RESUMO

AIM: To analyze the results of the restoration of the anterior abdominal wall in postoperative ventral hernias using mesh implants. MATERIAL AND METHODS: The study was of 680 patients, who underwent abdominal wall reconstruction using mesh implants. RESULTS: A total of 105 postoperative complications (15.44%) in 84 (12.35%) patients; the postoperative period was without complications in 596 (87.65%) patients. The greatest number of complications was noted after laparoscopic IPOM - 13 patients (20.31%); the smallest - after posterior component separation - 24 (7.5%). Also described 10 systemic complications: 8 of which were stopped by conservative therapy, in 2 cases repeated surgical intervention was required with a change in the type of wall reconstruction. Statistical analysis did not show statistically significant differences in the incidence of complications between the different versions of the performance of hernioplasty, except for the posterior muscular plastics. Statistical analysis also showed a link between the likelihood of various complications and the type of ventral hernia repair. CONCLUSION: Conducting different methods abdominal wall reconstruction using mesh implants is techniques with a low level of postoperative complications and comparable results when performing various types of plastics.

AIM: To analyze diagnostic and surgical tactics for suicidal and autoaggressive thoracic and abdominal lesions. MATERIAL AND METHODS: We have analyzed the features of emergency surgical care depending on type of patients' behavior at admission. There were 516 patients with suicidal and autoaggressive thoracic and abdominal lesions. RESULTS AND DISCUSSION: Depressive (34%) and adequate (46%) patients were examined routinely. Inadequate (aggressive) behavior is inappropriate for instrumental examination while medical sedation may be followed by significant hemodynamic disorders in patients with severe lesions. It was noted that indications for explorative surgery should be expanded in these patients and administration of sedatives should be combined with induction of general anesthesia. CONCLUSION: Severity of trauma and type of surgery affect mental state dynamics while the kind of psychopathy influences postoperative period course.

RESUMO

232 cases of septic complications after penetrating thoracic injury were analyzed. All patients were divided into groups according to the injury mechanism and inhabitancy. Patients with medico-social deviations, as well as patients with hemocontact infections, mental insanity and social deviations were marked out. 75% of patients were reported to have any medico-social deviations. Among them the distributing was as follows: persons with no fixed abode (100%), women (91%), patients over 50 years (86.2%) and patients after knife thoracic injury (77.4%). Among patients with hemocontact infection the majority were incomers from the near-abroad countries (37%), victims of assault (20%) and patients aged 31-40 years (18.4%).

RESUMO

The issue is devoted to the analysis of case reports of 305 patients with suicidal and autoagressive neck, thorax and abdomen injuries. The objective prevalence of penetrating injuries of the abdomen among men with depressive disorders was registered. The most complicated, both for surgeon and psychiatrist, were cases of combined neck, thorax and abdomen injuries in elderly patients.

231 cases of septic complications after the closed thoracic injury were analyzed. All patients were divided into groups according to the injury mechanism and inhabitancy. Patients with medico-social deviations, as well as patients with hemocontact infections, mental insanity and social deviations were marked out. The leading mechanisms of the closed thoracic injury were: fall from a height (34.2%), assault and battery (24.7%) and automobile accident (16.4%). 51.1% of patients were reported to have any medico-social deviations. Among the patients with socially important infections, the distribution was as follows: persons with no fixed abode (50%), incomers from the near-abroad countries (36.4%), victims of assault (28.1%) and pedestrians (21.4%).

RESUMO

The influence of the space charge of ions emitted from the surface of a conical spike on its shape has been studied. The problem of the calculation of the spatial distributions of the electric field, ion velocity field, and the space charge density near the cone tip has been reduced to the analysis of a system of ordinary differential equations. As a result of numerical solution of these equations, the criterion for the balance of the capillary and electrostatic forces on the conic surface of a liquid-metal anode has been determined. It has allowed us to relate the electrical current flowing through the system, the applied potential difference, and the cone angle. We have compared the results of our calculations with available experimental data concerning emission from the surface of pure liquid gallium, indium, tin, and some liquid alloys, such as Au+Si , Co+Ge , and Au+Ge . On the basis of the proposed model, explanations have been given for a number of specific features of the emissive behavior of different systems.

RESUMO

The paper presents the results of a study of the clinical features of psychoneurological disorders in 178 patients with acute intoxications by various psychotropic drugs. Hyperbaric oxygenation showed positive changes in psychopathological syndromic kinesis. A sanogenetic rationale is provided for the method of choice in treating this category of patients.

Formation of large-scale hydrodynamic convective patterns in plasma-like current-carrying media is considered. This process is shown to be described by the same equations, as Benard rolls, except that a temperature field must be replaced by a magnetic field. A simple low-mode model of spatial pattern formation for a case of cylindrical liquid-metal conductor with current is proposed and investigated. Nonlinear interaction of perturbations of the magnetic field and the velocity field results in an increase of effective conductor resistance even when transport coefficients are constant. In our opinion, it is this instability, that is of first importance at the initial stages of the electric explosion of conductors. In particular, it leads to conductor stratification and electric current interruption. (c) 1996 American Institute of Physics.
