RESUMO

ObjectiveTo observe the effect of Wenyang Jieyu decoction (WYJY) on the hippocampal structure of depressed rats with kidney-yang deficiency. MethodThe 105 SD rats were randomly divided into normal group, model group, fluoxetine group (4.17 mg·kg-1), Xiaoyaosan group (1.88 g·kg-1), and low-, medium- and high-dose (1.25,2.50,5.00 g·kg-1) WYJY groups,15 in each group. The depression model was induced by subcutaneous injection of corticosterone in rats except for those in the normal group and the rats were orally administered once a day for 28 days. The depression-like behaviors of rats were observed by sucrose preference test, novelty-suppressed feeding test, forced swimming test, and open field test. The morphology of hippocampal neurons was observed by hematoxylin-eosin(HE) staining, and the density of hippocampal neurons was detected by Nissl staining. The ultrastructure of hippocampal synapses was observed by transmission electron microscopy (TEM). The expression of synaptophysin (SYP), postsynaptic density-95 (PSD95), and apoptosis-related protein Caspase-3 in hippocampal neurons was observed by immunohistochemistry, and bromodeoxyuridine (BrdU) and doublecortin (DCX) were used to observe the apoptosis and regeneration of hippocampal neurons. ResultWYJY could improve weight loss in depressed rats. As revealed by the behavioral tests, the model group showed depression-like behaviors, which were relieved in the WYJY groups and the positive drug groups. HE staining showed that the nuclei of hippocampal neurons in the model group were constricted, deeply stained, and sparsely arranged, while the neurons in the WYJY groups and the positive drug groups were significantly improved. Nissl staining demonstrated that the cell density of the model group was lower than that of the normal group (P<0.05). Compared with model group, the groups with drug intervention showed increased cell density (P<0.05) and compact arrangement. According to the results in TEM, compared with normal group, the model group showed shortened synaptic active zone (P<0.05), widened synaptic cleft (P<0.05), and thinned tight zone (P<0.05). Compared with model group, the groups with drug intervention showed shortened synaptic active zone (P<0.05), narrowed synaptic cleft (P<0.05), and thickened tight zone (P<0.05). As displayed by the results of immunocytochemistry and immunofluorescence, compared with the normal group, the model group showed decreased protein expression of SYP, PSD95, BrdU, and DCX in the hippocampus (P<0.05) and increased protein expression of Caspase-3 (P<0.05). Compared with the model group, the groups with drug intervention showed increased protein expression of SYP, PSD95, BrdU, and DCX in the hippocampus (P<0.05) and decreased protein expression of Caspase-3 (P<0.05). ConclusionWYJY can promote the regeneration of hippocampal neurons in rats and improve the depression of rats.

ObjectiveTo analyze the re-examination results of malaria cases captured from the National Notifiable Communicable Disease Reporting System in Hubei Provincial Malaria Diagnostic Reference Laboratory from 2017 to 2019, so as to pro- vide the scientific evidence for improving the malaria control capability in the province. MethodsMicroscopy and nested PCR assay were performed to re-examine the diagnosis of malaria cases registered in the National Notifiable Communicable Disease Reporting System in Hubei Provincial Malaria Diagnostic Reference Laboratory from 2017 to 2019, and the coincidences of ma- laria diagnosis and malaria parasite species were evaluated. Results A total of 410 malaria cases were reported in Hubei Province from 2017 to 2019 according to the data retrieved from the National Notifiable Communicable Disease Reporting System. Among the 407 samples re-examined by Hubei Provincial Malaria Diagnostic Reference Laboratory from 2017 to 2019, the diag- nosis 374 malaria cases were confirmed, with an overall coincidence of 91.89% (374/407) for malaria diagnosis and 89.04% (333/374) for parasite species identification. The coincidence rates of malaria diagnosis and parasite species identification were 50.00% to 100.00% and 66.67% to 100.00% in 16 cities (prefectures) of Hubei Province during the re-examinations, which both varied in regions (χ2 = 40.46 and 42.30, both P values < 0.01). The coincidence rates of Plasmodium falciparum, P. vivax, P. malariae and P. ovale identification were 95.80%, 100.00%, 58.33% and 51.92% during the re-examinations, respectively (χ2 = 76.66, P < 0.01). The consistency rate between microscopic and nested PCR results was 89.83% (362/403). Conclusions The overall diagnostic quality of malaria is high in medical institutions at all levels in Hubei Province; however, the diagnostic capability of malaria remains to be improved in some regions.

OBJECTIVE: To investigate the space-time patterns of schistosomiasis after the stage of transmission controlled in Hubei Province, so as to provide the reference for precise controlling. METHODS: The data of human schistosomiasis cases in Hubei Province from 2015 to 2016 and basic information of human schistosomiasis cases and serum antibody titer of human schistosomiasis cases in 2016 were collected and analyzed. The spatial clustering of human schistosomiasis was detected by the Flexible spatial scan statistics and Kulldorff circular scan statistic, respectively. RESULTS: Totally 64 819 serological positive cases from 51 counties and 1 504 stool hatching positive cases from 17 endemic counties were reported in Hubei Province in 2015, and 39 365 serological positive cases were reported from 48 counties in 2016. All of them were identified as the research objects. No stool hatching positive cases were reported from the routine work in the whole province in 2016. There were 1 603 cases of the highest antibody titer (1∶80 or more), with the proportion of 4.07%. According to the results of Kulldorff spatial clustering analysis, there were eight and six spatial clustering areas in the distribution of serological positive cases and stool hatching positive cases in 2015, respectively. The numbers of spatial clustering areas in the distribution of serological positive cases and the cases of the highest antibody titer (1∶80 or more) were eight and five in 2016, respectively. According to the results of Flexible spatial clustering analysis, there were three and two spatial clustering areas in the distribution of serological positive cases and stool hatching positive cases in 2015, respectively. The numbers of spatial clustering areas in the distribution of serological positive cases and the cases of highest antibody titer (1∶80 or more) were two and one in 2016, respectively. CONCLUSIONS: The distribution of human schistosomiasis cases is not balanced, and there is spatial clustering in Hubei Province. So the key area for prevention and control is on the middle and lower reaches of the Yangtze River in the future.

OBJECTIVE: To evaluate the effect of clinical pathway (CP) on the implementation of advanced schistosomiasis patients with ascites. METHODS: Totally 1 129 cases of advanced schistosomiasis patients with ascites but without other complications were selected randomly from the Performance Evaluation and Management System for Medical Treatment of Advanced Schistosomiasis Patients in Hubei Province from year 2011 to 2013. Among the patients, 754 cases were treated by CP (CP group), and 375 cases were treated with traditional methods (NCP group), and the hospitalization days, hospitalization expenses, medicine proportions, treatment outcomes and degrees of satisfaction and health knowledge rates of the two groups were compared. Meanwhile, the variation of CP was calculated. RESULTS: The average length of hospital days of the CP group and NCP group were (13.85 ± 5.60) d and (17.92 ± 5.80) d, respectively, and the average hospitalization costs of the two groups were (4 699.14 ± 1 520.59) Yuan and (5 692.01 ± 1 616.66) Yuan, respectively, both the differences were statistically significant (both P < 0.05). Also the hospitalization cost structures of the two groups were remarkably different, the composition ratios of the inspection fee and accommodation fee in the CP group were lower than those in the NCP group, but the constituent ratios of the examination fee, diagnosis and treatment fee, drug charges and other expenses were higher than those in the NCP group (all P < 0.05). The awareness rate of health knowledge in the CP group was higher than that in the NCP group (P < 0.05), but there were no statistically significant differences in the treatment outcome and the degree of satisfaction between the two groups (both P > 0.05). The variation rate of CP was 9.02% (68/754). CONCLUSIONS: The implementation of CP can decrease the days of hospital stay and medical expenses, improve the awareness rate on health knowledge of the patients. The CP treatment with low variation rate is applicable to advanced schistosomiasis patients with ascites but without complications.

OBJECTIVES@#To analyze the cases of medical disputes involving prenatal examination from a point of view of forensic expertise, and to discuss the risk of medical disputes and the preventive measures.@*METHODS@#A retrospective analysis was conducted on 49 forensic expertise cases of medical disputes in prenatal examination which were identified in Academy of Forensic Science and Shanghai Di'an Forensic Science Limited Company from 2010 to 2017.@*RESULTS@#In recent years, the number of medical disputes involving prenatal examination showed an increasing trend year by year. The common causes of medical disputes were: uninformed or insufficiently informed disclosure (20 cases); the propaganda and application of three-dimensional, four-dimensional ultrasound were not standardized (14 cases); ultrasound examination and serological screening process were not standardized (12 cases); no antenatal counseling (2 cases), etc.@*CONCLUSIONS@#In order to minimize the occurrence of such medical disputes, hospitals or related associations should avoid the risk of prenatal examination through the standardization of management and operation.

OBJECTIVES@#To study the case characteristics of forensic medical identification of traumatic tympanic membrane perforations, and to discuss the key points of forensic medical identification and evaluations methods for tympanic membrane perforations.@*METHODS@#Twenty-four cases of traumatic tympanic membrane perforations accepted by the Academy of Forensic Science during 2017 were retrospectively analysed. The data of perforation size, form, predilection site, healing time and healing mode were evaluated.@*RESULTS@#For the traumatic tympanic membrane perforations, the study showed that the small size of perforation (<1/2 quadrant) with irregular shape was common. The location of perforations was almost on the anterior and inferior quadrant, and centripetal migration healing was common. The healing rate within 6 weeks was up to 90%.@*CONCLUSIONS@#In the identification cases of traumatic tympanic membrane perforations, the key is to determine whether it is traumatic and whether it will heal spontaneously within 6 weeks. It is suggested to check the tympanic membrane weekly by an otic endoscope combined with acoustic impedance measurement at the sixth week, which can improve the accuracy, objectivity and scientificity of the identification.

OBJECTIVES@#To study the injury manners of Bennett fracture and Rolando fracture and explore its identification principle of the first metacarpal base fractures.@*METHODS@#Totally 31 cases of Bennett fracture and Rolando fracture were collected. The injury manners of cases of 19 Bennett fractures and 12 Rolando fracture were classified, and various injury manners were analysed statistically.@*RESULTS@#The injury manners of the cases of 19 Bennett fracture and 12 Rolando fracture were divided into three types, including the first metacarpus hit hard objects during a punching, the first metacarpus hit hard objects when making fists and fell, or the first metacarpus was punched by hard objects when making fists.@*CONCLUSIONS@#The injury mechanism of Bennett fracture and Rolando fracture is formed by a force on the first metacarpus when making fists, which transmits to the basilar part along the vertical axis of metacarpus. The inference of injury manners should be focused on the confirmation of entrusted matters.

<p><b>OBJECTIVE</b>To investigate chondrocyte apoptosis and the expression of biochemical markers associated with apoptosis in Kashin-Beck disease (KBD) and in an established T-2 toxin- and selenium (Se) deficiency-induced rat model.</p><p><b>METHODS</b>Cartilages were collected from the hand phalanges of five patients with KBD and five healthy children. Sprague-Dawley rats were administered a selenium-deficient diet for 4 weeks prior to T-2 toxin exposure. The apoptotic chondrocytes were observed by terminal deoxynucleotidyl transferase dUTP nick end labeling staining. Caspase-3, p53, Bcl-2, and Bax proteins in the cartilages were visualized by immunohistochemistry, their protein levels were determined by Western blotting, and mRNA levels were determined by real-time reverse transcription polymerase chain reaction.</p><p><b>RESULTS</b>Increased chondrocyte apoptosis was observed in the cartilages of children with KBD. Increased apoptotic and caspase-3-stained cells were observed in the cartilages of rats fed with normal and Se-deficient diets plus T-2 toxin exposure compared to those in rats fed with normal and Se-deficient diets. Caspase-3, p53, and Bax proteins and mRNA levels were higher, whereas Bcl-2 levels were lower in rats fed with normal or Se-deficiency diets supplemented with T-2 toxin than the corresponding levels in rats fed with normal diet.</p><p><b>CONCLUSION</b>T-2 toxin under a selenium-deficient nutritional status induces chondrocyte death, which emphasizes the role of chondrocyte apoptosis in cartilage damage and progression of KBD.</p>

OBJECTIVE: To establish an expression platform of surveillance and forecast for schistosomiasis in key water regions of Hubei Province. METHODS: The platform of surveillance and forecast for schistosomiasis was established based on Google Earth, GIS Office software, network albums and network video stations. The results of the field surveillance and laboratory testing, and figures and videos were inputted into the established platform, and an information management file was set up to express the information of forecasting. RESULTS: The operation and expression platform based on Google Earth in key water regions of Hubei Province was successfully established. It was able to provide the forecasting data timely, to achieve off-site and real-time query and sharing. CONCLUSIONS: The platform based on Google Earth has preferable prospective on field application in Hubei Province.

RESUMO

OBJECTIVE: To improve the information level of schistosomiasis control by using the related functions of CorelDRAW software. METHODS: Combining with the requirement of schistosomiasis control, the sketch map was drawn according to the linear element drawing, the geometric drawing, the color rendering and the text adding. RESULTS: The schistosomiasis epidemic sketch map and the Oncomelania hupensis snail distribution sketch map at all levels were produced in CorelDRAW software. CONCLUSIONS: The sketch map drawing in CorelDRAW software is beautiful and standardizing, and it can improve the level of information management.

OBJECTIVE: To evaluate the effect of schistosomiasis control in"Oriental Star shipwreck event"in Jianli County, Hubei Province, so as to provide experiences for schistosomiasis prevention and control in rescue of emergency in the future. METHODS: According to the data of historical schistosomiasis prevalence and the results of the field survey in the townships in the upstream and downstream of the rescue spots, the emergency handling measures of schistosomiasis control were evaluated. Meanwhile, the Oncomelania hupensis snail situation, priority crowd chemotherapy, key aquatic monitoring, and illness monitoring of people and livestock were investigated to evaluate the schistosomiasis control effect after the events comprehensively. RESULTS: There were no schistosome-infected snails found in the place of shipwreck and the rescue spots. The average density of O. hupensis snails in 5 townships of Jianli County was 0.064 snails/0.1 m2, and there were no infected snails found. The positive rate of 2 090 migrants in the schistosomiasis serologic tests was 0.29% and there were no positive ones found in the fecal examination. There were no acute schistosomiasis cases found by the sentinel surveillance fever clinics, and also no positive cattle were found. The results of sentinel mice monitoring in the place of shipwreck and 4 villages in the upstream and downstream showed no infected mice were found. CONCLUSIONS: The schistosomiasis control measures taken after the shipwreck is effective, which achieves the goal of no schistosomiasis transmission after emergency.

OBJECTIVE: To analyze the Oncomelania hupensis snail situation in schistosomiasis surveillance sites in Hubei Province from 2009 to 2014. METHODS: From 2009 to 2014, 207 schistosomiasis endemic villages from 13 cities were selected as the surveillance sites in Hubei Province, where the surveillance of snail situation was performed, and the data about the indexes of snail situation during the 6 years were collected and analyzed statistically. RESULTS: From 2009 to 2014, totally 80 963.47 hm2 of snail areas were investigated, and 45 309.77 hm2 of area with snails as well as 364.93 hm2 of area with infected snails were found out, and 1 646 125 snails were captured. In the 207 surveillance sites, the density of living snails was reduced from 0.493 3 snail/0.1 m2 in 2009 to 0.339 3 snail/0.1 m2 in 2014, with a decline rate of 31.22%; the density of infected snails was reduced from 0.000 7 snail/0.1 m2 in 2009 to 0 in 2014; the infection rate of snails was reduced from 0.14% in 2009 to 0 in 2014. The infection rates of snails in the third-rank villages, endemic areas in inner embankment, ditch environment and weed environment were relatively higher. CONCLUSIONS: The snail situation of Hubei Province has been declined steadily. However, the environment of snail breeding has not been changed completely. In order to consolidate the control achievements further, the measures including mollusciciding and snail control projects should be strengthened.

OBJECTIVE: To evaluate the schistosomiasis transmission control in 19 counties (cities, districts) of Hubei Province in 2013. METHODS: The epidemic villages from different counties were randomly sampled by the cluster sampling method as the evaluation villages. The schistosome infection status of human and livestock, the Oncomelania hupensis snail status, the documents and data of schistosomiasis control, acute infection control and health education were investigated in the field. RESULTS: The serum examination of schistosome infection was performed to 29 631 residents, and 2 068 were positive, with a positive rate of 6.98% (0.78% -16.47%). The stool examination was performed to 2 021 sero-positive persons, and 47 cases were stool-positive, with an infection rate of 0.16% (0 - 0.82%). The cattle was the main domestic animals in 19 counties (cities, districts), and the measures of replacing cattle with machine for cultivation were done in 17 counties (cities, districts). Totally 105 cattle in Caidian District and Huangpi District received stool examinations and no positives were detected. A total of 1 579.37 hm2 and 31 829 frames in 154 environments were surveyed, and 4 857 snails were collected. Of the 1 935 living snails dissected, no infected snails were detected. The files regarding the schistosomiasis morbidity and snail status have been established in 19 counties (cities, districts) since 2009. CONCLUSIONS: The transmission of schistosomiasis in 19 counties (cities, districts) of Hubei Province has reached the standard of transmission controlled.

OBJECTIVE: To analyze the surveillance results of schistosomiasis and understand the transmission situation in Hubei Province in 2014. METHODS: According to the Scheme of Schistosomiasis Surveillance in Hubei Province (the 2009 edition), 207 endemic sites were selected, where the schistosome infections of residents and livestock, and the distribution of Oncomelania hupensis snails were investigated. RESULTS: In the 207 sites, there were 27 sites without positive results of the blood tests and 129 sites without positive results of the stool tests. The infection rates of residents and cattle were 0.17% and 0, respectively. The density of living snails was 0.339 3 snails/0.1m2, the rate of living snails was 10.12%, and the infection rate of snails was 0. The infection rate of residents in the inner embankment type was 0.22%, higher than the rates in the other types. In different age groups, the infection rate in the 50-59 years age group was higher than the rates in other age groups. The infection rates of residents in farmer and fisherman were higher than those in the other occupation groups. The infection rate of floating people was 0, and no acute schistosomiasis patients were found. CONCLUSIONS: The endemic situation of schistosomiasis has decreased obviously in Hubei Province, and the whole province has achieved the criteria for transmission control of schistosomiasis.

OBJECTIVE@#To analyze the latency of posture evoked response of normal lower limb muscle in different stimulations and explore its influencing factors.@*METHODS@#The normal lower limb was induced to produce postural evoked response by the dynamic posturography through two kinds of perturbations, the supporting surface rotation stimulation (Toes-up and Toes-down) and the horizontal perturbation stimulation (Forward and Backward). The latencies of tibialis anterior muscle and gastrocnemius muscle were recorded by surface electromyography acquisition system. The differences of the left and right limb, gender and height on the latency of postural evoked response were analyzed.@*RESULTS@#(1) Under the Toes-up and Backward perturbation, the latency of tibialis anterior muscle was longer than gastrocnemius muscle; under the Toes-down and Forward perturbation, the latency of gastrocnemius muscle was longer than tibialis anterior muscle. (2) The latencies of left limb and right limb had no significant difference. (3) The latency in male was longer than that in female. (4) The latency gradually increased with the increase of height.@*CONCLUSION@#In the postural evoked response, different perturbations, gender and height have significant impacts on the latency of posture evoked response of lower limb muscle. However, the effect of height and gender should be not considered referring to the same individual.

OBJECTIVE@#To explore the methods to assess the function disorder of patients with mild psychiatric impairment due to road traffic accidents.@*METHODS@#In contrast to that of the patient with physical impairment, the function disorder of the patient with mild psychiatric impairment was assessed by the standard assessment instruments, such as the Barthel index of ADL (BI), Functional Activities Questionnaire (FAQ), Social Disability Screening Schedule (SDSS), Activity of Daily Living Scale (ADL), Physical Self-maintenance Scale (PSMS), and Instrumental Activities of Daily Living Scale (IADL).@*RESULTS@#Except BI, the each total score of FAQ, SDSS, ADL, PSMS, and IADL correlated well with the rank of psychiatric impairment. The difference of each total score of rating scales among different physical impairment rank was not statistically significant. At the impairment rank of level 10, the difference of each total score of FAQ and ADL was not significant between the two groups; each total score of BI, SDSS, and IADL in psychiatric impairment was higher than that of physical impairment, while the total score of PSMS in psychiatric impairment was lower than that of physical impairment. At the level 9, except PSMS, the each total score of BI, FAQ, SDSS, IADL and ADL in psychiatric impairment was higher than that of physical impairment. At the level 8, except BI and PSMS, the each total score of FAQ, SDSS, IADL and ADL in psychiatric impairment was higher than that of physical impairment.@*CONCLUSION@#The different criteria for assessment of mild psychiatric and physical impairment may exist, and appropriate indexes for assessment between physical and psychiatric impairment need to be further elucidated.

OBJECTIVE@#To quantify the posture control ability of patients with peripheral vertigo by computed dynamic posturography (CDP).@*METHODS@#Ninety-one subjects diagnosed with peripheral vertigo by caloric test were divided into unilateral vestibular dysfunction group (unilateral group) and bilateral vestibular disorder dysfunction group (bilateral group). CDP tests including sensory organization test (SOT) and motor control test (MCT) were performed on all subjects. The CDP results were compared with the normal data by t-test.@*RESULTS@#In unilateral group, there was no statistically significant difference in SOT compared with normal data (P > 0.05). In bilateral group, the composite equilibrium score was lower than the normal data, especially during the moving of the platform and the changes of visual stimulation (P < 0.05). In MCT test, the latencies of both groups showed no statistically significant difference compared with normal data (P > 0.05).@*CONCLUSION@#During the non-acute period of peripheral vertigo, patients could maintain static equilibrium. Compared with normal people, dynamic equilibrium function is normal in patients with unilateral vestibular dysfunction, but declined in patients with bilateral vestibular dysfunction.

OBJECTIVE@#To analyze the difference of subjective hearing threshold and objective hearing threshold, and to discuss the importance of standard for hearing evaluation in forensic medicine.@*METHODS@#Three hundred and fifty-five cases (387 ears) of forensic medical identification with hearing impairment were retrospectively analyzed including the items entrusted and hearing test results. All cases were collected from 2004 to 2012 in the forensic science center.@*RESULTS@#In the 387 ears, 218 ears (56.3%) were evaluated the degree of disability and 106 ears (27.4%) were identified the degree of damage. In the disability degree evaluation, the subjective hearing threshold and the objective hearing threshold were significant different in 120 ears (55.0%), while in damage degree evaluation, the subjective hearing threshold and the objective hearing threshold were significant different in 69 ears (65.1%).@*CONCLUSION@#Because of camouflaging or exaggerating the hearing impairment by the wounded, the subjective hearing threshold can't accurately assess the existence and the degree of hearing impairment. In the forensic identification, auditory brainstem response, 40 Hz auditory event related potential and auditory steady-state response should be combined in the application to evaluate the hearing impairment for the wounded in order to ensure the reliability of the evaluation of hearing impairment.
