Introduction: With the acceleration of urbanization, public health issues have become increasingly prominent in smart city construction, especially in the face of sudden public health crises. A deep research method for public health management based on a 4M perspective (human, machine, materials, methods) is proposed to effectively address these challenges. Methods: The method involves studying the impact of human factors such as population age, gender, and occupation on public health from a human perspective. It incorporates a machine perspective by constructing a public health prediction model using deep neural networks. Additionally, it analyzes resource allocation and process optimization in public health management from the materials and methods perspectives. Results: The experiments demonstrate that the public health prediction model based on deep neural networks achieved a prediction accuracy of 98.6% and a recall rate of 97.5% on the test dataset. In terms of resource allocation and process optimization, reasonable adjustments and optimizations increased the coverage of public health services by 20% and decreased the response time to public health events by 30%. Discussion: This research method has significant benefits for addressing the challenges of public health in smart cities. It can improve the efficiency and effectiveness of public health services, helping smart cities respond more quickly and accurately to potential large-scale public health events in the future. This approach holds important theoretical and practical significance.

BACKGROUND AND AIMS: Low-grade squamous intraepithelial lesion (LSIL) is one of two categories of cervical intraepithelial lesions. Given that controversy exists regarding its management, this comparative study aimed to evaluate the effect of 5-aminolevulinic acid photodynamic therapy (ALA-PDT) in treating LSIL of the high-risk human papillomavirus (HR-HPV)-infected cervix. METHODS: A total of 218 patients (25-45 years old) with cervical LSIL associated with HR-HPV who underwent ALA-PDT, loop electrosurgical excision procedure (LEEP), or observation only were included. The clearance rates of cervical LSIL and HR-HPV between the ALA-PDT, LEEP, and observation groups were compared at 6 and 12 months follow-up. Adverse reactions were also compared. The factors affecting the clearance on ALA-PDT of cervical LSIL were evaluated. RESULTS: There were no statistically significant differences in lesion and HR-HPV clearance rates between the ALA-PDT and LEEP groups at 6 and 12 months. However, the lesion and HR-HPV clearance rates were significantly higher in the ALA-PDT group than that in the observation group. The adverse reaction rate was significantly lower in the ALA-PDT group than in the LEEP group. CONCLUSION: For patients with cervical LSIL, the lesion and HR-HPV clearance rates after ALA-PDT were close to those after LEEP and significantly higher than in the observation group. Moreover, the adverse reaction rate for ALA-PDT was much lower than that for LEEP. Therefore, ALA-PDT provides a new option for the minimally invasive treatment of cervical LSIL.

Depression is a complex and prevalent mental illness. Cherry leaf is a traditional Chinese herbal medicine, which has confirmed to exert a certain antidepressant effect, but its potential neural regulation mechanism is not clear. This paper aims to investigate the improved action of cherry leaf decoction (CLD) on chronic unpredictable mild stress (CUMS) rats and its potential neural regulation mechanism by verifying the role and function of NMDAR regulatory target α2δ-1 in depression due to CUMS. Male SD rats were subjected to random stressors persisting for 5 weeks to establish the CUMS depression rat model. CLD could effectively alleviate depression-like behaviors of CUMS rats in behavioral tests including sucrose preference test, forced swimming test, tail suspension test and open field test. After the administration of the CLD, the expression of corticotropic-releasing hormone (CRH) in the hypothalamus was inhibited. Moreover, the levels of CRH, adrenal cortical hormone (ACTH) and corticosterone (CORT) in serum also decreased significantly. CUMS upregulated the expressions of α2δ-1, N-methyl-d-aspartate receptor 1 (NR1), NR2A and NR2B, and enhanced the binding ability to of α2δ-1 and NR1, which were reversed by CLD. The results demonstrated that CLD could ameliorate depression-like behaviors due to CUMS, which was related to the fact that CLD down-regulated α2δ-1 level and interfered with α2δ-1 binding to NR1, thereby reducing NMDAR expression and ultimately inhibiting HPA axis activity.

Background: Ventilator-associated pneumonia (VAP) is among the most important hospital-acquired infections in an intensive-care unit setting. However, clinical practice lacks effective theoretical tools for preventing VAP in the elderly. Aim: To describe the independent factors associated with VAP in elderly intensive-care unit (ICU) patients on mechanical ventilation (MV) and to construct a risk prediction model. Methods: A total of 1851 elderly patients with MV in ICUs from January 2015 to September 2019 were selected from 12 tertiary hospitals. Study subjects were divided into a model group (n = 1219) and a validation group (n = 632). Two groups of patients were divided into a VAP group and a non-VAP group and compared. Univariate and logistic regression analyses were used to explore influencing factors for VAP in elderly ICU patients with MV, establish a risk prediction model, and draw a nomogram. We used the area under the receiver operating characteristic curve (AUROC) and the Hosmer-Lemeshow goodness-of-fit test to evaluate the predictive effect of the model. Findings regarding the length of ICU stay, surgery, C-reactive protein (CRP), and the number of reintubations were independent risk factors for VAP in elderly ICU patients with MV. Predictive-model verification results showed that the area under the curve (AUC) of VAP risk after MV in the modeling and verification groups was 0.859 and 0.813 (P < 0.001), respectively, while P values for the Hosmer-Lemeshow test in these two groups were 0.365 and 0.485, respectively. Conclusion: The model could effectively predict the occurrence of VAP in elderly patients with MV in ICUs. This study is a retrospective study, so it has not been registered as a clinical study.

Chronic obstructive pulmonary disease (COPD) is a chronic airway inflammatory disease characterized by incomplete reversible airflow limitation. The diagnosis of COPD is mainly based on pulmonary function examination. In recent years, it has been indicated that small airway dysfunction occurs in patients with all stages of COPD, even in high-risk smoking groups who have not yet met the diagnostic criteria for COPD. Early recognition of small airway dysfunction and early initiation of small airway targeted therapy have become foci of research. In the present review, the methods of evaluating small airway function were summarized and their merits and shortcomings were discussed. Furthermore, the potential of targeted treatment of small airways in patients with COPD was outlined.

Two-dimensional (2D) transition metal chalcogenides (TMCs) become more attractive upon addition of a third element owing to their unique structure and remarkable physical and chemical properties, which endow these materials with considerable potential for applications in nanoscale devices. In this work, a RexNb(1-x)S2-based saturable absorber (SA) device for ultrafast photonics applications is studied. The device is assembled by placing RexNb(1-x)S2 nanosheets with a thickness of 1-3 nm onto a microfiber to increase their compatibility with an all-fiber laser cavity. The prepared RexNb(1-x)S2-based device exhibits a modulation depth of 24.3%, a saturation intensity of 10.1 MW/cm2, and a nonsaturable loss of 28.5%. Furthermore, the RexNb(1-x)S2-based device is used to generate ultrashort pulses in an erbium-doped fiber (EDF) laser cavity. At a pump power of 260 mW, the EDF laser operates in a conventional soliton mode-locked region. The pulse width is 285 fs, and the repetition frequency is 61.993 MHz. In particular, the bound-state soliton mode-locking operation is successfully obtained in a pump power range of 300-900 mW. The bound-state pulses are formed by doubling identical solitons with a temporal interval of 0.8 ps. The output power is as high as 47.9 mW, and the repetition frequency is 123.61 MHz. These results indicate that the proposed RexNb(1-x)S2-based SAs have comparable properties to currently used 2D SAs and provide a basis for their application in the field of ultrafast photonics.

OBJECTIVE: It aimed to analyze the epidemic situation of new coronary pneumonia (COVID-19) based on the epidemiological Markov model, and to study the clinical risk factors of the patients based on the patient's cardinal data and clinical symptoms. METHODS: A total of 500 patients with COVID-19 diagnosed by nucleic acid testing in the X hospital from January 2020 to May 2020 were collected. According to the severity of the disease, they were classified into general group (200 cases) and acute critical group (300 cases). Markov model to predict the number of COVID-19 infections was constructed. Patient's general information, clinical characteristics, and prevention methods were analyzed. RESULTS: According to Markov model statistics, the developmental expected stay time of patients infected with COVID-19 was 14 days. 2. The two groups of patients had statistically considerable differences in complications such as gender, age, hypertension, coronary heart disease, shortness of breath, myocardial damage, and thrombocytopenia (P < 0.05). 3. Logistic multivariate regression analysis showed that the clinical risk factors for patients with COVID-19 mainly included the patient's gender, age, whether they were associated with hypertension, coronary heart disease, shortness of breath, myocardial damage, and thrombocytopenia. CONCLUSION: Markov model can be utilized to judge the time course of the COVID-19 in various development states. In addition, the COVID-19 spread rapidly and is extremely harmful. Clinically, through active prevention, the treatment effect can be improved, the patient's respiratory function, and the quality of life can also be improved.

Total suspended matters (TSMs), as the sediment precursor, directly affect the mass exchange and sedimentation in the East China Sea (ECS). Ultrafine suspended matters (USMs) are an important component of the TSMs, and may play a significant role in regulating pollutant transfer and shaping biological communities. However, the conventional filtration may cause the loss of USMs because the filter membranes with the pore size of 0.45 µm were adopted to collect TSMs; and consequently, no data on USMs are currently available in continental shelves. In this study, the TSMs and USMs in the wintertime in the ECS were collected by using the filter membranes with the pore size of 0.22 µm for investigating their compositions, distributions and exchanges for the first time. The results show that the TSMs consisted of mineral particles (35-80%), biological fragments (10-50%), and flocs (10-40%); and mainly accumulated along the coastal belt and in southwest of the Cheju Island. Comparatively, the USMs were composed of fine biological fragments (10-70%), mineral particles (15-70%), and unrecognizable particles with various shapes (15-35%). They exhibited a clear heterogeneous distribution, namely, accumulated along the coastal belt and outer shelf, but dispersed in the mid-shelf, implying that USMs might be jointly controlled by biological activities, terrestrial inputs and hydrodynamic system in the ECS and the Yellow Sea. The distinct distribution difference between TSMs and USMs denotes their different exchange styles, i.e., for TSMs active in north of the ECS, and weak along the coastal front zone and 100 m isobath; while for USMs almost inactive along the coastal front zone, and active in the outer shelf. Our results may provide a novel clue for evaluating the contribution of TSMs to sedimentation, pollutant transfer and maintenance of marine biological communities with emphasis on the new method for collecting TSMs and USMs in the ECS.

WD40 proteins belong to a big protein family with members identified in every eukaryotic proteome. However, WD40 proteins were only reported in a few prokaryotic proteomes. Using WDSP ( http://wu.scbb.pkusz.edu.cn/wdsp/ ), a prediction tool, we identified thousands of prokaryotic WD40 proteins, among which few proteins have been biochemically characterized. As shown in our previous bioinformatics study, a large proportion of prokaryotic WD40 proteins have higher intramolecular sequence identity among repeats and more hydrogen networks, which may indicate better stability than eukaryotic WD40s. Here we report our biophysical and structural study on the WD40 domain of PkwA from Thermomonospora curvata (referred as tPkwA-C). We demonstrated that the stability of thermophilic tPkwA-C correlated to ionic strength and tPkwA-C exhibited fully reversible unfolding under different denaturing conditions. Therefore, the folding kinetics was also studied through stopped-flow circular dichroism spectra. The crystal structure of tPkwA-C was further resolved and shed light on the key factors that stabilize its beta-propeller structure. Like other WD40 proteins, DHSW tetrad has a significant impact on the stability of tPkwA-C. Considering its unique features, we proposed that tPkwA-C should be a great structural template for protein engineering to study key residues involved in protein-protein interaction of a WD40 protein.

Clara cell protein (CC16) is an anti-inflammatory protein, which is expressed in the airway epithelium. It is involved in the development of airway inflammatory diseases, including chronic obstructive pulmonary disease and asthma. However, the exact molecular mechanism underlying its anti­inflammatory action remains to be fully elucidated. The aim of the present study was to define the protein profiles of the anti­inflammatory effect of CC16 in lipopolysaccharide (LPS)­treated rat tracheal epithelial (RTE) cells using shotgun proteomics. Protein extracts were obtained from control RTE cells, RTE cells treated with LPS and RTE cells treated with LPS and recombinant CC16 (rCC16). Subsequent label­free quantification and bioinformatics analyses identified 12 proteins that were differentially expressed in the three treatment groups as a cluster of five distinct groups according to their molecular functions. Five of the twelve proteins were revealed to be associated with the cytoskeleton: Matrix metalloproteinase­9, myosin heavy chain 10, actin­related protein­3 homolog, elongation factor 1­α­1 (EF­1­α­1), and acidic ribosomal phosphoprotein P0. Five of the twelve proteins were associated with cellular proliferation: DNA­dependent protein kinase catalytic subunit, EF­1­α­1, tyrosine 3­monooxygenase, caspase recruitment domain (CARD) protein 12 and adenosylhomocysteinase (SAHH) 3. Three proteins were associated with gene regulation: EF­1­α­1, SAHH 3 and acidic ribosomal phosphoprotein P0. Three proteins were associated with inflammation: Tyrosine 3­monooxygenase, CARD protein 12 and statin­related protein. ATPase (H+­transporting, V1 subunit A, isoform 1) was revealed to be associated with energy metabolism, and uridine diphosphate glycosyltransferase 1 family polypeptide A8 with drug metabolism and detoxification. The identified proteins were further validated using reverse transcription­quantitative polymerase chain reaction. These protein profiles, and their interacting protein network, may facilitate the elucidation of the molecular mechanisms underlying the anti­inflammatory effects of CC16.

OBJECTIVE: To compare the positive rates and complications of ultrasound-guided transrectal and transperineal prostate biopsies. METHODS: We retrospectively analyzed 156 cases of ultrasound-guided transrectal (n = 97) and transperineal (n = 59) prostate biopsy, and compared the positive rate and post-biopsy complications between the two approaches. RESULTS: The positive rates in the transrectal and transperineal groups were 48.4% and 44.1%, respectively, with no significant difference between the two approaches according to different PSA levels (P >0.05). No statistically significant differences were observed between the transrectal and transperineal groups in the post-biopsy incidence rates of such complications as hematuria (54.6% vs 42.4%, P >0.05), lower urinary tract symptoms (17.5% vs 22.0%, P >0.05), dysuria (9.3% vs 6.8%, P >0.05), and acute urinary retention (7.2% vs 6.8%, P >0.05). However, the incidence rates of post-biopsy infection and rectal bleeding were remarkably higher (15.5% vs 3.4%, P<0.05 and 50.5% vs 3.4%, P >0.01) while that of perineal swelling markedly lower in the former than in the latter (3.1% vs 13.6%, P <0.05). CONCLUSION: Transrectal and transperineal biopsies are both effective for the diagnosis of prostate cancer. Since their complications vary, the choice between the two methods depends on the specific condition of the patient.
