RESUMO

Results of conservative treatment of 87 patients with "mallet finger" injuries with metallic and plastic splints are analyzed. All the patients were divided into 3 groups depending on time of treatment procedure. The 1st group consisted of 49 patients resorted to medical assistance in first 4 days after trauma, 2nd group 31 patients with 5 -- 28 days prescription of injury, 3rd group -- 7 patients with more 28 days prescription of injury. Positive results at 1st group was at 97.96% cases (average score 4.35), at 2nd group 83.87% (average score 4.03), 3rd group -- 71.43 (average score 3.57). The positive results at patients resorted to medical assistance in first 6 weeks after trauma were achieved at 92.86% cases (average score 4.24). It is concluded that 6 weeks is the critical period for high effective conservative treatment.

RESUMO

Results of treatment of 61 patients with "mallet finger" injuries have been analyzed. After discontinuation of permanent immobilization the patients of the main group (n = 20) underwent "splint dishabituation" consequentially and bit by bit during 8 weeks, according to an original protocol developed with special reference to the stages of biological process of union of the damaged tendon. Following permanent immobilization patients of the group of comparison (n = 35) were recommended to use a removable splint at night during two weeks. The proposed protocol of "splint dishabituation" improved the results of treatment of injuries of the "mallet finger" type.

RESUMO

The effect of atropine on insulin secretion stimulated by glucose administration in the duodenum was studied in chronic experiments on dogs prior to and following supradiaphragmatic vagotomy. The control experiments on 10 dogs have demonstrated that in the first 10 min. after glucose administration in the duodenum pancreatic islet B-cells show a raised sensitivity to glucose expressed in a higher value of the insulinogenic index at this time than in later periods. After vagotomy no significant differences in insulin secretion and glucose tolerance in animals were found as compared to preoperative values. However the experiments with atropine administration have shown that the blockade of M-choline receptors results in considerable weakening of insulin secretion and glucose tolerance in animals irrespective of the integrity of the nerves vagus. It has been assumed that an important role in the mechanisms of the intestinoinsular axis is played by the nervous connections of the duodenum with the pancreas through interaction of the intraorganic neurons of these organs.
