RESUMO

The Advisory Committee on Vaccines of the Spanish Paediatric Association updates annually the immunization schedule, taking into account epidemiological data as well as evidence of the effectiveness and efficiency of vaccines. This vaccination schedule includes grades of recommendation. The committee has graded as universal vaccines those that all children should receive, as recommended, those with a profile of universal vaccines of childhood and as are desirable those that all children may receive, but that can be prioritized based on public funding resources and for risk groups, targeting those groups of people in epidemiological situations of risk. The Committee considers as a priority to achieve a common immunization schedule for Spain. The Committee reaffirms the recommendation to include pneumococcal vaccination in the routine vaccination schedule. Vaccination against varicella in the second year of life is an effective strategy and therefore a desirable goal. Given the morbidity and high burden on the health care system, vaccination against rotavirus is recommended for all infants. Due to the current problems of availability of both vaccines, associated with the recent finding of circovirus, the committee urges that rotavirus vaccination is restarted as soon as possible as it is considered a desirable health benefit for all children in our country. The Committee adheres to the recommendations of the National Health Coordination Council in reference to routine vaccination against HPV for all girls aged 11 to 14 years and stresses the need to vaccinate all patients with risk factors for these diseases against influenza and hepatitis A. Finally, it stresses the need to update incomplete immunizations using accelerated immunization schedules.

INTRODUCTION: The aim of this study was to describe the clinical and epidemiological features of confirmed influenza A virus infection in patients aged less than 7 years old. PATIENTS AND METHODS: We performed a retrospective, descriptive study of the clinical and epidemiological features of influenza A virus infection in patients aged less than 7 years old attending three primary care centers during the 2003-2004 influenza season. Patients with influenza A infection confirmed by immunofluorescence, cell culture, or polymerase chain reaction were interviewed to obtain clinical and epidemiological data on their disease. RESULTS: Influenza A virus was detected in 47 patients aged between 5 months and 7 years (mean: 37.3 +/- 5.7 months). Of these, 35 children (74.4 %) were aged between 0 and 4 years. The most frequent symptoms were fever (100 %), cough (95.7 %), and rhinorrhea (76.6 %). Gastrointestinal symptoms were present in 40.4 % of the patients. The mean duration of fever was 5.2 +/- 1.8 days (range 2-8). Complications occurred in nine patients (19.2 %). The frequency of influenza-like disease in parents was 9.1 % (8/88) at diagnosis of the index case and was 23.9 % (21/88) immediately after the episode in the child (p 5 0.01). The mean number of days of absence from school was 7.5 +/- 3.6 days. CONCLUSIONS: The mean age of our patients confirms that influenza is a frequent disease in infancy and childhood. The results demonstrate that the infection is transmitted from children to parents in the household setting, since the frequency of influenza-like symptoms in adults increases after influenza episodes in children.

OBJECTIVE: To compare self-perception of health with physical parameters in a population group with chronic illnesses when conducting a group intervention of supervised physical exercise. DESIGN: Intervention study with test-retest evaluation. SETTING: Serraparera Primary Care Centre (PCC) in the town of Cerdanyola del Vallès (24,735 inhabitants). PATIENTS: Patients between 30 and 65 with a chronic illness and attended at the PCC (n = 87). INTERVENTION: A seven-month programme, based on regular supervised physical exercise, three times a week for an hour, supplemented by organised monthly walks on varying routes. MEASUREMENTS AND MAIN RESULTS: Blood pressure, BMI, flexibility, sub-maximum and at-rest heart rates were measured, and the Nottingham Health Profile (NHP) self-perception test conducted. 87 patients began the programme and 51 reached the end. The following was observed in the 51: self-perception of overall improvement in health on the NHP of 9.51 points (CI 95%, 5.73-13.23); lowering of blood pressure, systolic 4.8 mmHg (CI 2.47-7.13) and diastolic 4.78 (CI 3.16-6.46); sub-maximum heart rate, with an average difference of 7.65 pulses per minute; and at-rest heart rate, with a difference of 4.61 ppm. Flexibility increased by 1.37 (CI 1.05-1.69). There was no significant reduction in BMI. CONCLUSIONS: Patients with chronic illnesses experienced a notable improvement in the control of their pathology and vital constants. There was also a significant difference detected in patients' perception of their own health.
