RESUMO

Familial Mediterranean fever (FMF) is an autosomal recessive autoinflammatory disease characterized by recurring serosal inflammation. Cardiac involvement in FMF commonly manifests as pericarditis and pericardial effusion; however, there is limited research on myocardial function. This study aimed to assess cardiac functions during active inflammation and remission periods of FMF patients and investigate the cardiac effects of inflammation during the attack period. Thirty-eight FMF patients without additional cardiac diseases were included in the study. Demographic characteristics, clinical symptoms, family history, and MEFV gene analysis results were obtained retrospectively. Blood tests, blood pressure measurements, electrocardiogram evaluations, conventional echocardiography, and speckle tracking echocardiography were performed during the attack and remission periods. Disease severity was assessed using the Pras scoring system. During the attack period, FMF patients exhibited significantly higher leukocyte count, neutrophil count, C-reactive protein, and erythrocyte sedimentation rate compared to the remission period (p < 0.005). Speckle tracking echocardiography revealed decreased function in the inferior segments of the left ventricle during the attack period (p < 0.005). Right ventricular function was more affected in the moderate disease group. FMF patients with lymphopenia during the attack demonstrated more impaired right ventricular function compared to those with normal lymphocyte count. Conclusions: FMF patients experience cardiac abnormalities during active inflammation, highlighting the importance of monitoring cardiac functions in these patients. Speckle tracking echocardiography can provide valuable insights into cardiac involvement in FMF. These findings emphasize the cardiac impact of FMF inflammation and the significance of long-term cardiac function monitoring in the management of FMF patients. What is Known: • The current literature lacks studies investigating myocardial function in the pediatric population during the attack period of this particular disease. • Our objective was to assess the alterations in cardiac function during the attack and remission periods, considering clinical manifestations, disease severity, acute phase reactant levels, and mutation type. We also evaluated the pattern of cardiac involvement and the affected cardiac areas by comparing remission and attack periods. What is New: • Several studies have demonstrated a rise in the prevalence of ischemic cardiac disease and mortality among individuals with FMF. • Investigating cardiac involvement during the attack period in FMF patients can provide valuable insights for the prevention of long-term complications.

OBJECTIVES: Diabetic kidney injury (DKI) is a serious microvascular complicationof type 2 diabetes mellitus (T2DM). Novel markers are being developedto make a timely diagnosis of this complication. Uric Acid to HDLratio (UHR) was reported to be associated with inflammatory andmetabolic diseases such as metabolic syndrome, type 2 diabetesmellitus, thyroiditis, and nonalcoholic hepatosteatosis, recently. Inthe present study, we aimed to show the diagnostic role of UHR indiabetic kidney injury (DKI), a condition characterized with chronicand low-grade inflammation. METHODS: We retrospectively analyzed data of 287 patients who already had T2D Min outpatient clinics of our institution. The study population was divided into two groups according to the presence of DKI. General characteristics and laboratory data, including UHR levels, of the diabetic subjects with DKI were compared to those of patients without DKI. RESULTS: The median UHR of the diabetic patients with DKI group (0,13 (0,06-0,33)) was significantly elevated compared to the UHR of diabetic patients without DKI (0,11 (0,04-0,34)) (p < 0.001). Median UHR was significantly and positively correlated with UACR (r=0.14, p=0.02), serum creatinine (r=0.18, p=0.002), and HbA1c (r=0.28, p<0.001) levels. There was also a significant inverse correlation between UHR and GFR (r=-0.19, p=0.001) values. Logistic regression analysis confirmed that a 0.1 point increase in UHR increases DKI odds by 2.3 times. UHR was also correlated with serum creatinine, GFR, and UACR, which are important signs for DKI. CONCLUSION: In conclusion, we think that UHR could be a diagnostic tool indiabetic kidney injury according to the preliminary results of the present study. UHR has an independent predictive role in DKI, and ithas significant correlation with other markers of kidney functions, therefore, we suggest routine evaluation of UHR in patients with DKIalong with other markers such as serum creatinine, GFR, and UACR.

Static and dynamic cues within certain spatiotemporal proximity are used to evoke respective global percepts of form and motion. The limiting factors in this process are, first, internal noise, which indexes local orientation/direction detection, and, second, sampling efficiency, which relates to the processing and the representation of global orientation/direction. These parameters are quantified using the equivalent noise (EN) paradigm. EN has been implemented with just two levels: high and low noise. However, when using this simplified version, one must assume the shape of the overall noise dependence, as the intermediate points are missing. Here, we investigated whether two distinct EN methods, the 8-point and the simplified 2-point version, reveal comparable parameter estimates. This was performed for three different types of stimuli: random dot kinematograms, and static and dynamic translational Glass patterns, to investigate how constant internal noise estimates are, and how sampling efficiency might vary over tasks. The results indicated substantial compatibility between estimates over a wide range of external noise levels sampled with eight data points, and a simplified version producing two highly informative data points. Our findings support the use of a simplified procedure to estimate essential form-motion integration parameters, paving the way for rapid and critical applications to populations that cannot tolerate protracted measurements.

Spatial cues presented prior to the presentation of a static stimulus usually improve its perception. However, previous research has also shown that transient exogenous cues to direct spatial attention to the location of a forthcoming stimulus can lead to reduced performance. In the present study, we investigated the effects of transient exogenous cues on the perception of briefly presented drifting Gabor patches. The spatial and temporal frequencies of the drifting Gabors were chosen to mainly engage the magnocellular pathway. We found better performance in the motion direction discrimination task when neutral cues were presented before the drifting target compared to a valid spatial cue. The behavioral results support the hypothesis that transient attention prolongs the internal response to the attended stimulus, thus reducing the temporal segregation of visual events. These results were complemented by applying a recently developed model for perceptual decisions to rule out a speed-accuracy trade-off and to further assess cueing effects on visual performance. In a model-based assessment, we found that valid cues initially enhanced processing but overall resulted in less efficient processing compared to neutral cues, possibly caused by reduced temporal segregation of visual events.

SARS-CoV-2 is still a major threat to the world. In this pandemic, transplantation activities have largely been affected worldwide. We are still facing with this pandemic; however, after regulations, we have started our transplantation activities. We report the first kidney transplantation whose recipient and living donor recovered from COVID-19. A 31-year-old male with renal failure was admitted for transplantation with an ABO-compatible relative. The recipient and the donor were tested for COVID-19 before transplantation, and they were both positive with a polymerase chain reaction. The recipient had minor symptoms and received therapy; the living donor also received therapy. Thirty days after recovery, surgery was performed successfully. The recipient was discharged with mycophenolate mofetil (MMF), tacrolimus, and steroid 15 days after surgery. In the follow-up, they were both negative for COVID-19 45 days after surgery. Although there is missing literature regarding safety concerns and short-term follow-up, living-donor transplantation may be considered for patients, who recovered from COVID-19, after careful selection with paying attention to precautions.
