RESUMO

Satisfaction with social support (pain-relevant social support) may influence pain experience and behaviour in patients with chronic pain. Prior studies on measurement of social support, however, have been limited by the use of general, rather than of pain-specific assessment instruments. In this study, a new pain-relevant social support instrument, the Social support and Pain Questionnaire (SPQ), is presented together with an evaluation of its psychometric properties. A literature search was performed to establish different aspects of social support. For each of the six aspects found, one item was selected for inclusion in the new questionnaire. The draft version of the questionnaire was field tested. Thereafter, the psychometric properties of the SPQ were assessed in 250 patients with oro-facial pain. Principal component analysis (n=250) showed that the SPQ had a one-factor structure. The test-retest reliability of the SPQ (in a subsample of 54 patients) was fair-to-good (R=0·70; P<0·000). Convergent validity, as compared with a non-specific social support instrument, was good (n=140; R=0·54; P<0·000). The SPQ is a valid and reliable instrument, which offers the possibility to explore the patient's satisfaction with pain-related social support. With the SPQ, a useful tool to assess the influence of social support in patients with various types of pain is provided.

OBJECTIVE: To determine whether assessment of antibodies directed against citrullin provides additional value in the diagnosis of rheumatoid arthritis (RA) in general practice. DESIGN: Retrospective. METHODS: In a 6-month period in 2004 (May-December), all sera sent to our laboratory for assessment of rheumatoid factor (RF-IgM), were also analysed for the presence of antibodies directed against citrullinated fibrinogen (anti-citrullin). We analysed 691 sera sent in by general practitioners using a homemade assay. To determine the disease classification, general practitioners were asked to provide information pertaining to the American College of Rheumatology disease classification criteria. The response was 97.6%. For patients who were referred to a rheumatologist in the last 2 years (December 2004-December 2006), the diagnosis of the rheumatologist was also considered in the analysis. RESULTS: A total of 28 patients (4%) were diagnosed with rheumatoid arthritis. Only 25% of these patients were positive for anti-citrullin, and only 25% were positive for RF-IgM. These 2 groups only partially overlapped. The positive and negative predictive values of anti-citrullin were 36 and 96%, respectively. CONCLUSION: The presence of anti-citrullin provided no additional value compared to rheumatoid factor in classifying RA in a general practice population.

We aimed to explore the level of inter- and intra-individual variation in applied force when listening at a surface, and assess the resulting variation in earprints. We further intended to identify possible sources of this variation. Forty subjects each listened twenty-four times at a surface while applied force was recorded. In between efforts the level and frequency of the target sound, and the level of ambient noise were varied. Each listening effort was characterized by two values: the mean of a series of force recordings ('functional force') and the highest force reading of the effort ('peak value'). A mixed model analysis of variance revealed that repetition during multiple efforts of listening and the level of the target sound significantly affected both values for applied force. The frequency of the target sound affected the peak value, but we assume this was due to confounding effects. The level of ambient noise did not affect applied force. To explore the correlation between values for applied force of various efforts by single ear, the intra-class correlation coefficient was calculated. For functional force it was 0.80; for the peak value it was 0.79. To study intra-individual variation in earprints, five prints from each ear were lifted and studied. Variation in prints is discussed.

RESUMO

Occasionally ear prints are found at crime scenes. The height of the ear imprint may provide the police with information regarding the stature of the perpetrator and may therefore help to narrow down the number of suspects. The research provides calculations for the determination of stature from the height of the tragus imprint found at crime scenes. It takes into account various variables such as age, stature and gender.

RESUMO

A description is given of some aspects of the normal human auricle. The physiognomy of the auricle is different for every individual which leads to the possibility of identifying people based on their auricles. Indeed this has even led to the reading of earprints similar to fingerprints, a fact not generally known amongst ENT-specialists. This highly specialized knowledge has been developed within a special branch of forensic medicine and criminology, called 'earology' or 'otomorphology'. Two illustrative case histories are presented.
