INTRODUCTION: Osteogenesis imperfecta (OI) is a congenital disease comprising a heterogeneous group of inherited connective tissue disorders. The main treatment in children is bisphosphonate therapy. Previous animal studies have shown that bisphosphonates delay tooth eruption. The aim of this study is to determine whether patients with OI treated with pamidronate and/or zoledronic acid have a delayed eruption age compared to a control group of healthy children. METHODS: An ambispective longitudinal cohort study evaluating the age of eruption of the first stage mixed dentition in a group of children with OI (n = 37) all treated with intravenous bisphosphonates compared with a group of healthy children (n = 89). Within the study group, the correlation (Pearson correlation test) between the type of medication administered (pamidronate and/or zoledronic acid) and the chronology of tooth eruption is established, as well as the relationship between the amount of cumulative dose received and tooth eruption. RESULTS: The age of eruption of the study group was significantly delayed compared to the age of eruption of the control group for molars and lateral incisors (p < 0.05). Patients who received higher cumulative doses had a delayed eruption age compared to those with lower cumulative doses (p < 0.05). There is a high positive correlation between age of delayed tooth eruption and Zoledronic acid administration. CONCLUSION: Patients with OI have a delayed eruption of the 1st stage mixed dentition compared to a control group of healthy children. This delayed eruption is directly related to the cumulative dose of bisphosphonates and the administration of zoledronic ac.

The aim of the study was to establish the normality ranges of the linear and angular measurements that can be carried out in orthopantomographies (OPGs) of a paediatric sample from Madrid, according to sex and dentition. OPGs performed in the Radiology Service of the Faculty of Dentistry of the Complutense University of Madrid (UCM) from caucasic children between 4-14 years old were selected. A total of 44 measurements were made in the OPGs, and the sex and type of dentition of each child were recorded. Panoramic Mandibular Index and Antegonial Index were also calculated. Statistical tests were performed with a confidence level of 95% (p < 0.05) and bilateral significance to analyse the differences between sex and type of dentition, and the correlation between the measurements of the right and left sides. A total sample of 160 OPGs (50% boys, 50% girls) were analysed. 16.25% of the sample was in primary dentition, 50% in first phase mixed dentition, 17.5% in second phase mixed dentition and 16.25% in permanent dentition. Statistically significant differences were found with respect to the sex of the subjects in 11 of the measurements; and 44 in relation to the dentition stages of the subjects. A strong correlation is found between the measurements on the right and left sides. The application of panoramic measurements as indicators of normality can help in the detection of craniofacial alterations in growth and development of the lower facial third.

AIM: To analyse differences in the eruption of primary teeth between both sexes. MATERIALS AND METHODS: A cross-sectional study was performed in a sample of 1250 children aged between 3 and 42 months. The clinical emergence of teeth was taken to estimate average ages for primary teeth eruption. The t-test was used to analyse gender and arch differences. RESULTS: Eruption of primary teeth began at the same time in girls as in boys; although the eruptive process was longer in girls. It was observed that almost all deciduous teeth emerged earlier in boys, except for the first molar (upper and lower) and the maxillary central incisors. The differences were statistically significant for almost all the primary teeth except for the central incisors, both upper and lower. CONCLUSIONS: By comparing the timing of the eruption of primary teeth in both sexes, it was found that eruption of almost all primary teeth took place earlier in boys than in girls and that the eruption process was shorter for boys than for girls. Most of the deciduous teeth erupted earlier in the right side in both sexes. No differences were found in the sequence of eruption of primary teeth between genders.

Introducción: La hipofosfatasia es un trastorno raro hereditario caracterizado por un defecto en la mineralización del hueso y de los dientes y una disminución de la fosfatasa alcalina sérica que cursa con una mineralización anómala del hueso, exfoliación prematura de los dientes y atrofia ósea severa. Mientras que las talasemias son un grupo de anemias hereditarias caracterizadas por la disminución de la síntesis de cadenas polipeptídicas de la hemoglobina que cursan con desarrollo puberal tadío, fragilidad ósea, insuficiencia cardiaca y anemia. El objetivo de la presente publicación es exponer un caso clínico con la asociación de ambas enfermedades y evaluar su manejo odontoestomatológico. Caso clínico: Se presenta el caso de una mujer de 55 años remitida por problemas de estabilidad en su prótesis inferior. La paciente ha sido diagnosticada de hipofosfatasia en forma adulta y talasemia menor con antecedentes quirúrgicos de cesárea y seis intervenciones por fracturas. Tras valorar las diferentes opciones terapéuticas y una vez informada y habiendo obtenido el consentimiento por parte de la paciente se procedió a la adaptación de su prótesis inferior mediante el ajuste con dos locators. Conclusiones: La hipofosfatasia es una enfermedad rara que debe estar en conocimiento de los profesionales de la salud bucodental (AU)


Introduction: The hypophospatasia is a rare inherited disorder characterized by a defect in the mineralization of bone and teeth and a decrease in serum alkaline phosphatase that causes anormal bone mineralization, premature exfoliation of teeth and severe bone atrophy. While the thalassemia are a group of inherited anemias characterized by decreased synthesis of polypeptide chains of hemoglobin that course with late puberal development , brittle bones, heart failure and anemia. The purpose of this publication is to present a case with the association of both diseases and to evaluate its dental management. Case Report: a case report of a 55 year old woman referred by stability problems in the lower denture. The patient has been diagnosed in adult form of hypophosphatasia and β-thalassemia minor. She refers having been subjected to a caesarean and six surgical interventions for bone fractures. After assessing the different therapeutic options and having obtained the informed consent from the patient, her lower overdenture was adapted adding two locators. Conclusions: hypophosphatasia is a rare disease that should be in the knowledge of the professionals of the oral healt (AU)

RESUMO

Niño, varón de 9 años de edad, que acude a consulta por primera vez a la Facultad de Odontología de la uCM, para recibir atención odontológica. La exploración clínica, pone de manifiesto que se encuentra en dentición mixta primera fase, y llama la atención la persistencia del incisivo central primario maxilar del lado izquierdo (6.1), estando erupcionados el resto de incisivos maxilares permanentes (Figura 1). En la radiografía panorámica solicitada para el diagnóstico de la situación y evaluación del desarrollo dentario, se confirma la presencia de un diente supernumerario que bloquea la trayectoria eruptiva del incisivo central maxilar izquierdo (retenido)(Figura 2). En colaboración con el Servicio de Cirugía Oral de la Facultad de odontología de la uCM se decide la exodoncia del diente primario, y tras la realización de un colgajo completo, la extracción también del supernumerario (Figuras 3 a 5). RESUMO

The Spanish Public Health System is stepping up its efforts to meet all the medical needs of the population. Oral health is of increasing interest for society, especially for parents who are keen for their children to have healthy teeth. Disabled children with both physical and mental disabilities do not always receive the dental care they need. The purpose of this bibliographical review is to evaluate the services provided by the Spanish Public Health System to such children. We have noted marked differences in the types of dental treatment given to these patients in the different Autonomous Communities of Spain. Some, such as Asturias, Navarra and Extremadura, offer specific care for disabled children. Others, such as Ceuta and Melilla, provide more general care.
