RESUMO

Despite being the busiest transient sea in the world due to the Suez Canal, radionuclide distribution studies in seawater and sediment of the Red Sea remain rare. A sampling expedition in the Red Sea was conducted from June 9 to July 6, 2021, visiting a transect of several deep sampling stations located along the central axis of the basin from the Gulf of Aqaba to the southern Red Sea (near Farasan Island, Saudi Arabia). The collected seawater profile samples were analyzed for tritium, radiocarbon and oxygen-18. The observed tritium levels in surface waters of the Red Sea peaked at 0.3-0.4 TU, similar to the values observed in the western Arabian Sea (decay corrected). The values observed at waters below 150 m were around 0.2 TU, however, at depths of 450 and 750 m, tritium minima (<0.2 TU) were observed, which could be associated with a partial return flow of bottom waters from the southern to the northern Red Sea. At two stations at the depth of about 550 m, deep Δ14C minima were observed as well (-4‰ and -10‰), documenting ongoing transport of carbon in the water column, important for sink of anthropogenic carbon.

Recent studies in the Middle East have shown an increased incidence of vitamin D deficiency across this region of year-round sunlight. There is scarcity of information, however, as to the levels of 1,25-dihydroxyvitamin D [1,25(OH)2D], the active form of vitamin D, and its associations with cardiometabolic parameters in an Arab cohort and this study aims to fill this gap. In a cross-sectional study, 33 male and 43 female (22 children and 54 adults, total 76) Saudis with previously established low levels of serum 25-hydroxyvitamin D [25(OH)D] (<50 ng/ml or 20 nmol/l) were recruited. Anthropometrics were obtained and fasting blood samples were taken for a routine measurement of glucose, lipid profile, calcium, and albumin, while serum 25(OH)D, 1,25-(OH)2D, and intact PTH were quantified using specific ELISAs. Serum calcium, intact PTH, and 1,25(OH)2D were all within the normal range in both children and adults in both genders. In all subjects, serum 1,25(OH)2D was not associated with intact PTH, while circulating 1,25(OH)D inversely correlated with systolic blood pressure (p=0.01) and waist circumference (p=0.04). Thus, vitamin D deficient Saudi children and adults with normal levels of 1,25-(OH)2D also had normal circulating calcium and PTH. This study suggests that local cutoffs should be set that will be of clinical significance in the identification of those at true risk for harder end-points, such as secondary hyperparathyroidism and bone-related diseases.

BACKGROUND: Hypovitaminosis D has been associated with an increased prevalence of Type 2 diabetes mellitus (DMT2) and metabolic syndrome manifestations. The purpose of this study was to examine the association between 25-hydroxy-vitamin D (25-OH-VitD) levels and indices of insulin resistance (IR), including adipocytokines, in a Saudi population with or without DMT2. SUBJECTS AND METHODS: A total of 266 subjects (153 DMT2 and 113 healthy controls) aged 26-80 yr were randomly selected from the existing Biomarkers Screening in Riyadh Program (RIYADH Cohort). Subjects were assessed clinically, anthropometry was performed, morning blood chemistries, including fasting glucose (FG), triglycerides, total cholesterol, LDL cholesterol (LDL-C), and HDL cholesterol were obtained. Homeostasis model assessment of IR (HOMA-IR) was calculated, and serum 25-OH-VitD, leptin, adiponectin, resistin, insulin, high sensitivity CRP (hsCRP), and tumor necrosis factor α concentrations were measured using specific assays. RESULTS: In DMT2 subjects, negative correlations between 25-OH-vitD and body mass index (BMI), FG, insulin, HOMA-IR, cholesterol, LDL-C, and hsCRP were observed, while a positive correlation between 25-OH-VitD and adiponectin was detected. The later remained significant after controlling for BMI. Interestingly, only weak and nonsignificant associations between 25-OH-VitD and metabolic parameters were observed in the control group, whereas, when the entire population was examined, negative correlations were evident primarily between 25-OH-VitD and FG, HOMA-IR, total cholesterol, LDL-C. These associations remained significant after controlling for BMI. CONCLUSIONS: These results suggest that hypovitaminosis D associations with metabolic disturbances are accentuated in DMT2. The BMIindependent positive correlation between 25-OH-VitD and adiponectin suggests a potential role for this adipocytokine as a link between 25-OH-VitD and IR in patients with DMT2.

Chronic pains management costs billions of dollars in medical exchequer to the world population. Additionally, 77% of people with chronic pains also have a degree of medically treatable depression. Opioids have a narrower safety index due to their side effects associated with its tolerance, hyperalgesia and subsequent dependence. Likewise, non steroidal anti-inflammatory drugs and anticonvulsants, also have limited safety and tolerability profile in the management of chronic pains. Bacopa monnieri, a renowned ayurvedic medicine has a strong antidepressant effect and significant antinociceptive effect, which is comparable to the effect of morphine via adenosinergic, opioidergic, and adrenergic mechanisms. BM has been also reported to be effective in neuropathic pains. Additionally, it has a strong anti-inflammatory effect mediated via COX-2 inhibitory mechanism. Apart from its effect of augmenting morphine analgesia, BM also inhibits opioid-withdrawal induced hyperalgesia, and acquisition and expression of morphine tolerance. BM is reported to have a strong protective effect against toxic effects of opiates on major organs like brain, kidneys and heart. BM is well documented to be safe and well tolerated herbal therapy in multiple clinical trials including various age groups. This minireview evaluated the preclinical data that highlights potential of BM as a future candidate for clinical management of chronic pains.

The study aimed to determine hospital-based prevalence of vertebral fractures in postmenopausal Saudi Arabian women. Chest radiographs from consecutive Saudi women over the age of 50 years visiting the emergency room at King Fahd Hospital of the University were evaluated. Of 785 radiographs analysed 159 (20.3%) patients had 198 vertebral fractures. The mean age of the women was 65.7 (SD 8.5) years. In only 37.8% of the radiographs with fractures was a vertebral fracture highlighted in the radiologist's report, and only 13.2% of the women with vertebral fractures were on antiresorptive therapy for osteoporosis.

Eight treatments of cardboard (CB) and date palm leaves (DPL) with or without alkali treatments plus molasses, dates, and ammonium sulfate were ensiled in laboratory silos. Latobacillus plantarum and Saccharomyces cerevisiae inoculants were added at an application rate of 10(5) cfu/g of silage to study their possible influence on the fermentation process. Silos were sealed and incubated at 30 degrees C. The anaerobic ensiling was evaluated for chemical and microbiological variables by opening the silos after 30, 60, and 90 days. After 90 days, significantly low pH values (5.2-3.5) were obtained. The percentages of hemicellulose, cellulose, lignin, and ash were determined. Concentrations of reducing sugars, ether extract, and crude protein tended to be acceptable among treatments. Silages showed a significantly high lactic acid concentration (up to 4.3%) and only traces of butyric acid (below 1%). Also changes in acetic and propionic acids concentrations were determined. Lactobacillus populations remained almost static while yeast and molds populations tended to be lower after 90 days of ensilage. Neither alkali treatment nor microbial inoculants showed a relationship with the ensiling process. Thus, overall, it was concluded that CB and DPL can be effectively ensiled and the resultant silages are acceptable to merit their use as ruminant feed in arid regions.

The present study was undertaken to evaluate the peroxidative damage and hypercholesterolemia induced in male Wistar albino rats by diets enriched either with 1% oxidized cholesterol (OC) (containing 49.8% of cholesterol oxidation products) or pure cholesterol (PC). The damage caused by the OC diet was revealed by a significant rise in red blood cell hemolysis, increased tissue lipid peroxidation and elevated aspartate amino transferase activity as compared with control and PC diets. Liver glutathione-S-transferase activity was decreased by both OC (P < 0.01) and PC (P < 0.05) diets, but glutathione was observed to be decreased only by the OC diet. Plasma triacylglycerol and cholesterol were increased significantly with both the OC and PC diets. Liver cholesterol and triacylglycerol were increased significantly with the OC diet only. These results indicate that the oxidative damage caused by the OC diet is much more pronounced than that caused by the PC diet.

Pan bread formulations based on raw wheat germ, vital wheat gluten, and enzyme-active soybean flour were optimized with the objective of developing a phytochemical-enriched designer food product with superior nutritional and sensory qualities. The objective texture values (measured as compression force, g) indicated that the test bread with 10% wheat germ addition was comparable (299.9 g) to the control (210.1 g), but the compression force was significantly higher (415.4 g) at 20% wheat germ level. With 0.5% sodium stearoyl-2-lactylate (SSL), 30 ppm potassium bromate and 50 ppm ascorbic acid, the test breads with 10 and 20% wheat germ had compression force values of 313.8 g and 367.7 g, respectively. Comparing the CIE L*a* values, the test bread samples having up to 20% wheat germ were slightly darker in crumb color than the white flour control bread, but were significantly lighter than the whole wheat flour bread. However, the addition of wheat germ increased the yellow color of bread crumb as indicated by the higher b* values of 11.4, 16.4 and 21.4, for control, 10% and 20% wheat germ breads, respectively. The physical texture and objective color measurements can be used in evaluating the quality of a phytochemical-enriched designer food (pan bread). It can be concluded that wheat germ-enriched bread can be prepared by using white flour, 20% raw wheat germ, 0.5% SSL, 30 ppm potassium bromate and 50 ppm ascorbic acid to provide consumers with a functional food.

Fifteen patients who were 2 years of age or older and had bilateral developmental dysplasia of the hip were reviewed. Thirty hips had required open reduction combined with osteotomy on the pelvic or femoral side or both as necessary. The results were assessed according to McKay and Severin classifications, and complications including asymmetric outcome were recorded. An asymmetric outcome existed if the clinical or radiologic result for the contralateral hip was more than one unit difference or a leg length discrepancy of more than 1 cm was observed. Before surgery, there was no significant difference clinically and radiologically between the right and left hips for all patients. Surgical procedures required for both hips also were symmetric in 14 (93%) patients. However, asymmetric outcome occurred in four (27%) patients, three of whom were older than 5 years. An asymmetric outcome as defined is a potential risk after surgical treatment of bilateral developmental dysplasia of the hip in children who are older than 5 years.

The objective of this study was to determine the changes in nutritional quality of protein in stored UHT milk. Commercial fresh and recombined ultra high temperature (FUHT and RUHT respectively) treated milk samples were used. Pasteurized milk (P-milk) samples were used for comparison. Milk samples were freeze-dried for the use in diet formulation. Pasteurized milk samples were freeze-dried within 24 h of production. FUHT and RUHT milk samples were freeze-dried after production (zero time), 3 and 6 months of storage at 37 degrees C. Three nitrogen balance experiments were done at zero time, after 3 and 6 months of storage. The diet used in all experiments were similar in composition except for protein source. In each experiment egg white (low protein), casein (control), P-milk, and either 0, 3 or 6 months old FUHT and RUHT milk samples were used as the source of protein in the diets. In each experiment 30 (six for each diet) male Sprague-Dawley rats were used. Apparent digestibility (AD), true digestibility (TD), biological value (BV), and net protein utilization (NPU) were calculated for protein in casein and all milk samples. The nutritional quality of protein in FUHT milk before storage was similar to that of protein in pasteurized milk, but it decreased during storage for 3 months and then remained at the same quality up to 6 months of storage. BV and NPU of protein in FUHT milk after storage for 3 and 6 months were lower than that of protein in pasteurized milk by 4% (not significant) and 5% respectively. On the other hand, the nutritional quality of protein in RUHT milk before storage was lower than that of protein in pasteurized milk and remained at the same level after storage for 3 months: however, it decreased significantly after storage for 6 months. AD and TD of protein in RUHT milk stored for 6 months were lower than that of protein in pasteurized milk by 5% and BV and NPU were lower than that of protein in pasteurized milk by 9 and 12% respectively. The above reduction in nutritional quality of protein in UHT milk may be of significant value in feeding young children who might be dependent on this type of milk in their diet.

Rhinoscleroma is a very rare cause of upper airway obstruction with only isolated reports in the literature of rhinoscleroma with isolated tracheal obstruction. The course is usually chronic with the presentation most often being non-specific. We report a 54-year-old woman with progressive shortness of breath and wheezing over 7 years' duration. She was diagnosed and treated as bronchial asthma without improvement in her symptoms. At the time of referral to our institution, her flow-volume loop revealed fixed upper airway obstruction. Her chest radiography and other laboratory tests were normal. Bronchoscopy revealed a 70-80% irregular concentric stenosis of the trachea beginning immediately below the vocal cords and extending 4 cm distally. Biopsy showed characteristic Mikulicz histiocytes containing numerous gram-negative intracellular coccobacilli consistent with a diagnosis of rhinoscleroma. The patient was treated with laser resection of the stenosis followed by a course of ciprofloxcin and trimethoprim-sulfamethoxazole. She has remained asymptomatic over a year follow-up period and repeated biopsies have shown no evidence of recurrence.

An experiment with male albino rats was conducted to examine the effect of dietary fat sources on growth performance and apparent digestibility of total fat and fatty acids. The effect on plasma and organ cholesterol concentrations as well as lipoproteins cholesterol and fatty acid profile of hepatic phospholipids was also examined. Forty 3 week old rats were fed purified diets containing 0.041% cholesterol and 7% fat (wt/wt) from soy oil (SO), or cow ghee (GH), or 25% fish oil (FO) plus soy oil (SF) or 25% fish oil plus 75% ghee (GF), or 50% soy oil plus 50% ghee (SG) for a period of 35 days. Final body weight, weight gain, organ weights and feed efficiency ratio showed no significant differences (P < 0.05) among the dietary treatments. The apparent digestibility of total dietary fat and saturated fatty acids in the animals fed diet containing cow ghee only or in combination with fish oil or soy oil was significantly lower than the other dietary groups. Consumption of ghee with combination of fish oil or soy oil lead to significant improvement in the fat apparent digestibility of dietary fat. Total cholesterol and triglyceride concentrations of plasma and organs as well as lipoprotein cholesterol levels were higher in animals fed diets containing ghee. For rats fed diet containing cow ghee in combination with fish oil or soy oil, the cholesterol content of total plasma, lipoprotein fractions (VLDL, LDL and HDL) and organs was decreased significantly. Furthermore, dietary fats altered the fatty acids composition of hepatic phospholipid fatty acids composition. Feeding dietary fish oil reduced arachidonic acid (20:4) and increased linoleic acid (18:2) and eicosapentaenoic acid (20:5) contents. The reduction in the arachidonic acid was being more pronounced in animals fed dietary ghee with combination of fish oil.

BACKGROUND: Although the cause of adolescent idiopathic scoliosis remains unclear, pinealectomy is followed by the development of scoliosis in chickens. Melatonin is the only hormone secreted by pineal gland. OBJECTIVE: To assess serum melatonin levels in adolescents with idiopathic scoliosis. METHODS: Serum melatonin was assayed once, at 12:00 hours, in each of 20 adolescents with idiopathic scoliosis and 10 age-matched siblings. In the patient group, mean age was 14.3 years and the mean Cobb angle was 54.5 degrees (range, 20 degrees-110 degrees). Mean age in the control group was 14 years. RESULTS: Serum melatonin was significantly lower in the patients (mean, 7.7 pg/mL; range, 4 to 13 pg/mL) than in the controls (mean, 29.9 pg/mL; range, 19.3 to 46 pg/mL) (P < 0.00001). The 12 surgically treated patients had a mean melatonin level of 8.1 pg/mL, versus 7.2 pg/mL in the eight other patients (nonsignificant P and regression coefficient values). Serum melatonin was not significantly correlated with the Cobb's angle (regression coefficient, 0.18; P < 0.44). CONCLUSION: Our data suggest that serum melatonin levels may contribute to the pathogenesis of idiopathic scoliosis.

A fast method for sample treatment of phosphate rock has been developed for the purpose of quantitative leaching (98-100%) of fluoride but less of the interfering cations such as iron and aluminum. Citric acid (0.5 M) was used to extract fluoride in 15-45 min. Leaching of iron and aluminum is minimal, and these ions are complexed with citric acid. The leaching method was optimized with respect to sample size, citric acid concentration, leaching time and temperature. The analysis was completed by the rapid determination of fluoride with ion-selective electrode. The proposed treatment method was applied to phosphate rock samples from Jordan and Morocco and yielded accurate results as compared to the standard steam distillation from strong acid solution followed by thorium nitrate titration.

Most intra-abdominal and other types of fluid collections are now successfully drained percutaneously under image guidance. The utility of percutaneous drainage of tuberculous abscesses, especially those associated with osseous changes, is, however, less well established. Six patients with tuberculous iliopsoas abscesses were successfully managed by percutaneous drainage combined with antituberculous therapy. The abscesses were bilateral in one patient and unilateral in the other five. Drainage was by needle aspiration under ultrasound (US) guidance in one patient, and by catheter under CT guidance in the other patients. Three patients had associated osseous changes. There were no procedural complications. Tuberculous iliopsoas abscess can be successfully treated by percutaneous drainage and appropriate antituberculous therapy.

Twenty-eight Saudi Arabian common foods were analyzed for their fatty acid contents by gas-liquid chromatography using capillary column. The predominant fatty acids were oleic, palmitic, linoleic and stearic acids. The polyunsaturated to saturated fats ratio (P:S) as well as the n-3:n-6 ratio were generally not compatible with recommended values. Assessment of C20:4 (n-6) content in those foods showed a noticeable variation ranging from 9 mg/100 gm in beef sirloin to 256 mg/100 g in kannad fish. Eicosapentaenoic acid (C20:5) n-3 content was highest in kannad (925 mg/100 g). Applying C20:4 (n-6) values determined in this work to Saudi Balanced Sheet, the mean C20:4 (n-6) intake for a Saudian was estimated as approximately 115 mg/day.
