RESUMO

The effect of bilateral tube insertion on language development was studied in a randomized controlled trial of 43 preschool children with persistent bilateral otitis media with effusion (OME). Improvement in language scores was observed in both the treatment and non-treatment groups. There was a minor difference in favour of the treatment group which did not reach statistical significance. Although there is a growing consensus that otitis media with effusion can have negative effects on language development in young children, it has not yet been proved that this effect can be reversed by treatment with ventilating tubes.

Data on middle ear disease and follow-up was recorded and analysed in 2225 ears operated on for chronic otitis media between 1958 and 1975. The closure rate of tympanic membrane perforations (70-80%) was not influenced by the presence of otorrhoea, but it was significantly lower in patients aged below 15 or over 40 yr and especially when tympanic membrane homograft had been used. For all types of tympanoplasty the mean improvement in air-conduction threshold was most marked at the lower frequencies, being some 10 dB in Type I, 15 dB in Type II and 8 dB in Type III and IV and it decreased by some 3 dB per octave. The combination of cholesteatoma and tympanosclerosis appeared to be rare. The post-operative incidence of otorrhoea was 10-15% after modified radical mastoidectomy and 20-25% after radical mastoidectomy.

RESUMO

A case is presented of a positive 'fistula sign' in a patient with an intact ear-drum and no history of previous ear-disease. The cause proved to be an eosinophil granuloma of the temporal bone which responded well to radiotherapy.

RESUMO

The results are presented of a study, designed to examine the relation between the site of insertion of transtympanic ventilating tubes into the tympanic membrane and the period, elapsing before extrusion takes place. The results suggest that tubes inserted into the anterior part of the tympanic membrane remain in place for a longer period than tubes inserted posteriorly.
