RESUMO

Objective:To explore any differential effect of training wearing a unilateral exoskeleton on the lower-limb motor function of stroke survivors. Methods:Forty stroke survivors were randomly divided into an exoskeleton group ( n=20) and a control group ( n=20). The control group performed conventional lower extremity exercise training while the exoskeleton group received exoskeleton-assisted lower-limb physical therapy. Each participant received eighteen 40-minute training sessions over three weeks. Before and after the intervention, the walking ability, lower-limb function, balance and ability in the activities of daily living of both groups were evaluated. Integrated electromyography (iEMG) of the rectus femoris and tibialis anterior of both legs was also recorded during sit-to-stand transitions to assess the activation of the affected muscles and the symmetry of bilateral muscle activation. Results:After the three weeks, significant improvement was observed in all of the measurements in both groups, but with the exoskeleton group scoring significantly better on average in functional ambulation category grading (1.63±0.72). Both groups′ iEMGs had also improved significantly compared with before treatment, but the exoskeleton group′s average result was by that time significantly better than the control group′s average. Conclusions:A wearable exoskeleton can effectively improve the rehabilitation of walking, lower limb movement, balance and skill in the activities of daily living of persons with subacute stroke. It better activates the affected lower limb muscles and improves the symmetry of bilateral lower limb muscle activation.

RESUMO

Objective:To observe any dependence of anticipatory postural adjustment (APA) on the difficulty of fine upper limb tasks and to document any effect of reticulospinal tract (RST) facilitation on APAs during such tasks. Methods:The study′s bivariate mixed design involved 4 different tasks and 3 different priming states. Thirteen healthy, male, right-handed subjects were recruited. They were asked to complete the 4 tasks of reaching, grasping a cup, pinching a card using the thumb or using the little finger, respectively for 10 times 1 in response to two different starting cues delivered through an earphone. Half of the trials with each task were initiated with 114dB white noise to startle and activate the reticulospinal tract (RST), while the others were activated with 80dB beeps as a control. Electromyographic signals were recorded from the bilateral sternocleidomastoid (SCM), lower trapezius (LT), latissimus dorsi (LD), lumbar erector spinae and right anterior deltoid muscles and also from the right flexor and extensor carpi radialis muscles (ECR/FCR). In the subsequent processing the electromyographic time domain and frequency domain indicators were converted into a pre-motor reaction time, a time to muscle peak contraction, an activation latency, and APA or compensatory postural adjustment (CPA) amplitude of the tested muscles. These were compared among the different tasks and stimuli. In addition, the 114dB test tasks were classified as two different priming status as SCM + and SCM - according to whether the sternocleidomastoid muscle (SCM) was activated in advance. Results:After RST activation the pre-motor reaction time and the time to peak contraction of all of the muscles were significantly shortened in all of the tasks. The deltoid muscle reaction times in the SCM + , SCM - and control states were (106.89±43.78)ms, (136.78±48.74)ms and (168.60±73.17)ms, respectively, and those differences are significant. The APA amplitudes of the contralateral LT and ipsilateral LD were significantly greater than normal, but the timing of muscle activation onset and the APA/CPA amplitudes of the ECR/FCR were not affected. The latency in the anticipatory muscle activation of the ECR in the little finger grip task was significantly shorter than that in reaching. Conclusions:The extensor carpi radialis show task-specific early activation in fine tasks of the upper limbs with different difficulties. RST activation can lead to early starting of expected actions, accelerate muscle contraction and increase APA amplitude of some trunk muscles, but it has no significant effect on APA/CPA amplitudes in the forearm muscles.

RESUMO

Objective:To observe the reliability and validity of using a head-neck relocation test (HRT) in assessing cervical position sense. Methods:A total of 62 subjects were recruited, including 32 with neck pain and 30 healthy subjects. All received two HRTs conducted by two independent assessors at a first session, and then they received another HRT 24 hours later administered by one of them. The joint position error (JPE) was measured in the four directions of cervical motion: flexion, extension, levorotation and dextrorotation. The intra- and inter-rater reliability was quantified in terms of intra-class correlation coefficients (ICCs) and the difference in JPE between the two groups was also determined for validity. Results:The intra- and inter-rater reliability for the HRT in the four directions had ICC values of 0.603-0.830 and 0.597-0.837 respectively. The corresponding standard error of measurement (SEM) values were 1.52-2.15cm and 1.69-2.09cm, while the minimal detectable change (MDC) values were 4.20-5.97cm and 4.69-5.79cm, respectively. The patients with neck pain had significantly larger JPE in cervical extension and right rotation compared to healthy individuals, on average. And their JPE for cervical extension had an area under the curve (AUC) of the receiver′s operating curve of 0.755.Conclusions:The HRT showed good intra- and inter-rater reliability in assessing cervical joint position sense. The JPE for cervical extension can be used to discriminate those with neck pain from healthy subjects.

RESUMO

Objective:To investigate the effects of different degrees of root canal filling therapy on pain severity, stress response and masticatory function in patients with dental diseases. Methods:A total of 120 patients with dental disease who received treatment in Department of Oral Surgery, Zhoushan Hospital between December 2019 and December 2020 were included in this study. They were randomly divided into study and control groups ( n = 60/group). Patients in the control group were given excessive root canal filling and those in the study group were given approximate root canal filling. Cortisol, adrenocorticotropic hormone, interleukin-1β and interleukin-10 levels, bite force, gingival index and masticatory efficiency were determined before and after treatment. Visual Analog Scale (VAS) was used to evaluate the severity of pain in each group. Clinical efficacy was compared between the two groups. Results:After treatment, interleukin-1β, cortisol and adrenocorticotropic hormone levels in the study group were (23.66 ± 6.94) μg/L, (129.61 ± 27.18) μg/L, (14.58 ± 4.11) ng/L, respectively, which were significantly lower than those in the control group [(31.31 ± 10.13) μg/L, (147.93 ± 30.26) μg/L, (17.16 ± 5.04) ng/L, t = 4.82, 3.45, 3.07, all P < 0.05]. Interleukin-10 level in the study group was significantly higher than that in the control group [(65.19 ± 16.06) ng/L vs. (56.61 ± 15.52) ng/L, t = 2.97, P < 0.05). Bite force and masticatory efficiency in the study group were (127.53 ± 33.20) 1bs and (84.73 ± 20.65)%, respectively, which were significantly higher than those in the control group [(115.25 ± 30.12) 1bs, (75.49 ± 18.14)%, t = 2.12, 2.60, both P < 0.05]. Gingival index and Visual Analog Scale score in the study group were (0.44 ± 0.12) and (2.73 ± 0.81) points, respectively, which were significantly lower than those in the control group [(0.44 ± 0.12), (2.73 ± 0.81) points, t = 7.92, 2.66, both P < 0.05]. Total response rate in the study group was significantly higher than that in the control group (96.67% vs. 81.67%, χ2 = 5.52, P < 0.05). Conclusion:Appropriate root canal filling therapy is highly effect on dental diseases. It can decrease stress response, inhibit inflammation, alleviate pain, and improve masticatory function.

RESUMO

BackgroundCOVID-19 is a highly transmissible infectious disease that has infected over 122 million individuals worldwide. To combat this pandemic, governments around the world have imposed lockdowns. However, the impact of these lockdowns on the rates of COVID-19 transmission in communities is not well known. Here, we used COVID-19 case counts from 3,000+ counties in the United States (US) to determine the relationship between lockdown as well as other county factors and the rate of COVID-19 spread in these communities. MethodsWe merged county-specific COVID-19 case counts with US census data and the date of lockdown for each of the counties. We then applied a Functional Principal Component (FPC) analysis on this dataset to generate scores that described the trajectory of COVID-19 spread across the counties. We used machine learning methods to identify important factors in the county including the date of lockdown that significantly influenced the FPC scores. FindingsWe found that the first FPC score accounted for up to 92.81% of the variations in the absolute rates of COVID-19 as well as the topology of COVID-19 spread over time at a county level. The relation between incidence of COVID-19 and time at a county level demonstrated a hockey-stick appearance with an inflection point approximately 7 days prior to the county reporting at least 5 new cases of COVID-19; beyond this inflection point, there was an exponential increase in incidence. Among the risk factors, lockdown and total population were the two most significant features of the county that influenced the rate of COVID-19 infection, while the median family income, median age and within-county move also substantially affect COVID spread. InterpretationLockdowns are an effective way of controlling the COVID-19 spread in communities. However, significant delays in lockdown cause a dramatic increase in the case counts. Thus, the timing of the lockdown relative to the case count is an important consideration in controlling the pandemic in communities. Research in contextO_ST_ABSEvidence before this studyC_ST_ABSWe searched PubMed using the term "coronavirus", OR "COVID-19", OR "COVID-19 infection", OR "SARS-CoV-2" combined with "Lockdown" or "sociodemographic factor" or "Vulnerability" for original articles published before March 18, 2021. Similar searches were done in medRxiv, Google Scholar, and Web of Science. Only papers published in English were reviewed. The most similar relevant works to our study were Acharya et al.1 and Karmakar et al.2, which investigated the associations between population-level social factors and COVID-19 incidence and mortality. Unlike our current study, which employed a longitudinal design, both of studies were cross-sectional in nature and thus fixed on a single time point. In addition, neither of these studies investigated the impact of lockdown measures on COVID-19 infection patterns. Another relevant study is Alfano et al.s work3, which focused on the efficacy of lockdown on COVID-19 case rates. However, this study did not evaluate the timing of lockdown on this endpoint. Added value of this studyTo our knowledge, this is the first study to use functional principal component analysis (FPCA) to investigate COVID-19 infection trajectories (in a longitudinal manner) and their relationships with different sociodemographic factors and lockdown policy at a county level. The FPCA transformed a longitudinal vector with high-dimensions into a "single" surrogate variable, which retained 93% of the information. We used an advanced statistical model (segmented regression) to investigate the effects of lockdown on incidence of COVID-19 across the US. We found that the relationship had a "hockey stick" appearance with an inflection point at [~]7 days prior to a county reporting at least 5 cases of COVID-19. We also applied a machine learning model (i.e., elastic net) to explore joint effects of lockdown and other sociodemographic factors on COVID-19 infection patterns, which estimated the impact of each of factors, adjusted for each other. Implications of all the available evidenceOur study suggests that lockdown is an effective policy to reduce case counts of COVID-19 in communities; however, significant delays in its implementation result in exponential growth of COVID-19. The inflection point is approximately 7 days prior to a county reporting at least 5 cases of COVID-19. These data will help policy-makers to determine the optimal timing of lockdowns for their communities.

RESUMO

Objective:To explore the appropriate use of a self-developed scoliosis rehabilitation robot in treating adolescent idiopathic scoliosis (AIS), and also its safety and effectiveness. Methods:The scoliosis rehabilitation robot consists of a closed-loop pneumatic control system and multi-segment torso, pelvis and upper extremity fixation devices. It provides three-dimensional synchronous correction. Eighteen AIS subjects first received 30min of robot-assisted treatment using the maximum tolerable orthotic force. The angles of their spinal processes were evaluated using ultrasound before the treatment and after 30 seconds, 5 minutes and 30 minutes of treatment, then 5min later. In a second 30sec course of treatment the transverse orthotic force was 10%, 15%, 20%, 25%, and 30% of the patient′s body weight. Any adverse effects were observed and recorded. Results:The robot ran smoothly and could apply intelligent and precise correction. No severe adverse effects were reported. The mean correction of the spinal process angles showed a significant cumulative effect with treatment time, reaching 104% at 30min. The mean process angle correction increased with the applied force. Force at 25% of the patient′s weight produced an average correction of 104% in patients with mild AIS and 65% in those whose AIS was moderate. Conclusion:The scoliosis rehabilitation robot is safe and immediately effective. Setting the transverse force at 25% of a patient′s weight gives the best corrective effect with mild AIS. Moderate AIS requires more force.

RESUMO

Myoclonus dystonia syndrome (MDS) is an inherited movement disorder, and most MDS-related mutations have so far been found in the ε-sarcoglycan (SGCE) coding gene. By generating SGCE-knockout (KO) and human 237 C > T mutation knock-in (KI) mice, we showed here that both KO and KI mice exerted typical movement defects similar to those of MDS patients. SGCE promoted filopodia development in vitro and inhibited excitatory synapse formation both in vivo and in vitro. Loss of function of SGCE leading to excessive excitatory synapses that may ultimately contribute to MDS pathology. Indeed, using a zebrafish MDS model, we found that among 1700 screened chemical compounds, Vigabatrin was the most potent in readily reversing MDS symptoms of mouse disease models. Our study strengthens the notion that mutations of SGCE lead to MDS and most likely, SGCE functions to brake synaptogenesis in the CNS.

RESUMO

Objective To assess scoliosis in adolescence and adults by three-dimensional ( 3-D ) ultrasound imaging . Methods A commercial available ultrasound instrument with a magnetic sensor system was used for long distance 3-D ultrasound imaging .Specific phantoms were imaged to evaluate the precision of this imaging system . Twenty healthy adults and 20 patients with scoliosis were imaged by 3-D ultrasonography and radiography . The deformity angle of the patients with scoliosis was measured on the reconstructed coronal plane ( C plane) of 3-D ultrasound imaging . The relevance between the curvature measured by ultrasound and the Cobb angles originated by radiography were evaluated . Results There was no significant difference between the angles of the phantom measured by the 3-D ultrasound imaging system and that measured by an angle gauge ( P = 0 .479) . The transverse process ,vertebral arch and acoustic shadow of the central spinous process of spines in healthy adults can be displayed on the reconstructed C plane by 3-D ultrasound imaging . The morphology of scoliosis in all the twenty patients can be observed on the reconstructed C planes ( multi-layers) by 3-D ultrasound imaging ,which accorded well with that observed on radiograph . In 9 patients ,part of the spinal segment could not be totally displayed on one single coronal plane and no additional measurement was carried out . In 11 patients ,all the segments of the spine could be clearly displayed on a single coronal plane and the curvature of scoliosis was then measured . There were no significant differences between the angle measured on 3-D ultrasound imaging and the Cobb angle measured on radiograph( P =0 .974) and the two measurement methods have high correlation( r =0 .991 , P <0 .0001) . Conclusions The commercial available 3-D ultrasound imaging system applied in this study offers a viable method to assess scoliosis in adolescence and adults in a reliable and radiation-free manner .

RESUMO

BACKGROUND: Insufficient bone mass in the posterior maxilla region often increases the difficulty of dental implantation. The application of maxillary sinus floor elevation successfully solves this problem. In the surgery bone grafting can be used to obtain sufficient bone mass. However, nonuse of graft materials can shorten the cycle of patient's treatment and reduce trauma and cost. OBJECTIVE: Combining the latest relevant research trends, to review the advances in the study of maxillary sinus floor elevation with and without bone grafting. METHODS: The first author searched PubMed, VIP, WanFang and CNKI databases using computer for relevant articles published from January 1985 to August 2018. The key words were "dental implant, maxillary sinus, bone augmentation, bone substitute" in English and Chinese, respectively. Finally, 78 articles were included in result analysis after screening and summarizing. RESULTS AND CONCLUSION: Ideal results can be achieved in the maxillary sinus floor elevation with or without simultaneous bone grafting In addition, the nonuse of grafting materials have the advantages of simplifying surgical procedures, shortening treatment cycles, reducing trauma and pain, reducing treatment costs, reducing complications and failure rates, thus gaining the favor of doctors and patients. Therefore, the maxillary sinus floor elevation without simultaneous bone grafting will be the primary choice of the doctors engaged in dental implantation in the future.

RESUMO

Objective This study aimed to investigate the associations between adipose tissue distribution and risks of macro-vascular complications in type 2 diabetic patients. Methods The study included T2DM patients either previously diagnosed by questionnaire or newly diagnosed with fasting plasma glucose(FPG)≥7. 0 mmol/L and/or 2 hour postprandial glucose ( 2h PG )≥11. 1 mmol/L from residents older than 40 years from Jiading Community, Shanghai, China(n=10 375). Each participant had gone through anthropometric measurements, blood tests, vascular function tests including carotid intima-media thickness ( CIMT ) , ankle-brachial index ( ABI ) , branchial-ankle pulse wave velocity( baPWV) as well as carotid plaques. Results WHR quartiles was significantly associated with high risks for ten-year cardiovascular disease risks [ ASCVD high risk: OR:1. 17 ( 1. 05-1. 31 );Framingham high risk:1. 13(1. 00-1. 29)]. However, WHR has no significant relations with risks of increased carotid intima-media thickness( CIMT) , abnormal ankle-brachial index( ABI) , increased brachial ankle pulse wave velocity( baPWV) or carotid plaques. Conclusions In type 2 diabetic patients, abdominal adipose tissue has an independent role in high risk for ten-year cardiovascular diseases. However it has no relations with other vascular functions including increased CIMT, abnormal ABI, increased baPWV, or carotid plaque.

RESUMO

Objective To study any effect of repetitive magnetic stimulation (rMS) on the differentiation and apoptosis of rat neural stem cells in vitro. Methods The bilateral hippocampus of a 3-day old Sprague-Dawley rat was used to culture neural stem cells (NSCs) in vitro.P2 NSCs were differentiated to neurons or astrocytes in differentiation medium and then divided into a control group in which the NSCs differentiated naturally, and an rMS group in which 1000 impulses/day of rMS were applied at 10 Hz once a day for 7 days at 50% of maximum output. One hour after the last stimulation, immunofluorescence was used to analyze the ratio of neurons and astrocytes, and Western blotting was employed to evaluate the expression of glial fibrillary acidic protein (GFAP),β-Ⅲ tubulin and brain-derived neurotrophic factor (BDNF).NSCs which had differentiated for 7 days without stimulation were then selected and divided into an apoptosis group and an apoptosis+rMS group. The same rMS protocol was applied to the latter group 1h after the apoptosis, and 4h later flow cytometry (anexin V-FITC) was employed to evaluate the apoptosis ratio.Bcl-2,Bax and caspase-3 protein expression were analyzed using Western blotting. Results There were no significant differences between the control and rMS groups in the proportion of NSCs differentiating to neurons or in β-Ⅲ tubulin,GFAP or BDNF protein expression. The cell apoptosis rate of the apoptosis+rMS group was significant lower than in the apoptosis group.Caspase-3,Bcl-2 and Bax protein expression were also significantly different between the two groups. Conclusion rMS at 10Hz for 7 days has no effect on the differentiation of NSCs,but it has a protective effect on neural cells and decreases the apoptosis rate.

RESUMO

@#Objective To analyze the data of concentric (c) and eccentric (e) peak torque of hamstring (H) and quadricep (Q) in differ-ent gender and angle after 4-week concentric and eccentric isokinetic exercise training on knee, in order to provide suggestions for the knee extensor and flexor in rehabilitation exercise prescription. Methods 28 healthy college students from May to June 2014 in our college partic-ipated in maximum concentric (n=14) or eccentric (n=14) resistance training of the right knee extensors and flexors 3 times per week for 4 weeks. The concentric strength was measured with BIODEX isokinetic dynamometer assessment system 4 at 30 °/s, 60 °/s, 90 °/s, and ec-centric strength was measured at 60 °/s, 120 °/s, 180 °/s. PT, H/Q (He/Qc, Hc/Qc) ratio and H/H (He/Hc) or Q/Q(Qe/Qc) ratio were ana-lyzed. Results Concentric isokinetic training could improve the Hc and He at 60 °/s and 90 °/s (P<0.05), and the Qe and Qc increased at 30 °/s, 60 °/s, 90 °/s respectively (P<0.01). There was no significant improvement after eccentric training (P>0.05). After concentric isokinet-ic training, the He/Qc ratio decreased at 90 °/s (P<0.05), the Hc/Qc ratio decreased at 30 °/s (P<0.05). However, there was no difference in He/Qc and Hc/Qc after eccentric contraction (P>0.05). The Qc/Qe ratio increased at 60 °/s and 90 °/s (P<0.05), and the Hc/He ratio de-creased at 90 °/s (P<0.05) after concentric training. There was no difference in Hc/He and Qc/Qe after eccentric contraction (P>0.05). There were significant differences in Hc, He, Qc, Qe and He/Qc ratio between different genders (P<0.05). Conclusion Concentric training and ec-centric training have different influences on the muscle strength after 4 weeks training. Eccentric training only can reduce the knee stability. Hc/He ratio and Qc/Qe ratio can be used to evaluate the knee muscle function. Gender should be considered into training.

Objective To study the mechanism of rats' neural stem cells (NSCs) proliferation in vitro after repetitive magnetic stimulation (rMS).Metbods The bilateral hippocampus of neonatal Sprague-Dawley rats (<3 d) was taken out to culture NSCs in vitro. The OD value was evaluated with Cell Counting Kit-8 (cck-8) and cell growth curve was generated. The NSCs cultured were divided into a control group and an rMS group.rMS (3 days, once per day) was applied on p2 NSCs at 10 Hz,50% machine output and 200 pulses per day.One hour after the last rMS,the cck-8 was used to test the cell proliferation, and the western blotting was applied to detect the protein expression of c-fos and p-CREB.Results The nestin fluorescent staining of p2 neurospheres was proved to be neural stem cells. The growth curve indicated that their viability reached the peak on the third day.The OD value in the rMS group (0.309 ± 0.043) showed a significant difference (P < 0.05) after rMS compared with the control group (0.256± 0.043).So did the c-fos and p-CREB protein expression between the two groups (P < 0.01).Conclusion The rMS at 10 Hz can promote rats' NSCs proliferation in vitro, which may be related to the increased expression of p-CREB and c-fos after rMS.
