RESUMO

PURPOSE: Medicines derived from natural sources have been used for thousands of years throughout the world. Because natural compounds are thought to have less toxic effects and fewer side effects, these products are becoming more popular by the day. CASE REPORT: In this case report, we presented a case of acute kidney injury, rhabdomyolysis, and hepatotoxicity after ingestion of black seed oil. Although black seed oil is widely used around the world, there is currently limited knowledge on its adverse effects. CONCLUSION: It is important to keep in mind that rhabdomyolysis, acute renal damage, and hepatotoxicity might occur following the use of black seed oil. Black seed oil ingestion should be considered when making a differential diagnosis for these conditions in patients suspected of taking herbal products.

El intervencionismo coronario se asocia a la aparición de nefropatía inducida por contraste. El propósito del estudio fue evaluar el riesgo de desarrollar nefropatía inducida por contraste (NIC) en pacientes con obstrucción coronaria significativa y su relación con factores de riesgo conocidos para esta nefropatía. Se diseñó un estudio de cohorte prospectiva con 160 pacientes atendidos en el cardiocentro del hospital "Hermanos Ameijeiras", Cuba, a los cuales se les realizó una coronariografía invasiva, entre enero 2016 y julio 2017. La edad promedio fue de 61,6 ± 9,2 años; el 70,6% eran hombres. Predominaron los antecedentes patológicos personales de cardiopatía isquémica (85,6%), y de hipertensión arterial (75,6%). El 75% de los casos presentó una oclusión coronaria significativa. La frecuencia de nefropatía por contraste fue de 42,5%. Los factores que guardaron importante relación estadística con la presencia de oclusión arterial significativa fueron la cardiopatía isquémica conocida (p<0,001), el intervencionismo coronario percutáneo previo (p=0,007), la creatinina después (p=0,043) y la NIC (p=0,016) así como el volumen de contraste administrado (p=0,006). En el subgrupo de pacientes con oclusión significativa el hematocrito bajo (p=0,025) y el intervencionismo coronario percutáneo de urgencia (p=0,007) fueron los factores más influyentes. Se concluye que los pacientes con oclusión coronaria significativa tienen un riesgo aumentado para el desarrollo de la nefropatía por contraste. La corrección de aquellos factores de riesgo que sean modificables (como el hematocrito bajo) y la correcta aplicación del protocolo de hidratación son esenciales para prevenir esta complicación.


Coronary intervention is associated with the appearance of contrast-induced nephropathy. The purpose of the study was to assess the risk of developing contrast-induced nephropathy in patients with significant coronary obstruction and its relationship with known risk factors for this nephropathy. A prospective cohort study was designed with 160 patients treated at the cardiocenter of the "Hermanos Ameijeiras" hospital, Cuba, who underwent invasive coronary angiography, between January 2016 and July 2017. The average age was 61.6 ± 9 ,2 years; 70.6% were men. The personal pathological history of ischemic heart disease (85.6%) and arterial hypertension (75.6%) predominated. 75% of the cases presented a significant coronary occlusion. The frequency of contrast nephropathy was 42.5%. The factors that had an important statistical relationship with the presence of significant arterial occlusion were known ischemic heart disease (p <0.001), previous percutaneous coronary intervention (p = 0.007), creatinine after the procedure (p = 0.043) and CIN (p = 0.016) as well as the volume of contrast administered (p = 0.006). In the subgroup of patients with significant occlusion, low hematocrit (p = 0.025) and emergency percutaneous coronary intervention (p = 0.007) were the most influential factors. It is concluded that patients with significant coronary occlusion have an increased risk for the development of contrast nephropathy. The correction of those risk factors that are modifiable (such as low hematocrit) and the correct application of the hydration protocol are essential to prevent this complication

RESUMEN Introducción: En el Hospital General Docente "Dr. Agostinho Neto", de Guantánamo, no se ha caracterizado la problemática del daño renal agudo en la unidad de terapia intensiva. Objetivo: caracterizar esta afección en pacientes ingresados en la citada unidad en el periodo 2018-2019. Método: Se realizó un estudio descriptivo, retrospectivo y longitudinal, aprobado por el comité de ética. El universo de estudio se constituyó por el total de pacientes con este diagnóstico según la clasificación Acute Kidney Injury Network (AKIN). Se estudiaron las características de los pacientes (edad, sexo, comorbilidad, etiología, estadía en la unidad de terapia intensiva, necesidad de hemodiálisis, estado al egreso) y del daño renal agudo (estadio según los criterios de la escala AKIN). Resultados: En el 35,6 % de los pacientes se diagnosticó esta enfermedad, sobre todo en hombres (56,7 %) y con edad de 66,3 ± 24,3 años. El 41,4 % padeció hipertensión arterial sistémica. En el 48,9 % la enfermedad se presentó un en estadio 1, y en el 69,4 % la sepsis fue la principal causa. La mortalidad al egreso hospitalario fue de 16,4 %, y a los 30 días fue de 25,4 %. Se realizó hemodiálisis al 13,8 % de los pacientes, en los que el riesgo de muerte fue superior. Conclusiones: En la unidad de terapia intensiva es elevada la proporción de pacientes con daño renal agudo y resulta útil la escala que se utilizó para el diagnóstico y la evaluación de la gravedad y el pronóstico de los pacientes afectados.


ABSTRACT Introduction: Acute kidney injury in the intensive care unit in the General Teaching Hospital ¨Dr. Agostinho Neto¨ in Guantanamo has not been characterized. Objective: To characterize this disease in patients in the intensive care unit in the mentioned institution in the period 2018-2019. Method: A descriptive, retrospective and longitudinal study was undertaken, all approved by the ethics committee. The study population was made out of the total amount of the patients diagnosed according to the classification of the Acute Kidney Injury Network (AKIN). The variables taken into account were: age, gender, comorbidity, etiology, time in the intensive care unit, requirement for hemodialysis and status of the patient at the time of discharge, plus the stages of the acute kidney injury according to the AKIN scale. Results: Acute kidney injury was diagnosed in the 35.6% of the patients, especially in male patients (56.7%) with ages between 66.3 ± 24.3 years. 41.4% of the patients suffered of systemic arterial hypertension. Stage 1 was found in the 48.9% of the cases, and sepsis was the main cause in 69.4%. Mortality at the time of discharge represented the 16.4%; and after the following 30 days went up to 25.4%. Hemodialysis was required in the 13.8% of the patients; in wich the risk of death was significatively higher. Conclusions: There is a high number of patients with acute kidney injury in the intensive care unit, and it was really useful the scale implemented for diagnosis and evaluation of the severity of the condition and the prognosis of the patients.

Introducción: los pacientes necesitados de angioplastia transluminal percutánea tienen una elevada probabilidad de presentar daño renal inducido por medios de contraste. Objetivo: determinar los factores de riesgo asociados al daño renal agudo inducido por dichos medios. Métodos: estudio descriptivo de 57 pacientes con cardiopatía, quienes requirieron de angioplastia transluminal percutánea en el Cardiocentro del Hospital Clinicoquirúrgico Hermanos Ameijeiras de Ciudad de la Habana, desde enero hasta diciembre de 2016. Para ello se conformaron 2 grupos (con nefropatía y sin ella). Se consideró la existencia de daño renal si la creatinina sérica se incrementaba en 25 por ciento o más a las 72 horas de realizado el proceder. Para estratificar el riesgo se empleó la escala de Mehran modificada. Resultados: existió daño renal en 49,1 por ciento de los pacientes. Los factores asociados fueron la angiografía previa, la angioplastia transluminal percutánea urgente y el volumen de contraste empleado. Según la escala de Mehran hubo un predominio de los afectados en los grupos de riesgo bajo y moderado; sin embargo, teniendo en cuenta los factores de riesgo identificados, estos pacientes prevalecieron en los grupos de riesgo alto y muy alto. Conclusiones: la angiografía previa, la realización urgente de angioplastia transluminal percutánea y el volumen de contraste empleado aumentan el riesgo de daño renal. Se determinó que la escala de Mehran modificada no fue útil para estratificar a los pacientes de bajo riesgo


Introduction: patients in need of transluminal percutaneous angioplasty have a high probability of presenting renal damage induced by contrast media. Objective: to determine the risk factors associated with the acute renal damage induced by this contrast media. Methods: descriptive study of 57 patients with heart disease who required transluminal percutaneous angioplasty in the Cardiology Center of Hermanos Ameijeiras Clinical Surgical Hospital of Havana City, from January to December, 2016. Two groups were conformed for that purpose (with nephropathy and without it). It was considered the existence of renal damage if the serum creatinine increased in 25 percent or more at the 72 hours of having carried out the procedure. The Mehran scale modified was used to stratify the risk. Results: there was renal damage in 49.1 percent of the patients. The associated factors were the previous angiography, the emergency percutaneous transluminal angioplasty and the volume of contrast used. According to the scale of Mehran there was a prevalence of the affected patients in the groups of low and moderate risk; however, taking into account the identified risk factors, these patients prevailed in the groups of high and very high risk. Conclusions: the previous angiography, the emergency of transluminal percutaneous angioplasty and the volume of contrast used increase the risk of renal damage. It was determined that the modified Mehran scale was not useful to stratify the patients of low risk.

The review presents literature data on the efficacy of new and potential biomarkers for the early diagnosis of acute renal damage. Early non-invasive and cost-effective kidney damage biomarkers are vital for determining the tactics, effectiveness and prognosis of treatment. Their use will not only reduce morbidity and mortality, but also reduce health care costs.

AIM: To investigate experimentally ultrastructural and biochemical signs of acute injury to the renal parenchyma after warm renal ischemia of various duration and subsequent reperfusion. MATERIALS AND METHODS: The experiments were performed on 44 healthy conventional female rabbits of the "Chinchilla" breed weighted 2.6-2.7 kg, which were divided into four groups. In the first, control, group included pseudo-operated animals. In the remaining three groups, an experimental model of warm ischemia of renal tissue was created, followed by a 60-minute reperfusion. The renal warm ischemia time was 30, 60 and 90 minutes in the 2nd, 3rd and 4th groups, respectively. Electron microscopy was used to study ultrastructural disturbances of the renal parenchyma. Biochemical signs of acute kidney damage were detected by measuring the following blood serum and/or urine analytes: NGAL, cystatin C, KIM-1, L-FABP, interleukin-18. The glomerular filtration was evaluated by creatinine clearance, which was determined on days 1, 5, 7, 14, 21 and 35 of follow-up. RESULTS: A 30-minute renal warm ischemia followed by a 60-minute reperfusion induced swelling and edema of the brush membrane, vacuolation of the cytoplasm of the endothelial cells of the proximal tubules, and microvilli restructuring. The observed disorders were reversible, and the epithelial cells retained their viability. After 60 minutes of ischemia and 60 minutes of reperfusion, the observed changes in the ultrastructure of the epithelial cells were much more pronounced, some of the epithelial cells were in a state of apoptosis. 90 min of ischemia and 60 min of reperfusion resulted in electron-microscopic signs of the mass cellular death of the tubular epithelium. Concentration in serum and/or biochemical urine markers of acute renal damage increased sharply after ischemic-reperfusion injury. Restoration of indicators was observed only in cases when the renal warm ischemia time did not exceed 60 minutes. The decrease in creatinine clearance occurred in the first 24 hours after the intervention, lasting not less than two weeks after a 30-minute warm ischemia, at least 3 weeks after a 60-minute warm ischemia and continued more than a month after a 90-minute renal artery occlusion. CONCLUSION: Intraoperative warm ischemia and subsequent reperfusion are the actual reasons for the alteration of the ultrastructure of the renal tissue and the impairment of the filtration function. The severity of the disorders depends on the duration of the damaging factors. After a 30-60-minute ischemia, the structural and functional changes in the renal tissue are reversible. The mass death of nephrocytes-effectors is possible only after warm renal ischemia longer than 60 min.

Introducción: La elevada prevalencia de enfermedad renal crónica y otras enfermedades no transmisibles crónicas, unido al incremento en la edad de embarazo ha generado nuevas investigaciones y evidencias de la relación entre la enfermedad renal crónica, el embarazo y los resultados para la madre y el feto. Objetivo: Exponer las mejores prácticas actuales y ofrece una aproximación al diagnóstico, evaluación y tratamiento de la enfermedad renal en el contexto del embarazo y su repercusión en términos de morbilidad y mortalidad para la madre y el feto. Material y Métodos: Se realizó un análisis de la literatura describiendo los mejores resultados clínicos basado en los avances científicos a la fecha actual. Resultados: Se enfatiza la evolución continua entre el daño renal agudo (incipiente) y la falla renal aguda (con necesidad de métodos dialíticos sustitutivos), también de importancia pronóstica con incrementos en la mortalidad materna asociados a pequeños incrementos en la creatinina sérica. Conclusiones: Tanto el desarrollo de un daño renal agudo, la falla renal aguda y la enfermedad renal crónica son causas importantes de morbilidad y mortalidad materno fetal(AU)


Introduction: The high prevalence of chronic kidney disease and others non contagiables diseases, jointly with its increasing frequency in gestational age women have generated new investigations and evidences of the relationship between the chronic renal diseases, the pregnancy and its consequence for the mother and fetus. Objective: This paper summarizes the best practice up to this date and provides a reasonable approach to the diagnosis, evaluation, and treatment of the Renal Disease Disorders of Pregnancy to evaluate the impact of them on maternal and fetal morbidity and mortality. Material and Methods: An analysis of the published literature of the subject was performed, describing the best clinical results based on scientific advances available today. Results: Was emphasized the continuum evolution between acute kidney damage (incipient injury) and acute kidney failure (need to dialysis) also of importance for prognosis, with increasing of the mortality associated with small increases in serum creatinine. Conclusions: The acute kidney diseases/ acute renal failure and chronic renal diseases are important causes of maternal and perinatal morbidity - mortality(AU)
