RESUMO

The communication of results of HIV tests and chest-X-rays because of persistent coughing are of particular interest because potential life-threatening disease may be disclosed. For HIV tests it is recommended that the result is communicated to the patient in the doctor's office face to face. In this questionnaire study based on two simulated case-stories with a 63 year-old man referred to chest-X-rays because of persistent coughing, and a 27 year-old man, who had been living in Africa for some time, now wanting a HIV test performed, we found that only half (53%) of the general practitioners (GP) did communicate HIV test results in the consultation office. X-ray test results were only communicated in the consultation office by 18% of the GPs. Communication of test results which might have serious implications to the patient should preferably not be done by telephone. Patients should be told of potentially serious results in person by their own physician.

With the first report of an HIV-positive Inuit in 1985 Greenland was considered a high risk area for a self-sustained heterosexual HIV-epidemic, due to high incidences of sexually transmitted diseases. In 1986 a national HIV-intervention programme began with two major aims: Firstly, improved STD treatment and incitation to change of sexual high-risk behaviour, secondly, close monitoring of HIV-antibody test activity, partner notification and repeated surveys of knowledge and sexual behaviour in selected populations. Very early the spread of HIV in Greenland became heterosexually transmitted and self-sustained, with repeated local small-scale epidemics in a subpopulation characterized by frequent STD's, alcoholism, lack of education and vulnerable social and economic resources. From 1994 to 1997 a marked increase in new cases was reported, and in November 1997 the total number was 76 HIV-positives in the Greenlandic population of 50,000 Inuits. The successful defeat of STD's, such as gonorrhoea and syphilis has not been followed by a change in sexual behaviour among the youth in Greenland. High incidences of genital chlamydial infections, early sexual debut and high frequencies of excessive promiscuity render Greenland an alarming potential for a future HIV-epidemic. This threat calls for an intensified national prevention programme, with close monitoring of the known HIV-positives and the potentially infected persons in the population at risk, and new campaigns to reduce high-risk sexual behaviour among the youth.

A method for the use of a biotinylated antibody (DAKO-ER 1D5) to quantify oestrogen receptors (ER) on tumour cells by flow cytometry is described. ER quantification was determined after treatment with saponin rendering cells permeable to ER antibody. Use of dual parameter labelling was performed utilizing a FITC-conjugated antibody (NCL-5D3) directed against cytokeratin 8/18. This allowed selection of breast cancer cells of epithelial origin by gating to exclude contaminating inflammatory and stromal cells. Use of such a gating technique was seen to identify cells with a higher level of ER expression. Using QC quantum bead standards, the number of ER binding sites per cell was assessed. Results were compared with conventional ER quantification using a radio-ligand binding assay. A high degree of correlation was found between the two methods. The flow cytometric method for ER quantification described is simple, rapid, and reproducible. The assay may be of particular value in measuring ER on urgent clinical samples. Advantages of this assay over the radio-ligand binding assay include reduction in use of radio-labelled iodine compounds, a decrease in analysis time, and reduced cost and quantity of material needed for assay.

In 4 inbred strains of guinea pig the tendency to develop peripheral high-eosinophilia was shown to be genetically controlled. The development of eosinophilia with age and following immunization was examined in high- and low-eosinophilic parental strains, in F1-hybrids and in backcross offspring. The results show that probably only one or a very few segregating genes control eosinophilia, and they also indicate that different genes are involved in the determination of spontaneous (age-related) and immunization-induced eosinophilia.

Development of eosinophilia was studied in four strains of guinea pigs (gp), selectively bred for either high or low respiratory anaphylactic reactivity. One high-asthma strain (IMM/S 209) and one low-asthma strain (IMM/R 203) developed spontaneous high blood eosinophilia. The 2 other gp strains - one high-asthma strain (IMM/S 740) and one low-asthma strain (IMM/R 201-16) - maintained normal low levels of eosinophilic granulocytes (eos). The levels of eos in various tissues showed similar differences between the gp strains. Following immunization with ovalbumin/Al(OH)3 the levels of blood eos increased significantly only in gp of strain 209. The blood eos levels in gp of all 4 strains decreased significantly following immunization with ovalbumin in Freund's complete adjuvant (FCA).
