[Is cobalt-binding capacity of serum a new perspective diagnostic marker?].

The aim of this study was to determine the reference interval of serum cobalt-binding capacity (CoBC) and to estimate the effect of factors unrelated to oxidative modification of serum albumin on this diagnostic marker. A group of healthy volunteers (n=194), a group of patients with autoimmune diseases (n=44) and a group of patients with diabetes type 2 (n=50) participated in this study. The regional reference interval was found to be 0,462-0,744 mmol Co 2+ /l of serum. The study of serum CoBC in two groups of patients showed that the CoBC level strongly depends on the serum protein profile. Therefore, this diagnostic test may be used for diagnosing ischemia, but other pathologies associated with changes in a ratio of blood protein fractions can also influence the serum CoBC level.

[The algorithms of evaluation of economic effectiveness of laboratory technologies].

The article considers several practical situations requiring estimation of economic effectiveness. The protocols of estimation, costs and effectiveness accounting requirements of practical public health are proposed. The necessity of development of correct epidemiological model is demonstrated to estimate profit of innovations of diagnostic stage. The estimation "costs-effectiveness" is made for troponins at diagnostic of cardiac infarction and prostate-specific antigen under screening of prostate cancer.

[The population study of total antioxidant activity of human blood serum and urine].

Many diseases are accompanied with the development of oxidation stress. The analysis of total antioxidant activity is one of the techniques to assess the oxidation-reduction balance. In spite of availability of normative documents recommending to apply this examination the antioxidant activity analysis technique is used in practice not very often. The absence of reference values of this indicator is one of the reasons of this situation. The article presents the results of the study of total antioxidant activity of human blood serum and single portion of urine. On the basis of the results the reference limits were estimated. The correlation analysis of interrelationship between antioxidant activity analysis technique assessment and age, gender and content of substances mostly inputting into analysis results.

[Determination of troponin in the diagnosis of acute myocardial infarction in practice: clinico-economic analysis].

The authors first made a clinicoeconomic analysis of benefits from the introduction a rapid test for cardiac troponins in Russian routine clinical practice for the diagnosis of acute myocardial infarction. The economic gains associated with the reduced number of both optional hospitalizations and unwarranted refusals of hospitalization were calculated. The probable economic benefit of the improved diagnosis of myocardial infarction after total introduction of the test for troponins was 324.86 rbl. per patient. The importance of a clinicoeconomic analysis is shown to solve the problem of introducing the diagnostic tests into medical practice.

[Influence of tryptophan content in proteins on the quantitative determination of this proteins using photochemical p-dimethylaminobenzaldehyde].

The interaction of proteins containing various number of Trp residues with p-dimethylaminobenzaldehyde (pDABA) was studied. A positive correlation between tryptophan content in proteins and absorbance of protein-pDABA reaction products was found (Spearman's correlation coefficient R = 0.936; p < 0.001). Using the observed correlation we were able to predict a possible range for the absorbance of reaction products for each serum globulin and estimate the contribution of different globulins in the pDABA serum assay. We propose that the described method can be used for the determination of the globulin content in blood serum.

Enantioconvergent synthesis of (-)-(S)- and (+)-(R)-2-acetyl-3,6-dihydroxycyclohex-2-enone starting from rac-6-hydroxy-3-methoxycyclohex-2-enone.

The synthesis of enantiomerically pure (-)-(S)- and (+)-(R)-2-acyl-3,6-dihydroxycyclohex-2-enone starting from diastereomerically pure N-tosyl-(S)-proline esters 3-methoxy-6-hydroxycyclohex-2-enone 1 is presented. An enantioconvergent synthesis of either (-)-(S)- and (+)-(R)-2-acyl-3,6-dihydroxycyclohex-2-enone starting with the racemic alpha-ketol 1 through a conversion of ( approximately 1:1) mixture of diastereomeric esters into one diastereomer by a repeated crystallization, followed by dimethylaminopyridine-catalyzed equilibration as key steps is described.

[Morphological characteristics of chronic hepatopathy in combination of HCV and chronic alcoholic intoxication]. / Morfologicheskie osobennosti khronicheskoi gepatopatii pri sochetanii HCV-infektsii i khronicheskoi alkogol'noi intoksikatsii.

Biopsies from 35 patients and autopsy material from 20 patients who died of chronic hepatopathy were studied. Hydropic dystrophy and inflammation were predominant in HCV infection. Chronic alcoholic intoxication (CAI) was characterized by diffuse large-droplet fat dystrophy and liver fibrosis. In combination of HCV-infection with CAI sclerotic processes were pronounced and therefore liver cirrhosis is more frequent than in other groups.

[Ambulatory surgical interventions in patients with proctological diseases]. / Ambulatornye khirurgicheskie vmeshatel'stva u bol'nykh proktologicheskimi zabolevaniiami.

The results of 629 operations for the most frequent proctological diseases (hemorrhoid, anal scissure, rectal fistula, acute paraproctitis) were analyzed. 134 operations were performed in outpatient clinic, 225--in one-day hospital and 270 (control group)--in coloproctology department. The place of operation did not influence postoperative period and temporary disability time. The mean stay of patient in outpatient department and hospital surgical unit was 3.8 and 324 hours, respectively. Since the second day after operation in specialized department it is possible to perform therapeutic manipulations outpatient in coloproctology room without negative sequences for patient's health. Cost of the ambulatory operation constituted 28.4% of hospital stay for patients with similar interventions. Surgical treatment of the above diseases could be performed in outpatients setfing in more than 50% patients. The bed resources must be used for patients who need long-term and intensive treatment in specialized department.

[The interrelationship of the intensity of free-radical oxidation to the level of serum bilirubin in lesions of the hepatobiliary system]. / Vzaimosviaz' intensivnosti svobodnoradikal'nogo okisleniia s urovnem syvorotochnogo bilirubina pri porazheniiakh gepatobiliarnoi sistemy.

The level of lipid peroxidation (LPO) products increases in the blood of patients with chronic diseases of the liver and acute cholecystitis with mechanical jaundice. The values normalize after surgery for acute cholecystitis, whereas therapy for chronic diseases of the liver virtually does not correct them. These data demonstrate the relationship between intensity of free-radical oxidation and severity of hepatobiliary process and between plasma content of LPO products and level of serum bilirubin.

[Protein composition of venoms from several species of tropical ants and their effect on mitochondrial H+-ATPase]. / Issledovanie belkovogo sostava iadov nekotorykh vidov tropicheskikh murav'ev i ikh vliianiia na H+-ATPazu mitokhondrii.

Protein compositions of venoms of South-American stinging ants, Ectatomma tuberculatum, Paraponera clavata (subfamily Ponerinae), and "tangarana" were analyzed. The venom of E. tuberculatum displayed the most complex protein composition (more than 15 polypeptides). The water-soluble fraction of the venoms of P. clavata and "tangarana" contained acidic proteins (pI < 3.5 to 5.2), whereas the venom of E. tuberculatum contained predominantly basic proteins (pI 8 to > 9.5). N-Terminal residues and N-terminal sequences of a number of polypeptides were determined. High-molecular-mass polypeptides of the P. clavata venom slightly stimulated the ATPase activity of mitochondrial F1-ATPase. Low-molecular-mass nonprotein components of this venom significantly inhibited the ATPase activity of submitochondrial particles and F1-ATPase from bovine heart mitochondria. The venoms of E. tuberculatum and "tangarana" produced no effect on the ATPase activity.

[Methodical aspects of formation of diagnostically related groups for inpatient therapeutic-prophylactic institutions]. / Metodicheskie aspekty razrabotki diagnosticheski sviazannykh grupp dlia statsionarov lechebno-profilakticheskikh uchrezhdenii.

The authors discuss problems concerning the methodology of the evolution of standard sets of therapeutic-prophylactic measures and diagnostically related groups (DRG) for in-patient hospitals. The peculiarities of the formation of DRG for colo-proctological patients in a proctological clinic are shown as an example and DRG of the surgical and therapeutic classes are indicated. The results of studying the frequency of combined pathology in diseases of the large intestine are discussed.

[Study of the structural organization of OSCP--subunits of H+-ATPase of porcine heart mitochondria]. / Issledovanie strukturnoi organizatsii OSCP--sub''edinitsy H+-ATP-azy mitokhondrii serdtsa svin'i.

Unmodified and citraconilated OSCP of the pig heart mitochondrial H(+)-ATPase were hydrolysed by proteinase from Staphylococcus aureus V8 and trypsin, respectively. To purify the individual peptides, various types of HPLC and covalent chromatography on SH-Sepharose were used. By the automatic Edman method complete or partial amino acid sequences of the peptides obtained were determined, thus allowing for the reconstruction of the primary structure of pig OSCP. A linear antigenic determinant recognizable by A1 monoclonal antibody against bovine OSCP, was localized. Studies showed Gly43 residue (bovine OSCP) to be replaced by Ala43 (pig OSCP), which is responsible for a decrease of the affinity of the monoclonal antibody A1 to pig OSCP. Comparative analysis of primary structures of bovine and pig OSCP was carried out.
