RESUMO

The construction of compact county medical consortium is an effective way to integrate county medical resources, improve county diagnosis and treatment capabilities, and establish and improve the hierarchical diagnosis and treatment system. It is the guidance of national policy and the demand of social practice. Based on the reality of Jiangyin′s developed local economy, sufficient overall high-quality medical resources and high health demand of residents, Jiangyin People′s Hospital Medical Group explored the construction practice of compact county medical consortium according to local conditions. The group focused on implementing the functional positioning of medical institutions, improving the primary service ability, and straightening out the two-way referral process. Through the four major measures of establishing mechanism, strengthening the primary level, optimizing the process, and focusing on convergence, the group improved the organization structure and operation mechanism, continuously improved the management level and service connotation of the medical consortium. The goal of establishing an overall management mode, improving primary service capabilities, and forming an up-down cooperation mechanism had been initially realized.

RESUMO

A newly identified novel coronavirus (2019-nCoV) has caused numerous acute respiratory syndrome cases in Wuhan China from December 2019 to Feb 2020. Its fast spreading to other provinces in China and overseas is very likely causing a pandemic. Since the novel coronavirus has been reported to be capable of endangering thousands of lives, it is extremely important to find out how the coronavirus is transmitted in human organs. Apart from fever and respiratory complications, gastrointestinal symptoms are observed in some patients with 2019-nCoV but the significance remains undetermined. The cell receptor angiotensin covering enzyme II (ACE2), which is the major receptor of SARS-nCoV, has been reported to be a cellular entry receptor of 2019-nCoV as well. Here, to more precisely explore the potential pathogen transmission route of the 2019-nCoV infections in the gastrointestinal tract, we analyzed the ACE2 RNA expression profile in the colon tissue of healthy adults and colorectal cancer patients of our cohort and other databases. The data indicates that ACE2 is mainly expressed in epithelial cells of the colon. The expression of ACE2 is gradually increased from healthy control, adenoma to colorectal cancer patients in our cohort as well as in the external Asian datasets. According to the expression profile of ACE2 in colon epithelial cells, we speculate adenoma and colorectal cancer patients are more likely to be infected with 2019-nCoV than healthy people. Our data may provide a theoretical basis for the classification and management of future 2019-nCoV susceptibility people in clinical application.

RESUMO

Objective To explore the basic functional orientation of countywide healthcare alliances and how to maintain their stable operation and sustainable development direction. Methods 42 government officials, experts, employees of countywide healthcare alliances and residents were interviewed. The grounded theory was used to analyze data from the interview, with the core categories gradually extracted. Results Three basic functions were defined for such healthcare alliances, namely basic functions, stable operation and sustainable development. Conclusions The healthcare alliances prove an effective way to build the hierarchical medical system and improve people's health in general. Such alliances rely on good governance mechanism, reasonable balance mechanism of interests and scientific resource sharing mechanism.

RESUMO

Literature review to build optional indexes pool,Delphi method to screen indexes,fuzzy model used to calculate the weight,for building such a system.An index system centering on public wellbeing is thus constructed,comprising three level 1 indicators,six level 2 indicators,40 level-3 indicators.The system can satisfactorily reflect the vision of county-level public hospitals reform,and showcase outcomes of such a reform.

Background and purpose:The prognosis in patients with early breast cancer(EBC) was poorly defined by clinical and histopathological features.There were more than 200 genes that might be correlated with development,progression,recurrence and prognosis of breast cancer in the published literature.This study evaluated the prognosis of patients with EBC at molecule level according to the protein expressions of 16 selecting genes(HER2,ER,PR,BCL2,Ki-67,BMYB,Cyclin B1,STK15,MMP11,BAG1,NM23,PTEN,P53,P27,VEGF,PCN A).Methods:The immunohistochemical method was used to detect the protein expressions of these genes in 76 patients with EBC and the statistical analysis was performed by COX proportional hazards model.Results:COX proportional hazard model revealed that BCL2,VEGF and STK15 were independent prognostic factors for patients with EBC.A formula of PI was set up according the three genes.The predictive outcomes with PI were compared with the actual follow-up outcomes in 48 patients with EBC.As a result,the predictive accuracy of good and bad prognosis was 86.67% and 91.67%,respectively.Conclusion:A formula of PI derived from protein expressions of the 16 genes correlated with breast cancer was ultimately set up and might be used to predict the outcomes of the patients with EBC.
