We evaluated the characteristics of high-risk human papillomavirus (Hr-HPV) infection in different grades of vaginal intraepithelial neoplasia (VaIN). 7469 participants were involved in this study, of which 601 were diagnosed with VaIN, including single vaginal intraepithelial neoplasia (s-VaIN, n = 369) and VaIN+CIN (n = 232), 3414 with single cervical intraepithelial neoplasia (s-CIN), 3446 with cervicitis or vaginitis and 8 with vaginal cancer. We got those results. First, the most popular HPV genotypes in VaIN were HPV16, 52, 58, 51, and 56. Second, our study showed that higher parity and older age were risk factors for VaIN3 (p < 0.005). Third, the median Hr-HPV load of VaIN+CIN (725) was higher than that of s-CIN (258) (p = 0.027), and the median Hr-HPV load increased with the grade of VaIN. In addition, the risk of VaIN3 was higher in women with single HPV16 infections (p = 0.01), but those with multiple HPV16 infections faced a higher risk of s-VaIN (p = 0.003) or VaIN+CIN (p = 0.01). Our results suggested that women with higher gravidity and parity, higher Hr-HPV load, multiple HPV16 infections, and perimenopause or menopause status faced a higher risk for VaIN, while those with higher parity, single HPV16 infections, and menopause status are more prone to VaIN3.

Purpose: Obstructive sleep apnea (OSA) is a disease with high morbidity and is associated with adverse health outcomes. Screening potential severe OSA patients will improve the quality of patient management and prognosis, while the accuracy and feasibility of existing screening tools are not so satisfactory. The purpose of this study is to develop and validate a well-feasible clinical predictive model for screening potential severe OSA patients. Patients and Methods: We performed a retrospective cohort study including 1920 adults with overnight polysomnography among which 979 cases were diagnosed with severe OSA. Based on demography, symptoms, and hematological data, a multivariate logistic regression model was constructed and cross-validated and then a nomogram was developed to identify severe OSA. Moreover, we compared the performance of our model with the most commonly used screening tool, Stop-Bang Questionnaire (SBQ), among patients who completed the questionnaires. Results: Severe OSA was associated with male, BMI≥ 28 kg/m2, high blood pressure, choke, sleepiness, apnea, white blood cell count ≥9.5×109/L, hemoglobin ≥175g/L, triglycerides ≥1.7 mmol/L. The AUC of the final model was 0.76 (95% CI: 0.74-0.78), with sensitivity and specificity under the optimal threshold selected by maximizing Youden Index of 73% and 66%. Among patients having the information of SBQ, the AUC of our model was statistically significantly greater than that of SBQ (0.78 vs 0.66, P = 0.002). Conclusion: Based on common clinical examination of admission, we develop a novel model and a nomogram for identifying severe OSA from inpatient with suspected OSA, which provides physicians with a visual and easy-to-use tool for screening severe OSA.

BACKGROUND: Hepatocellular carcinoma is one of the most common and deadly cancers. The aim of this study was to elucidate the role of tRNA methyltransferase 6 (TRMT6) during HCC progression. METHODS: The role of TRMT6 in the progression and prognosis of HCC was confirmed by analysis of online databases and clinical human samples. The effects of up-regulation or down-regulation of TRMT6 on HCC cell proliferation and PI3K/AKT pathway-related protein expressions were verified. The molecular mechanism was investigated in vivo by constructing subcutaneous xenograft tumor model. RESULTS: TRMT6 was overexpressed in HCC tissues and associated with Tumour-Node-Metastasis (TNM) stage, primary tumor (T) and regional lymph node (N) classification. TRMT6 expressions in HCC cell lines were higher than that in normal liver cell. TRMT6 overexpression can promote HCC cell proliferation, increase the number of S phase cells. Interference with TRMT6 reduced the PI3K/AKT pathway-related protein expressions, and was reversed by the addition of IGF1. Interference with TRMT6 inhibited tumor growth in vivo and was related to PI3K/AKT pathway. CONCLUSIONS: Overexpression of TRMT6 promote HCC cell proliferation in vivo and in vitro through PI3K/AKT/mTOR axis, which provides a potential choice for the treatment of HCC in clinical practice.

Elsholtzia rugulosa Hemsl., a Chinese herbal medicine, may have the potential to treat COVID-19. The geographical origin has a significant influence on the quality and application of E. rugulosa. In this paper, gas chromatography-mass spectrometry (GC-MS) combined with principal component analysis (PCA) and hierarchical cluster analysis (HCA) and other multivariate statistical analyses were performed for the identification of E. rugulosa. origins. The results showed that the volatile components of E. rugulosa. from different origins were significantly different. PCA and HCA can clearly distinguish the E. rugulosa of Lijiang and Fumin, and Dali and Yongsheng can be distinguished but with a certain overlap. The correlation of different components of was investigated by Pearson correlation. The results showed that E. rugulosa. characteristic component Elsholtzia ketone is regulated by terpenoid metabolism. The discriminant functions of different origins are constructed by Fisher stepwise discrimination, and its initial verification accuracy and leave-one-out cross-validation accuracy were 100% and 87.5%, respectively.

Objective. Retinal fluid mainly includes intra-retinal fluid (IRF), sub-retinal fluid (SRF) and pigment epithelial detachment (PED), whose accurate segmentation in optical coherence tomography (OCT) image is of great importance to the diagnosis and treatment of the relative fundus diseases.Approach. In this paper, a novel two-stage multi-class retinal fluid joint segmentation framework based on cascaded convolutional neural networks is proposed. In the pre-segmentation stage, a U-shape encoder-decoder network is adopted to acquire the retinal mask and generate a retinal relative distance map, which can provide the spatial prior information for the next fluid segmentation. In the fluid segmentation stage, an improved context attention and fusion network based on context shrinkage encode module and multi-scale and multi-category semantic supervision module (named as ICAF-Net) is proposed to jointly segment IRF, SRF and PED.Main results. the proposed segmentation framework was evaluated on the dataset of RETOUCH challenge. The average Dice similarity coefficient, intersection over union and accuracy (Acc) reach 76.39%, 64.03% and 99.32% respectively.Significance. The proposed framework can achieve good performance in the joint segmentation of multi-class fluid in retinal OCT images and outperforms some state-of-the-art segmentation networks.

Luminescent crystals with high efficiency have huge potential in applications for white light-emitting diodes (LEDs). Herein, organic-inorganic hybrid crystals doped with Mn4+, [N(CH3)4]2XF6:Mn4+ (X = Si, Ge, and Ti), were grown under mild conditions. Their crystal structural evolution was confirmed by single-crystal X-ray diffraction at different temperatures. These crystals exhibit intense red emission with a high external quantum efficiency (73.0% for [N(CH3)4]2TiF6:Mn4+) and good thermal stability. The warm white LEDs were fabricated by combining these red-emitting phosphors with a YAG:Ce3+ ceramic chip. As-grown crystals can significantly optimize the performances of white LEDs (colour rendering index up to 95). Hence, this work provides a new strategy to explore Mn4+-activated organic-inorganic hybrid materials for white LEDs.

The non-equivalent doping of Mn4+ in red-emitting fluoride phosphors effectively shortens the fluorescence lifetime. Herein, we successfully synthesized Rb2NaInF6:Mn4+ phosphors by an ion-exchange method. The compensation mechanism of Mn4+ local symmetry and charge balance in the phosphor were studied in detail by theoretical calculations based on density functional theory. The phosphor Rb2NaInF6:Mn4+ (5.59 mol%) exhibits intense red emission with a short fluorescence lifetime (1.45 ms). Meanwhile, it has excellent colour stability and thermal quenching stability at elevated temperatures. Using Rb2NaInF6:Mn4+, ß-SiAlON:Eu2+ and a blue-emitting GaN chip as red, green, and blue components, the white light-emitting diode (w-LED) exhibits a wide colour gamut of 94.1% of the National Television System Committee (NTSC) standard value. So, this work provides valuable information to investigate novel red-emitting fluoride phosphors for LED backlighting.

RESUMO

Two new benzoic acid derivatives, named methyl(S)-3-hydroxy-4-(2- hydroxy -6-methylheptan-2-yl)benzoate (1) and 2-hydroxy-3-(6- hydroxy-6-methylhept-1-en-2-yl)benzoic acid (2), were isolated from the ethanol extract of an endophytic fungus Aspergillus versicolor derived from the medicinal plant Euphorbia royleana. The structures of compounds (1-2) were elucidated using NMR and MS methods.

The zero-phonon line (ZPL) of Mn4+, which highly depends on its local environment, is usually much weaker than the vibrational phonon sidebands. In this work, an ultraintense ZPL emission, coming from a brand new red-emitting Rb2LiGaF6:Mn4+ (RLGFM) phosphor upon blue light excitation, is presented. The interesting spectral characteristic originates from the nonequivalent substitution of Mn4+ for Ga3+ in a rigid octahedral structure with a low symmetry, which induces neighboring cation vacancies that distort the local symmetry of the [MnF6] octahedra. Benefiting from the ultraintense ZPL emission, a wide color gamut is achieved using RLGFM and ß-SiAlON:Eu2+ as color converters. Moreover, a comprehensive investigation on the thermal quenching behavior is also conducted to provide detailed insights to explore novel Mn4+ red phosphors for high-quality backlight display applications.

BACKGROUND: Numerous pieces of clinical evidence have shown that many phenotypic traits of human disease are related to their gut microbiome, i.e., inflammation, obesity, HIV, and diabetes. Through supervised classification, it is feasible to determine the human disease states by revealing the intestinal microbiota compositional information. However, the abundance matrix of microbiome data is so sparse, an interpretable deep model is crucial to further represent and mine the data for expansion, such as the deep forest model. What's more, overfitting can still exist in the original deep forest model when dealing with such "large p, small n" biology data. Feature reduction is considered to improve the ensemble forest model especially towards the disease identification in the human microbiota. METHODS: In this work, we propose the kernel principal components based cascade forest method, so-called KPCCF, to classify the disease states of patients by using taxonomic profiles of the microbiome at the family level. In detail, the kernel principal components analysis method is first used to reduce the original dimension of human microbiota datasets. Besides, the processed data is fed into the cascade forest to preliminarily discriminate against the disease state of the samples. RESULTS: The proposed KPCCF algorithm can represent the small-scale and high-dimension human microbiota datasets with the sparse feature matrix. Systematic comparison experiments demonstrate that our method consistently outperforms the state-of-the-art methods with the comparative study on 4 datasets. CONCLUSION: Despite sharing some common characteristics, a one-size-fits-all solution does not exist in any space. The traditional depth model has limitations in the biological application of the unbalanced scale between small samples and high dimensions. KPCCF distinguishes from the standard deep forest model for its excellent performance in the microbiota field. Additionally, compared to other dimensionality reduction methods, the kernel principal components analysis method is more suitable for microbiota datasets.

MOTIVATION: Modeling microbiome systems as complex networks are known as the problem of network inference. Microbial association network inference is of great significance in applications on clinical diagnosis, disease treatment, pathological analysis, etc. However, most current network inference methods focus on mining strong pairwise associations between microorganisms, which is defective in reflecting the comprehensive interactive patterns participated by multiple microorganisms. It is also possible that the microorganisms involved in the generated network are not dominant in the microbiome due to the mere focus on the strength of pairwise associations. Some scholars tried to mine comprehensive microbial associations by association rule mining methods, but the adopted algorithms are relatively basic and have severe limitations such as low calculation efficiency, lacking the ability of mining negative correlations and high redundancy in results, making it difficult to mine high-quality microbial association rules and accurately infer microbial association networks. RESULTS: We proposed a microbial association network inference method 'MANIEA' based on the improved Eclat algorithm for mining positive and negative microbial association rules. We also proposed a new method for transforming association rules into microbial association networks, which can effectively demonstrate the co-occurrence and causal correlations in association rules. An experiment was conducted on three authentic microbial abundance datasets to compare the 'MANIEA' with currently popular network inference methods, which demonstrated that the proposed 'MANIEA' show advantages in aspects of correlation forms, computation efficiency, adjustability and network characteristics. S100 calcium binding protein A16 (S100A16) is the most recent member of the S100 calcium-binding protein family. The function of S100A16 has been associated with various types of cancer; however, its role in colorectal cancer (CRC) remains unknown. Therefore, the aim of the present study was to investigate the role of S100A16 in CRC progression. The Oncomine dataset used in the current study revealed that the expression of S100A16 was decreased in CRC compared with normal colorectal tissues. Similar results were also determined via immunohistochemistry. In addition, a negative association was identified between S100A16 expression and the prognosis of patients with CRC. Further functional experiments revealed that S100A16 knockdown promoted the proliferation, migration and invasion of HCT116 and SW480 cells, and vice versa in Lovo cells. Epithelial-mesenchymal transition (EMT) was promoted and the JNK/p38 MAPK pathway was activated in HCT116 cells following S100A16 knockdown, as determined via western blotting. Furthermore, S100A16 silencing promoted the migration and invasion of cells. EMT was also reversed when cells were treated with the JNK inhibitor (SP600125) or the p38 inhibitor (SB203580). In summary, the results of the present study demonstrated that S100A16 suppressed the proliferation, migration and invasion of CRC cells partially via the JNK/p38 MAPK signalling pathway and subsequent EMT mediation.

A new isoquinoline alkaloid(1) has been isolated from the whole plant of Thalictrum glandulosissimum by using various chromatographic techniques, including silica gel, sephadex, MCI-gel resin, and RP-HPLC, and its structure was determined as 1-(6-hydroxy-7-methylisoquinolin-1-yl) ethantone by physicochemical properties and spectroscopic data. This compound was evaluated for anti-tobacco mosaic virus(TMV) activity. The results showed that it had prominent anti-TMV activity with inhibition rates of 28.4%. This rate was closed to that of positive control.

STUDY OBJECTIVES: To investigate the fiber-type distribution in palatopharyngeal muscle via adenosine triphosphatase and quantitative real-time polymerase chain reaction in children with severe obstructive sleep apnea (OSA). METHODS: Study participants were 12 children with severe OSA and 15 children with simple snoring as the control group. Both groups were diagnosed by polysomnography and treated with tonsillectomy. The samples of palatopharyngeus muscle were studied under adenosine triphosphatase staining and quantitative real-time polymerase chain reaction to classify the different fiber types. RESULTS: There were no differences in baseline age, body mass index, tonsil size, or sleep stage constitution between the 2 groups. Dominance (>60%) of type I fiber was observed both in children with simple snoring (3/15, 20%) and in those with severe OSA (1/12, 8.3%) via adenosine triphosphatase staining. Predominance of type II fibers was seen in 3/15 (20%) in the control group and 6/12 (50%) in the severe OSA group, respectively. Type grouping was also seen in 8/15 (53.3%) in non-OSA and 6/12 (50%) in severe OSA groups, respectively. There was no difference in distribution of subtype fibers assessed by quantitative real-time polymerase chain reaction between the 2 groups; the mean percentages of type I fibers were 25.8% ± 19.5% and 20.9% ± 16.6%, respectively (P > .05), similar to type IIa fibers (35.2% ± 23.4% and 40.9% ± 28.8%) (P > .05). There was a decrease in the percentage of type I fibers between children younger and older than 12 years (P < 0.05), although this was not due to OSA (P > 0.05). CONCLUSIONS: There were no specific changes via adenosine triphosphatase staining or a difference in distribution of subtype fibers via quantitative real-time polymerase chain reaction between children with severe pediatric OSA and those with simple snoring, whereas the percentage of type I fiber decreased dynamically due to age but not OSA. A new isobenzoisofuran(1) has been isolated from the whole plant of Cassia pumila using various chromatographic techniques, including silica gel, Sephadex, MCI-gel resin, and RP-HPLC, and its structure was determined as 9-(2-hydroxyethyl)-2,2-dimethyl-2H-furo[3,4-g]chromen-6(8H)-one. This compound was also evaluated for its antibacterial activity. The results showed that it had prominent antibacterial activity with MIC_(90) value of(45.2±4.2) µg·mL~(-1) for methicillin resistant Staphylococcus aureus(MRSA) strain. This value was closed to that of levofloxacin [with MIC_(90) value(48.5±4.3) µg·mL~(-1)].

A new flavone( 1) has been isolated from the whole plant of Cassia nomame by using various chromatographic techniques,including silica gel,Sephadex,MCI-gel resin,and RP-HPLC,and its structure was determined as 8,4'-dimethoxy-7-( 2-oxopropyl)-flavone based on spectroscopic data. The biological activity test showed that this compound displayed potent cytotoxicity against NB4,SH-SY5 Y,PC3,A549 and MCF-7 cell lines with IC50 values 2. 2,1. 8,3. 4,4. 5 and 1. 6 µmol·L-1,respectively.

A new isobenzofuranone derivative was isolated from Chaenomeles sinensis by using various chromatographic techniques,including silica gel,Sephadex LH-20,MCI-gel resin and RP-HPLC. This compound was determined as 2,2-dimethyl-5-( 2-oxopropyl)-2 H-furo[3,4-h]chromen-7( 9 H)-one( 1) by NMR,MS,IR and UV spectra,and was also evaluated for its antibacterial activity. The results showed that it showed prominent antibacterial activity with MIC90 value of( 53. 7±4. 5) mg·L-1 for methicillin resistant Staphylococcus aureus( MRSA) strain. This value is close to that of levofloxacin [with MIC90 value( 50. 2± 4. 2) mg·L-1].

This research was carried out to study the secondary metabolites of endophytic fungus Aspergillosis fumigatus from Euphorbia royleana. The endophytic fungus A. fumigatus was fermented by solid fermentation,and purified by various chromatographic methods after extraction. The structures of the compounds were identified by1 H-NMR,13 C-NMR and HSQC,HMBC spectra and physicchemical properties. Three compounds were isolated and their structures were identified as 3-( 3,4-dihydroxybenzoyl)-5-( 3,4-dihydroxyphenyl)-6-methyl-5,6-dihydro-2 H-pyran-2-one( 1),hydroxysydonic acid( 2) and 11-hydroxysydonic acid( 3). Compound 1 is a new compound.

RESUMO

A new benzylated alkamide, N-(3,4-dimethoxybenzyl)-9Z-oleamide (1), along with two known ones (2 and 3) were isolated from the roots of Lepidium meyenii collected from Lijiang, Yunnan Province of China. Their structures were elucidated by extensive spectroscopic analyses and the new compound further confirmed by a one-step synthesis. All the isolated alkamides were evaluated for their cytotoxicity against five human cancer cell lines. However, no significant activities were detected at concentrations up to 40 µM.

RESUMO

China formally established a system of national standardized medical residency training in 2014, which affects the health of its 1.4 billion people. Accompanying this system were new guidelines and standards for internal medicine residency training. However, the majority of the standards focused on process measurements, such as minimum case requirements of diseases and procedural skills, rather than describing broader physician competencies in the domains of professionalism, patient care, communication, teamwork, quality improvement, and scholarship. While China has taken a large step forward with standardization of certain aspects of internal medicine residency training, the next step should focus on outcome measures and creating a system that is competency-based.
