The safety of early administration of oral fluid following general anesthesia in children undergoing tonsillectomy: a prospective randomized controlled trial.

BACKGROUND: The feasibility and safety of administrating a small amount of oral fluid to children in the early recovery period following tonsillectomy under general anesthesia to reduce the thirst and its associated restlessness remain unknown. METHODS: This study was approved by the institutional ethics committee and adhered to the CONSORT guidelines. Pediatric patients undergoing tonsillectomy who met the inclusion and exclusion criteria of our study were randomized into the study and control groups. In the study group, patients were given a small amount of water instantly after recovering from general anesthesia, which included the recovery of the cough and deglutition reflex, and attaining grade V of muscle strength. The control group was given a small amount of water at 4 to 6 h after the operation. The incidence of nausea and vomiting and the degree of thirst relief were measured and compared between the two groups. RESULTS: Three hundred patients were randomized into each group. There was no significant difference in the incidence of nausea and vomiting at 20 min after drinking water between the two groups (P > 0.05). The thirst score of children over 5 years old in the study group was significantly lower than that of the control group (P < 0.05). CONCLUSION: Early administration of a small amount of oral fluid to children following tonsillectomy and recovering from general anesthesia is not only safe but also effective in reducing postoperative thirst. Trastuzumab-Induced Severe Thrombocytopenia:A Case Report and Literature Review.

We present a 29-year-old woman with pT2N0M0 breast cancer, histological diagnosis of invasive ductal carcinoma, ER and PR low positive, and HER-2 (3+). The patient developed trastuzumab-induced thrombocytopenia in 6 hours after an intravenous infusion of trastuzumab at the second cycle of trastuzumab treatment with the symptom of abnormal uterine bleeding. Laboratory exam revealed a sharp drop of platelet count down to 3×109/L. With the treatment of single-donor platelet transfusions, glucocorticoids, oxytocin and thrombopoietic drugs, the platelet count recovered completely in 11 days. This case was confirmed to be severe thrombocytopenia induced by trastuzumab, and retreatment with trastuzumab was not attempted. With increasing clinical utilization of trastuzumab, clinicians are likely to encounter more life-threatening trastuzumab induced severe thrombocytopenia. By this case report and literature review, we hope to increase the awareness, attach the attentions to this condition, and help with the effective treatment.

[Feeding ecology of Conger myriaster and structure of the food webs in artificial reef zone, Laoshan Bay, China]. / 崂山湾人工鱼礁区星康吉鳗摄食生态及食物网结构.

Based on cage net and longline fishing surveys in Laoshan Bay, China, from April 2015 to January 2017, a total of 279 Conger myriaster stomach samples were analyzed to study their feeding ecology, including diet composition, feeding type, feeding grade, feeding intensity, trophic niche, and trophic level. The stomach content analysis of nine key fish species (including Sebastes schlegelii, Hexagrammos otakii, Hexagrammos agrammus, Sebastiscus mamoratus, Lateolabrax maculatus, etc.), were conducted to examine the food webs in artificial reef zone. The results showed that the diet of C. myriaster consisted of more than 30 species belonging to seven orders, in which shrimps were the most dominant prey species, followed by fishes and cephalopods. The dominant species in the diet of C. myriaster were H.otakii, Enedrias fangi, Trachypenaeus curvirostris, Ammodytes personatus and Alpheus japonicas. The diet composition of C. myriaster varied with season and anal length. Fishes and shrimps were the two important groups throughout the years. Cephalopods were predominant in spring. Cephalopods and crabs were predominant in Autumn. Fish eggs were eaten mostly during winter. Fish eggs and T. curvirostris were the dominant prey groups of the C. myriaster with anal length ≤120 mm, whereas A. personatus and A. japonicas were the dominant prey groups of the C. myriaster with anal length 120-130 mm, H.otakii and E.fangi were the dominant prey of the C. myriaster with anal length ＞130 mm. The percentage of empty stomach of C. myriaster varied significantly with season, but the mean stomach fullness index did not. The percentage of empty stomach and mean stomach fullness index did not differ significantly among different anal length groups. The trophic levels of the key fish species were higher than level 3, with C. myriaster at the top of food webs (with a trophic level of 4.636). Decapoda, crabs, amphipoda and molluscs were the main prey of key fish species, while Crustaceans, E. fangi, H. otakii and A. personatus were the main preys of L. maculatus and C. myriaster.

Electro-optic coupling of wide wavelength range in linear chirped-periodically poled lithium niobate and its applications.

We theoretically investigate the electro-optic coupling in an optical superlattice of linear chirped-periodically poled lithium niobate. It is found that the electro-optic coupling in such optical superlattice can work in a wide wavelength range. Some of examples, with bandwidths of 20, 40, 80, 120 nm, are demonstrated. The way to determine the electric field for perfect conversion between o- and e-ray and the method using apodized crystals of tanh profile to reduce the ripples are shown. As one of its applications, one kind of broadband Solc-type bandpass filter in optical communication range is proposed.
