point-of-care ultrasound for the diagnosis of acute cholecystitis when compared with a reference standard of final diagnosis (informed by available surgical pathology, discharge diagnosis, and radiology-performed ultrasound). We completed a systematic review and meta-analysis, registered in PROSPERO, in adherence to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched 7 databases as well as gray literature in the form of select conference abstracts from inception to February 8, 2023. Two independent reviewers completed study selection, data extraction, and risk of bias (QUADAS-2) assessment. Disagreements were resolved by consensus with a third reviewer. Data were extracted from eligible studies to create 2 × 2 tables for diagnostic accuracy meta-analysis. Hierarchical Summary Receiver Operating Characteristic models were constructed. Of 1855 titles/abstracts, 40 were selected for full-text review. Ten studies (n=2356) were included. Emergency physician-performed point-of-care ultrasound with final diagnosis as the reference standard (7 studies, n=1,772) had a pooled sensitivity of 70.9% (95% confidence interval [CI] 62.3 to 78.2), specificity of 94.4% (95% CI 88.2 to 97.5), positive likelihood ratio of 12.7 (5.8 to 27.5), and negative likelihood ratio of 0.31 (0.23 to 0.41) for the diagnosis of acute cholecystitis. Emergency physician-performed point-of-care ultrasound has high specificity and moderate sensitivity for the diagnosis of acute cholecystitis in patients with clinical suspicion. This review supports the use of emergency physician-performed point-of-care ultrasound to rule in a diagnosis of acute cholecystitis in the emergency department, which may help expedite definitive management.

This study examined lower extremity joint kinematics in individuals with chronic ankle instability (CAI) and controls during unanticipated and anticipated single-leg drop landings onto a laterally inclined platform. Physically active adults with CAI 15 (n = 15) and controls (n = 15) performed an unanticipated and anticipated 30 cm single-leg drop landing onto a 20° laterally inclined platform. Three-dimensional ankle, knee and hip-joint kinematics were recorded 200 ms pre- to 200 post-landing and analysed with a 2 (group) × 2 (landing condition) SPM ANOVA (p < 0.05). Results revealed individuals with CAI displayed significantly greater ankle internal rotation post-landing across both landing conditions. Anticipated landings elicited significantly greater pre-landing ankle inversion and external rotation, knee abduction and hip adduction. Additionally, significantly less ankle inversion, knee and hip flexion, and knee adduction and hip abduction were present during post-landing of the anticipated landing. Greater ankle internal rotation during landing may contribute to the ankle 'giving way' in individuals with CAI. However, preparatory and reactive proximal-joint kinematics were similar in both groups during landing. This highlights the possible role of the knee and hip joints in assisting with ankle-joint stability during anticipated inversion perturbations.

Shoulder and elbow injuries among baseball pitchers of various ages and abilities continue to rise despite exhaustive efforts analysing pitch type and count; however, it has yet been determined if footwear plays a role in altering pitching mechanics and subsequently injury susceptibility. Therefore, the purpose of the study was to investigate the effect footwear might have on youth baseball pitching mechanics. Data were collected on eleven youth baseball pitchers wearing two different shoes on two different surface inclinations. A repeated measures ANOVA was utilised to determine differences between upper and lower extremity joint kinematics and kinetics (p < 0.05). Results indicate wearing moulded cleats elicited significantly greater amounts of shoulder internal rotation torque, angle and velocity as well as elbow varus torque. Turf shoes caused an increased plantarflexion joint angle in the stride leg ankle and shoulder external rotation torque, angle and velocity. The findings of this study suggest that the footwear worn by a youth baseball pitcher does alter the shoulder and elbow dynamics in the dominant throwing arm as well as the amount of ankle plantarflexion in the stride leg.

Peptide transporters play important nutritional and cell signalling roles in Bacillus subtilis, which are pronounced during stationary phase adaptations and development. Three high-affinity ATP-binding cassette (ABC) family transporters are involved in peptide uptake - the oligopeptide permease (Opp), another peptide permease (App) and a less well-characterized dipeptide permease (Dpp). Here we report crystal structures of the extracellular substrate binding proteins, OppA and DppE, which serve the Opp and Dpp systems, respectively. The structure of OppA was determined in complex with endogenous peptides, modelled as Ser-Asn-Ser-Ser, and with the sporulation-promoting peptide Ser-Arg-Asn-Val-Thr, which bind with K d values of 0.4 and 2 µM, respectively, as measured by isothermal titration calorimetry. Differential scanning fluorescence experiments with a wider panel of ligands showed that OppA has highest affinity for tetra- and penta-peptides. The structure of DppE revealed the unexpected presence of a murein tripeptide (MTP) ligand, l-Ala-d-Glu-meso-DAP, in the peptide binding groove. The mode of MTP binding in DppE is different to that observed in the murein peptide binding protein, MppA, from Escherichia coli, suggesting independent evolution of these proteins from an OppA-like precursor. The presence of MTP in DppE points to a role for Dpp in the uptake and recycling of cell wall peptides, a conclusion that is supported by analysis of the genomic context of dpp, which revealed adjacent genes encoding enzymes involved in muropeptide catabolism in a gene organization that is widely conserved in Firmicutes.

The vertical jump has been shown to be an effective tool in assessing neuromuscular fatigue. The two most common iterations of the vertical jump are the countermovement and squat jumps. This investigation sought to identify if differences exist between the two jumping strategies with regard to electromyography (EMG) and kinetics in a group of recreationally trained males. Twenty-two participants completed one experimental session, where three countermovement (CMJ) and three squat jumps (SJ) were performed using a counterbalanced within-subject design. Jump performance was evaluated with data obtained using a force platform. Additionally, EMG was collected on the vastus lateralis (VL), vastus medialis (VM), semitendinosus (ST) and medial gastrocnemius (MG). Greater EMG values were seen in the CMJ for ST as well as percentage of activation in the MG (p < 0.05). Increased values of mean force and mean power were observed in the SJ, while the CMJ showed greater peak and mean velocity. Greater jump heights in the CMJ were present as well (p < 0.05). These findings suggest that the increase in CMJ jump height due to the increase in propulsive velocity is not due to increases in knee extensors muscle activation.

BACKGROUND: Mal de debarquement (MdD), or often called 'sea legs', is the perception of self-motion after exposure to passive movement such as being on a boat at sea. Previous studies highlight sensory re-organization difficulties and postural control impairments after disembarking from sea travel in experienced crew members. However, the impact of MdD in individuals with minimal offshore experience, defined as participating in less than 2 offshore excursions per year, has not been investigated. RESEARCH QUESTION: Does exposure to boat motion while at sea alter static postural control after disembarking in individuals with minimal offshore experience? METHODS: Healthy adults (n = 24) with minimal offshore experience had their static balance assessed on a force platform before (PRE) and after (POST) a 7-h deep sea fishing excursion. Static balance was tested in eyes open (EO), eyes closed (EC), eyes open on a foam surface (EOF), and eyes closed on a foam surface (ECF) conditions. Sway excursions, sway velocity and sway variability in the medial-lateral (ML) and anterior-posterior (AP) directions were computed and then compared PRE/POST using a paired t-test (p < 0.05). RESULTS: Significant increases in ML sway excursion (p = 0.004), ML sway range (p < 0.001), ML sway variability (p < 0.001), AP sway excursion (p = 0.045), AP sway range (p = 0.020), and AP sway variability (p = 0.030) were observed at POST during EOF. Significant increases in ML sway excursion (p = 0.027), AP sway excursion (p = 0.020), and AP sway variability (p = 0.014) at POST were also observed during ECF. No differences were found in the EO condition (p > 0.05). SIGNIFICANCE: Increases in postural sway excursion and variability were observed in individuals with minimal offshore experience after disembarking. Our findings suggest sensory re-organization difficulties in order to maintain an upright posture in challenging sensory conditions are dependent on vestibular and somatosensory inputs following exposure to boat motion at sea.

This study examined the effects of medial longitudinal arch (MLA) flexibility on kinetics during the eccentric and concentric subphases of a drop vertical jump (DVJ). Physically active adults with flexible (n = 16) and stiff (n = 16) MLA completed DVJs onto a force platform from a height of 30 cm. Eccentric and concentric subphases of the DVJ were identified from the vertical ground reaction force (GRF) data. Jump height, ground contact time, reactive strength index (RSI), vertical center-of-mass depth, vertical stiffness and time of the eccentric and concentric subphases were evaluated. Amortization force, peak vertical GRF and vertical impulse were also obtained for the eccentric and concentric subphases of the DVJ. Dependent variables were compared between groups using independent samples t-tests (p < 0.05). Significantly greater vertical stiffness (p = 0.048; ES = 0.63) was found in the stiff arch group (-173.91 ± 99.73 N/kg/m) compared to the flexible arch group (-122.95 ± 63.42 N/kg/m). A moderate-magnitude difference (ES = 0.58) was observed for RSI between flexible (0.89 ± 0.39) and stiff arch (1.20 ± 0.70) groups, but was not significant (p = 0.063). The active and passive structures supporting the MLA may be used differently to achieve similar vertical jump height during a DVJ. Additional research is warranted to further understand the contributions of MLA flexibility to jumping performance.

BACKGROUND: As physical fitness has become more of a central component of competitive golf it is important to have an understanding of the relationship between anthropometric and physical performance on actual golf performance. Thus, the purpose of this investigation was to determine the relationship between measures of anthropometrics and physical performance to golf swing performance. METHODS: Fourteen division I collegiate golfers performed a battery of tests including the vertical jump, grip strength, rotational medicine ball toss, and sit and reach test in addition to anthropometric measure measurements. Golf specific variables included clubhead speed (CHS), ball velocity (BV) and carry distance (CD) using the participants own driver. Pearson product moment correlations were used to assess the level of relationship between all variables. RESULTS: Statistically significant large positive relationships were found between grip strength and all golf specific variables. Height also showed large significant positive relationship with both CHS and BV. CONCLUSIONS: The association between anthropometrics and physical performance and golf specific variables appear to be limited in a group of collegiate male golfers, outside of grip strength. However, this does not mean that physical fitness is irrelevant to golf performance, but caution should be taken in assuming increase in physical performance will have a direct impact on the golf specific variables.

The vertical jump is commonly used as a means of evaluating athlete readiness. Athletes have been shown to arrive to training and competition in a hypohydrated state. Thus, this investigation sought to examine the impact of hydration status on both countermovement (CMJ) and squat jump (SJ) performance. Twenty-five recreationally trained males completed three CMJ and SJ in a euhydrated, hypohydrated and control condition. Conditions were separated by a minimum of 24 hours. Hydration status was assessed using urine specific gravity. Jump performance was evaluated using both kinematic and kinetic data obtained from a force platform. A repeated-measures ANOVA was performed for each variable of interest in both the CMJ and SJ. CMJ peak and mean force values were significantly greater in the euhydrated condition compared to the hypohydrated condition (p < 0.05), with no differences between the control condition and either experimental condition. SJ showed reductions in jump height, peak and mean velocity, peak and mean power and impulse from control and euhydrated conditions (p < 0.05). The findings of this investigation show that when performing jump testing, specifically SJ, that hydration status of the individual may impact commonly used variables to assess the readiness of the individual for a given day.

This study assessed ankle kinematics, surface electromyography, and center-of-pressure (COP) progression relative to the medial border of the foot during a side-cutting task in individuals with and without chronic ankle instability (CAI). Thirty participants (CAI = 15; Controls = 15) performed a side-cutting task on a force platform while 3-dimentional ankle kinematics, COP position, and surface electromyography from the tibialis anterior, medial gastrocnemius, fibularis longus, fibularis brevis, vastus medialis, and semitendinosus were recorded on the testing leg. Ankle kinematics, root-mean-square muscle activity and COP position relative to the medial boarder of the foot were compared between CAI and healthy controls (p < 0.05). Significantly greater ankle internal rotation from 35-54% of the stance phase (p = 0.032) was found for the CAI group compared to controls. Furthermore, significantly greater tibialis anterior muscle activity from 86-94% of the stance phase (p = 0.022) and a more medial COP position from 81-100% (p < 0.05) and of the stance phase was also observed in the CAI group. Less lateral COP progression and increased tibialis anterior activation in the CAI group could reflect a protective movement strategy during anticipated side-cutting to avoid recurrent injury. However, greater ankle internal rotation during mid-stance highlights a potential 'giving way' mechanism in individuals with CAI.

OBJECTIVES: This study evaluated center-of-pressure (COP) and time-to-boundary (TTB) measures of postural control during a Lateral Step-Down Test in participants with chronic ankle instability (CAI). DESIGN: Cohort study. SETTING: Biomechanics laboratory. PARTICIPANTS: Physically active adults with CAI (n = 15) and matched controls (n = 15). MAIN OUTCOME MEASURES: Traditional COP and TTB measures of postural control were computed in the medial/lateral (ML) and anterior/posterior (AP) directions. RESULTS: No significant results were found for the traditional COP measures (p > 0.05). The CAI group exhibited a lower TTB ML absolute minimum on their affected limb compared to the matched limb of the control group (p < 0.001). Additionally, on average the CAI group displayed significantly lower TTB ML mean of minima (p = 0.004) and TTB standard deviation of minima in the ML (p < 0.001) and AP directions (p = 0.002) regardless of limb. CONCLUSIONS: Sensorimotor impairments associated with CAI negatively alter spatiotemporal postural control and may cause a maladaptive reorganization of centrally mediated motor control strategies that results in bilateral postural control deficits during the Lateral Step-Down Test. In addition, traditional COP measures did not reveal any postural control deficits suggesting that a spatiotemporal analysis should be used when assessing postural control in participants with CAI.

Anticipatory responses to inversion perturbations can prevent an accurate assessment of lateral ankle sprain mechanics when using injury simulations. Despite recent evidence of the anticipatory motor control strategies utilized during inversion perturbations, kinetic compensations during anticipated inversion perturbations are currently unknown. The purpose of this investigation was to examine the influence of anticipation to an inversion perturbation during a single-leg drop landing on ankle joint and impact kinetics. Fifteen young adults with no lateral ankle sprain history completed unanticipated and anticipated single-leg drop landings onto a 25° laterally inclined platform from a height of 30 cm. One-dimensional statistical parametric mapping (SPM) was used to analyze net ankle moments and ground reaction forces (GRF) during the first 150 ms post-landing, while peak GRFs, time to peak GRF, peak and average loading rates were compared using a dependent samples t-test (p ≤ 0.05). Results from the SPM analysis revealed significantly greater plantar flexion moment from 58 to 83 ms post-landing (p = 0.004; d = 0.64-0.77), inversion moment from 89 to 91 ms post-landing (p = 0.050; d = 0.58-0.60), and medial GRF from 62 to 97 ms post-landing (p < 0.001; d = 1.00-2.39) during the unanticipated landing condition. Moreover, significantly greater peak plantarflexion (p < 0.001; d = 1.10) and peak inversion moment (p = 0.007; d = 0.94), as well as greater peak (p = 0.002; d = 1.03) and average (p = 0.042; d = 0.66) medial loading rates, were found during the unanticipated landing condition. Our findings suggest alterations to ankle joint and impact kinetics occur during a single-leg drop landing when inversion perturbations are anticipated. Researchers and practitioners using drop-landings onto a tilted surface to assess lateral ankle sprain injury risk should consider implementing protocols that mitigate anticipatory responses.

While neuromuscular control deficits during inversion perturbations in chronic ankle instability (CAI) cohorts are well documented in the literature, anticipatory motor control strategies to inversion perturbations in CAI are largely unknown. The purpose of this study was to examine neuromuscular control and ankle kinematics in individuals with CAI (n = 15) and matched controls (n = 15) during unexpected and expected single leg drop-landings onto a tilted surface rotated 20° in the frontal plane. Muscle activity from 200 ms pre- to post-landing was recorded from the tibialis anterior (TA), medial gastrocnemius (MG), peroneus longus (PL) and peroneus brevis (PB). Mean muscle activity, co-contraction index (CCI), and peroneal latency was analyzed. Ankle inversion angle at initial contact, time to maximum inversion angle, maximum inversion angle and velocity were also assessed. Significantly longer PL latency, less time to maximum inversion and greater maximum inversion angle was found in CAI compared to controls. Regarding landing condition, significantly greater maximum inversion angle, less inversion at initial contact, longer PB latency, less TA activity and frontal plane CCI during the post-landing phase was found during the unexpected perturbation. Prolonged PL latency and altered ankle kinematics suggests reduced frontal plane ankle stabilization in CAI. However, similar motor control strategies were utilized in both groups during the ankle inversion perturbations.

The effects of various weighted implements on baseball swing kinematics in collegiate baseball players. J Strength Cond Res 33(5): 1347-1353, 2019-The purpose of this study was to investigate the effects of different warm-up (WU) devices on bat swing parameters including maximal resultant velocity (MRV), resultant velocity at ball contact (RVBC), time difference between MRV and RVBC, bat angle at MRV, bat angle at RVBC, and perceptual differences of each WU implement used by National Collegiate Athletic Association Division-I baseball players. Fifteen varsity baseball players completed 1 experimental session during fall training. Retroreflective markers were placed on the bat and tee to measure basic bat kinematics during the swing. Participants completed a general calisthenics WU before being counter-balanced into 1 of 4 WU conditions: standard bat (SB) (33 in/30 oz), fungo (10.6 oz), weighted gloves with SB (weighted gloves) (55.6 oz) and donut with SB (donut) (55.6 oz). Each participant was asked to perform their normal on-deck routine over a 2-minute period, finishing with 5 practice swings with the designated condition. After completion of the WU, a 1-minute rest period (simulating normal game conditions) was given to allow each participant to get set to perform 5 maximal swings with a SB. Five, 1 × 4 (group × condition) repeated measures analysis of variance examined the aforementioned variables. There were no significant differences in MRV, RVBC, time difference between MRV and RVBC, and bat angle at MRV and RVBC between all WU conditions. If presented with the current options, athletes should choose the WU implement with which they are most comfortable using before an at-bat situation.

Postural stability has been shown to be impacted by footwear and task performed. This study analysed the impact of two military footwear, standard boot (STB) and minimalist boot (MTB) on postural stability, before (PRE) and after (POST) a load carriage task. Sixteen participants were tested for postural stability using sensory organisation and motor control tests on Neurocom Equitest™. Postural sway, equilibrium scores and postural latencies were analysed using a two-factor repeated measures ANOVA: boot type (STB-MTB) × time (PRE-POST) load carriage task. Significantly greater postural sway variables, lower balance scores and slower postural latencies were seen in STB and POST load carriage conditions (p < .05). The results suggest that MTB exhibited greater balance compared to STB in balance conditions that rely on somatosensory feedback and that balance is lowered after a load carriage task. Decrements in postural stability could be attributed to boot design characteristics and muscular exertion due to the load carriage task. Practitioner Summary: Maintaining optimal postural stability is crucial in military. Impact of military footwear types and load carriage task on postural stability are addressed. Findings provide footwear design and physical exertion implications on postural stability leading to potential interventions that reduce postural stability decrements; thereby, reducing potential falls and fall related injuries.

The use of vibrating platforms has become increasingly available, and popular at sports and rehabilitation institutes. Given the discrepancies in the literature regarding whole body vibration (WBV) and human reflexive responses, the purpose of this study was to examine the acute effects of WBV on postural response latencies, as well as associated electromyography measures of the lower extremities during balance perturbations. Reflexive responses during backward and forward balance perturbations were examined before, after, and 10 min after a bout of WBV. The findings suggest that following an acute bout of whole body vibration, muscle activity of the lower extremities is decreased during a reflexive response to an unexpected perturbation, and may be associated with faster reaction time.

OBJECTIVE: Studies have looked at the individual associations of sensory impairment on balance, but no population-based studies have examined their combined association on balance and difficulty with falls. Thus, the purpose of this study was to examine both the independent associations and combined associations of visual impairment, peripheral neuropathy, and self-reported hearing loss with the odds of reporting difficulty with falls and functional balance. METHODS: Data from the 2003-2004 National Health and Nutrition Examination Survey were used. Vision and peripheral neuropathy were objectively measured, and hearing was self-reported. Balance testing consisted of a modified Romberg test. After exclusions, 1662 (40-85years of age) participants provided complete data on the study variables. RESULTS: Sensory impairment was associated with perceived difficulty of falls and functional balance. Participants who presented a single sensory impairment had 29% reduced odds of having functional balance (95% CI=0.54-0.93, p=0.01) and increased odds of reporting difficulty with falls by 61% (95% CI=0.99-2.60, p=0.05). Moreover, our multisensory models showed some evidence of a dose-response relationship, in that sensory impairment of multiple sensory systems was associated with worse balance (OR =0.59, CI=0.35-1.00, p=0.05) and perceived difficulty of falls (OR =5.02, 95% CI=1.99-12.66, p=0.002) when compared to those with less sensory impairment. CONCLUSION: Multiple sensory impairment is associated with significantly higher odds of both reporting difficulty with falls and balance dysfunction, which may lead to a subsequent fall, ultimately compromising the individual's health.
