RESUMO

Fibroepithelial polyps are a benign neoplasm of mesodermal origin that usually occur in the urinary tract. However, a giant fibroepithelial polyp of the glans penis is very rare and is strongly linked with long-term condom catheter use. In this article, we report a case of a 45-year-old man who presented with a giant fibroepithelial polyp originating from the glans penis. Physical examination showed a 6x3 cm sized mass lesion overlying the ventral surface of the glans near the urethral meatus without urethral communication. For the previous 10 years, the patient had been required to use a condom catheter secondary to paraplegia sustained during a T12-L1 spinal cord injury. He was successfully treated by wide local excision of the mass and suprapubic cystostomy placement. There was no evidence of recurrence after 12 months of follow-up.

RESUMO

PURPOSE: We investigated the relationship of prostate volume with metabolic and anthropometric parameters in men who visited a health promotion center. MATERIALS AND METHODS: From January 2004 to July 2007, among 16,236 men between 30 to 69 years old who visited our health promotion center for a general check-up, 1,033 men(6.4%) agreed to have their prostate evaluation included in this study. They underwent anthropometric measurements, basic laboratory tests, and transrectal ultrasonography. We evaluated the relationship of prostate volume with metabolic and anthropometric factors. RESULTS: In bivariate analysis, prostate volume positively correlated with height, weight, body mass index(BMI), body surface area(BSA), serum prostate-specific antigen(PSA), triglyceride, and blood pressure, and negatively correlated with high-density lipoprotein cholesterol(HDL-C). In multivariate analysis, age, BSA, serum PSA and HDL-C significantly correlated with prostate volume, whereas BMI did not(p=0.765). Prostate volume in patients with metabolic syndrome(28.0ml) was significantly larger than those without(25.4ml), but there was no difference in PSA (p=0.976). CONCLUSIONS: Because height positively correlated with prostate volume (p=0.010), BSA(calculated as height(0.725)xweight(0.425)x0.007184) but not BMI (as: weight/height2) correlated with prostate volume in multivariate analysis. Metabolic syndrome increased prostate volume.

PURPOSE: Alfuzosin XL (Sanofi-Aventis, UroXatral(R)) has to be taken just after a meal for the best efficacy due to its specific pharmacokinetics. We studied patient compliance and associated factors with regard to the correct instructions for taking alfuzosin XL. MATERIALS AND METHODS: Alfuzosin XL was prescribed to 62 patients from February to May 2008. At the first visit, we provided a prescription for alfuzosin XL with the instructions to "take one tablet just after dinner once a day". At the second visit, we evaluated patient compliance and the factors that influenced the patient compliance for taking the alfuzosin XL. The physician explained the instructions to the patients. At the third visit, we compared the compliance and associated factors with the results from the first visit. RESULTS: A total of 50 patients completed the study. After the first visit, twenty one patients (42.0%) were not taking the alfuzosin XL according to the prescription, that is, 20 patients took alfuzosin XL before going to bed, and one patient before meals. (p<0.05) After direct instructions by a physician, 49 patients (98.0%) took the alfuzosin XL correctly. (p<0.001) CONCLUSIONS: Taking alfuzosin XL as prescribed was accomplished in only 58% of patients. When alfuzosin XL is prescribed, the physicians should be aware of the importance of providing instructions directly to patients.

BACKGROUND: Bowen's disease is a form of intraepidermal (in situ) squamous cell carcinoma in histopathology, the representative premalignancy of the cutaneous squamous cell carcinoma and is usually persistent and progressive, with a small potential for invasive malignancy by 3-5%. OBJECTIVE: The purpose of this study was to investigate the clinical and histopathological characteristics of Bowen's disease. METHODS: A total of 45 patients with Bowen's disease were enrolled in this study. The hospital charts and histopathological slides were reviewed at Chungnam university hospital from 1983 to 2002. RESULTS: The male and female ratio was 1: 1.14. The mean age at diagnosis was 63 years. Twenty-two cases (58%) of solitary bowen's disease were located in non sun exposed area. Trunck, extremities, genital area followed in decreased order. Sixteen cases (42%) of solitary bowen's diseases occurred on the sun-exposed area. Face, dorsum of hand followed in decreased order. Thirty-three cases(73%) of single lesions and seven cases(27%) of multiple lesion. Analysis of clinical character of the Bowen's disease was performed and results were as follows: the most common type were patch(20 cases; 44%) and plaque (18 cases; 40%). And nodule, ulcer and papule type in decreased order. Analysis of histopathological character of the Bowen's disease was performed and results were as follows: the most common type were psoriasiform(15 cases; 41%) and irregular (12 cases; 32%). And verrucous, atrophic and combined type in decreased order. CONCLUSION: In Bowen's disease among Koreans, there were many distinctive clinical and histopathological features compared to those of non-Korean cases. In Korean cases, Bowen's diseases were located on sun exposed area only 42%, pigmented lesions were 20% and sex ratio 1: 1.14(male: female).

RESUMO

BACKGROUND: The cutaneous squamous cell carcinoma(SCC) is a malignant proliferation of the keratinocyte of the epidermis. It may have the potential to metastasize distally in contrast to the cutaneous basal cell carcinoma (BCC). Our lives were being lengthened gradually and our society was being polluted by many causative factors. Because many kinds of cutaneous malignant tumors occurred to the aged and resulted from the severe pollution, a lot of people have been interested in SCC. OBJECTIVE: In this study we investigated the recent trend of the cutaneous squamous cell carcinoma in clinical and histopathological aspects. METHODS: Seventy one cases of SCC who had visited the Department of Dermatology, Chungnam University Hospital during the last 17 years(between 1983 and 1999) were confirmed in histopatho-logical review. The observation of occurrence rate in years, age and sex distribution, onset, site, cause, histopathological malignancy and treatment was done, retrospectively. RESULTS: 1. The occurrence of the cutaneous squamous cell carcinoma in males was one and half times more than in females(M>F :1.63x). Age distribution showed almost above 60 years old(about 74%). 2. The common sites (in decreasing order) were face(56%), extremities(upper>lower), other sites and most etiologic factors were idiopathic(71%), in 2 cases metastatic. 3. Histopathologically, the subtype of SCC was verrucous(7%), acantholytic(10%), spindle cell(6%), and the degree of differentiation was well-, moderate-, poor-differentiated (respectively 25%, 32%, 20%), and SCC invaded to full dermis(84%) and subcutis(9%). 4. Various treatments were done and among them the simple excision(53%) was common. 5. One case recurred and four cases metastasized to adjacent lymph node. One case expired. CONCLUSION: We investigated the recent trend of the cutaneous squamous cell carcinoma through our cases of SCC who had visited the department of dermatology, chungnam university hospital.

RESUMO

We report a case of multiple piloleiomyoma in a 50-year-old man. About 30 years ago, multiple red colored firm papules with pain developed on the left lower leg. The skin lesions gradually increased in number and size arranging in a group. Some lesion became more painful after cold exposure. About a few years ago, new lesions developed on the left back and flank abdomen. The histologic findings showed poorly demarcated and were composed of interlacing bundles of smooth muscle fibers with varying amounts of collagen bundles being intermingled. Masson's trichrome and smooth muscle actin stain showed typical leiomyoma, and s-100 stains revealed an increased amount of nerve fibers interlacing within muscle fibers and the surrounding tissue. We diagnosed our case as unilateral multiple piloleiomyoma arising from arrector pilorum muscle. Pain was ameliorated with oral nifedipine alone.

BACKGROUND: Clinical features of malignant melanoma (MM) are known to be different according to racial and environmental factors. Recently, there have been reports of an increase of MM. OBJECTIVE: We evaluated the incidence of tumors, age and sex distribution, tumor sites, and histopathological patterns. METHODS: Clinical records were reviewed for 35 cases of MM with a histologically confirmed diagnosis for patients who visited the Department of Dermatology, Chungnam National University Hospital during the 20 years from 1980 to 1999. Histopathologic and clinical characteristics were also reviewed. RESULTS: A total of 35 malignant melanoma cases diagnosed at Chungnam National University Hospital during the past 20 years from 1980 to 1999 were included for the clinico-histopathologic study. The ratio of man to woman with MM was 1 : 1.33 with a mean age of 55 years. Acral lentigenous melanoma was the most common clinico-histopathologic pattern. CONCLUSION: The incidence of MM has increased during the past 20 years. The common site, and histopathologic patterns show a significant difference from Caucasians.

RESUMO

The staphylococcal scalded skin syndrome encompasses a spectrum of blistering skin disease that ranges in severity from localized bullous impetigo to a generalized syndrome with cutaneous tenderness, widespread blistering, and superficial denudation or desquamation. This disorder is induced by the exfoliative toxin of Stapylococcus aureus. The intermediate form of staphylococcal scalded skin syndrome may be seen that begin as localized bullous impetigo but evolve to produce regionally limited bullae and denuded areas that may or may not harbor causative microorganism. We report six cases of the intermediate form of staphylococcal scalded skin syndrome in Daejeon for a short time. All cases occurred in children under 5 years of age and showed localized bullae and erythroderma with positive Nikolsky's sign.
