RESUMO

Incidence of the benign and malignant tumors of smooth muscles of the alimentary tract is relatively low. Both kinds of tumors cause haemorrhage, which often urges a prompt surgical intervention. The paper deals with the problems encountered in diagnosis and treatment of 20 patients with the above tumors, operated on at the Department of Gastroenterological Surgery, University Medical Centre of Ljubljana, during the last decade.

RESUMO

Operations on the biliary system rank among the most frequent abdominal surgical procedures. Discovery of the chenodeoxycholic acid brought about no changes in treatment of cholelithiasis. The paper deals with the problems of residual stones, injuries to the hepatic artery and bile ducts, surgical treatment of stenosis of the bile ducts, cholecystitis, cholangitis and papila of Vater conditions. Since the paper is conceived as a basis for the round table duscussion, it only indicates the issues to be dealt with.

RESUMO

Abscess of the spleen is an uncommon entity that seems even less common as it still represents a diagnostic problem. The most common cause of splenic abscess is metastatic hematogenous seeding of the diseased spleen especially of the infarcted areas or traumatic hematomas. It can result also from the direct spread of infection from surrounding structures. Many patients with splenic abscess have a rapidly progressive generalized sepsis and even the combination of well-timed surgery and antibiotic therapy is not always curative. Local symptoms may be mild and overlooked and there may be only general symptoms of suppuration present. X-rays investigations often yield valuable information about the location of the abscess. By far the most promising technique is splenic scanning with the use of radioisotopes. Our case of splenic abscess following appendicitis has been described. The course and the diagnosis has been established using liver-spleen scanning. The patient was treated with Obracin and Dalacin and the diseased spleen has been removed. After drainage of the left subphrenic abscess the recovery was uneventful.

RESUMO

Extensive fat necrosis may be sometimes seen in patients suffering from acute pancreatitis even if pancreas itself is not seriously damaged. Infection of the necrotic areas of fat may produce serious intoxication and even death of the patient. If the infection occurs, evacuation of the necrosis should be carried out, even several times if necessary. In some cases fat necroses colliquate. The liquid may be reabsorbed or pseudocyst formation follows. In the cases of colliquation without infection a close observation of the patient is necessary to choose the time for surgery.
