RESUMO

OBJECTIVE@#To study the effective method of comprehensive evaluation and analysis of hospital valuable medical equipment performance.@*METHODS@#The operation performance of 6 valuable equipment was evaluated by cost-benefit method, comprehensive index method and public evaluation method.@*RESULTS@#Utilize equipment information management methods for data collection and evaluation, and construct an assessment data model based on evaluation indicators from three aspects: equipment operation status, profitability status, and scientific research contribution.@*CONCLUSIONS@#Through the performance analysis of different types of valuable medical equipment, a more real and comprehensive quantitative analysis is carried out, which plays a key role in the reasonable purchase, efficient operation and avoiding idling.

【Objective】 To explore the emergency treatment of penoscrotal avulsion injury based on the American Association for Surgery and Trauma (AAST) penoscrotal injury grade. 【Methods】 Data of 30 patients with penoscrotal avulsion injury treated in our hospital with in-situ suture, skin grafting or skin flap during Oct.2003 and Dec.2017 were reviewed. 【Results】 Among the 30 patients, 29 received emergency surgery, including in-situ suturein 15 grade Ⅰ-Ⅲ cases, skin graft in 8 grade Ⅳ-Ⅴ cases, and skin flap in 6 cases;1 case received delayed operation due to wound infection on admission. In the perioperative period,25 cases had more than 90% wound healing, 3 cases had 70% wound healing after wound dressing change, and 2 cases had complete necrosis. Second-stage skin graft yielded satisfactory penoscrotal appearance. 【Conclusion】 According to the specific conditions of patients, reasonable surgical methods can be selected. For grade Ⅰ-Ⅲ patients, in-situ suture can be used, while for grade Ⅳ-Ⅴ patients, wound condition, age, fertility and other factors should be taken into consideration so as to preserve the function of testis to the maximum extent.

RESUMO

A synchronous-traveling carrier photodetector (STC-PD) with a thick absorption region and large diameter is presented and investigated for 100 Gbps PAM4 applications. In the STC structure, a suitable electric field is introduced to synchronize the traveling of electrons and holes, which achieves high bandwidth while maintaining large responsivity. The characteristics of STC-PD, including electric field distribution, energy band diagram, responsivity, junction capacitance, and frequency response, have been studied carefully.

We demonstrate a new, to the best of our knowledge, dimensional "horizontal optimization" scheme, which can improve the high-speed characteristics of a PIN photodetector by designing the incident optical field distribution. First, coaxially incident faculae with the same peak and same power are studied and simulated, revealing that the bandwidth of the photodetector illuminated by uniform light is higher than that of the device illuminated by nonuniform light. Next, an annular optical field is designed incident to the photodetector, and the bandwidth is further improved. For a PIN photodetector, by carefully optimizing the incident optical field distribution, the photodetector bandwidth under an annular optical field can be increased significantly compared with that under conventional coaxial illumination.

RESUMO

Objective: To systematically review the safety and efifciency for dual antiplatelet therapy (DAPT) ≤ 6 months and DAPT ≥ 12 months in patients after drug eluting stent (DES) implantation. Methods: We collected the data for randomized clinical trials for DAPT ≤ 6 months and DAPT ≥ 12 months in patients after DES implantation up to 2015-01 by searching the literatures of PubMed, EMBASE, Cochrane Library, Scopus and Chinese literature database, and meanwhile collected the relevant reporting cases from both domestic and international cardiovascular conferences for this study. There were 2 investigators independently conducted the literature screening, data extraction and quality evaluation, Meta analysis was performed with STATA 12.0 software. Results: A total of 15,378 patients from 7 eligible studies were enrolled and the patients were divided into 2 groups: DAPT ≤ 6 months group,n=7672 and DAPT ≥ 12 months group,n=7706. Meta analysis indicated that DAPT ≤ 6 months could effectively reduce the major bleeding (OR=0.58, 95% CI 0.37-0.91,P=0.017). While the other incidences between 2 groups were similar as all cause death (OR=0.90, 95% CI 0.73-1.11,P=0.314), cardiac death (OR=0.93, 95% CI 0.70-1.24,P=0.617), myocardial infarction (OR=1.13, 95% CI 0.91-1.41,P=0.275), in stent thrombosis (OR=1.21, 95% CI 0.79-1.85,P=0.382) and cerebrovascular accidents (OR=1.00, 95% CI 0.66-1.51,P=1.000). Conclusion: The incidence rates of cardiovascular and cerebrovascular events are similar in patients with DAPT ≤ 6 months and DAPT ≥ 12 months after DES implantation. DAPT ≤ 6 month had the lower risk of bleeding, which is rather suitable for the patients received new generation of DES, with higher risk of bleeding, lower risk of thrombosis and with poor compliance to medication; however, the large and randomized clinical trials are needed to make ifnal conclusion.

RESUMO

<p><b>OBJECTIVE</b>To develop a novel thermal treatment modality against metastatic tumor, and to verify the hypothesis that the extent of tumor angiogenesis damage and tumor cell necrosis, accompanied with immune suppression cells relief is deterministic to enhance therapeutic effect in the thermal treatment.</p><p><b>METHODS</b>The thermal treatment system was developed in our laboratory. The treatment including hyperthermia and alternate treatment, were locally applied to 4T1 murine mammary carcinoma. The extent of tumor necrosis was examined. Further investigations were performed to study the changes of MDSCs in peripheral blood and spleen.</p><p><b>RESULTS</b>The alternate treatment caused more damage to tumor microvasculature and tumor cell necrosis. Immunosuppression cells significantly reduced in peripheral blood and spleen. Moreover, it highly increased the survival rate of tumor-bearing mice.</p><p><b>CONCLUSIONS</b>The greatest destruction of primary tumor induced by the alternate treatment led to a relief of immune suppression in tumor bearing mice, and significantly increased therapeutic effect, especially for metastatic tumor.</p>

Objective To investigate the suitable mode of cognitive-behavioral therapy for lung transplant recipients during waiting for lung and evaluate the effect to improve patients' physical and mental state.Methods 50 cases were randomly divided into the experimental group and the control group,each group with 25 cases.The control group received conventional treatment.The experimental group received cognitive-behavioral therapy in addition to conventional treatment.SAS and vital signs were measured in the two groups on admission,2 weeks and 4 weeks after admission.Results The SAS score of the experimental group was lower than the control group 2 weeks and 4 weeks after admission and SAS score decreased as the intervention extended.The SBp of the experimental group was lower than the control group 4 weeks after admission.There was no significant difference of DBp between two groups.The heart rate of the experimental group was lower than the control group 4 weeks after admission.The heart rate of the experimental group was lower on 2 weeks and 4 weeks after admission than on admission.4weeks after admission,the respiratory rate of the experimental group was lower than the control group and on admission.Conclusions Cognitive-behavioral therapy can reduce the level of anxiety and keep vital signs stable with good feasibility and effectiveness.

@#Objective To investigate the mental health status of patients with coronary artery stent implantation. Methods Symptom Checklist (SCL-90) was used to evaluate the mental health conditions of 218 patients after coronary artery stent implantation with man-machine conversation and computer evaluation. Results It was found that 48 cases in excellent mental health, accounting for 22.02%; 56 cases in good mental health condition, accounting for 25.69%; 82 cases in normal mental health condition, accounting for 37.61%; and 32 cases in poor mental health, accounting for 14.68%. In addition to the interpersonal factor, hostility and psychosis factors, the other six factors’overall average score was more than 2. More than 50% of the staff scored greater than 3, and their average positive factors were greater than 43 items in somatization, depression, anxiety and fear. Conclusion The patients after coronary artery stent implantation suffered from many psychological problems.

@#Objective To observe the psychological state in patients accepting prosthesis and the effect of psychological intervention on them. Methods38 patients were assessed with Symptom Checklist 90 (SCL-90) before, after prosthesis fitting, and after rehabilitation training and psychological intervention. ResultsThere were 33 cases with somehow disorder in some factors of SCL-90 before prosthesis fitting. Except somatization, interpersonal and psychotic factors, all the other factors increased after fitting (P<0.05). All the factors scored lower after rehabilitation and intervention (P<0.05). ConclusionPatients accepting prosthesis appear with some psychological disorders, and get worst after prosthesis fitting. Rehabilitation training and psychological intervention can relieve these disorders.

BACKGROUND: The preventative capabilities of enamel demineralization, rising from the fixed orthodontic treatment, have drawn tremendous attention in recent years. Despite a variety of investigations carried out by both domestic and foreign scholars, whereas, the results of ways to reduce the incidence of demineralization are never fully satisfactory or unified. OBJECTIVE: To evaluate the inhibitory effect of Fluor protector on enamel demineralization based on clinical practices during fixed orthodontic treatment.DESIGN, TIME AND SETTING: Contrast observation was performed in the Department of Stomatology, Qingdao Second Sanatorium, Jinan Military Area Command of Chinese PLA from January 2006 to January 2009. PARTICIPANTS: A total of 132 participants (856 teeth, aged 7-18 years, average age 10.5 years, 76 females and 56 males), who had wall-developed hardened dental tissues, no tetracycline pigmentation teeth, no dental fiuorosis, or anything that constricted in their teeth, were involved in the study. And all subjects were volunteers who had signed the informed consent form and agreed to receive dental treatment and periodic review.METHODS: All the teeth were randomly divided into two groups: experimental and control groups. Teeth for the experimental group were bonded with the supporting brackets with the Fluor protectors, whereas teeth for the control group did basically the same only without the Fluor protectors. After the fixed orthodontic treatment, the supporting devices were removed, all the teeth were fully washed, and then the conditions of the tooth surfaces were observed and documented in both experimental and control groups.MAIN OUTCOME MEASURES: The detection rate of orthodontic enamel demineralization in the two groups alter the fixed orthodontic treatment.RESULTS: After evaluation of the results of both groups, the incidence rate of enamel demineralization in the experimental group (5.51%) was obviously lower than control group (15.1%).CONCLUSION: Fluor protector has a significantly positive effect on preventing enamel demineralizaUon dudng fixed orthodontic treatment.

<p><b>OBJECTIVE</b>To explore the possible relationship between erectile dysfunction (ED) and benign prostate hyperplasia (BPH) in men, and to assess the positive effect of Sildenafil on the lower urinary tract symptoms (LUTS) from BPH while treating ED.</p><p><b>METHODS</b>Thirty-two patients with ED and BPH were offered oral Sildenafil and reviewed before and six months after the administration of Sildenafil by the International Index of Erectile Function-5 (IIEF-5) and the International Prostate Symptom Score (IPSS) questionnaires. Scores were tested by chi-square.</p><p><b>RESULTS</b>IIEF-5 scores were increased by 42.36% and IPSS scores declined by 20.14%, with statistical significance (P < 0.01).</p><p><b>CONCLUSION</b>Treatment of ED with Sildenafil appears to improve urinary symptom scores. A lower IPSS at baseline seems to predict a better response to Sildenafil therapy for ED.</p>

<p><b>OBJECTIVE</b>To evaluate the effects of combined treatment of hormone therapy with strontium-89 for pain from bone metastases in patients with prostate cancer.</p><p><b>METHODS</b>Sixteen patients with metastasis prostate cancer received hormone therapy with strontium-89 chloride (89Sr), and 4 of them received radiotherapy.</p><p><b>RESULTS</b>The pain was relieved. Relief occurred in 75.6% of the patients for 3 months, 80.5% for 6 months and 63.4% for 9 months.</p><p><b>CONCLUSION</b>Hormone therapy enhances the effect of 89Sr without obvious toxicity. The combined treatment not only significantly raises pain palliation, prolongs pain-free survival, but improves the patients life quality.</p>
