RESUMEN Introducción: La resección anterior del recto ultrabaja laparoscópica con anastomosis coloanal evita la realización una colostomía definitiva. Objetivo: Determinar las indicaciones, describir la técnica quirúrgica y mostrar los resultados a largo plazo obtenidos en la realización de este proceder en el tratamiento del cáncer del recto bajo. Método: Se realizó un estudio observacional descriptivo y prospectivo de 53 pacientes con cáncer del recto bajo en el período comprendido entre octubre 2007 y noviembre 2018 en el Centro Nacional de Cirugía de Mínimo Acceso. Todas las resecciones fueron llevadas a cabo por un grupo dedicado a la cirugía colorrectal, en todos los casos se realizó la excisión total del mesorrecto. Resultados: Se operaron mediante esta técnica 53 pacientes, 30 masculinos y 23 femeninos con un promedio de edad de 57 años (rango 23-81) y de ellos 42 (79,2 por ciento) después de terapia neoadyuvante. El promedio de tiempo quirúrgico fue de 195 min (rango 90-360) y las pérdidas hemáticas estimadas, de 72 mL. La incidencia de morbilidad mayor fue de 16,9 por ciento (9/53) y la media de estadía hospitalaria de 6,3 días. La media del periodo de seguimiento fue de 40 meses (rango 1-132) con una recidiva local de 9,4 por ciento (5/53) y una supervivencia global a los 5 años de 80,3 por ciento. Conclusiones: La resección anterior del recto ultrabaja laparoscópica con anastomosis coloanal es una técnica segura con excelentes resultados en cuanto a recidiva local y supervivencia global(AU)


ABSTRACT Introduction: Laparoscopic ultralow anterior rectal resection with coloanal anastomosis prevents the performance of a definitive colostomy. Objective: To determine the indications, describe the surgical technique and show the long-term outcome of performing this procedure in the treatment of cancer of the lower rectum. Method: A descriptive and prospective observational study of 53 patients with low rectal cancer was carried out in the period between October 2007 and November 2018, at the National Center for Minimum Access Surgery. All resections were carried out by a group dedicated to colorectal surgery. In all cases, the total mesorectal excision was performed. Results: 53 patients (30 males and 23 females), with an average age of 57 years (range 23-81) and 42 (79.2 percent) after neoadjuvant therapy were operated using this technique. The average surgical time was 195 min (range 90-360) and the estimated blood loss was 72 mL. The incidence of major morbidity was 16.9 percent (9/53) and the average hospital stay was 6.3 days. The mean follow-up period was 40 months (range 1-132), with a local relapse of 9.4 percent (5/53) and a 5-year overall survival of 80.3 percent. Conclusions: Laparoscopic ultralow anterior resection of the rectum with coloanal anastomosis is a safe technique with excellent outcomes in terms of local relapse and overall survival(AU)

AIM: To assess the results of treatment for colorectal (CRA), coloanal (CAA) or ileal pouch-anal (IPAA) anastomotic stenosis (AS). METHOD: All patients operated on for AS from 1995 to 2014 were included. Success was defined as the absence of an additional surgical procedure for AS during 12 months after the last procedure and the absence of a stoma at the end of follow-up. RESULTS: Fifty consecutive patients presenting with AS after CRA (n = 16, 32%), CAA (n = 18, 36%) or IPAA (n = 16, 32%), performed for colorectal cancer (n = 28, 56%), familial adenomatous polyposis (n = 5, 10%), inflammatory bowel disease (n = 8, 16%), diverticulitis (n = 4, 8%), benign colorectal neoplasia (n = 3, 6%) or other (n = 2, 4%) underwent a total of 99 procedures including digital (n = 14, 14%), instrumental (n = 38, 38%) or endoscopic dilatation (n = 5, 5%), transanal AS stricturoplasty (n = 9, 10%), transanal circular stapler resection (n = 11, 11%) or transabdominal redo-anastomosis (n = 22, 22%). Overall the per-procedure success rate was 53% (52/99). Success rates were 36% (5/14) for digital dilatation, 40% (15/38) for instrumental dilatation, 20% (1/5) for endoscopic dilatation, 64% (7/11) for circular stapler resection, 89% (8/9) for stricturoplasty and 73% (16/22) for transabdominal redo-anastomosis. After a mean follow-up of 46 months, 42/50 (84%) patients had treatment that was considered successful. Multivariate analysis identified redo-anastomosis [OR = 5.1 (95% CI: 1.4-18.7), P = 0.003] as the only independent prognostic factor for success. CONCLUSION: AS should be managed according to a step-up strategy. Conservative procedures are associated with acceptable success rates. If these fail, transabdominal redo-anastomosis is associated with the highest probability of success.

BACKGROUND: The surgical treatment for low rectal cancer involves an ultra-low anterior resection with complete mesorectal resection and coloanal anastomosis. Two-stage coloanal anastomosis such as the Turnbull-Cutait technique represents an option for patients with low rectal cancer. CLINICAL CASE: A 69 year-old female patient with a diagnosis of adenocarcinoma (T2N1), located 4 cm from the anal margin. She received neoadjuvant radiotherapy. An ultra-low anterior resection and total resection of the mesorectum were performed. The intestinal transit was reconstructed by coloanal anastomosis using the Turnbull-Cutait technique. CONCLUSION: Coloanal anastomosis with the Turnbull-Cutait technique represents a primary option for patients with low rectal cancer, avoiding a loop ileostomy, its economic impact and on their quality of life.

Rectal ultralow, intersphincteric anterior resection (RIE) can be used in selected cases with the intention of improving the quality of life of patients avoiding permanent colostomy. RIE is indicated for tumors that are located from 1-2 cm above the anorectal ring to the top of the internal anal sphincter without involvement of the pelvic floor, puborrectalis muscle or external anal sphincter. RIE aims to get a free distal margin tumour not less than 1cm. Correct preoperative staging and anatomical tumour location and relation with adjacent structures and organs is fundamental. Intestinal transit reconstruction can be performed manually with a coloanal anastomosis according with Parks and with a lateral ileostomy or, alternatively, by a two-stage coloanal anastomosis technique as Turnbull-Cutait avoiding the stoma protection. Postoperative morbidity and mortality and the rate of local recurrence and overall disease-free survival at 5 years after RIE are comparable to those observed in standard ultra low anterior resection. Postoperative functional alterations of the RIE can affect the quality of life of patients regardless of reconstructive technique. Published studies do not provide sufficient data to establish the most efficient reconstruction method in terms of functional outcomes.

Fundamento: El hemangioma rectal es una enfermedad infrecuente que afecta a adultos jóvenes. El signo de presentación más frecuente es el sangrado rectal indoloro. Presentación del caso: Paciente femenina de 43 años con antecedente de rectorragias de causa no precisada desde hace tres años, a pesar de estudios endoscópicos que fueron normales. Se diagnostica el caso como una posible colitis ulcerativa; la paciente continuó con sangrado rectal que le produjo anemia severa y tenesmo intenso, por lo que se decide tratamiento quirúrgico. El estudio anatomopatológico informó la presencia de un hemangioma cavernoso del recto. Este caso es novedoso por lo infrecuente a nivel mundial. Conclusiones: La paciente evolucionó favorablemente y sin complicaciones postoperatorias (AU)


Background: Rectal hemangioma is a rare disease that affects young adults. The most common presenting sign is painless rectal bleeding. Case presentation: 43 year old female patient with a history of rectal bleeding of an undefined cause for three years, despite endoscopic studies were normal. The case is diagnosed as a possible ulcerative colitis. The patient continued with rectal bleeding which caused her severe anemia and intense tenesmus, therefore, a surgical treatment was applied. The pathological study confirmed the presence of a cavernous hemangioma of the rectum. This case is novel as it is rare worldwide. Conclusions: The patient improved and had no postoperative complications

INTRODUCCIÓN. La proctosigmoidectomía con resección mesorrectal total, reservorio en J y anastomosis coloanal es útil en los pacientes con cánceres de recto medio y bajo, para evitar la colostomía terminal definitiva. El objetivo de este trabajo fue analizar la factibilidad de dicha técnica quirúrgica, el tratamiento integral multidisciplinario y los resultados obtenidos. MÉTODOS. Se estudiaron 15 pacientes que padecían adenocarcinomas de recto medio y bajo, tratados entre enero de 1996 y diciembre de 2002 en el servicio de Cirugía Esplácnica del Instituto Nacional de Oncología y Radiobiología de La Habana. El tratamiento consistió en una combinación de radioterapia más quimioterapia concurrentes neoadyuvantes, seguidas de cirugía y quimioterapia adyuvante. RESULTADOS. La edad promedio de los pacientes fue de 56 años. El adenocarcinoma fue el tipo histológico diagnosticado en todos los pacientes. La estadificación de los tumores fue la siguiente: T1 y T2, 4 pacientes (27 por ciento, respectivamente); T3, 7 pacientes (46 por ciento). Cuatro pacientes (20 por ciento) se complicaron como consecuencia del tratamiento radiante y 5 (33,3 por ciento), a causa del tratamiento quirúrgico. La mortalidad quirúrgica fue de 1 paciente (6,6 por ciento) y 11 pacientes (73,3 por ciento) sobrevivieron más de 5 años. Ningún paciente presentó recidiva tumoral pélvica ni de la anastomosis coloanal. Se logró buena continencia esfinteriana. CONCLUSIONES. La resección mesorrectal total y anastomosis coloanal con reservorio colónico en J evita la colostomía terminal definitiva, cura a un alto porcentaje de pacientes con cánceres de recto medio y bajo, no transgrede los principios de la cirugía oncológica, es bien aceptada por los pacientes y es factible en nuestro medio(AU)

Four patients (20 percent) had complications due to radiation treatment and five (33,3 percent), by surgical treatment. Surgical mortality occurred in one patient (6,6 percent) and eleven patients (73,3 percent) survived over 5 years. Neither patient had pelvic tumor relapse or by colorectal anastomosis. There was good sphincter continence. CONCLUSIONS: Total mesorectum resection and colorectal anastomosis with a colonic reservoir in J prevent the definite terminal colostomy, to cure a high percentage of patients with low and middle rectum cancer without respecting the oncology surgery principles, is well accepted by patients and it is feasible in our practice(AU)
