RESUMO

PURPOSE: This study assessed the prevalence of posttraumatic stress symptoms in young adult survivors of childhood cancer and the association of posttraumatic stress with anxiety, adjustment, perceptions of illness and treatment, and medical data extracted from oncology records. PATIENTS AND METHODS: Seventy-eight young adults (ages 18 to 40 years) who had been treated for childhood cancer completed questionnaires and psychiatric interviews assessing posttraumatic stress, anxiety, perceptions of their illness and treatment, and symptoms of psychologic distress. Data on treatment intensity and severity of medical late effects were collected via chart review. RESULTS: Of the patient sample, 20.5% met American Psychiatric Association Diagnostic and Statistical Manual criteria for posttraumatic stress disorder (PTSD) at some point since the end of their treatment. Clinically significant levels of intrusive (9%) and avoidant (16.7%) symptoms were reported. Participants also reported elevated state and trait anxiety. Participants with PTSD reported higher perceived current life threat, more intense treatment histories, and higher (and clinically significant) levels of psychologic distress than those who did not have PTSD. CONCLUSION: One-fifth of this sample of young adult survivors of childhood cancer met criteria for a diagnosis of PTSD, with clinically significant symptoms of intrusion and avoidance reported. As in other samples, PTSD in young adult survivors was associated with anxiety and other psychologic distress. Survivors' perceptions of treatment and its effects were more highly associated with posttraumatic stress than were more objective medical data. The data suggest that cancer-related posttraumatic stress may emerge in young adulthood and may affect the achievement of developmental milestones and orientation toward health care.

RESUMO

An education specialist in follow-up clinics for childhood cancer survivors assists these individuals in meeting their complex educational needs. Most survivors present with unique educational profiles as a result of their cancer diagnosis or treatment. Children at risk for neurocognitive late effects are especially in need of ongoing educational consultation. Lack of funding for educators in the clinical setting results in the need for nurse practitioners to take on this role. However, nurse practitioners may feel inadequate in regard to their knowledge of the educational system. This article presents a model for targeting high-risk patients and monitoring their educational needs and gives practical advice in advocating needed services in the school system.

Preliminary results of a long-term, prospective therapeutic trial with mebendazole in 28 patients mainly with inoperable echinococcosis are reported. The course of disease was monitored closely and plasma mebendazole levels were checked regularly. A major problem is the lack of reliable methods for defining and measuring early success or failure of therapy. Most of the patients improved clinically. No marked change of parasitic lesions was observed except in one patient with constantly low plasma mebendazole levels who deteriorated. No serious side effects occurred. The preliminary results are encouraging and suggest that mebendazole has a "parasitostatic" effect on larval growth.

A total of 77 adult and 123 juvenile willow grouse ( Lagopus lagopus ) from the island of Karlsöy, and 75 adult and 63 juvenile willow grouse from Sennaland on the mainland of North Norway were examined for helminths. Raillietina urogalli and Hymenolepis microps , and Ascaridia compar and Syngamus trachea were found. All four species occurred regularly in grouse from Karlsöy between April and September-October, but only H. microps was frequently found in birds from Sennaland. S. trachea only parasitised juvenile grouse.

RESUMO

21.4% of the willow grouse (Lagopus lagopus) examined from a coastal locality in northern Norway were infected with Syngamus trachea. None of the willow grouse examined from an inland locality with colder and longer winters were infected with the parasite. It is suggested that the parasite may influence survival of the grouse population.
