RESUMO

A 36-year-old male patient presented with a decrease in vision after undergoing scleral suturing for a left eye injury caused by an iron hook, combined with intravitreal injection of cefuroxime. Ocular examination revealed extensive gray-white edematous areas in the macular region, along with focal serous shallow retinal detachment in the posterior pole. Following admission, comprehensive ophthalmic examinations were conducted, leading to the diagnosis of toxic retinal damage in the left eye. Treatment with oral corticosteroids and interventions to improve microcirculation were initiated, resulting in improved visual acuity. At the six-month follow-up, the patient's visual acuity had recovered to 0.5.

RESUMO

The common data model (CDM) is an important tool to facilitate the standardized integration of multi-source heterogeneous healthcare big data, enhance the consistency of data semantic understanding, and promote multi-party collaborative analysis. The data collections standardized by CDM can provide powerful support for observational studies, such as large-scale population cohort study. This paper provides an in-depth comparative analysis of the data storage structure, term mapping pattern, and auxiliary tools development of the three international typical CDMs, then analyzes the advantages and limitations of each CDM and summarizes the challenges and opportunities faced in the CDM application in China. It is expected that exploring the advanced technical concepts and practical patterns of foreign countries in data management and sharing will provide references for promoting FAIR (findable, accessible, interoperable, reusable) construction of healthcare big data in China and solving the current practical problems, such as the poor quality of data resources, the low degree of semantization, and the inabilities of data sharing and reuse.

Urethroplasty is an important method to treat male urethral stricture. Nowadays, urethroplasty mainly includes two types: anastomotic urethroplasty and substitution urethroplasty. Anastomotic urethroplasty mainly includes primary anastomosis urethroplasty and non-transecting anastomotic urethroplasty. Substitution of urethroplasty mainly includes staged urethroplasty and one-stage urethroplasty. Substitution materials always are chosen by pedicle skin flap and free mucosal graft. Anastomosis urethroplasty has shown good results in short bulbar urethral stricture and posterior urethral stricture after pelvic fracture. Among them, non-transecting anastomosis urethroplasty has become a new surgical method for iatrogenic, single, short or non-occluded stenosis. At present, the one-stage substitution urethroplasty is the most widely used. However, there are still many complicated cases that must be solved by staged urethroplasty. Pedicle skin flap and oral mucosa are widely used as substitutes at present. How to select the best surgical procedure and substitute materials individually would be the problem worthy of attention in the future. Accumulating more long-term follow-up data is helpful for objective comparison of various surgical procedures and grafts.

Objective: To analyze influence on HIV related fatality of implementation of 'Four Free Services One Care' policy in China during 1993-2012, and evaluate effectiveness for the policy of HIV/AIDS prevention and treatment. Methods: The data during 1993-2003 (before the implementation of the policy) and during 2005-2012 (after the implementation of the policy) were collected from the national HIV/AIDS Comprehensive Response Information Management System for the analysis on HIV related fatality in Chinese, especially in rural residents. Interrupted time series analysis was used to analyze the trends of HIV related fatality during 1993 and 2012. Results: During the twenty years from 1993 to 2012, the HIV related fatality in China decreased, and the HIV related fatality in rural areas was higher than the overall HIV related fatality. Compared with the period before the implementation of the policy, the overall HIV related fatality decreased by 1.5% (P=0.156) and the HIV related fatality in rural residents decreased by 2.6% in 2004 when the policy was implemented (P=0.094). As for the change of slope, the overall slope rate after the policy implementation was -0.466 (P=0.005), indicating a decrease trend of HIV related fatality. The slope rate after the policy implementation in rural residents was-0.592 (P=0.005), indicating a decrease trend of HIV related fatality. At the same time, the slope rate after the policy implementation in rural residents infected through blood transmission route was-0.766 (P<0.001), indicating a decrease trend of HIV related fatality. Conclusions: The implementation of the 'Four Free Services One Care' policy in China has made remarkable progress in free HIV antiviral therapy.

Objective: The prevalence of HIV/AIDS in people aged ≥50 years is increasing in China, resulting in another focus for AIDS prevention and control. A Meta-analysis was conducted to evaluate the HIV infection, its area and time distributions in people aged ≥50 years in China to provide evidence for the prevention and control of HIV/AIDS. Methods: Information on HIV infection rate in people aged ≥50 years in China were collected from Wanfang digital database, China National Knowledge Infrastructure (CNKI), PubMed and Embase. The R Studio1.1.456 software was used to calculate the pooled infection rate, and assessed the publication bias. Results: A total of 30 studies which covered 13 provinces were selected. From 2010 to 2018, the pooled HIV infection rate in people aged ≥50 years was 1.68% (95%CI:1.00%-2.79%). The HIV infection rate (2.60%) was higher in eastern China than in central and western China (0.16%, 2.13%). Conclusion: The overall HIV infection rate in people aged ≥50 years is generally high in China. It is important to control the HIV infection in this population to facilitate the AIDS prevention and control in China.

Objective: To understand the AIDS awareness and related factors as well as the demands for AIDS-related health education programs, among first-grade students from three senior high schools in Tianjin. Methods: A cross-sectional study using both stratified cluster and convenient sampling methods was conducted. Anonymous questionnaires including social demographic characteristics, AIDS-related knowledge and needs for AIDS health education were distributed to the first-grade students from two general middle schools and one vocational senior middle school in Tianjin. Results: A total of 1 082 students were responding to the questionnaires. Among them, the overall awareness rates of AIDS-related knowledge were 34.3%(371/1 082), 71.9%(778/1 082) of the students had received knowledge about AIDS and 59.4% (643/1 082) of the students had received knowledge about sexual health in the past, from the three schools. There appeared statistical differences among the two types of schools. 7.0% (76/1 082) of the students reported having had sexual behaviors. Results from the multivariate logistic regression analysis showed that, in vocational senior middle school, the awareness rate of AIDS knowledge was lower (compared with general middle school, OR=0.41, 95%CI: 0.29-0.59) and in the following items, including no need in receiving safe sex health knowledge (compared with needs of safe sex health knowledge, OR=0.62, 95%CI: 0.43-0.91), no interests in reading AIDS-related health education materials (compared with attention to AIDS health education materials, OR=0.41, 95%CI: 0.30-0.56), not aware of safe sex behavior (compared with aware of safe sex behavior, OR=0.55, 95%CI: 0.39-0.77), not aware of correct condom use, compared with aware of correct condom use (OR=0.33, 95%CI: 0.24-0.46). Conclusions: The overall awareness rate of AIDS knowledge was low among the first-grade students in Tianjin. Some students reported of already having had insertive sex, sexual health knowledge has a significant impact on AIDS knowledge, suggesting that sexual health education and AIDS health education among high school students be strengthened in Tianjin.

BACKGROUND: Chronic hepatitis B patients carry a high risk of developing hepatocellular carcinoma (HCC). α-Fetoprotein (AFP) is one of the most commonly used and reliable biomarkers for HCC. However, the AFP level during different phases of CHB is not well understood. We aimed to identify the AFP levels during the different infection phases of CHB patient and explore which phase is at high risk of developing HCC. METHODS: Three hundred and fifty-five CHB patients were divided into four groups: a. immune tolerant HBeAgpositive phase (IT); b. immune reactive HBeAg-positive phase (IR), c. inactive carrier state (IC), d. HBeAg-negative activation phase (ENA). The risk of development of HCC in different group is assessed by the serum AFP levels. An electrochemiluminescence assay was used to analyze serum AFP levels. RESULTS: Mean AFP levels were different in each phase of CHB (p < 0.001): IT (9.6 ng/mL), IR (33.7 ng/mL), IC (3.2 ng/mL), and ENA (71.6 ng/mL). The ENA phase had the highest AFP level and IC phase has the lowest. There was no correlation between serum AFP level and HBV viral load. A significant correlation between serum ALT levels and HBV viral load was observed (r = 0.272, p < 0.01). CONCLUSIONS: These findings suggest that high levels of AFP during HBeAg-negative activation phase (ENA) may be associated with a high risk of developing of HCC. Furthermore, higher burden of HBV viral load is associated with more severe liver damage.

The practical use of bone marrow mesenchymal stem cells (MSCs), considered to be the best candidate in the field of regenerative medicine, is limited by the low efficiency of MSC differentiation. Sphingosine 1-phosphate (S1P) could promote proliferation, survival, and differentiation of many types of cells, but its effects on MSCs remain elusive. In this study, S1P was added during primary MSCs (PR-MSCs) culture and the effects of S1P on proliferation, survival, and differentiation of PR-MSCs were evaluated. The results showed that S1P could improve PR-MSCs proliferation activity in a concentration-dependent manner, and the apoptosis of PR-MSCs cultured in hypoxia was significantly reduced in the S1P-treated group compared to the control group. After being cultured with vascular endothelial growth factor for 7 days, the specific genes of endothelial cells were highly expressed in S1P-treated PR-MSCs compared to control group, which coincided with the augumented production of hepatocyte growth factor, stromal cell-derived factor-1, and insulin-like growth factor-1. In summary, our results suggest that S1P can promote proliferation, survival, and differentiation into vascular endothelial cells of PR-MSCs. These results will promote the clinical application of PR-MSCs and deepen our understanding of the function mechanism of S1P.

Iron is important general well being, to prevent or treat anemia, and is a cofactor of many enzymes in the anti-oxidant process. Effect of sodium iron ethylenediaminetetraacetate (NaFeEDTA) and ferrous sulfate on iron bioavailability and oxidative stress in anemic pregnant women was evaluated. A 2-month randomized controlled trial was conducted on 153 anemic pregnant women, with 80 <= Hb <110 g/L. They were randomly allocated to three groups: group C (n=51) was the placebo control group, group I (n=51) was supplemented daily with 60 mg iron as ferrous sulfate, and group IE (n=51) with 60 mg iron as NaFeEDTA. Blood samples were collected before and at the end of the intervention for measurements of hematological indices and oxidative stress parameters. Considerable increases of hematologic indicators were observed: 20.5 and 21.8 g/L for Hb (both p values <0.001); 4.81 and 7.19 µmol/L for plasma iron (both p values <0.001), 2.63 and 8.99 µg /L for ferritin (both p values <0.05) in I and IE groups, respectively, compared with the control group. Glutathione peroxidase (GSH-Px) activities increased by 32.6 and 75.3 IU/ml, and malondialdehyde (MDA) levels decreased by 0.70 and 1.12 µmol/L in I and IE groups, compared with the C group (p values <0.05). Moreover, differences of plasma iron, ferritin and GSH-Px activity were 2.38 µmol/L, 6.36 µg /L and 42.7 IU/ml were also significantly greater in the IE group than in the I group. Moderate iron supplementation may be beneficial to improving iron deficiency and oxidative stress, and NaFeEDTA is better than ferrous sulfate.

BACKGROUND: Iron and vitamin A deficiencies impact anemia and the immune system. OBJECTIVE: to investigate the effect of iron combined with retinol supplementation on iron status, IL-2 level and lymphocyte proliferation. METHODS: a double-blind randomized trial conducted over 2 months. We randomly allocated 186 anemic pregnant women with 80 ≤ Hb 0 < 110 g/L into four groups. Group I (n=47) was supplemented daily with 60 mg iron as ferrous sulfate, IF (n=46) with 60 mg iron and 0.4 mg folic acid, IR (n=46) with 60 mg iron, 2.0 mg retinol and 0.4 mg folic acid and C (n=47) was the placebo group,. RESULTS: after the 2 months trial, there were considerable increases of iron status in Hb, plasma iron and ferritin in the I, IF and IR groups compared with Group C. Increases in plasma iron and ferritin in the IR group were also significantly greater than in Groups I and IF. Compared with group C, increases of IL-2 levels were 119, 184 and 206 ng/L; and lymphocyte proliferation increased by 0.095, 0.112 and 0.219 in Groups I, IF and IR, respectively. Increases of IL-2 were 65.3 ng/L and 87.5 ng/L in Groups IF and IR, greater than in Group I (both p values <0.01); and lymphocyte proliferation in Group IR were 0.124 and 0.107, also greater than in Groups I and IF, respectively. CONCLUSION: iron combined retinol supplementation was more beneficial to improving iron status and lymphocyte proliferation during pregnancy than iron alone.

Benzene is a recognized haematotoxin and leukaemogen, but its mechanism of action and the role of genetic susceptibility are still unclear. Cytochrome P450 2E1 (CYP2E1) and myeloperoxidase (MPO) are involved in benzene activation; and NAD (P)H:quinine oxidoreductase 1 (NQO1), glutathione S-transferase theta 1 (GSTT1) and glutathione S-transferase mu 1 (GSTM1) participate in benzene detoxification. The common, well-studied single-nucleotide polymorphisms (SNPs) were analysed in these genes drawn from the toxicant-metabolizing pathways. A total of 100 workers with chronic benzene poisoning (CBP) and 90 controls were enrolled in China. There was a 2.82-fold (95% CI = 1.42-5.58) increased risk of CBP in the subjects with the NQO1 609C > T mutation genotype (T/T) compared with those carrying heterozygous (C/T) and wild-type (C/C). The subjects with the GSTT1 null genotype had a 1.91-fold (95% CI = 1.05-3.45) increased risk of CBP compared with those with GSTT1 non-null genotype. There was no association of CYP2E1 and MPO genotype with CBP. A three genes' interaction showed that there was a 20.41-fold (95% CI = 3.79-111.11) increased risk of CBP in subjects with the NQO1 609C > T T/T genotype and with the GSTT1 null genotype and the GSTM1 null genotype compared with those carrying the NQO1 609C > T C/T and C/C genotype, GSTT1 non-null genotype, and GSTM1 non-null genotype. The study provides evidence of an association of a gene-gene interaction with the risk of CBP.

The carcass characteristics of 334 Chinese Yellow Cattle, and their Simmental and Limousin crosses, were investigated in abattoirs in Hebei and Sandong provinces of China. The overall slaughter age was 4.8±2.2 years at a mean liveweight of 527.28±77.0 kg. Mean ages and liveweights for Yellow Cattle and its crosses were, respectively, 5.29±2.03 and 2.85±1.71 years and 519.43±78.7 and 563.13±56.8 kg. Carcass weights varied considerably and, consequently, so did subcutaneous fat depths and dressing percentages. Overall, mean carcass weights, subcutaneous fat depths and, dressing percentages and eyemuscle areas were 283.9±64.1 kg, 14.0±8.0 mm, 54.0±4.0%, and 62.7 ±13.3 cm(2), respectively. Compared with pure Yellow Cattle, the crosses were slaughtered at younger age, and had larger live and carcass weights, higher dressing percentages, less subcutaneous fat depth and bigger eyemuscle areas. It is concluded that crossbreeding can significantly improve Yellow Cattle' meat productivity, however feeding systems need to be improved and an appropriate grading system developed to improve beef production and quality in China.

The relationship between the thyroid functional test in 107 malignant tumour patients' sera with Deficiency-Excessiveness Syndromes in TCM, was studied. The results revealed that there were significant differences between the FT3, FT4 in tumour and control groups (P < 0.01) as well as between the rT3, FT3/rT3 in each type of tumour and control groups (P < 0.05-0.01). There was also definite relationship between the FT3, rT3, FT3/rT3 and the degree of differentiation, rate of progression and malignancy of tumours as well. rT3 elevated and FT3 reduced in accordance with the lowering of body resistance and reinforcing of pathogenic factors. FT3, rT3, FT3/rT3 are essential factors that reflecting the wax and wane of body resistance in the cases with malignant tumours in modern medicine.
