RESUMO

[Purpose] The purpose of this study was to determine whether strict adjustment of the pillow height using the Set-up for Spinal Sleep method improves clinical outcomes like neck pain and somatic symptoms. [Participants and Methods] A total of 84 participants with chief complaints of stiff shoulders and neck pain were evaluated using the numerical rating scale and the Somatic Symptom Scale-8. Questionnaires were used to collect data at the baseline, after two weeks, and after three months of adjusting the pillow height. [Results] Forty-two participants (50%) achieved the minimal clinically important difference for neck pain with a decrease of three points or higher in the numerical rating scale. The baseline neck pain scores were significantly higher in the group that achieved the minimal clinically important difference. The three-month change in neck pain and the Somatic Symptom Scale-8 was significantly greater in participants who were satisfied with treatment. There was a significant positive association between improvement in the numerical rating scale score and improvement in the Somatic Symptom Scale-8 at three months. [Conclusion] Strict adjustment of the pillow height using the Set-up for the Spinal Sleep method significantly improved both physical neck pain and somatic symptoms related to psychological and social problems.

RESUMO

The arcuate nucleus (ARC) of the hypothalamus has been identified as a prime feeding regulating center in the brain. Several feeding regulating peptides, such as neuropeptide Y (NPY) and proopiomelanocortin (POMC), are present in neurons of the ARC, which also serves as a primary targeting site for leptin, a feeding inhibiting hormone secreted predominantly by adipose tissues, and for orexin (OX)-containing neurons. OX is expressed exclusively around the lateral hypothalamus, an area also established as a feeding regulating center. Some recent physiological analyses have shown that NPY- and POMC-containing neurons are activated or inactivated by leptin and OX. Moreover, we have already shown, using double immunohistochemical staining techniques, that NPY- and POMC-containing neurons express leptin receptors (LR) and orexin type 1 receptors (OX-1R). However, no morphological study has yet described the possibility of whether or not these arcuate neurons are influenced by both leptin and OX simultaneously. In order to address this issue, we performed histochemistry on ARC neurons using a triple immunofluorescence method. We found that 77 out of 213 NPY- and 99 out of 165 POMC-immunoreactive neurons co-localized with both LR- and OX-1R-immunoreactivities. These findings strongly suggest that both NPY- and POMC-containing neurons are regulated simultaneously by both leptin and OX.

Galanin-like peptide (GALP) is a newly discovered 60 amino acid peptide from the porcine hypothalamus. GALP has been shown to be expressed predominantly in the arcuate nucleus (ARC) of the rat hypothalamus, a region considered to be one of the most important feeding-regulating centers in the brain. GALP-containing neurons in the ARC express leptin receptors, but relationships between GALP and other feeding-regulating neurons have not yet been fully elucidated. Given that Orexin (OX)-containing neurons make synaptic inputs to the ARC, we thus examined the relationship between GALP and OX in the ARC by use of a dual immunostaining technique. OX-immunoreactive fibers appeared to be closely apposed to GALP-immunoreactive cell bodies and their processes. We also examined whether the OX receptor, OX(1)-R was expressed in the GALP-containing neurons. Immunoreactivity for both OX(1)-R and GALP was detectable in 9.6 % neurons (range 4.2-14.6%) in the ARC. These findings strongly suggest that GALP may participate in the regulation of feeding behavior under the influence of leptin and OX.

The distribution and ultrastructural localization of the orexin-1 receptor (OX(1)R) in the rat arcuate nucleus were studied using immunocytochemical techniques. OX(1)R-containing neurons were found throughout the nucleus, but were concentrated in its posterior region. Both OX(1)R-positive perikarya and dendrites were found to receive synapses from other unknown axon terminals. In addition, a small number of OX(1)R-positive axon terminals were observed. These OX(1)R-immunoreactive axon terminals were often found to make synapses on unidentified immunonegative dendritic processes. The present results indicate that orexin may act on food intake regulating neurons through both pre- and post-synaptic OX(1)R.

Immunohistochemistry and Western blotting were used to determine the distribution of orexin receptors in the rat brain. Strong orexin receptor 1 (OX1R) immunoreactivity was detected in the hypothalamus including the arcuate, ventromedial, and tuberomammillary nuclei that are involved in feeding regulation. The neuropeptide Y- and proopiomelanocortin-containing neurons of the arcuate nucleus, which act to stimulate or to inhibit feeding, respectively, displayed intense OX1R immunoreactivity by double immunostaining. Western blotting analysis yielded a 50-kDa major band of OX1R.
