Diagnostic evaluation of institutions as a basis for designing the Brazilian maturity model of telehealth services.

BACKGROUND: The number and specificities of telehealth service units that expanded their services and diversified with the COVID-19 pandemic in Brazil need to be discovered. The objective of this manuscript is to present a methodology for the diagnostic evaluation of 19 telehealth units from different regions of the country for federal governmental decision-making. METHODS: A cross-sectional quantitative and qualitative study was carried out in the form of a census based on administrative records with an online survey and in-depth interviews with local telehealth managers. RESULTS: Despite the discontinuity of regular funding, the results point to a diversity of initiatives and advances. Citizenship, sustainability, security, and budget management are recurring themes in the maturity analysis of telehealth services after the advent of the pandemic. CONCLUSION: It is necessary for Brazil to build a resilient model of the maturity of telehealth services that contemplates the different regional scenarios.

Bancos de leche humana y promoción de políticas públicas favorables a la salud materno-infantil / Human milk bank and promotion of public policies in favor of maternal-child health

La política pública de salud orientada al incentivo de la lactancia materna en Brasil, tiene a lo largo de las últimas décadas su consolidación en los bancos de leche humana. Desde 1998 la Fundación “Oswaldo Cruz”, viene coordinando la elaboración y ejecución del proyecto de la Rede Nacional de Bancos de Leite Humano. Se identificó que el conocimiento es uno de los principales elementos que asegura la legitimación y sustentación académica de esta red. En este sentido el estudio y el análisis realizado identifican que, además de los movimientos de los actores sociales participantes, hay una dinámica de la producción de conocimiento que debe ser entendida. Se concluye que, en consecuencia de la comprensión de la génesis de la red y del análisis de una parte de su producción científica reciente, es posible percibir su potencial contribución para la promoción de transformaciones sociales y en la formulación de políticas públicas dirigidas hacia el área de la salud de la mujer y del infante.

The public health policy, aimed at encouraging breastfeeding in Brazil , has been consolidating in the human milk banks in the last few decades. Since 1998, “Osvaldo Cruz” Foundation has been coordinating the preparation and implementation of the project called National Network of Human Milk Bank . It was found that knowledge is one of the main elements assuring legitimization and academic substantiation of this network. In this regard, the conducted study and analysis identify that, in addition to the movement of the participating social actors, there is a dynamic production of pieces of knowledge that should be understood. It was concluded that, on the basis of the understanding of the network genesis and of the analysis of part of the recent scientific production, it is possible to perceive the potential contribution of this network to the promotion of social changes and the drawing up of public policies in the field of woman's and child's health.
