RESUMO

Wheat aging plays an important role in assessing storage wheat quality and its subsequent processing purposes. The conventional detection methods for wheat aging are mainly involved in chemical techniques, which are time-consuming as well as waste part of wheat samples for each detection. Although some physical detection methods have obtained gratifying results, it is extremely hard to expand their application fields but to stay in the theory stage. For this reason, a novel nondestructive detection model for wheat aging based on the delayed luminescence (DL) has been proposed in this paper. Specifically, after collecting enough sample data, we first took advantage of certain hyperbolic function to fit DL signal, and then used four parameters of the hyperbolic function to feature the decay trend of the DL signal. Secondly, in order to better feature the DL signal, we extracted other six features together with above four features to form the input feature vector. Finally, as the bidirectional long short-term memory (Bi-LSTM) network lacked error-correcting performance, the Bi-LSTM network based on Walsh coding (Walsh-Bi-LSTM) mechanism was proposed to establish the detection model, which made the detection model have the error-correcting performance by reasonably splitting the multi-classification target task. Shown by experimental results, the newly proposed wheat aging detection model is able to achieve 94.00% accuracy in the testing dataset, which can be used as a green and nondestructive method to timely reflect wheat aging states.

RESUMO

Background/Aims@#Roxadustat, an oral medication for treating renal anemia, is a hypoxia-inducible factor prolyl hydroxylase inhibitor used for regulating iron metabolism and promoting erythropoiesis. To investigate the efficacy and safety of roxadustat in patients undergoing peritoneal dialysis (PD) with erythropoietin hyporesponsiveness. @*Methods@#Single-center, retrospective study, 81 PD patients (with erythropoietin hyporesponsiveness) were divided into the roxadustat group (n = 61) and erythropoiesis-stimulating agents (ESAs) group (n = 20). Hemoglobin (Hb), total cholesterol, intact parathyroid hormone (iPTH), brain natriuretic peptide (BNP), related indicators of cardiac function and high-sensitivity C-reactive protein (hs-CRP) were collected. Additionally, adverse events were also recorded. The follow-up period was 16 weeks. @*Results@#The two groups exhibited similar baseline demographic and clinical characteristics. At baseline, the roxadustat group had a mean Hb level of 89.8 ± 18.9 g/L, while the ESAs group had a mean Hb level of 95.2 ± 16.0 g/L. By week 16, the Hb levels had increased to 118 ± 19.8 g/L (p 0.05) in the ESAs group. The efficacy of roxadustat in improving anemia was not influenced by baseline levels of hs-CRP and iPTH. Cholesterol was decreased in the roxadustat group without statin use. An increase in left ventricular ejection fraction and stabilization of BNP were observed in the roxadustat group. @*Conclusions@#For PD patients with erythropoietin hyporesponsiveness, roxadustat can significantly improve renal anemia. The efficacy of roxadustat in improving renal anemia was not affected by baseline levels of hs-CRP0 and iPTH.

RESUMO

BACKGROUND:Due to the sudden release and the rapid removal by proteases,platelet-rich plasma hydrogel leads to shorter residence times of growth factors at the wound site.In recent years, researchers have focused on the use of hydrogels to encapsulate platelet-rich plasma in order to improve the deficiency of platelet-rich plasma hydrogels. OBJECTIVE:To prepare self-assembled polypeptide-platelet-rich plasma hydrogel and to explore its effects on the release of bioactive factors of platelet-rich plasma. METHODS:The self-assembled polypeptide was synthesized by the solid-phase synthesis method, and the solution was prepared by D-PBS.Hydrogels were prepared by mixing different volumes of polypeptide solutions with platelet-rich plasma and calcium chloride/thrombin solutions,so that the final mass fraction of polypeptides in the system was 0.1%,0.3%,and 0.5%,respectively.The hydrogel state was observed, and the release of growth factors in platelet-rich plasma was detected in vitro. The polypeptide self-assembly was stimulated by mixing 1%polypeptide solution with 1%human serum albumin solution,so that the final mass fraction of the polypeptide was 0.1%,0.3%,and 0.5%,respectively.The flow state of the liquid was observed, and the rheological mechanical properties of the self-assembled polypeptide were tested. The microstructure of polypeptide(mass fraction of 0.1%and 0.001%)-human serum albumin solution was observed by scanning electron microscope and transmission electron microscope. RESULTS AND CONCLUSION:(1)Hydrogels could be formed between different volumes of polypeptide solution and platelet-rich plasma.Compared with platelet-rich plasma hydrogels,0.1%and 0.3%polypeptide-platelet-rich plasma hydrogels could alleviate the sudden release of epidermal growth factor and vascular endothelial growth factor, and extend the release time to 48 hours.(2)After the addition of human serum albumin, the 0.1%polypeptide group still exhibited a flowing liquid, the 0.3%polypeptide group was semi-liquid,and the 0.5%polypeptide group stimulated self-assembly to form hydrogel. It was determined that human serum albumin in platelet-rich plasma could stimulate the self-assembly of polypeptides. With the increase of the mass fraction of the polypeptide, the higher the storage modulus of the self-assembled polypeptide, the easier it was to form glue.(3)Transmission electron microscopy exhibited that the polypeptide nanofibers were short and disordered before the addition of human serum albumin. After the addition of human serum albumin, the polypeptide nanofibers became significantly longer and cross-linked into bundles, forming a dense fiber network structure.Under a scanning electron microscope, the polypeptides displayed a disordered lamellar structure before adding human serum albumin. After the addition of human serum albumin, the polypeptides self-assembled into cross-linked and densely arranged porous structures.(4)In conclusion, the novel polypeptide can self-assemble triggered by platelet-rich plasma and the self-assembly effect can be accurately adjusted according to the ratio of human serum albumin to polypeptide. This polypeptide has a sustained release effect on the growth factors of platelet-rich plasma, which can be used as a new biomaterial for tissue repair.

RESUMO

BACKGROUND:A large number of domestic and international documents have confirmed that elevated interleukin-1β is associated with primary frozen shoulder.Interleukin-1B gene polymorphisms can affect the transcription and protein expression of interleukin 1β-related genes, resulting in altered levels of cytokines in vivo, and thus altering the incidence of primary frozen shoulder.Through the study of interleukin-1B gene polymorphism and susceptibility to primary frozen shoulder, this study aimed to explore new breakthroughs in the pathogenesis of primary frozen shoulder from the perspective of molecular biology, and to search for susceptibility genes of primary frozen shoulder. OBJECTIVE:To explore the association between linkage disequilibrium of three gene loci in interleukin-1B gene and susceptibility to primary frozen shoulder. METHODS:A case-control study was conducted. There were two groups in this study. One group consisted of 184 patients with primary frozen shoulder, while the other group included 260 healthy controls. The genotypes of interleukin-1B gene loci-511C/T(rs16944),+3954C/T(rs1143634),and-31C/T(rs1143627)were detected by polymerase chain reaction and restriction fragment length polymorphism. The correlation between the probability of linkage disequilibrium and haplotypes and the risk of primary frozen shoulder disease was compared and analyzed. RESULTS AND CONCLUSION:Unconditional Logistic regression analysis showed that the proportion of CT genotypes at rs1143634 and rs1143627 sites increased significantly in the primary frozen shoulder.Linkage disequilibrium analysis showed that rs16944,rs1143634 and rs1143627 tended to be balanced in the control group(D'value<0.1),while there was a certain degree of linkage disequilibrium at rs1143627 and rs1143634 sites in the primary frozen shoulder group(D'value=0.595).Haplotype TTT increased the risk of primary frozen shoulder by 6.66 times compared with CCT type(TTT,OR=6.66,95%CI=1.59-27.88,P=0.009 7).To conclude, there is a certain degree of linkage disequilibrium between interleukin-1B gene loci rs1143627and rs1143634 in patients with primary frozen shoulder;haplotype TTT formed by these three gene loci may increase the risk of developing primary frozen shoulder.

RESUMO

Objective:To explore the possible risks of re-fracture after bone healing in children with congenital pseudarthrosis of the tibia, who were treated with combined surgery.Methods:56 children (35 males and 21 females, with an average age of 38.0±9.2 months ranging from 18 to 66 months) with re-fracture after bone healing were retrospectively enrolled from January 2007 to August 2016, during which a total of 142 children with CPT underwent combined surgical treatment in the orthopedics department of Hunan Children's Hospital. Complete healing of the tibial pseudarthrosis, trauma after surgery, breakage of fibula, and the distal end of the tibial intramedullary rod located in the tibial medullary cavity or not were hypothesized as 4 risk factors. Univariate logistic regression analysis was conducted to investigate the correlation between these factors and re-fractures after tibial pseudarthrosis healing. Results:The average follow-up was 81.7±10.8 months ranging from 60 to 120 months with at least 5 years after bone healing. For complete or partial healing of the tibial pseudarthrosis after surgery, the number of re-fractures was 48 and 8, respectively; For with or without a history of trauma, the number of re-fractures was 50 and 6, respectively; for with an intact or broken fibula after surgery, the number of re-fractures was 7 and 49, respectively; For entry or no entry of the intramedullary rod into the tibial medullary cavity, the number of re-fractures was 44 and 12, respectively. The results of univariate logistic regression analysis showed that partial healing of the tibial pseudarthrosis after surgery [ OR=0.255, 95% CI (0.107, 0.605), P=0.002], history of trauma [ OR=36.458, 95% CI (13.332, 99.701), P<0.001], incomplete fibula [ OR=0.267, 95% CI (0.108, 0.661), P=0.004], and intramedullary rod insertion into the tibial medullary cavity [ OR=2.640, 95% CI (1.224, 5.695), P=0.013] were associated with re-fracture after bone healing. The number and proportion of cases with recurrent fractures occurring ≤1, 1-3, 3-6, ≥6 years after bone healing were 5 cases, 9% (5/56), 14 cases, 25% (14/56), 22 cases, 39% (22/56), 15 cases, and 27% (15/56), respectively, the difference was statistically significant (χ 2=11.569, P=0.009). With the extension of follow-up time, the number of cases of re-fractures after bone healing increases, mostly occurring more than one year after bone healing. There were 44 cases (47%, 44/94) and 12 cases (25%, 12/48) of re fractures after bone healing in 94 cases of distal intramedullary rods in the tibial medullary cavity and 48 cases of cross ankle joint fixation, respectively. The difference in the incidence of re-fractures was statistically significant (χ 2=6.327, P=0.018). The incidence of intramedullary rod displacement in cases where the distal end of the intramedullary rod is located within the tibial medullary cavity was 100%. Conclusions:Factors of partial healing of the tibial pseudarthrosis, a history of trauma, incomplete status of the fibula after surgery, and intramedullary rod's entry into the tibia were risk factors for re-fracture after bone healing treated with combined surgery for CPT. After the healing of the tibial pseudarthrosis, it is not advisable to push the tibial intramedullary rod into the tibial medullary cavity, which can cause unstable fixation of the tibial intramedullary rod and result in displacement, and even affect the development of the tibial mechanical axis or the occurrence of re-fractures.

RESUMO

Plastics have brought invaluable convenience to human life since it was firstly synthesized in the last century. However, the stable polymer structure of plastics led to the continuous accumulation of plastic wastes, which poses serious threats to the ecological environment and human health. Poly(ethylene terephthalate) (PET) is the most widely produced polyester plastics. Recent researches on PET hydrolases have shown great potential of enzymatic degradation and recycling of plastics. Meanwhile, the biodegradation pathway of PET has become a reference model for the biodegradation of other plastics. This review summarizes the sources of PET hydrolases and their degradation capacity, degradation mechanism of PET by the most representative PET hydrolase-IsPETase, and recently reported highly efficient degrading enzymes through enzyme engineering. The advances of PET hydrolases may facilitate the research on the degradation mechanism of PET and further exploration and engineering of efficient PET degradation enzymes.

RESUMO

Objective@#To understand the longitudinal relationship between body shame, abnormal eating behavior and eating disorder tendency of adolescents, so as to provide reference for the intervention of eating problems among adolescents.@*Methods@#From September 2020, a total of 1 097 students from two high schools and two universities in Heilongjiang Province were investigated for three times (T1, T2 and T3) with an interval of 9 months. Adolescents completed the Chinese Body Shame Scale, the Chinese Version of Dutch Eating Behavior Questionnaire and the Sick, Control, One, Fat, Food(SCOFF) questionnaire. A cross lag analysis was used to explore the relationship between body shame, abnormal eating behavior and eating disorder tendency.@*Results@#The body shame, abnormal eating behavior and eating disorder tendency of adolescents showed an upward trend, but only the main effect of time on abnormal eating behavior was statistically significant ( F=3.78, P<0.05, η 2=0.18), and the main effect of gender on three variables were statistically significant ( F=18.06, 30.48, 25.09, P <0.01). There were significant and positive correlations between body shame, abnormal eating behavior and eating disorder tendency at three wave survey (T1: r =0.34-0.58, T2: r =0.35-0.56, T3: r =0.33-0.53, P <0.01). Body shame could predict abnormal eating behavior across time ( β T1-T2 =0.13, β T2-T3 =0.08, P < 0.05), and the predictive effect was stronger in female ( β T1-T2 =0.16, β T2-T3 =0.12, P <0.05), only the effect between T1 and T2 was significant in male ( β T1-T2 =0.09, P <0.05). Abnormal eating behavior ( β T1-T2 =0.14, β T2-T3 =0.15, P <0.01) and eating disorder tendency ( β T1-T2 =0.26, β T2-T3 =0.24, P <0.01) had cross time predictive effect. Abnormal eating behavior showed a mediating effect on the association between body shame and eating disorder tendency ( β ab =0.019, 95% CI=0.006-0.047, P <0.05), and the mediating effect was stronger in female ( β ab =0.029, 95% CI=0.008-0.053, P <0.05).@*Conclusion@#Adolescents body shame positively predicts abnormal eating behavior. Abnormal eating behavior and eating disorder tendency interact with each other, and body shame can indirectly affect eating disorder tendency through acting on abnormal eating behavior.

RESUMO

The incidence of chronic kidney disease (CKD) is increasing worldwide and the current prevalence rate is 13.4%. There are > 120 million CKD patients in China and this number is expected to increase. One of the main abnormalities in patients with CKD and kidney impairment is decreased synthesis of erythropoietin (EPO), which causes anemia and affects iron metabolism. The probability of developing is higher in anemia patients with CKD than in the general population, and the incidence increases as kidney function decreases. Deficient EPO production by the kidney is the most important cause of renal anemia. Notably, anemia in patients with CKD has multiple causes, such as bleeding caused by platelet dysfunction, iron deficiency due to digestive and absorption disorders of the gastrointestinal tract, and shorter red blood cell life. Anemia is also a leading cause of hospitalization in patients with CKD. A new oral medication to treat renal anemia, the hypoxia-inducible factor prolyl hydroxylase inhibitor called roxadustat (FG-4592), regulates iron metabolism and promotes erythropoiesis. This drug has a therapeutic effect on patients with CKD. Roxadustat showed advantages over EPO in clinical experiments. This review summarizes the mechanisms of action, clinical applications, effectiveness, and safety of roxadustat.

RESUMO

Objective To analyze the risk factors and build a clinical prediction model for hemodynamic depression (HD) after carotid artery stenting (CAS). Methods A total of 116 patients who received CAS in the Department of Vascular Surgery,Drum Tower Clinical College of Nanjing Medical University and the Department of Vascular Surgery, the Affiliated Suqian First People's Hospital of Nanjing Medical University from January 1,2016 to January 1,2022 were included in this study. The patients were assigned into a HD group and a non-HD group. The clinical baseline data and vascular disease characteristics of each group were collected, and multivariate Logistic regression was employed to identify the independent predictors of HD after CAS and build a clinical prediction model. The receiver operating characteristic (ROC) curve was drawn, and the area under the ROC curve (AUC) was calculated to evaluate the predictive performance of the model. Results The HD group had lower proportions of diabetes (P=0.014) and smoking (P=0.037) and higher proportions of hypertension (P=0.031),bilateral CAS (P=0.018),calcified plaque (P=0.001),eccentric plaque (P=0.003),and the distance<1 cm from the minimum lumen level to the carotid bifurcation (P=0.009) than the non-HD group. The age, sex, coronary heart disease, symptomatic carotid artery stenosis, degree of stenosis, and length of lesions had no statistically significant differences between the HD group and the non-HD group (all P>0.05).Based on the above predictive factors,a clinical prediction model was established, which showed the AUC of 0.807 and the 95% CI of 0.730-0.885 (P<0.001).The model demonstrated the sensitivity of 62.7% and the specificity of 87.7% when the best cut-off value of the model score reached 12.5 points. Conclusions Diabetes, smoking, calcified plaque, eccentric plaque, and the distance<1 cm from the minimum lumen level to the carotid bifurcation are independent predictors of HD after CAS.The clinical prediction model built based on the above factors has good performance in predicting the occurrence of HD after CAS.

RESUMO

Myopia has become a serious public health problem, but its pathogenesis is still unclear, and effective interventions are relatively scarce. It is recognized that myopia is influenced by both genetic and environmental factors, in which epigenetics may play a key role.Epigenetics refers to the changes in gene expression and function that do not involve DNA sequence variation.Mainly including DNA methylation, non-coding RNA (microRNA, long non-coding RNA and circular RNA, etc.), histone modification and mRNA modification, epigenetic modifications interact to form a complex regulatory network in the pathophysiological process of myopia.By controlling the process of scleral matrix remodeling, eye cell proliferation and retinal development, the morphological characteristics of the eye are jointly regulated, ultimately affecting the onset and development of myopia.Epigenetics has provided new targets of myopia intervention and has become a hotspot in the field. In this paper, we reviewed the current findings of myopia epigenetics to provide a reference for related research.

RESUMO

We established an indirect ELISA method using Trichinella spiralis trehalase(TsTRE)protein expressed in prokaryotic cells. The TsTRE gene was amplified by RT-PCR and ligated into the pCold I plasmid, which was expressed in E.coli BL21 competent cells. The rTsTRE protein was purified through affinity column chromatography. The TsTRE protein was localized with immunofluorescence techniques, and the immunogenicity of rTsTRE was detected by westernblotting.Subse-quently,rTsTRE protein was used as a coating antigen to establish an indirect ELISA.We optimized the antigen-coating con-centration,serum dilution concentration,antigen-coating incubation time, type of blocking solution, blocking incubation time,HRP-labeled goat anti-rabbit IgG serum dilution concentration,HRP-labeled goat anti-rabbit IgG serum incubation time and response time of TMB.Subsequently, the critical value, repeatability, sensitivity, specificity and clinical detection rate of the ELISA were evaluated.Immunofluorescence indicated that trehalase was abundant in the rod-shaped body, tail and epidermis of Trichinella spiralis muscle larvae.Western-blot indicated that rTsTRE protein combined with the positive serum of mice infected with T.spiralis for 42 d;the band was approximately 60 kDa.The established indirect ELISA had a positive threshold of 0.384;the intra-run and inter-run coefficients of variation were 5.504%-7.630% and 4.664%-9.929%,and did not exceed 10%.The lowest detectable titer was 1:1 280.No cross reaction was observed with antibodies to Clonorchissinensis,Schistosoma ja-ponicum,Ascaris suum,Toxocara gondii and Toxocara canis, and the clinical negative detection rate was 0%.Thus,we suc-cessfully expressed the rTsTRE protein. Moreover, the established indirect ELISA method using the TsTRE protein as the coating antigen had good repeatability, sensitivity, specificity and clinical detectability, and can be applied to the detection of clinical samples.

RESUMO

Objective:To explore the clinical efficacy of 4-in-1 surgery in the treatment of Crawford Ⅳ congenital pseudarthrosis of the tibia (CPT) in children. Methods:A retrospective study was conducted to analyze the clinical data of 86 patients with Crawford Ⅳ CPT who had been treated by 4-in-1 surgery at Orthopedic Department, Hunan Children's Hospital from January 2013 to December 2017. There were 47 boys and 39 girls, with an age of (40.7 ± 9.7) months at the time of surgery. Of them, 63 were complicated with type I neurofibromatosis. The 4-in-1 surgery consisted of resection of the tibial pseudarthrosis and surrounding hamartoma-like tissue, internal fixation with tibial intramedullary rod, grafting with wrapped autologous iliac bone, and Ilizarov external fixation. Their bone union, postoperative complications, and follow-up results were recorded. Results:The mean postoperative follow-up time was (80.1 ± 12.4) months for this cohort. The initial bone union rate was 96.5% (83/86), and the initial bone union time (4.8 ± 0.7) months. The last follow-up revealed 20 re-fractures, 15 cases of leg length discrepancy (longer tibia on the affected side in 5 ones and longer tibia on the healthy side in 10 ones), 10 cases of proximal tibial valgus, 12 cases of ankle valgus, 9 cases of bone bridge formation of the distal tibial epiphyseal plate, 10 cases of Ilizarov pin infection, and 2 cases of tibial osteomyelitis. Ankle stiffness occurred in 35 cases, and deviation of the tibial intramedullary rod from the center of the tibial medullary cavity in 60 cases, of which the intramedullary rod had to be removed in 20 because the rod protruded the tibial bone cortex. At the last follow-up, there were 15 cases of skeletal maturity and 71 cases of immature skeleton. Conclusions:In the treatment of Crawford Ⅳ CPT in children, the 4-in-1 surgery can lead to a high rate of initial bone union and definite primary clinical efficacy, but further improvements are necessary because postoperative complications are numerous.

RESUMO

Objective:To explore the risk factors associated with tibia fractures in children with congenital anterolateral bowing of the tibia (ALBT).Methods:A retrospective analysis was conducted on data from 87 children diagnosed with ALBT at the Children's Hospital of Hunan Province from January 2012 to January 2020. The collected data included age at initial diagnosis, affected limb side, whether there was a concomitant type I neurofibromatosis, whether there was a concomitant fibular pseudoarthrosis, whether there was concomitant ankle joint deformity, whether there was bone cystic change in the region of tibial bowing deformity, location of the apex of the bowing deformity, diameter of the tibial bowing deformity on the affected side, diameter on the healthy side in the same plane as the tibial bowing deformity, angle of lateral bending deformity of the tibia, angle of anterior bending deformity of the tibia, occurrence of tibia fracture, history of trauma before fracture, location of fracture, and age at the time of fracture. The follow-up endpoint was January 2023. Receiver operating characteristic (ROC) curves were used to determine the optimal cutoff values for the angles of lateral and anterior bending deformity of the tibia and the ratio of cross-sectional areas. The correlation between the above factors and tibial fractures in children was analyzed by single factor survival analysis, and the indicators with statistical significance were included in multivariate Cox proportional risk regression analysis to determine the risk factors for tibial fractures in children with ALBT.Results:Of the 87 children diagnosed with ALBT, the median age at initial diagnosis was 14.0 months (range, 1-93 months), with 42 males and 45 females, 44 left-sided and 43 right-sided cases. The median follow-up time for non-fracture cases was 42.0 months (range, 1-124 months). At the last follow-up, 43 children had experienced fractures, while 44 had not. The average time to fracture-free survival was 70.3 months, the median fracture-free survival time was 55.0 months, and the median survival time without fractures was 42.0 months. The ROC curve results indicated a cutoff value of 25.55° for the lateral bending angle of the tibia and 32.63° for the anterior bending angle of the tibia, with no statistically significant significance for the cross-sectional area ratio [AUC=0.54, 95% CI (0.42, 0.66), P=0.530]. Single-factor analysis of fracture-free survival suggested that there were statistically significant differences in the intergroup fracture-free survival rates of four factors: lateral bending angle of the tibia (χ 2=7.06, P=0.008), anterior bending angle of the tibia (χ 2=8.96, P=0.003), history of trauma (χ 2=18.26, P<0.001), and tibial bone cystic change (χ 2=4.30, P=0.038). The results of the multivariate Cox proportional hazards regression analysis showed that a lateral bending angle of the tibia≥25.55° ( HR=2.73, P=0.007), tibial bone cystic change ( HR=2.35, P=0.018), and history of trauma ( HR=2.65, P=0.004) were all positively correlated with fractures. Conclusion:The main risk factors for tibia fractures in children with ALBT include trauma, tibial bowing deformity with concomitant bone cystic change, and lateral bending angle of the tibia≥25.55°.

RESUMO

The Evidence-based guidelines for diagnosis and treatment of age-related macular degeneration in China (2023) is the first evidence-based clinical practice guidelines for the diagnosis and treatment of age-related macular degeneration (AMD) in China that strictly follows the WHO guideline formulation and international guideline standards. Based on the latest evidence-based medical evidence and the rapid development of ocular fundus imaging technology, the new version of the guidelines has been updated in the diagnosis, treatment and follow-up of AMD. Based on the origin and presence of neovascularization in retinal tissue, the macular neovascularizations are divided into type 1, type 2 and type 3 macular neovascularization (MNV). Optical coherence tomography (OCT) plus OCT angiography is recommended as the useful diagnostic method in neovascular AMD patients. Antioxidant vitamins, zinc, lutein, zeaxanthin, or a mix of antioxidant vitamins and minerals are recommended to prevent the progression of early to intermediate AMD. Intravitreal administration of anti-vascular endothelial growth factor (VEGF) agents is the first-line treatment for neovascular AMD, and once a month for the first 3 months plus treatment and prolonged administration regimen shows benefit to a certain extent. For poor response and non-response neovascular AMD patients, the clinician should decide the next treatment plan based on comprehensive consideration. Close observation is recommended for non-exudative MNV, and anti-VEGF therapy should be used promptly once non-exudative MNV transfer to exudative MNV. It is hoped that this guideline will improve the diagnosis, treatment, prevention and follow-up of AMD in China.

RESUMO

Objective: To explore the long-term effect of combined surgery for the treatment of congenital tibial pseudarthrosis in children. Methods: The clinical data of 44 children with congenital tibial pseudarthrosis who underwent combined surgery (tibial pseudarthrosis tissue resection, intramedullary rod fixation, Ilizarov external fixator fixation, wrapped autologous iliac bone graft) from August 2007 to October 2011 at the Department of Pediatric Orthopedics, Hunan Children's Hospital were collected retrospectively. There were 33 males and 11 females. The age at the time of surgery was (3.7±2.2)years (range:0.6 to 12.4 years), including 25 cases under 3 years old and 19 cases above 3 years old.Among them, 37 cases were complicated with neurofibromatosis type 1.The operation status, postoperative complications and follow-up results were recorded. Results: The follow-up time after surgery was (10.9±0.7)years (range:10 to 11 years).Thirty-nine out of 44 patients (88.6%) achieved initial healing of tibial pseudarthrosis, with an average healing time of (4.3±1.1)months (range:3 to 10months).In the last follow-up, 36 cases (81.8%) had unequal tibial length, 20 cases (45.4%) had refractures, 18 cases (40.9%) had ankle valgus, 9 cases (20.4%) had proximal tibial valgus, and 11 cases (25.0%) had high arched feet.Nine cases (20.4%) developed distal tibial epiphyseal plate bridging.17 cases (38.6%) had abnormal tibial mechanical axis.Seven cases (15.9%) developed needle infection, and one case (2.3%) developed tibial osteomyelitis. 21 patients (47.7%) had excessive growth of the affected femur.Five patients (11.3%) had ankle stiffness, and 34 patients (77.2%) had intramedullary rod displacement that was not in the center of the tibial medullary cavity.Among them, 8 cases (18.1%) protruded the tibial bone cortex and underwent intramedullary rod removal.18 children have reached skeletal maturity, while 26 children have not been followed up until skeletal maturity. Conclusion: Combined surgery for the treatment of congenital pseudarthrosis of the tibia in children has a high initial healing rate, but complications such as unequal tibia length, refracture, and ankle valgus occur during long-term follow-up, requiring multiple surgical treatments.

RESUMO

Objective:To explore the mediating effect of experiential avoidance and emotional eating between intolerable uncertainty and life satisfaction of college students. Methods:From October to December 2021, a total of 890 college students in the Beijing-Tianjin-Hebei region were tested by the intolerable uncertainty scale (simplified version), the acceptance action scale second edition, the Dutch eating behavior questionnaire and the life satisfaction scale. All data processing and analysis were preformed by SPSS 22.0 software, and the mediating effect was tested by Pearson correlation analysis and Bootstrap analysis. Results:The intolerable uncertainty score was (26.43±7.64), the experiential avoidance score was (22.63±9.67), the emotional eating score was (26.43 ±11.83), and the life satisfaction score was (16.74±6.03). Intolerable uncertainty was significantly and negatively correlated with life satisfaction( r=-0.76, P<0.01), and was significantly and positively correlated with experience avoidance and emotional eating( r=0.66, 0.78, both P<0.01). Experience avoidance was significantly and positively correlated with emotional eating( r=0.70, P<0.01), and was significantly and negatively correlated with life satisfaction( r=-0.62, P<0.01). Emotional eating was significantly and negatively correlated with life satisfaction( r=-0.68, P<0.01). Intolerable uncertainty affects life satisfaction through four paths. The direct effect value of intolerable uncertainty on life satisfaction was -0.53, accounting for 70.23% of the total effect. The separate mediating effect value of experience avoidance was -0.11, accounting for 50.00% of the total indirect effect. The separate mediating effect value of emotional eating was -0.08, accounting for 36.36% of the total indirect effect, and the chain mediating effect value of experience avoidance and emotional eating was -0.03, accounting for 13.64% of the total indirect effect. Conclusion:Intolerable uncertainty can directly affect life satisfaction of college students and indirectly through experiencing avoidance and emotional eating.
