RESUMO

Medicine is the subject of studying human and its inherent humanistic attribute endows medicine with the temperature it deserves. However, with the continuous improvement of medical testing technology and treatment technology, medical workers pay more attention to diseases and ignore humanistic care, which has become an important factor in the aggravation of patients’ burden and the tension relationship between doctors and patients. There are many virtues in Chinese traditional culture. Sun Simiao’s medical ethics thought of "great doctor with professionalism and sincerity" embodies the core value of the humanistic spirit of Chinese traditional medicine. In his medical ethics thought, the moral and ethics of "great doctor with professionalism and sincerity", the medical practice attitude of "being cautious and diligent", the values of "benevolence for the world" and the professional conduct of "honesty and truth" still have strong practical significance at present. It is the good material to cultivate the humanistic spirit of medical students. This is of great value to integrate Sun Simiao’s medical ethics thought into cultivate medical students with both "professionalism" and "sincerity", practice the original intention of medical and health undertakings, and carry forward cultural self-confidence.

RESUMO

OBJECTIVES: The purpose of this study was to explore the effects of inhalational anaesthetics on the male reproductive system in neonatal rats. METHODS: Postnatal day 6 (P6) male rat pups were assigned to 2 groups of 40 rats each. The control group did not receive anaesthesia, and the test group was exposed to air containing 3% sevoflurane in a chamber. After sevoflurane exposure, the effects on the male reproductive system in neonatal rats exposed to inhalational anaesthetics were tested. The rats were maintained to P60. The fertility, sperm and spermatid count, sperm motility, organ weights and histological changes in rats were determined. RESULTS: Compared with those of the control group, the sperm count and motility of the sevoflurane group were significantly decreased (p < 0.05), and there was also a decrease in the number of foetal rats from the sevoflurane group (p < 0.05). Furthermore, the arrangement of the seminiferous tubule was regular in the control group, whereas the arrangement of the seminiferous tubule was distorted and the spermatocytes were detached and irregularly lined in the sevoflurane group. TUNEL analysis showed that the number of apoptotic cells in the testes of the sevoflurane group was significantly increased (p < 0.05). SOD and MDA analyses showed that SOD was decreased, while MDA expression was increased. CONCLUSIONS: These data indicated that young rats suffered as a result of inhalation of the anaesthetic sevoflurane and had reduced reproductive system function during adolescence in males. These studies are useful as a foundation for clinical studies.

RESUMO

Objective:To study the predictive value of the amplitude-integrated electroencephalography (aEEG) within 6 hours and 3 days after birth and magnetic resonance imaging(MRI) on the adverse neurobehavioral development of asphyxiated preterm infants at the correction age of 6 months. Methods:From December 2017 to June 2019, 50 asphyxiated preterm infants who were delivered at the obstetrical department transferred to the division of neonatology in the Third Affiliated Hospital of Anhui Medical University were monitored by aEEG within 6 hours after birth, then once a day for at least 4 h. MRI was administered at 40 weeks of corrected age, neuromotor developmental function of the infants was assessed by the Geisel developmental diagnostic scale at 6 months of corrected age, then the infants were divided into good prognosis group and poor prognosis group according to the assessment results. SPSS 19.0 software was used for statistical analysis. The software of SPSS 19.0 was used to analyze the data.Independent sample t-test and χ 2 test were used to analyze the difference between the two groups. The relationship between aEEG grading and MRI, and their predictive value for adverse neurobehavioral development were analyzed at 6 months of corrected age. Results:The degree of white matter damage( H=24.896) and intracranical hemorrhage( H=29.245) of premature infants with different aEEG clinical grades were different (both P<0.01) on MRI. The sensitivity of aEEG within 6 hours and 3 days after birth on predicting poor prognosis was 96.2% and 97.8%, the specificity was 56.2% and 62.5%, the negative predictive value was 98.2% and 99.0%, the positive predictive value was 37.8% and 52.3%, the correct index was 52.4% and 60.3%, respectively. The aEEG was combined with MRI, the sensitivity (90.0%, 97.0%), specificity (89.0%, 99.0%), negative predictive value (99.2%, 99.5%), positive predictive value (80.6%, 88.5%), and correct index (79%, 96%) were all improved. Conclusion:The combination of aEEG grading and MRI can improve the prognostic value on neurodevelopmental prognosis, and provide a better evaluation basis for clinical follow-up and intervention of asphyxiated premature infants with brain injury.

RESUMO

Cubic spline function was used in a genetic evaluation to model the change of yolk proportion over the lay life. A total of 19,862 yolk proportion records of 2,324 hens was used. The evaluated submodels consisted of 3 to 6 knot models. The same knots were fitted for genetic and permanent environmental splines. The residual effects were specified to be independently and normally distributed, but with heterogeneous variance for each test week. (Co)variance components were estimated by the average information restricted maximum likelihood (AIREML) method. The best fitting random regression model (RRM) was a submodel with 4 knots at 32, 36, 52, and 72 wk of age for genetic and permanent environmental effects. The estimate of genetic variance was larger than that of permanent environmental variance at the same time point. The heritability of yolk proportion ranged from 0.32 to 0.55, and the repeatability ranged between 0.45 and 0.73. The genetic correlations between test wk were from moderate to unity. To the best of our knowledge, this was the first report on the use of a RRM to evaluate yolk proportion. The results of this study showed that random regression models with the spline function could be used for improvement of yolk proportion.

RESUMO

Tu-Jia medicine is an important part of traditional Chinese medicine (TCM).“San-Yuan doctrine”is the guiding ideology of Tu-Jia medicine, but there is no systematic exposition of its literature origin. This paper ar-gued that“San-Yuan doctrine”of Tu-Jia medicine was originated in the“San-Yuan”of Taoism. Its formation was closely related to the impact of Taoism and Tu-Jia culture, the understanding of human physiology and pathology in Tu-Jia medicine. This paper expounded the origin of“San-Yuan doctrine”, the guiding ideology of Tu-Jia medi-cine and Taoism culture through citing a number of theoretical bases. First of all, the concept of“San-Yuan”in Tu-Jia medicine originated in Taoism. Secondly, legends about the Lord Lao-Zi and Medicine King Bodhisattva in Tu-Jia medicine were closely related with Taoism. Moreover, there existed important links between Tu-Jia medicine and Taoist medicine. For example, both of them are aware of the importance of brain. The reason, I think, is firstly due to the penetration of Taoism in Tu-Jia culture. Taoism culture has a profound influence on Tu-Jia culture. Sec-ondly, the understanding of human physiology and disease in Tu-Jia medicine is closely related to“three”. There-fore, Tu-Jia medicine took“San-Yuan doctrine”as its own guiding ideology. It is of great significance to the study of ethnic minority medicine culture and inheriting its academic thoughts by the study of Tu-Jia medicine origin.

RESUMO

BACKGROUND: Vasopressin is widely used to treat various type of hypotension, but the effect of vasopressin on coronary artery bypass grafting surgery (CABG) patients is not clear. This study was to investigate the effect of vasopressin on the hemodynamics in CABG patients. METHODS: Twenty coronary artery disease (CAD) patients were randomly divided into two groups: norepinephrine group and vasopressin group. During the anesthesia and the operation, the central venous pressure (CVP) and pulmonary capillary wedge pressure (PCWP) were maintained to 8-10cmH2O, and the hemocrit was maintained above 30% through lactate ringer's mixture, artifact colloid and red blood cells. The invasive artery blood pressure (IBP) was maintained by appropriate anesthetic depth and norepinephrine or vasopressin respectively. The target IBP was 70 mmHg, and heart rate (HR) was 60 bpm. The MAP (mean artery pressure), HR, ST-T, CVP, PAP (pulmonary artery pressure), PCWP, SVR (systemic vascular resistance), PVR (pulmonary vascular resistance), CO (cardiac output), urine output, blood gas analysis, surgery duration and blood loss were monitored. RESULTS: The MAP, HR, and ST-T were stable in either group during the operation. CVP, PCWP and SVR increased but CI deceased during the posterior descending artery (PDA) was grafted in both groups and without any significant difference between them. PAP increased during PDA was grafted in either group and there was significant difference between the two groups. PVR increased during ADA and PDA being grafted in norepinephrine group but not in vasopressin group. Metoprolol usage was 11.2 mg and 5.9 mg in norepinephrine group and vasopressin group respectively. CONCLUSION: Vasopressin was better than norepinephrine.to keep the hemodynamics stability of patients undergoing CABG surgery.
