RESUMO

Attributions regarding the nature, cause, and best treatment of mental health problems were examined in a total of 563 adults with serious mental illness from four ethnocultural groups, 185 family members, and 43 mental health practitioners. Attributions agreed in many areas, such as stress as a cause, and medical intervention and counseling as best treatment. Differences were found between the practitioner and other groups in attributions regarding the senses in the nature of mental illness, and in drugs and alcohol as a cause. The role of ethnicity was examined and treatment implications discussed.

OBJECTIVES: The new curriculum at King's College School of Medicine and Dentistry, which commenced in September 1996, requires all medical undergraduates to have a general practice placement throughout the 5 years of their medical education. DESIGN: This paper discusses recruitment, training and support of teaching practices for the new curriculum, reviews the distribution of single-handed general practices in the network and, via a selection of monitoring and evaluation procedures, discusses the implications of a policy which is inclusive of single-handed practices. The findings relate to the experience of the first semester of the first year of the new curriculum. It also examines the contributions that single-handed practices have made to the teaching network and the kind of support needed, if single-handed practices are to continue to contribute to the King's teaching network. SETTING: King's College School of Medicine and Dentistry. SUBJECTS: Medical undergraduates. RESULTS: The findings of this paper revealed that over a third of general practice provision is via single-handed practices in South-east London. Within the undergraduate teaching network, 10% of practices are single-handed. Students are welcomed and receive a learning experience comparable to those students in larger practices. Attendance at training events has proved difficult for some of these tutors, but the extra input from the department, in order to address this deficit, has not been onerous. Indeed, single-handed practices have not been unique with regard to difficulties in attendance at training events. CONCLUSIONS: The study concludes that single-handed practices can make satisfactory provision for undergraduates in the new curriculum and there is no evidence from this study to suggest otherwise.

Lever pressing in rats was maintained by continuous and intermittent schedules of food while defecation was monitored. In Experiment 1, reinforcement densities were matched across variable-ratio and variable-interval schedules for three pairs of rats. Defecation occurred in all 3 rats on the variable-ratio schedule and in all 3 rats on the yoked variable-interval schedule. In Experiment 2, fixed-ratio and fixed-interval schedules with similar reinforcement densities maintained lever pressing. Defecation occurred in 3 of 4 rats on the fixed-ratio schedule and in 4 of 4 rats on the fixed-interval schedule. Almost no defecation occurred during continuous reinforcement in either experiment. These results demonstrate that defecation may occur during both ratio and interval schedules and that the inter-reinforcement interval is more important than the behavioral requirements of the schedule in generating schedule-induced defecation.

Defecation rate was monitored during daily 30-min periods as 16 rats were exposed to different sequences of the following three experimental conditions: (a) a fixed-time 60-s food delivery schedule, (b) a massed-food presentation baseline, and/or (c) a no-food baseline. All food delivery was response independent. Rate of defecation increased during fixed-time 60-s food delivery when compared to baseline rates of defecation established during no-food and massed-food baselines. This effect was present for 12 of 16 rats during four alternative sequences of experimental conditions. Within-subject reversals established reliability of this effect. Schedule induction of defecation is clearly demonstrated under these conditions.

Prior clinical research suggests that superimposition and subsequent removal of a schedule of continuous reinforcement (CRF) may be a viable rate-decreasing procedure in that an extinction-like condition is arranged. The arrangement of similar conditions in the laboratory, however, resulted in the quick recovery of baseline rates. Lever-pressing patterns of eight male rats maintained by different schedules of variable-ratio and variable-interval food reinforcement were examined in an A-B-A experimental design of CRF food superimposition and removal. Responding was substantially reduced during the superimposition of CRF. Upon removal of the superimposed schedule, responding quickly approached presuperimposition baseline rates.
