RESUMO

Small cell transformation was one mechanism by which EGFR-mutation NSCLC acquired resistance after tyrosine kinase inhibitors (TKIs) treatment. A few reports of small cell transformation occurred in other oncogene-driven lung cancers. We found the first case of transformation of a RET-rearranged lung adenocarcinoma to SCLC after selpercatinib, a novel highly selective RET TKIs. Whole-exome sequencing (WES) was used to explore alteration in gene expression in tumor tissue at initial diagnosis and after transformation into small cell carcinoma. We found that transformed into SCLC tumor tissue had inactivation of RB1 and TP53, with RET fusion was still present. In addition, the APOBEC family of cytidine deaminases appeared amplification. Although RET rearrangement still existed, using another RET TKIs was ineffective, and etoposide plus platinum might be an effective rescue treatment.

RESUMO

Objective To investigate the potassium currents of the cardiac pacemaking cells induced and differentiated from rat mesenchymal stem cells (MSCs) modified by HCN4 gene. Methods Identified cardiac pacemaking cells were adopted as the experiment group, and the sinoatrial node cells of original infant rat cultured in the same period were regarded as the control group. Whole cell patch was used to measure the action potential of the pacemaking cells and sinoatrial node cells. Results Action potential of automatic depolarization at dilatation was recorded in both the differentiated cardiac pacemaking cells and sinoatrial node cells. There was no significant difference on amplitudes of resting potential, amplitudes and cycle of action potential [(-50?2.8) vs (-55?5.5),(-60?2.5) vs (-65?2.5),(240?57) ms vs (250?60) ms], but the field potential was much lower in cardiac pacemaking cells than the control group[(-30?2.5) vs (-55?5.5),P

Objective To evaluate the abilities of plasma perfusion and plasma exchange to remove blood endotoxin and cytokines from hepatic failure patients. Methods Plasma perfusion and plasma exchange were performed on 7 cases and 10 cases for the treatment of chronic severe hepatitis, respectively. Changes of blood endotoxin and cytokines before and after treatment were analyzed. Results The levels of cytokines and endotoxin in patient blood decreased greatly, but bilirubin decreased slightly after treatment with plasma perfusion. The changes of interleukin 6 were very limited. Compared with plasma perfusion, the removal of bilirubin was higher but that of cytokines was lower after plasma exchange. Conclusion Plasma perfusion can remove cytokines with high efficiency and retard the development of hepatic failure.
