OBJECTIVES: The aims of this study were (1) to identify attributes for patient safety at a primary healthcare level and (2) to analyze conceptions of patients, professionals, and managers about how these attributes are being addressed. STUDY DESIGN: This was a qualitative study. METHODS: Participants were recruited from three primary care settings in Brazil. A total of 37 subjects (four physicians, three nurses, three dentists, three managers, five community assistants, and 19 patients) participated on interviews about their perceptions of safety attributes at the primary care settings involved in the study. Some of these participants attended a focus group meeting. A thematic categorical analysis was carried out to interpret the interviews. RESULTS: The main attributes for patient safety were valued by the participants. However, barriers such as discontinuity of care, interruptions during consultations, breakdowns in the communication, and ineffective teamwork were reported as frequent sources of patient safety issues. Reports of patients left unattended for excessive time because of the lack of accurate information and disruptions that took up to 35 min show that there is still a long way to go for primary care to be safe and effective in the study settings. CONCLUSIONS: It is necessary that the strategies meet the patient safety needs more effectively and efficiently. Further research is needed to understand the complex nature of the problems that affect patient safety in these settings so that appropriate decisions can be made.

PURPOSE: To evaluate macular and retinal nerve fibre layer (RNFL) measurements in patients with cyanotic congenital heart disease (cCHD) using spectral domain optical coherence tomography (OCT). PATIENTS AND METHODS: Thirty patients with cCHD (18 females and 12 males, mean age 10.9 years) and 60 healthy controls (35 females and 25 males, mean age 11.2 years) underwent complete ophthalmologic examination and OCT measurements of macular and peripapillary RNFL thickness. RESULTS: Patients with cCHD had significantly thinner measurements in all macular subfields compared with healthy controls (P<0.001). There was no significant difference in peripapillary RNFL thickness between the two groups, with the exception of the upper quadrant, for which thickness measurements were higher in patients with cCHD (P=0.021). CONCLUSIONS: Patients with cCHD showed a significant decrease in macular thickness and a thickened superior quadrant RNFL thickness when compared with healthy controls. This may represent the damage caused by the effect of hypoxia.

Reaction-diffusion systems may lead to the formation of steady-state heterogeneous spatial patterns, known as Turing patterns. Their mathematical formulation is important for the study of pattern formation in general and plays central roles in many fields of biology, such as ecology and morphogenesis. Here we show that Turing patterns may have a decisive role in shaping the abundance distribution of predators and prey living in patchy landscapes. We extend the original model proposed by Nakao and Mikhailov [Nat. Phys. 6, 544 (2010)] by considering food chains with several interacting pairs of prey and predators distributed on a scale-free network of patches. We identify patterns of species distribution displaying high degrees of apparent competition driven by Turing instabilities. Our results provide further indication that differences in abundance distribution among patches can be generated dynamically by self organized Turing patterns and not only by intrinsic environmental heterogeneity.

We consider the classical dynamics of two particles moving in harmonic potential wells and interacting with the same external environment HE, consisting of N noninteracting chaotic systems. The parameters are set so that when either particle is separately placed in contact with the environment, a dissipative behavior is observed. When both particles are simultaneously in contact with HE an indirect coupling between them is observed only if the particles are in near-resonance. We study the equilibrium properties of the system considering ensemble averages for the case N=1 and single trajectory dynamics for N large. In both cases, the particles and the environment reach an equilibrium configuration at long times, but only for large N can a temperature be assigned to the system.

This study describes the oral and maxillofacial pathological characteristics of a series of odontogenic tumours in children and adolescents from three Brazilian reference centres. The records were reviewed for all odontogenic tumours in patients up to 18 years old based on criteria proposed by the World Health Organization (WHO) in 2005. Data concerning sex, age, skin colour and tumour location were collected and plotted. Four hundred and thirty one odontogenic tumours in children and adolescents were found, accounting for 37.5% of the total number of odontogenic tumours diagnosed. Benign tumours were predominant (99.8% of the cases), and odontoma was the most frequent type (41.4%), followed by keratocystic odontogenic tumours (25.5%) and ameloblastoma (14.6%). Odontogenic tumours were rarely detected in early childhood, and their prevalence increased with age. An almost equal distribution was observed with respect to sex and the site of the lesions. This study is the largest reported retrospective analysis describing odontogenic tumours in children and adolescents to date. The authors detected some variation in the relative frequency of odontogenic tumours compared with similar reports. Additional studies should be conducted based on the new WHO classification and predetermined age parameters to enable comparative analysis among different worldwide populations.

We study the flow of energy between a harmonic oscillator (HO) and an external environment consisting of N two-degrees-of-freedom nonlinear oscillators, ranging from integrable to chaotic according to a control parameter. The coupling between the HO and the environment is bilinear in the coordinates and scales with system size as 1/âN. We study the conditions for energy dissipation and thermalization as a function of N and of the dynamical regime of the nonlinear oscillators. The study is classical and based on a single realization of the dynamics, as opposed to ensemble averages over many realizations. We find that dissipation occurs in the chaotic regime for fairly small values of N, leading to the thermalization of the HO and the environment in a Boltzmann distribution of energies for a well-defined temperature. We develop a simple analytical treatment, based on the linear response theory, that justifies the coupling scaling and reproduces the numerical simulations when the environment is in the chaotic regime.

We investigated the phenomenon of population outbreaks in a spatial predator-prey model, and we found that pattern formation and outbreaks occur if the predators have a limited neighbourhood of interaction with the preys. The outbreaks can display a scale-invariant power-law tail, indicating self-organized criticality. We have also studied the system from an evolutionary point of view, where the predator home range is a hereditary trait subjected to mutations. We found that mutation drives the predator home range area to an optimal value where pattern formation and outbreaks are still present, but the latter are much less frequent. We developed analytical approximations using mean field and pair correlation techniques that indicate that the predation strategy is crucial for existence of this optimal home range area.

The nature and frequency of cystic fibrosis mutations in Brazil is not uniform due to the highly varied ethnic composition of the population. The average frequency of the F508del mutation has been reported to be 48.6%. Other common mutations in Brazil are G542X, R1162X, and N1303K. The aim of this study was to analyze the frequency of 8 mutations (F508del, G542X, R1162X, N1303K, W1282X, G85E, 3120+1G>A, and 711+1G>T) in a sample of 111 newborn patients with cystic fibrosis diagnosed by the Cystic Fibrosis Neonatal Screening Program of Minas Gerais State. The mutations were tested by allele-specific oligonucleotide PCR with specially designed primers. An allele frequency of 48.2% was observed for the F508del mutation, and allele frequencies of 5.41, 4.50, 4.05, and 3.60% were found for the R1162X, G542X, 3120+1G>A, and G85E mutations, respectively. The genotypes obtained were in Hardy-Weinberg equilibrium. These data demonstrate that the 8-mutation panel studied here has extensive coverage (68%) for the cystic fibrosis mutations in Minas Gerais. These data improve our knowledge of cystic fibrosis in Brazil, particularly in this region. In addition, this investigation contributed to the establishment of a sensitive and population-specific mutation panel, which can be helpful for molecular diagnosis of cystic fibrosis.

We study the stability and synchronization of predator-prey populations subjected to noise. The system is described by patches of local populations coupled by migration and predation over a neighborhood. When a single patch is considered, random perturbations tend to destabilize the populations, leading to extinction. If the number of patches is small, stabilization in the presence of noise is maintained at the expense of synchronization. As the number of patches increases, both the stability and the synchrony among patches increase. However, a residual asynchrony, large compared with the noise amplitude, seems to persist even in the limit of an infinite number of patches. Therefore, the mechanism of stabilization by asynchrony recently proposed by Abta [Phys. Rev. Lett. 98, 098104 (2007)], combining noise, diffusion, and nonlinearities, seems to be more general than first proposed.

We study the dynamics of populations of predators and preys using a mean field approach and a spatial model. The mean field description assumes that the individuals are homogeneously mixed and interact with one another with equal probability, so that space can be ignored. In the spatial model, on the other hand, predators can prey only in a certain neighborhood of their spatial location. We show that the size of these predation neighborhoods has dramatic effects on the dynamics and on the organization of the species in space. In the case of a three species food chain, in particular, the populations of predators display a sequence of apparently irregular outbreaks when the predation neighborhood has intermediate values, as compared to the size of the available space. Nonetheless, further increasing their size makes the outbreaks disappear and the dynamics approach that of the mean field model. Our study of synchronization also shows that the periodic behavior displayed by the average populations in a spatially extended system may hide the existence of patches that oscillate out of phase in a highly coordinated fashion.

AIM: To investigate the accuracy of histological diagnosis of oral hemangioma, oral vascular malformation and oral pyogenic granuloma according to immunohistochemical evaluation of the human erythrocyte-type glucose transporter protein (GLUT-1), and to observe the immunoexpression of this protein in oral varix. MATERIALS AND METHODS: Immunohistochemistry for GLUT-1 was performed in 93 histologically diagnosed cases of oral benign vascular lesions: 17 vascular malformations, 19 hemangiomas, nine varix, and 48 pyogenic granulomas. Descriptive analyses were performed. RESULTS: None of the cases of the oral benign vascular lesions evaluated were immunopositive to GLUT-1. The 19 cases histologically diagnosed as oral hemangioma that showed negative staining to GLUT-1 were reclassified as oral pyogenic granuloma or oral vascular malformations. The histological evaluation itself is not enough to obtain the correct diagnosis of oral HEM as none of the sample cases were true hemangioma. All sample cases with initial vascular malformation or pyogenic granulomas classification were negative to GLUT-1, demonstrating the accuracy of histological diagnosis of these lesions itself. Oral varix showed negative staining to GLUT-1 in blood vessels. CONCLUSIONS: GLUT-1 is an useful, effective and important auxiliary marker for the diagnosis of oral benign vascular lesions. CLINICAL RELEVANCE: This study showed that histological diagnosis alone is not sufficient to correct diagnoses of oral hemangioma. Moreover, immunohistochemistry to GLUT-1 is a useful and easy diagnostic method that may be used to avoid such misdiagnosis. Accurate diagnosis of these oral lesions has an important clinical relevance allowing: (1) correct management, (2) adequate communication among the multidisciplinary team (dentist, dermatologist, pediatrist, radiologist, pathologist, and surgeon), (3) understanding of the biological behavior of the lesions, and (4) facilitate the development of new therapeutic modalities. Thus, supporting the use of this marker in medical and dentistry communities is warranted.

We propose a conjugate application of the Bargmann representation of quantum mechanics. Applying the Maslov method to the semiclassical connection formula between the two representations, we derive a uniform semiclassical approximation for the coherent-state propagator which is finite at phase space caustics.

Two cases are presented of a nasolabial cyst (NC) on which a radiographic contrast medium was used in order to define the real extension of the lesion. Use of contrast is important for visualizing the definite extensions of NC primarily in cases when plain radiographs may not show any detectable changes and CT can not be used.

The semiclassical limit of the coherent state propagator

Holoprosencephaly (HPE) is genetically heterogeneous with four genes, SIX3, SHH, TGIF, and ZIC2 that have been identified to date and that are altered in 12% of patients. To analyze this prevalence in a South American population-based sample (57 HPE cases in 244,511 live and still births or 1 in 4300), we performed a mutational study of these genes in 30 unrelated children (26 newborns and 4 non-newborns) with HPE being ascertained by ECLAMC (Latin American Collaborative Study of Congenital Malformations). We identified three novel mutations: two were missense mutations of the SHH gene (Cys183-->Phe; His140-->Pro); the third mutation was a 2-bp deletion in the zinc-finger region of the ZIC2 gene. These molecular results explained 8% (2/26 newborn samples) of the HPE cases in this South American population-based sample, a proportion similar to our previously published data from a collection of cases.

OBJECTIVE: The purpose of this study was to evaluate retrospectively the profile of patients with oral paracoccidioidomycosis referred to two Dental Schools in Belo Horizonte (MG, Brazil) between 1955 and 1998. Despite the importance of the oral manifestations of this disease, few papers in the English literature have provided epidemiological data. SUBJECTS AND METHODS: The medical records of 62 patients presenting oral paracoccidioidomycosis were reviewed in detail. Patient age, gender, race, occupation, site of lesion and type of clinical manifestation of the disease were tabulated. RESULTS: There was a predominance of male patients (97%), with a male:female ratio of 30:1. The mean age was 40 years. Most of the patients were farm workers (53%). Some patients presented multiple oral lesions (19 cases, 30%). The fungal lesions were found principally in the alveolar process and gingiva, but were also seen on the palate, lip and buccal mucosa. All patients had chronic proliferative mulberry-like ulcerated oral lesions and the diagnosis was confirmed histologically. The clinical records did not contain notes about pulmonary involvement by the lesions. CONCLUSION: The purpose of this study was to describe the epidemiological profile of a specific population with the diagnosis of oral paracoccidioidomycosis. The major goal is to establish a scientific basis for initiating educational programs for prevention and early diagnosis of oral paracoccidioidomycosis.

We study the near equilibrium dynamics of nonhomogeneous elastic filaments in viscous media using the Kirchhoff model of rods. Viscosity is incorporated in the model as an external force, which we approximate by the resistance felt by an infinite cylinder immersed in a slowly moving fluid. We use the recently developed method of Goriely and Tabor [Phys. Rev. Lett. 77, 3537 (1996); Physica D 105, 20 (1997); 105, 45 (1997)] to study the dynamics in the vicinity of the simplest equilibrium solution for a closed rod with nonhomogeneous distribution of mass, namely, the planar ring configuration. We show that small variations of the mass density along the rod are sufficient to couple the symmetric modes of the homogeneous rod problem, producing asymmetric deformations that modify substantially the dynamical coiling, even at quite low Reynolds number. The higher-density segments of the rod tend to become more rigid and less coiled. We comment on possible applications to DNA.

OBJECTIVE: To perform a retrospective analysis of patients with nasopalatine duct cysts (NPDC) in Brazil and compare with previous analyses. MATERIALS AND METHODS: The files of the Laboratory of Oral Pathology (Minas Gerais Federal University) from 1966 to 1997 were reviewed. Demographic, clinical and histologic data of patients with nasopalatine duct cysts were collected. RESULTS: The mean age of patients with nasopalatine duct cysts at the time of diagnosis was 37.4 years and with a predilection for males. The majority of cases were asymptomatic. Histologically, non-keratinized stratified squamous epithelium alone or in combination with other epithelia was observed in 93% of the cases. Recurrence was not recorded. CONCLUSION: The demographic, histopathological, radiographic, and clinical data of the NPDC in our series are similar to previous studies in other populations.

The dentinogenic ghost-cell tumor is a rare solid variant of the calcifying odontogenic cyst. Few peripheral cases of this tumor with clinical and radiographic documentation have been reported. A case of peripheral dentinogenic ghost-cell tumor is presented and the literature is reviewed.
