RESUMO

OBJECTIVE: To explore whether electroacupuncture (EA) could alleviate osteoarthritis (OA) through affecting the DNA methylation regulated transcription of miR-146a and miR-140-5p. METHODS: Sixty male eight-week-old Sprague-Dawley rats were divided into three groups: normal group (normal healthy rats; no treatment), model group (OA rats; no treatment) and EA group (OA rats treated with EA). Safranin O staining and modified Mankin's score were performed to evaluate the histopathological alterations and degeneration of cartilage 8 weeks after 8 consecutive weeks of treatment. Quantitative real time polymerase chain reaction (qRT-PCR) assay was employed to evaluate the expression of miR-146a in the cartilage tissue and miR-140-5p in the synovium tissue, respectively. The bisulfite sequencing analysis and quantitative methylation specific PCR (qMSP) were used to analyze the status of methylation in the regulatory regions of miR-146a and miR-140-5p. Chromatin immunoprecipitation (ChIP) assay were performed to assess the binding of nuclear factor-kappa B (NF-κB) and signal transducer and activator of transcription 3 (SMAD-3) in the regulatory regions of miR-146a and miR-140-5p. Western blot analysis was performed to detect the expressions of DNA Methyltransferase 1 (DMNT1), DNA Methyltransferase 3A (DMNT3A), and DNA Methyltransferase 3A (DMNT3b), NF-κB, SMAD3 levels. RESULTS: Our results showed that EA treatment significantly upregulated miR-146a and miR-140-5p expressions. qMSP analysis showed that EA significantly decreased methylation levels of miR-140-5p regulated region and miR-146a promoter in OA cartilage and synovium. Bisulfite DNA sequencing (BDS) and ChIP analysis showed that EA significantly increased binding affinity of SMAD3 and NF-kB on the hypermethylated miR-140 regulatory region and miR-146a promoter, respectively. Western Blot analysis demonstrated that EA also significantly decreased expressions of methylation related proteins- DMNT1, DMNT3a, and DMNT3b as well as NF-κB and SMAD3. CONCLUSIONS: Electroacupuncture stimulating Neixiyan (EX-LE5) and Dubi (ST35) may alleviate OA affecting the DNA methylation regulated transcription of miR-146a and miR-140-5p.

The study aims at exploring the composition and characteristics of the medicinal plant resources of six major ethnic minorities living in Xishuangbanna, as well as the characteristics of traditional utilization of ethnic medicine, and promote the rational protection and inheritance and utilization of ethnic medicine resources and traditional knowledge. Based on the fourth survey of traditional Chinese medicine resources in Xishuangbanna, Yunnan province, through the methods of literature collection, interviewing and field investigation, this paper analyzed the data of the types, medicinal parts and attending diseases of medicinal plant resources of Dai, Hani, Jinuo, Wa, Lahu and Bulang nationalities. The results showed that there were 1 048 medicinal plant resources of the six ethnic minorities, and the medicinal plants were 735, 497, 272, 295, 280 and 49 in the six nationalities, respectively. The cross-use of medicinal plants among the six nationalities accounted for 62.45%, 78.07%, 88.60%, 87.46%, 82.25% and 95.92%, respectively. The main medicinal plants used by the six major ethnic minorities are root and rhizome, whole grass, leaf and stem. Various ethnic medicinal plants are mainly used in the treatment of diseases of digestive system, respiratory system, rheumatic disease, trauma, snake and insect bite and other external diseases. This study showed that Xishuangbanna has rich resources of ethnic medicinal plants. The ethnic medicine culture with Dai medicine as the main part has a long history and diversified theories, forming a unique ethnic characteristic culture. However, all ethnic medicine resources and the application of traditional knowledge are faced with the risk of gradually decreasing, it is suggested to strengthen the mining, Xishuangbanna folk medicine of ethnic culture protection and inheri-tance, and to develop regional economy and the international community has a positive influence.

The default mode network (DMN) has been extensively investigated in the literature. However, previous studies have mainly focused on age-related changes in the DMN between old and young participants. Age-dependent changes in specific regions within the DMN have not been adequately explored across the entire adult lifespan. Thus, in the present study, we performed a seed partial least squares (PLS) analysis to investigate lifespan-wide changes in the regions of the functional and structural DMNs using resting-state functional magnetic resonance imaging (fMRI) and structural magnetic resonance imaging (MRI) data from healthy subjects aged 16-85 years. The posterior cingulate area was selected as the seed region based on prior fMRI studies. The single-group functional connectivity analysis showed a stable connection between the seed and the posterior cingulate cortex (PCC), middle temporal gyrus (MTG) and inferior temporal gyrus (ITG); a decreased connection between the seed and the medial prefrontal cortex (MPFC), anterior cingulate cortex (ACC) and superior frontal gyrus (SFG); and an increased connection between the seed and the precuneus (PreC), inferior parietal lobule (IPL) and middle frontal gyrus (MFG) across the entire lifespan. In contrast, in the single-group structural covariance analysis, the covariance connections of the seed to the DMN regions demonstrated a stable covariance trend to the PCC, MTG, superior temporal gyrus (STG) and ITG; an inverted U-shaped covariance trend to the MPFC, ACC, SFG, MFG and inferior frontal gyrus (IFG); and a U-shaped covariance trend to the PreC with age. Full-group analyses found significant linear decreases in functional and structural DMN integrity. Our findings provide crucial information regarding the influence of age on the function and structure of the DMN and may contribute to the understanding of the underlying mechanism of age-related changes in the DMN over the lifespan.

Metastases to the gingival soft tissues are rare in hepatocellular carcinoma (HCC). To the best of our knowledge, only 13 cases have been reported in English literature to date. The present study described the case of a 43-year-old Chinese man who was admitted to Tangdu Hospital (Xi'an, China) due to the presence of a gingival tumor that was initially diagnosed as granulation tissue by a dental surgeon. Examination of the patient's medical history revealed that a solid mass, measuring 1.5 cm in diameter, was identified in the right lobe of the liver 2 years prior to presentation at the current hospital; however, no biopsy was performed. Thus, the tumor was resected and histological examination resulted in an initial diagnosis of atypical squamous cell carcinoma. However, the histopathological characteristics, immunohistochemical features and serum α-fetoprotein expression levels supported a diagnosis of metastatic HCC. In conclusion, the present case study highlights the difficulties in diagnosing metastatic HCC without a history of primary HCC, and the importance of excluding a diagnosis of metastatic tumor when a lesion is identified in the gingival. Furthermore, it was determined that a final diagnosis of gingival metastasis of HCC predominantly depends on pathological characteristics and immunohistochemical features.

INTRODUCTION: Surgical excision of the ununited ossicles has been suggested for unresolved sequelae of Osgood-Schlatter disease in adults resistant to conservative measures. We report a case where arthroscopy was used to excise the ossicles. A bird eye view from the superolateral portal was helpful in the arthroscopic procedure for excision of the deep low lesion. CASE SERIES: A 32-year-old, male driver had anterior knee pain during walking and sports activity that had been treated conservatively for 3 months. On physical examination, there was a prominent tibial tubercle, but without palpable pain. There was obvious pain when the knee was approaching full extension. On image, a huge ununited ossicle was seen behind the patellar tendon, intruding into the joint space, and there was another two small ununited ossicles beneath the bow-shaped patellar tendon. Arthroscopy was performed through a three portals technique, and a bird eye view was achieved from the superolateral portal. The ossicles were separated from the surrounding soft tissue with a motorized shaver. The small ununited ossicles were removed by use of a grasper. The huge ossicle was removed by use of a motorized bur, and the contouring of the irregular surface of the tibial tubercle was performed. After 3 months, the patient returned to sports activities without any restrictions. CONCLUSION: This report shows that a huge ossicle can cause impingement in anterior knee compartment, and it can be easily removed arthroscopically under assistance of an additional portal.
