The stimulating effect of curiozin gel and plaster application on the engraftment of skin autografts from various areas of body was studied in rats. Curiozin gel in a single dose of 75 mg was used for the daily treatment of autografts over the entire postoperation period of time. The adhesive plaster was applied onto skin autografts for 5 days after skin transplantation. The skin autografts in the test group exhibited normalized volume blood flow dynamics, reduced duration of the inflammatory reaction, and improved the quality of engraftment as compared to the untreated control group. The best healing and blood-flow-normalizing effect was achieved by combined application of curiozin gel and adhesive plaster.

The authors have made an analysis of 325 esophagoplasties at the period from 1950 till 1988. In 65 patients 87 reconstructive operations were fulfilled, 19 most difficult patients being transferred from surgical hospitals after numerous surgical interventions to the Savinykh Clinic of the Medical Institute. The authors consider that patients with diseases of the esophagus should be directed to special medical institutions and reconstructive operations must be performed by most experienced surgeons in such centers.

The paper presents the results of the examination and treatment of 59 and 44 patients subjected to gastric resection for gastroduodenal ulcer early and late after the surgery, respectively. Special courses of aftertreatment are suggested. Electrophoresis of Karachi [correction of Karachinskaya] mud-water solution was explored as a component of the treatment.

The observations on 54 patients with a gastric cancer recurrence and 121 patients with the gastric cancer recurrence passing to the esophagus are presented. The time of the onset, clinical manifestations, histologic structure, location of recurrences were analysed. The results of surgical, combined and conservative (with the use of local SHF-hyperthermia and artificial hyperglycemia) treatment are presented. The effectiveness of the therapy performed is shown.

Results of the treatment of 70 patients with chemical burns of the stomach and esophagus were studied. Isolated burns of the stomach were diagnosed in 33 patients (47%), associated burns of the stomach and esophagus--in 37 patients (53%). The operation of choice for isolated burns of the stomach may be Billroth-2 resection, while for complete cicatrization of the stomach it was gastrectomy. In patients with associated injuries of the stomach and esophagus the curative methods should be chosen individually. In most cases due to pronounced emaciation they are to be divided into 2 steps. At the first step the conditions for nutrition of the patients are created (gastrostoma, enterostoma). At the second step--reconstructive operations are performed (plasty of the esophagus, gastroenteroanastomosis etc.).

The development of the method of diaphragmocrurotomy widely used in many surgical clinics of the world was considered on the basis of an analysis of case histories and results of autopsies of patients with diseases of the cardial part of the stomach and esophagus. The article proposes practical recommendations for the fulfillment of this operation.

The structure of the gastroduodenal passage from the surgical viewpoint was studied in 50 preparations taken from corpses of people from neonatal age to 80 years. There is a well pronounced layer of the connective tissue on the border between the stomach and duodenum which may be of significance for pathogenesis of traumatic abruption of the duodenum from the stomach. The pyloric sphincter is characterized by asymmetry of the wall thickness and oblique disposition. The pylorus canal lumen after Heineke-Mikulicz pyloroplasty became about 7.7 times larger, that after Jadd-Tanaka was 2.6 times larger.

An analysis of 158 completed esophagoplastics has shown that necroses of the transplant made their appearance in 7.8% of cases, fistulas of anastomoses--in 38%. The basis of pathogenesis of these complications is microthromboses of venules and small veins of the intramural blood circulation of the mobilized small intestine transplant (syndrome of local hypercoagulation). The novocain-heparin blockade and postoperative antiaggregation infusion therapy is an effective method for prevention of intestinal necroses in the postoperative period.

Free aminoacids were studied in the serum of portal and peripheral blood of 54 patients with complicated ulcer disease of the stomach and duodenum. It was established that a disbalance of FFA was more pronounced in the portal blood serum after resection of the stomach in the early postoperative period. The changes revealed should be taken into consideration in the period of preoperative preparation and rehabilitation.

The remote results following the small intestine esophagoplasty in 110 out of 211 patients subjected to the operation are described. The artificial esophagus diseases were found in 24%. Methods for the treatment and prevention of the disease of the artificial esophagus are proposed.

A classification of diseases of the artificial esophagus is made on the basis of the observation of 319 patients with the artificial esophagus. Causes of the disease are analysed in 75 patients. Methods of diagnostics, treatment and prevention of the disease are recommended.
