Objective: Few studies have examined the association between post-stroke depression (PSD), aphasia, and physical independence in Chinese patients. This study investigated the above association in stroke patients in China at 3-month follow-up. Methods: Altogether 270 patients within 14 days after ischemic stroke were recruited and followed up at 3 months. PSD, aphasia, and physical functional status were measured using the Stroke Aphasia Depression Questionnaire (SADQ), Western Aphasia Battery (WAB), and modified Rankin Scale (mRS), respectively. Patients with mRS total score >2 were considered as having "physical dependence." Results: Out of 248 patients at 3-month follow up, 119 (48%) were rated as having physical dependence. Multiple logistic regression analyses revealed that female (p = 0.04; OR = 2.2; 95% CI: 1.0-5.1), more severe stroke at admission (p < 0.01; OR = 1.4; 95% CI: 1.3-1.5), and more severe PSD at 3 months (p = 0.01; OR = 1.05; 95% CI: 1.01-1.1) were independently associated with physical dependence at 3 months. Conclusions: Greater PSD and stroke severity were independently associated with physical dependence at 3 months after stroke. Aphasia was also associated with physical dependence but the relationship was not significant. Early and effective depression screening, treatment and stroke rehabilitation appear to be important to improve the physical outcome and reduce the burden of stroke survivors.

Poly(acrylic acid) (PAA) hydrogels with superior mechanical properties, based on a single network structure with dual cross-linking, are prepared by one-pot free radical polymerization. The network structure of the PAA hydrogels is composed of dual cross-linking: a dynamic and reversible ionic cross-linking among the PAA chains enabled by Fe(3+) ions, and a sparse covalent cross-linking enabled by a covalent cross-linker (Bis). Under deformation, the covalently cross-linked PAA chains remain intact to maintain their original configuration, while the Fe(3+)-enabled ionic cross-linking among the PAA chains is broken to dissipate energy and then recombined. It is found that the mechanical properties of the PAA hydrogels are significantly influenced by the contents of covalent cross-linkers, Fe(3+) ions and water, which can be adjusted within a substantial range and thus broaden the applications of the hydrogels. Meanwhile, the PAA hydrogels have excellent recoverability based on the dynamic and reversible ionic cross-linking enabled by Fe(3+) ions. Moreover, the swelling capacity of the PAA hydrogels is as high as 1800 times in deionized water due to the synergistic effects of ionic and covalent cross-linkings. The combination of balanced mechanical properties, efficient recoverability, high swelling capacity and facile preparation provides a new method to obtain high-performance hydrogels.

STUDY OBJECTIVE: To investigate the effects of stellate ganglion block (SGB) on cardiovascular response and heart rate (HR) variability in elderly patients during anesthesia induction and endotracheal intubation. DESIGN: A randomized, double-blinded, and placebo-controlled study. SETTING: University-affiliated teaching hospital. PARTICIPANTS: Eighty elderly patients (American Society of Anesthesiologists grades I and II) receiving elective surgery during general anesthesia. INTERVENTIONS: Right stellate ganglion injection (SGB) was performed in all patients using 10 mL of 1% lidocaine or normal saline. MEASUREMENTS: Systolic blood pressure (BP), diastolic BP, HR, and calculated rate pressure product. HR variability at the following time points: conscious status before induction (T0); immediately before intubation (T1); immediately after intubation (T2); and 1, 3, and 5 minutes postintubation (T3, T4, and T5). MAIN RESULTS: No significant differences in BP and HR were observed between the 2 groups. Rate pressure product values significantly increased in the control group compared with baseline and SGB group values. Low-frequency power (LF) and LF/high-frequency power (HF) significantly increased, and HF and normalized units of HF significantly decreased in the control group compared with baseline values. LF, normalized units of LF, and LF/HF in the SGB group significantly decreased compared with those of the control group. CONCLUSION: SGB protects the myocardium and effectively suppresses stress responses during anesthesia induction and tracheal intubation in elderly patients.

PURPOSE: Study of the molecular variation in pre-eclampsia placenta based on micro-Raman spectroscopy. METHODS: Five pregnant women with pre-eclampsia from Nanfang hospital were selected as study group whose average age is 28.5 years and 38 ± 2 weeks gestation. The same period of healthy pregnant women, whose average age is 27.6 years and pregnant 39 ± 1 weeks, as control group (n = 5). The normal and pre-eclamptic placental tissues are detected by micro-Raman spectroscopy with the spectrum resolution of 1 cm(-1). RESULTS: We find that the protein structure of α-helix, ß-pleated sheet and ß-turn is overlying in pre-eclamptic placenta, which lead to a disorder of protein structure. The Raman peaks assigned to tryptophan indole ring and phenylalanine in pre-eclamptic placental tissue are more higher than that in normal tissue. CONCLUSIONS: Results suggest that the ordered structures of the main chain in protein molecules are reduced significantly, and the amino acid of side chains is damaged obviously. And a principal component analysis is used to classify the Raman spectra between normal and pre-eclamptic placental tissues. This study presents that Raman spectroscopy has a great potential on the mechanism research and diagnosis of placental lesions.

RESUMO

Laser Raman spectrum technology was used to distinguish fritillaria cirrhosa from its adulterants rapidly and accurately. The study is based on that different traditional Chinese medicine contains different chemical compositions, and the differences could be displayed in Raman spectra. The Raman spectra of fritillaria cirrhosas shows that several characteristic strong peaks could be found at 442, 480, 863, 941, 1 083, 1 129, 1 342, 1 463 and 2 910 cm(-1), and a few obvious peaks appear at 111, 302, 360, 409, 527, 579, 618, 718, 767, 1 052, 1 083, 1 207 and 1 261 cm(-1). According to the Raman spectra, a Raman fingerprint of fritillaria cirrhosa was set up, which could be used to distinguish its adulterants. This analysis could be used in identifying fritillaria cirrhosa rapidly, accurately and nondestructively.

OBJECTIVE: To evaluate the effects of ulinastatin (UTI) on lung injury induced by simultaneous pancreas-kidney transplantation in piglet. METHODS: With reproduction of simultaneous pancreas-kidney transplantation model in piglets, 12 couples of piglets were randomly divided into two groups (n=12). Piglets in UTI group were given a constant infusion of UTI 15 kU x kg(-1) x h(-1) by means of a pump during operation. Control group were treated with constant pumped infusion of 0.9% saline in equal volume. The blood samples were collected to measure the plasma concentrations of superoxide dismutase (SOD), malondialdehyde (MDA), tumor necrosis factor-alpha (TNF-alpha) and interleukin-8 (IL-8) before operation (baseline levels, T0), at the time of re-establishment of circulation with successful an anastomosis of arteries and veins (T1), at 1 hour and 2 hours after re-establishment of circulation (T2 and T3), and at the end of operation (T4). The pathological changes in the lungs were examined. RESULTS: In control group, plasma MDA concentration was significantly increased from T1 till T4 as compared with T0 (all P<0.05), whereas in group UTI it did not change significantly (all P>0.05). In group UTI, SOD activity was significantly increased at T1-2 and T4 as compared with T0 (all P<0.05), whereas in control group it did not change significantly (all P>0.05). The plasma MDA concentration was significantly decreased at T1 and T2, and SOD activity was increased at T2 in group UTI than those in control group (all P<0.05). In control group, plasma TNF-alpha concentration was significantly increased from T2 to T4 as compared with T0 (all P<0.05), whereas it did not change significantly in group UTI (all P>0.05). The plasma IL-8 concentration was significantly decreased at T1-2 and T4 in group UTI compared with those in control group (both P<0.05). CONCLUSION: UTI can inhibit neutrophil aggregation in lungs and expression of harmful inflammatory cytokines, and it reduces production of oxygen free radical, so that it can protect lung tissue from injury induced by simultaneous pancreas-kidney transplantation.

OBJECTIVE: To investigate diagnostic histopathology and ultrastructure features of primary pulmonary cryptococcosis (PC). METHODS: Clinical data and pathologic findings of 27 cases of PC were retrospectively reviewed, light and electron microscopic evaluations and histochemistry stain studies were performed. RESULTS: The specimens consisted of 2 fine-needle aspiration lung biopsies and 25 cases of open lung biopsies. Cryptococcosis granuloma formation was identifiable by histopathological examination in 25 of 27 cases, with gum-like lesion and fungi in the remaining 2 cases. The detection rates of cryptococcus neoformans (CN) by mucicarmine (MC), periodic acid-Schiff (PAS), alcian blue (AB) and Grocott methenamine-silver (GMS) were 87.0% (20/23), 100% (27/27), 66.7% (18/27), and 100% (23/23) respectively. Under the electron microscope, most CN had a simple structure with a few organelles. The capsule was seen in all organisms. A percentage of the organisms showed nuclei, nucleoli, mitochondria and vacuoles. The detection rate of CN by EM was 91.7% (11/12). CONCLUSIONS: The clinical manifestation and imaging of PC are nonspecific for PC. Lung biopsy is the major diagnostic modality. The detection rate by electron microscopy was quite high. Therefore, a correct diagnosis of pulmonary cyrptococcosis should rely on the combination of histopathological evaluation, histochemistry staining and/or electron microscopic examination.
