RESUMO

Sleep disorders are common among pregnant females. However, its association with postpartum depression (PPD) is unknown. We aimed to assess if sleep disorders during pregnancy increase the risk of PPD by a systematic review. The databases of PubMed, CENTRAL, ScienceDirect, Embase, and Google Scholar were searched for studies reporting the association between any type of sleep disorder during pregnancy and the risk of PPD. Effect sizes were pooled in a random-effects model. Sixteen studies with data of 12,614 women were included. Meta-analysis indicated that sleep disorders during pregnancy resulted in a statistically significant increased risk of PPD (OR: 2.36 95% CI: 1.72, 2.32). The overall result had high heterogeneity (I2 = 84%). Sub-group analysis based on study location (Asian vs Western), sample size (> 500 vs < 500), depression scale, and PPD assessment time did not change the results. However, we found that only poor sleep quality but not insomnia was associated with PPD. The risk was also increased only with sleep disorders measured in the 3rd trimester but not for the 1st and 2nd trimesters. Evidence suggests that sleep disorders during pregnancy may increase risk of PPD. The risk is high for sleep disorders occurring in the 3rd trimester. Based on these findings, there is a need for thorough screening and subsequent corrective measures to ensure adequate and quality sleep among pregnant females.

Objective: Red cell distribution width (RDW) and serum calcium (Ca) levels are predictors of in-hospital mortality in acute myocardial infarction (AMI) patients. However, their sensitivity and specificity are limited. Therefore, this study aimed to determine whether the RDW to Ca ratio (RCR) acquired on admission can be used to predict the in-hospital mortality of AMI patients. Methods: This retrospective cohort study extracted clinical information from the Medical Information Market for Intensive IV (MIMIC-IV) database on 2,910 AMI patients enrolled via propensity score matching (PSM). Prognostic values were assessed using a multivariate logistic model and three PSM approaches. Analysis was performed based on stratified variables and interactions among sex, age, ethnicity, anemia, renal disease, percutaneous transluminal coronary intervention (PCI), coronary artery bypass grafting (CABG), atrial fibrillation, congestive heart failure, dementia, diabetes, paraplegia, hypertension, cerebrovascular disease, and Sequential Organ Failure Assessment (SOFA) score. Results: A total of 4,105 ICU-admitted AMI patients were analyzed. The optimal cut-off value of the RCR for in-hospital mortality was 1.685. The PSM was performed to identify 1,455 pairs (2,910) of score-matched patients, with balanced differences exhibited for nearly all variables.The patients' median age was 72 years (range, 63-82 years) and 60.9% were male. The risk of in-hospital mortality incidence increased with increasing RCR levels. After adjusting for confounders, the risk ratio for the incidence of in-hospital mortality for high RCR was 1.75 [95% confidence interval (CI): 1.60-1.94, P = 0.0113] compared to that associated with low RCR in the PSM cohort. High RCR was also substantially implicated in in-hospital mortality incidence in the weighted cohorts [odds ratio (OR) = 1.76, 95% CI: 1.62-1.94, P = 0.0129]. Assessment of RCR in three groups showed that patients with high RCR also had a higher risk of in-hospital mortality (OR = 3.04; 95% CI, 2.22-4.16; P < 0.0001) than in patients with RCR in the adjusted model. In the sensitivity analysis, both the original and weighted groups showed similar results. Conclusion: The RCR at admission may be useful for predicting in-hospital mortality in ICU-admitted AMI patients.

Background: With the gradual penetration of network media into various fields of people's life, the relationship between network behavior and the sense of meaning of life is bound to be closer and closer. The purpose of this study is to explore the mediating role of state anxiety between cyber loafing and the sense of meaning of life, and the moderating role of psychological flexibility in this mediating relationship. Methodology: With 964 undergraduates recruited as subjects three-wave-time-lagged quantitative research design was conducted in China. All participants were required to complete a self-reported electronic questionnaire. Then, the mediating mechanism and moderating effect were explored with utilization of SPSS25.0. Results: The results showed that cyberloafing had significant negative correlation with the sense of meaning of life. Our analysis testing the mediating effect showed that state anxiety partially mediated the relationship between cyberloafing and the sense of meaning of life (indirect effect = -0.05, p < 0.01,), while the mediating effect was 31.25% of the total effect. Our analysis testing the moderating effect showed that psychological flexibility significantly moderated the relationship between cyberloafing and state anxiety (interaction effect = -0.26, p < 0.01). And our analysis testing the moderated mediating effect showed that psychological flexibility played a moderating role in the mediating effect of state anxiety. Conclusion: Based on the findings of this study, college students' cyberloafing negatively affects their sense of meaning of life. Therefore, appropriate measures should be taken to supervise and restrict college students' Internet use and provide them with corresponding guidance; certain psychological adjustment measures should also be taken when necessary to help college students with low psychological flexibility in reducing their state anxiety and improving their sense of meaning of life.

OBJECTIVE: To investigate the mechanism of the ring chromosome 9 formation by cytogenetic analysis of one case affected with ring chromosome 9 syndrome. METHODS: Routine chromosome GTG-binding analysis and dual-color fluorescence in situ hybridization (FISH) with TelVision 9p and 9q probes were applied to characterize the case. RESULTS: The G-binding revealed that the patient had ring chromosome 9 with the following karyotype: 45,X,-9/46,XX,r(9)(p24q34)/46,XX,r(9;9)(p24q34;p24q34)[4/92/4]. The dual-color FISH analysis with TelVision 9p and TelVision 9q probes failed to detect a hybridization signal on the ring chromosome in the case, which indicated that at least 115 kb were deleted on the terminal 9p and 95 kb were deleted on the terminal 9q. Comparing to the cases reported in the literatures, our patient shared some common features of the 9p- and 9q- syndrome. CONCLUSION: The clinical features of patients with identical r(9) breakpoints present variable phenotypes. The possible cause might be the submicroscopic variation in the deletion breakpoints, variation in the ring stability, the modification of the expression of the deleted by the individual's genetic background, or the effect of changes in the fetal environment. The haploinsufficiency of genes located in the deleted regions may play critical roles in the patient phenotype as well.

RESUMO

We propose a hybrid-guidance photonic crystal fiber (HG-PCF) that can guide light by the simultaneous effect of index guidance and photonic bandgap guidance at all wavelengths and directions. Index guidance is the dominant guidance mechanism at short wavelengths and photonic bandgap guidance is the dominant guidance mechanism at long wavelengths. The transmission spectrum of the Bragg grating in such a HG-PCF is also investigated.
