Time course of changes in hemostasis were investigated in 68 rheumatoid arthritis (RA) patients. DIC syndrome and anemia were recorded in 23 and 20 patients, respectively. Three RA sufferers were free of anemia. A course of plasma-pheresis proved effective against DIC syndrome in 12 anemic and 2 nonanemic patients. Alleviation of DIC syndrome occurred in association with improvement in the red cell count and inhibition of rheumatoid process activity. In persistent DIC syndrome a trend to normalization of the above parameters was insignificant.

Regularities in the development of anemia in patients with chronic hepatitis or hepato-cirrhosis were studied with regard to the pattern and stage of the affection of the liver. The impact of inefficient erythropoiesis on anemia development in patients with chronic diffuse diseases of the liver was demonstrated. Reasons of the red cells imperfection that caused their rapid hemolysis were indicated. In the bone marrow of chronically ill patients there were lower levels of early precursors of erythropoiesis with their proliferative activity reduced. On the basis of the data obtained the authors gave practical recommendations for anemia treatment.

The results of the clinico-laboratory investigations conducted in time course have evidenced a high therapeutic effectiveness of plasmapheresis in the complex treatment of patients with chronic active hepatitis and biliary cirrhosis of the liver. Plasmapheresis use has been validated and indicated in patients with a manifest biliary component, a high degree of the process activity and ineffectiveness or low effectiveness of the previous treatment.

Parameters of iron metabolism and humoral immunity were studied in patients with chronic diffuse diseases of the liver (cirrhosis, chronic hepatitis), beta-thalassemia major, dyserythropoiesis, hereditary hemochromatosis. High ferritin content has been recorded in the plasma of these patients, that leads to the formation of antibodies to this protein followed by the production of circulating immune complexes inducing metabolic disorders that aggravate the pathologic process. Plasmapheresis and deferoxamine therapy result in a decrease of ferritin and circulating immune complex content in the plasma, that produces a favourable effect on the patients' condition.

Iron metabolism was studied in patients with chronic diffuse diseases of the liver. A sharp increase of ferritin content at the expense of its release from affected hepatocytes, and formation of antibodies to ferritin of the splenic type have been recorded in patients with cirrhosis of the liver. The observed growth of ferritin content in red blood cells can be explained by functional deficiency of red blood cells and by the presence of hemolytic process (twofold decrease of haptoglobin level). A conclusion has been made that ferritin content in the blood serum does not correlate with the total iron reserves, and serum ferritin level should be considered not as a sign of the pathologic process, but as an index of iron reserves capable of participating in metabolic processes.

Optimum programs have been developed for the differential intensive transfusion-infusion therapy of patients with cirrhosis of the liver, based on the characteristics of the clinical course of the disease and the directed action of varying current solutions. Overtime clinical investigations were conducted in 296 patients with cirrhosis of the liver. The results of the study have shown a favourable effect of the components, blood preparations and hemocorrectors on the clinical signs of the disease, on the function of the liver, kidneys, hemostasis system and microcirculation.

The results of the overtime combined clinico-laboratory investigations have evidenced a high medical effectiveness of red blood cell pack transfusions in the treatment of anemia in patients with varying diseases of internal organs.

Therapeutic action of blood components, preparations and substitutes was studied in liver cirrhosis patients with deep anemia, marked hemorrhagic complications, endogenous toxicosis, protein insufficiency and rheological disorders. A high efficacy of different media was shown in urgent therapy of liver cirrhosis patients. A choice of drugs was determined by the patients' initial status and specificities of purpose-oriented action of modern transfusion and infusion media.
