RESUMO

A group of 133 patients treated for bleeding peptic ulcer in our Department, is reviewed. Within several hours of admission, all patients underwent upper gastrointestinal tract gastroscopy and obliteration of the bleeding ulcer. Bleeding gastric ulcers were found in 41 patients, and duodenal ulcers in 92 patients. Patients were classified according to the Forrest scale: IA - 11 patients, IB - 49 patients, IIA - 35 patients, lIB - 40 patients. In 126 (94.7%) patients the bleeding was stopped, and 7 required urgent surgery: 3 patients with gastric ulcer underwent gastrectomy, and 4 with duodenal ulcer - truncal vagotomy with pyloroplasty and had the bleeding site underpinned. Fifty-five patients underwent elective surgery: gastrectomy and vagotomy (18 patients with gastric ulcer), highly selective vagotomy (25 patients with duodenal ulcer) and truncal vagotomy and pyloroplasty (12 patients with duodenal ulcer). None of the patients was observed to have recurrent bleeding.

Sixty-two outpatients were assessed and divided into the following groups: 20 patients who had had partial gastrectomy (PG group), 22 patients who had had truncal vagotomy and pyloroplasty (TV group) or high selective vagotomy (HSV group), and 20 patients who had had cholecystectomy (CH group). The patients' age ranged from 35 to 64 years (mean 45 years), and the average postoperative period was 9 years. None of the patients evidenced clinical or biochemical symptoms of malnutrition or malabsorption or of diseases affecting vitamin D metabolism. The function of the kidneys and the liver was normal. An age-matched group of volunteers served as a control group. The calcium dietary intake was determined using a standardized questionnaire; and the levels of serum calcium (Cas), phosphate (Ps), alkaline phosphatase (AP), and 25-hydroxyvitamin D [25(OH)D] and the excretion of Ca in a sample of fasting urine corrected for concurrent creatine excretion (FuCa/cr) were assessed by means of standard laboratory techniques. The bone mineral density (BMD) of the lumbar spine (L2-4) and femoral neck (neck-L) was determined by means of dual energy x-ray absorptiometry (DXA). The daily Ca dietary intake was lower than recommended (RDA) in 80% of the patients, with most of them ingesting less than 300 mg daily. The mean values of Cas, Ps, AP, and FuCa/cr did not differ from those in the controls. Significantly reduced 25(OH)D levels were observed in the PG group (7.0 ng/ml) (p < 0.001) and CH group (12.5 ng/ml) (p < 0.01) compared with the values in the control group (20.0 ng/ml).(ABSTRACT TRUNCATED AT 250 WORDS)

This study was designed to look for the presence of Campylobacter pylori in patients who had Roux-en-Y procedure to control symptoms of severe alkaline reflux gastritis (ARG) and to correlate findings with endoscopy and histology. Twenty five patients, who had been operated on for ARG during the last 15 years, were recruited into this study (mean age: 53 years). Surgical procedures were performed by a single surgeon (K.B.). Clinical symptoms of ARG improved after corrective surgery in all cases, but endoscopic pictures remained abnormal in 14 of 25 patients and histological gastritis was observed in all cases. Helicobacter pylori were detected in 8 patients using an urease test performed on gastric mucosa biopsies (CLO-Gistbrocades test). All 8 patients had their biliary diversion 10 or more years earlier, suggesting that H-pylori colonisation of the gastric mucosa develops only slowly once the bile reflux has been stopped. Longitudinal studies, looking for the presence of H-pylori in postoperative stomachs, may help in determining whether they have a role in the aethiology of the peptic ulcer and gastritis, or whether it really is only an innocent bystander.
