RESUMO

Clinical efficiency of If-inhibitor ivabradin (Coraxan, Servier) in 40 patients with cardiorespiratory pathology (CRP) was studied. It was shown, that use of ivabradin in dose of 5 mg two times a day leaded to significant decrease in number of angina attacks in a week, and also in time of painless myocardial ischemia, decrease in heart rate at a day and during physical exercises, increase in 6 minutes walking distance and circadian index, oxygen saturation and partial tension, decrease in average pressure in pulmonary artery, increase in ejection fraction of left ventricle. Thus, ivabradin (Coraxan, Servier) is an effective antianginal drug for CRP patients, it improves life quality and do not has an influence on external respiration function. Ivabradin in dose of 5 mg two times a day can be used for CRP patients and as alternative to beta-adrenoblockers.

RESUMO

The aim of the study was to evaluate the endothelial function in patients with coronary heart disease and an exacerbation of chronic obstructive pulmonary disease (COPD), and the role of endothelial dysfunction (ED) in the pathogenesis of reciprocal exacerbation syndrome in cases of combined cardiovascular pathology. The subjects were 366 patients with single or combined cardiovascular diseases, in whom non-invasive evaluation of the endothelial regulation of the vascular tone was performed, and serum levels of NO were measured. The study revealed the biggest number of cases with an altered endothelium-dependent vasodilatation, and the most prominent decrease in NO level in patients with CHD and COPD. All the patients were followed up during two years in order to compare clinical manifestations of single and combined cardiovascular pathology. The results show that ED may be considered a predictor of complicated clinical course of COPD in patients with CHD.

RESUMO

The patients with myocardial infarction have marked changes in the structure of both protein and lipid parts of the membrane. These changes are connected with destruction of antioxide cell protection against the background of developing hypoxia and "oxide stress". In such patients the increase in HbA2 and HbF factors may be regarded as the adaptive mechanism of hypoxia conditions that develop due to decrease in myocardial propulsive ability and deterioration of blood rheological properties. Change of structural and functional characteristics of erythrocyte membranes leads to increase in their hardness that is connected with the increase in blood viscosity, pressure upon the impaired myocardium and extension of necrotic zones. Similar structures of erythrocyte and cardiomyocyte membranes allow to draw a conclusion about the presence of similar disorders in the latter membranes. It shows the necessity to introduce into the treatment scheme of myocardial infarction patients the drugs inhibiting lipid oxidation.
