ABSTRACT Objective: To validate a checklist for safe bathing in critically ill patients. Methods: This is a methodological and quantitative study. Researchers developed a checklist for safe bathing in critically ill patients consisting of 41 items, which were submitted to the apparent and content validation process, evaluated by eleven judges, and interobserver reliability. For reliability analysis, the instrument was applied in 54 bed bath procedures in the ICU; Kappa and CHF tests were used. Results: In the apparent and content validation, adjustments were made according to the judges' suggestions. Kappa values ranged from moderate to almost perfect (0.462 to 0.962), and, in some items, there was 100% agreement; the reliability of the instrument was excellent (ICC = 0.962). Conclusion: The instrument proved to be dependable and easy to apply. Its use will contribute to safe bed bathing and subsidize interventions aimed at increasing the quality of care.
RESUMEN Objetivo: Validar lista de verificación para el baño seguro en pacientes críticos. Métodos: Estudio metodológico y cuantitativo. Se elaboró una lista de verificación para baño seguro en paciente crítico compuesta por 41 ítems, que fue sometida al proceso de validación aparente y de contenido, evaluada por 11 jueces, y confiabilidad interobservadores. Para el análisis de confiabilidad, el instrumento fue aplicado en 54 procedimientos de baño en el lecho en UTI; utilizados las pruebas de Kappa e ICC. Resultados: En la validación aparente y de contenido, fueron realizados ayustes conforme sugestiones de los jueces. Los valores de Kappa variaron de moderado a casi perfecto (0,462 a 0,962); y, en algunos ítems, hubo 100% de concordancia; la confiabilidad del instrumento fue excelente (ICC = 0,962). Conclusión: El instrumento se demostró confiable y de fácil aplicación. Su utilización contribuirá para el baño en el lecho seguro y subsidiará intervenciones que objetiven aumentar la calidad asistencial.

Elaborou-se uma lista de verificação para banho seguro no paciente crítico composta por 41 itens, que foi submetida ao processo de validação aparente e de conteúdo, avaliada por 11 juízes, e confiabilidade interobservadores. Para a análise de confiabilidade, o instrumento foi aplicado em 54 procedimentos de banho no leito em UTI; utilizaram-se os testes de Kappa e ICC. Resultados: Na validação aparente e de conteúdo, foram realizados ajustes conforme sugestões dos juízes. Os valores de Kappa variaram de moderado a quase perfeito (0,462 a 0,962); e, em alguns itens, houve 100% de concordância; a confiabilidade do instrumento foi excelente (ICC = 0,962). Conclusão: O instrumento demonstrou-se confiável e de fácil aplicação. Sua utilização contribuirá para o banho no leito seguro e subsidiará intervenções que visem aumentar a qualidade assistencial.
RESUMO
Green Analytical Chemistry (GAC) metrics include a variety of criteria, such as the regent amounts and toxicity, energy consumption, generated waste, among others. The analytical greenness metric (AGREE) and its variant for sample preparation (AGREEprep) cover different aspects that contribute to the environmental sustainability of sample preparation. White Analytical Chemistry (WAC) considers not only environmental aspects but also analytical and practical aspects with a holistic vision based on a Red-Green-Blue color model. A case study is presented to assess the green and white profile of a method based on ultrasound-assisted extraction and determination of Mn and Fe in beef using microwave-induced plasma atomic emission spectroscopy (MP AES). The method was validated and resulted simple, fast without external heating using diluted acids. It was concluded that we should think in green sample preparation with the AGREEprep tool, as well as in white holistic assessments (WAC) as both constitute complementary tools.

PURPOSE: The study aimed to investigate the effect of repeated cold-water immersion (CWI) after high-intensity interval exercise sessions on cardiac-autonomic modulation, neuromuscular performance, muscle damage markers, and session internal load. METHODS: Twenty-one participants underwent five sessions of high-intensity interval exercise (6-7 bouts of 2 min; pause of 2 min) over a two-week period. Participants were allocated randomly into either a group that underwent CWI (11-min; 11 °C) or a group that performed passive recovery after each exercise session. Before the exercise sessions were performed, countermovement jump (CMJ) and heart rate variability were recorded (i.e., rMSSD, low and high frequency power and its ratio, SD1 and SD2). Exercise heart rate was calculated by recording the area under the curve (AUC) response. Internal session load was evaluated 30 min after each session. Blood concentrations of creatine kinase and lactate dehydrogenase were analyzed before the first visit and 24 h after the last sessions. RESULTS: The CWI group presented higher rMSSD than the control group at each time point (group-effect P = 0.037). The SD1 was higher in CWI group when compared to the control group following the last exercise session (interaction P = 0.038). SD2 was higher in CWI group compared to the control group at each time point (group-effect P = 0.030). Both groups presented equal CMJ performance (P > 0.05), internal load (group-effect P = 0.702; interaction P = 0.062), heart rate AUC (group-effect P = 0.169; interaction P = 0.663), and creatine kinase and lactate dehydrogenase blood concentrations (P > 0.05). CONCLUSION: Repeated post-exercise CWI improves cardiac-autonomic modulation. However, no differences in neuromuscular performance, muscle damage markers, or session internal load were demonstrated between the groups.

Rice is among the main foods produced in the world and is part of the daily diet of most families. The main waste from rice processing is rice husk (RH), which has been used as biomass for energy generation through combustion. In this process, rice husk ash (RHA) is generated as a residue, and its silica (SiO2) content varies from 85 to 98%. The present work describes the study of the extraction of silica from RHA by the ultrasound-assisted sol-gel method. An experimental design based on the response surface methodology (RSM) with the symmetrical, second-order rotational central composite design (RCCD) was applied to determine the best extraction conditions considering extraction time and molar ratio (n) as variables = nNaOH/nSilica). These optimal conditions were then applied to three ash samples, two obtained by the combustion process in a boiler furnace, with a mobile grate system (RHAC1 and RHAC2), and one obtained by the pyrolysis process (RHAP) carried out in a fixed bed reactor. Results showed that a molar ratio of 4.4, and an extraction time of 107 min were the best extraction conditions, leading to a yield of 73.3% for RHAP, 43.9% for RHAC1, and 31.1% for RHAC2. It was found that the extraction yield and textural properties of the silica obtained depend on the characteristics of the ash used. The silica extracted from RHAC1 presented a surface area of 465 m2.g-1, mesopores of 4.69 nm, purity greater than 95%, and an ultra-fine granulometric distribution, reaching nanoparticle dimensions, characteristics comparable to commercially available silicas.

ABSTRACT Objective: To describe the elaboration and content validity stages of a digital educational technology for nursing professionals about Japanese bathtub (ofurô) in newborns in neonatal care units. Method: A methodological study conducted between August 2019 and July 2021, in a public maternity hospital in Manaus, Amazonas, developed in two stages. In the first, the e-book images, text and editing were produced. In the second, material content validity was carried out, through the opinion of 15 expert judges, nurses and neonatal care experts. In data analysis, the percentage of agreement was applied with agreement score estimation. Items with 80% or more agreement were considered valid. Results: The e-book "Japanese bathtub: manual for nursing professionals" was produced, organized into nine sessions, which describe the technique execution. Content was considered valid by expert judges (general score 90%). Conclusion: The e-book was considered suitable for use in training nursing professionals, with potential for dissemination of knowledge regarding humanization of care for premature babies.
RESUMEN Objetivo: Describir las etapas de elaboración y validación de contenido de una tecnología educativa digital para profesionales de enfermería sobre el baño ofurô en recién nacidos en Unidades de Cuidados Neonatales. Método: Estudio metodológico realizado entre agosto de 2019 y julio de 2021, en una maternidad pública de Manaus, Amazonas, desarrollado en dos etapas. En la primera se produjeron las imágenes, el texto y la edición del e-book. En el segundo, se realizó la validación de contenido del material, a través de la expresión de 15 jueces expertos, enfermeros y experimentados en cuidados neonatales. En el análisis de datos se aplicó el porcentaje de concordancia con estimación del puntaje de concordancia. Se consideraron válidos los ítems con un 80% o más de acuerdo. Resultados: Se elaboró el libro electrónico "Banho de ofurô: manual para profissionais de enfermagem" (Baño de Ofurô: manual para profesionales de enfermería), organizado en nueve sesiones, que describen la ejecución de la técnica. El contenido fue considerado válido por los jueces expertos (puntuación general 90%). Conclusión: El e-book se consideró apto para ser utilizado en la formación de profesionales de enfermería, con potencial para la difusión de conocimientos sobre la humanización del cuidado del recién nacido prematuro.

Conclusão: O e-book foi considerado adequado para ser utilizado em treinamento de profissionais de enfermagem, com potencial para a difusão do conhecimento quanto à humanização da assistência ao recém-nascido prematuro.

Background: Fluid overload (FO) is a common problem in patients with peritoneal dialysis (PD), it is associated with adverse outcomes and may persist despite adjustements in PD therapy. Objective: To evaluate the feasibility and safety of stimulated diaphoresis to reduce FO with the use of a portable sauna bath. Methods: Open-label pilot study in patients on continuous ambulatory peritoneal dialysis (CAPD) and FO. The primary outcome was the treatment-related adverse events; secondary outcomes were changes in over-hydration (OH), body weight and blood pressure, FO symptoms, and sleep quality. Dialysis prescription and daily data were recorded. The intervention period consisted in a 30-min, 45°C sauna bath, daily for 10 days, using a portable sauna bath. Results: Fifty-one out of 54 total sauna bath sessions were well tolerated. In three (5.5%) sessions adverse effects were reported: transient dizziness in two cases, and a second-degree skin burn in a patient with advanced diabetic neuropathy. OH (6.3 ± 1.2 L vs. 5.5 ± 1.3 L, p = 0.05), body weight (67.7 ± 11.4 vs. 66.8 ± 3.8 kg, p = 0.003), diastolic blood pressure (92 ± 13.5 vs. 83 ± 13.3 mmHg, P = 0.003) and PSQI score (7.3 ± 3.7 vs. 5.1 ± 3.2, p = 0.02) improved significantly between the control and intervention period, respectively. Conclusions: Stimulated diaphoresis with a portable sauna bath could be a novel, safe, and effective alternative way to reduce FO in CAPD patients. Larger studies are needed to confirm our results. Despite the recent promising results of MDMA (3,4-methylenedioxy-methamphetamine) as a psychotherapeutic agent and its history of misuse, little is known about its molecular mode of action. MDMA enhances monoaminergic neurotransmission in the brain and its valuable psychoactive effects are associated to a dual action on the 5-HT transporter (SERT). This drug inhibits the reuptake of 5-HT (serotonin) and reverses its flow, acting as a substrate for the SERT, which possesses a central binding site (S1) for antidepressants as well as an allosteric (S2) one. Previously, we characterized the spatial binding requirements for MDMA at S1. Here, we propose a structure-based mechanistic model of MDMA occupation and translocation across both binding sites, applying ensemble binding space analyses, electrostatic complementarity, and Monte Carlo energy perturbation theory. Computed results were correlated with experimental data (r = 0.93 and 0.86 for S1 and S2, respectively). Simulations on all hSERT available structures with Gibbs free energy estimations (ΔG) revealed a favourable and pervasive dual binding mode for MDMA at S2, i.e., adopting either a 5-HT or an escitalopram-like orientation. Intermediate ligand conformations were identified within the allosteric site and between the two sites, outlining an internalization pathway for MDMA. Among the strongest and more frequent interactions were salt bridges with Glu494 and Asp328, a H-bond with Thr497, a π-π with Phe556, and a cation-π with Arg104. Similitudes and differences with the allosteric binding of 5-HT and antidepressants suggest that MDMA may have a distinctive chemotype. Thus, our models may provide a framework for future virtual screening studies and pharmaceutical design and to develop hSERT allosteric compounds with a unique psychoactive MDMA-like profile.

RESUMEN Introducción: El Camphenol Plus es un derivado clorofenólico empleado como medicación intraconducto durante los tratamientos pulporradiculares en Estomatología. Son escasos los reportes científicos sobre el papel de los canales de iones potasio en la dinámica contráctil del músculo liso arterial inducida por dicho medicamento. Objetivo: Determinar el papel de los canales de iones potasio en la dinámica contráctil del músculo liso arterial inducida por Camphenol Plus. Método: Se realizó una investigación experimental preclínica en el Instituto de Fisiología "Oscar Langerdorff", Facultad de Medicina, Universidad de Rostock, Alemania, entre octubre y diciembre de 2018, con el empleo de 30 anillos de aorta obtenidos de 10 ratas Wistar (n=10). Las preparaciones biológicas se colocaron en baño de órganos y se preactivaron con solución Krebs concentrada en iones potasio, registrándose luego la tensión desarrollada por el músculo liso vascular tras la adición de soluciones de Camphenol Plus durante diferentes intervalos de tiempo. Se utilizaron las pruebas de Wilcoxon y U de Mann-Whitney. Resultados: El 31,4 % de la musculatura lisa vascular se relajó por acción del Camphenol Plus tras la preactivación con solución Krebs concentrada en iones potasio. El mayor descenso del tono vascular se produjo con el uso de soluciones del medicamento al 7 % entre el primer y tercer minutos. Conclusiones : El efecto vasorrelajante in vitro producido por Camphenol Plus sobre el músculo liso arterial está mediado por canales de iones potasio sensibles a voltaje, a calcio y a trifosfato de adenosina del endotelio vascular y el sarcolema.
ABSTRACT Introduction: Camphenol Plus is a chlorophenolic derivative commonly used as an intra - duct medication for pulporadicular treatments in Dentistry. Scientific reports about the use of this medication on the role of potassium ion channels in the contractile dynamics of induced arterial smooth muscle are low. Objective: To determine the role of potassium ion channels in the contractile dynamics of Camphenol Plus - induced arterial smooth muscle. Method: A preclinical experimental investigation was performed at the "Oscar Langerdorff" Institute of Physiology, Rostock University Medical Center, Rostock, Germany, between October and December 2018. A total of 30 aortic rings obtained from 10 Wistar rats (n=10) were used. The biological preparations were placed in an organ bath and preactivated with Krebs solution concentrated in potassium ions, afterwards it was recorded the tension developed by the vascular smooth muscle after applying the Camphenol Plus solutions in different time intervals. The Mann-Whitney U test and Wilcoxon test were applied. Results: The 31.4% of vascular smooth muscle was relaxed by the effect of Camphenol Plus after preactivation with Krebs solution concentrated in potassium ions. The greatest decrease in vascular tone occurred between the first and third minutes after the use of the drug solutions prepared at 7 %. Conclusions: The in vitro vasorelaxant effect produced by the Camphenol Plus medication on arterial smooth muscle is mediated by the potassium ion channels sensitive to voltage, calcium and the adenosine triphosphate of the vascular endothelium and sarcolemma.
RESUMO Introdução: Camphenol Plus é um derivado clorofenólico utilizado como medicação intracanal durante tratamentos pulporradiculares em Estomatologia. Existem poucos relatos científicos sobre o papel dos canais iônicos de potássio na dinâmica contrátil do músculo liso arterial induzida pela referida droga. Objetivo: Determinar o papel dos canais iônicos de potássio na dinâmica contrátil do músculo liso arterial induzida por Camphenol Plus. Método: Uma investigação experimental pré-clínica foi realizada no Instituto de Fisiologia "Oscar Langerdorff" da Faculdade de Medicina da Universidade de Rostock, Alemanha, entre outubro e dezembro de 2018, utilizando 30 anéis aórticos obtidos de 10 ratos Wistar (n=10). As preparações biológicas foram colocadas em banho de órgãos e pré-ativadas com solução de Krebs concentrada em íons potássio, registrando-se então a tensão desenvolvida pelo músculo liso vascular após a adição de soluções de Camphenol Plus em diferentes intervalos de tempo. Foram utilizados os testes U de Wilcoxon e Mann-Whitney. Resultados: 31,4% da musculatura lisa vascular relaxada pela ação do Camphenol Plus após pré-ativação com solução de Krebs concentrada em íons potássio. A maior diminuição do tônus vascular ocorreu com o uso de soluções medicamentosas a 7% entre o primeiro e o terceiro minutos. Conclusões: O efeito vasorrelaxante in vitro produzido pelo Camphenol Plus no músculo liso arterial é mediado por canais de íons de potássio sensíveis à voltagem, trifosfato de cálcio e adenosina do endotélio vascular e do sarcolema.

Abstract Objective: Evaluate the effectiveness of a children's soap with physiological pH in maintaining cutaneous pH and moisture of the newborn (NB)'s skin after the first bath. Methods: Randomized, controlled and double-blind clinical trial in a rooming-in of a tertiary maternity hospital in southern Brazil with 204 newborns > 34 gestational weeks. Gestational and obstetric history was evaluated, and newborns were randomized into two groups according to the product applied in the bath: the control group (CG), which used common liquid soap with pH 7.0 and experimental group (EG), which used children's liquid soap with pH 5.8. Evaluation was made immediately before and after bath with skin pH measurement, corneometry and clinical parameters (erythema, scaling and moisture), on the forehead, abdomen and thigh. Results: There was no difference between groups regarding gestational, obstetric and family history (p > 0.05). In CG, skin pH increased in the abdomen and thigh (p < 0.05). In EG there was an improvement in clinical parameters after bathing with: increased moisture, less erythema and less scaling (p < 0.05). On the forehead, there was a significant increase in pH after bathing (p < 0.001) similar in both groups, although no use of soap. There was no difference in corneometry between groups after bathing. Conclusions: Children's liquid soap with physiological pH maintained the acidic skin pH and moisture of the newborn's skin after the first bath, which reinforces the importance of using products with physiological pH in the hygiene of newborns. Due to the scarcity of information regarding the control of Rhipicephalus microplus on bovines with different tick burdens, tick counts and the efficacy of a commercial spray formulation (aspersion bath with organophosphate + pyrethroid) were evaluated for animals with a low and high tick burdens for 35 days in a stall test. Thirty-two crossbreed Bos taurus indicus x Bos t. taurus experimentally infested with R. microplus larvae were divided into four groups: T01 high infestation control, T02 high infestation treated, T03 low infestation control, and T04 low infestation treated. The bovines were kept individually in pens and R. microplus females were collected daily. All data were statistically analyzed with a significance level of 5% (P ≤ 0.05). For linear regressions, variables with P≤0.05 and the highest coefficients of determination (R2 ≥ 0.70) were considered the best descriptors. The reliability level was 95%. Tick counts differed (P<0.0001) between T01 and T02 (high infestation control vs. treated), except at day 2 (P >0.05) post-treatment, and efficacy ranged from 85.2 to 50.6%. The number of collected engorged females was lower (P<0.0001) for T04 than for T03 (low infestation treated vs. control) animals from day 3 to 29, and efficacy ranged from 95.2 to 69.8%. In addition, tick burden and efficacy were negatively correlated for both the group of animals with a high tick burden (r = -0.5256; p = 0.0012; R² = 0.2762) and the group of animals with a low tick burden (r = -0.9817; p < 0.0001; R² = 0.9638). In conclusion, a high tick burden on bovines decreases the efficacy of the tested spray acaricide.

Resumo Objetivo Verificar positividade de culturas microbiológicas de pacientes hospitalizados, com prescrição de enfermagem para banho no leito, em três períodos disjuntos e sequenciais das modalidades de banho no leito: convencional (BLC), descartável (BLD) e descartável acrescido de clorexidina degermante à 2% (BLD-CX). Métodos Estudo ecológico, tipo séries temporais de 48 meses, em três períodos (P1=BLC; P2=BLD; P3=BLD-CX), com dados secundários de prontuário eletrônico, de pacientes que estiveram internados em hospital do estado de São Paulo, Brasil. Resultados Nos períodos de banhos descartáveis, o percentual de positividade foi, em média, 14,6% menor quando comparado aos meses cujo banho de leito foi convencional. No período de BLD-CX o percentual de positividade foi, em média, 19,3% menor quando comparado aos meses do período de BLC. Contudo, não há evidências de diferença no percentual de positividade entre o BLD (b = -14,6%; IC95% = (-18,9% a -10,3%) e o BLD-CX (b = - 19,3%; IC95% = (-24,4% a -14,22%). A cada ano a mais na idade média do paciente, o percentual de positividade aumenta, em média 0,3% (p=0,060). Não houve associação sazonal para positividades das culturas microbiológicas nas modalidades de banho. Conclusão A positividade de culturas microbiológicas, em pacientes com prescrição de enfermagem para banho no leito, é menor quando se utiliza as modalidades descartáveis. Recomenda-se adotar rotineiramente o BLD, deixando a prescrição de BLD-CX, somente para degermação da pele para procedimentos invasivos, operatórios e higienização das mãos de profissionais de saúde.
Resumen Objetivo Verificar los resultados positivos de culturas microbiológicas de pacientes hospitalizados, con prescripción de enfermería de baño en cama, en tres períodos disjuntos y secuenciales de las modalidades de baño en cama: convencional (BCC), descartable (BCD) y descartable con clorhexidina al 2 % (BCD-CX). Métodos Estudio ecológico, tipo series temporales de 48 meses, en tres períodos (P1=BCC; P2=BCD; P3=BCD-CX), con datos secundarios de historia clínica electrónica, de pacientes que estuvieron internados en hospital del estado de São Paulo, Brasil. Resultados En los períodos de baños descartables, el porcentaje de resultados positivos presentó un promedio 14,6 % inferior en comparación con los meses en que el baño en cama fue convencional. Durante el período de BCD-CX el porcentaje de resultados positivos fue, en promedio, 19,3 % inferior en comparación con los meses del período de BCC. Sin embargo, no hay evidencias de diferencias en el porcentaje de resultados positivos entre el BCD (b = -14,6 %; IC95 % = (-18,9 % a -10,3 %) y el BLD-CX (b = - 19,3 %; IC95 % = (-24,4 % a -14,22 %). Por cada año que se suma a la edad promedio del paciente, el porcentaje de resultados positivos aumenta en promedio 0,3 % (p=0,060). No se verificó una asociación estacional en las respuestas positivas de las culturas microbiológicas en las modalidades de baños. Conclusión Los resultados positivos de culturas microbiológicas de pacientes con prescripción de enfermería de baño en cama son menores cuando se utilizan las modalidades descartables. Se recomienda adoptar de forma rutinaria el BCD y dejar la prescripción de BCD-CX solo para la eliminación de los gérmenes de la piel en procedimientos invasivos, quirúrgicos e higienización de manos de los profesionales de salud.
Abstract Objective To verify microbiological culture positivity of hospitalized patients, with a nursing prescription for bed bath, in three disjoint and sequential periods of bed bath modalities: conventional (CBB), disposable (DBB) and disposable plus 2% chlorhexidine degerming (DBB-CX). Methods This is an ecological, time series study of 48 months, in three periods (P1=CBB; P2=DBB; P3=DBB-CX), with secondary data from electronic medical records of patients who were admitted to a hospital in the state of São Paulo, Brazil. Results In the periods of disposable baths, the percentage of culture positivity was, on average, 14.6% lower when compared to the months in which bed bath was conventional. In the DBB-CX period, the percentage of culture positivity was, on average, 19.3% lower when compared to the CBB period months. However, there is no evidence of difference in the percentage of culture positivity between DBB (b = -14.6%; 95%CI = (-18.9% to -10.3%) and DBB-CX (b = - 19.3%;95%CI = (-24.4% to -14.22%) For each year more in patients' mean age, the percentage of culture positivity increases by an average of 0.3% (p=0.060). There was no seasonal association for microbiological culture positivity in bath modalities. Conclusion Microbiological culture positivity in patients with a nursing prescription for bed bath is lower when disposable modalities are used. It is recommended to routinely adopt DBB, leaving the prescription of DBB-CX only for skin degermation for invasive and operative procedures and hand hygiene of health professionals.

Resumo Objetivo Produzir um vídeo instrucional para enfermeiros sobre o banho de ofurô em recém-nascidos pré-termo e validar seu conteúdo. Método Estudo metodológico de desenvolvimento tecnológico com ênfase na validação de conteúdo, conduzido em três etapas: revisão exploratória da literatura, produção da tecnologia educacional do tipo vídeo instrucional e validação por especialistas. Os dados foram sistematizados e organizados em planilhas e as análises foram conduzidas no pacote estatístico R. Para validar o conteúdo, aplicaram-se o Índice Kappa, para medir a concordância entre as respostas, e o Índice de Validade de Conteúdo, para estimar a representatividade do item. Resultados As publicações sobre a técnica (n=3) identificadas na revisão de literatura foram utilizadas para a produção do vídeo. O vídeo simula o cuidado em unidade neonatal e tem duração de 13 minutos. No processo de validação, o vídeo foi considerado, no geral, com representatividade de conteúdo adequada e obteve índice de concordância entre as respostas quase perfeito. Dois itens foram avaliados como "adequados com alterações" e modificados. Conclusão e implicações para a prática O material produzido foi considerado adequado para ser utilizado na capacitação de enfermeiros sobre a técnica e compartilhamento de informações, contribuindo para o cuidado de enfermagem humanizado e seguro em unidades neonatais.

Resultados Para la producción del video se utilizaron publicaciones sobre la técnica (n = 3) identificadas en la revisión de la literatura. El video simula la atención en una unidad neonatal y tiene una duración de 13 minutos. En el proceso de validación, el video se consideró, en general, con adecuada representación de contenido y con un índice de concordancia casi perfecto entre las respuestas. Dos ítems se evaluaron como "adecuados con cambios" y se modificaron. Conclusión e implicaciones para la práctica El material producido fue considerado por los especialistas como adecuado para uso en la formación de enfermeros sobre la técnica y el intercambio de información, contribuyendo para la atención de enfermería humanizada y segura en las unidades neonatales.
Abstract Objective To produce an instructional video for nurses about hot tub bath for preterm newborns and to validate its content. Method Methodological study of technological development with emphasis on content validation, conducted in three stages: exploratory literature review, production of an instructional video educational technology and expert validation. The data were systematized and organized in spreadsheets and the analyses were carried out in the statistical package R. For content validation, the Kappa Index was calculated to measure response agreement and the Content Validity Index was employed to estimate item representativeness. Results Publications about the technique (n=3) identified in the literature review were used for video production. The video simulates care in a neonatal unit and lasts 13 minutes. In the validation process, the video was considered, in general, to have an adequate content representation and had an almost perfect response agreement index. Two items were assessed as "adequate after revision" and modified. Conclusion and implication for the practice The produced material was considered by the experts as suitable for use in training nurses on the technique and sharing information, contributing to a humanized and safe nursing care in neonatal units.

BACKGROUND: Bathing with 2% chlorhexidine (CHG) wipes is an important measure regarding infection prevention in critically ill patients. The aim of this study was to evaluate the impact of CHG wipes bath to prevent central-line associated bloodstream infection (CLABSI) in critically ill patients and determine if such measure is cost-saving. METHODS: a quasi-experimental study, conducted from July 2017 to April 2019. Daily bath with 2% CHG was used in all patients at the unit in the intervention period. The following were evaluated: CLABSI incidence density in both periods, 30- day mortality, guided antimicrobials used to treat CLABSI and 2% CHG costs. RESULTS: CLABSI incidence density dropped from 8.69 to 1.83 per 1.000 central line-days (pâ¯=â¯0.001), mainly by Klebsiella pneumoniae Carbapenen Resistant (Kp-KPC) (pâ¯=â¯0.05). Costs with guided antimicrobials for the treatment in pre-intervention were US$ 46,114.36, and in the intervention period, US$ 4,177.50. The 2% CHG monthly cost was US$ 2,698.00, achieving 30% savings when comparing both periods. DISCUSSION: An expressive reduction of 79% in CLABSI incidence density was observed, mainly due to Kp-KPC infection and also a reduction in guided antimicrobial costs. CONCLUSIONS: Bathing with 2% CHG led to evident CLABSI reduction.

The complexity of aerodynamic particulate matter's (PM) matrices poses a challenge for the extraction and quantification of metals, especially for analytes with low concentration. Aiming to solve this issue, a precise and accurate protocol with the ultrasound extraction combined with microwave radiation digestion (USMW), was applied to PM samples with excellent compensations in sample throughput, digestion efficiency, and energy consumption. After the digestion and extraction procedures, the inorganic analytes, including rare earth elements, were determined by ICP OES. Two types of particulate matter sampled from two stations, Gobernacion (GOB10 and GOB2.5) and Milan (MIL10), corresponding to PM2.5 and PM10, were digested with a combination between HF, HNO3, and H3BO3. The absolute limits of detection ranged from 0.42 pg m-³ for V, to 3459 pg m-³ for As. The accuracy of the experimental study was assessed using two certified reference materials (CRMs), Coal Fly Ash (NIST1633b) and Fly Ash (BCR176). The method presented good accuracy, with recoveries ranging from 90 to 115%, except for Al (120%) and Fe (123%). Considering the replicates for the determination of analyte elements, the repeatability was below 10% for the relative standard deviation (RSD). A cloud point extraction (CPE) procedure, with parameters optimized for the determination of Pd and Pt, was successfully applied in digested PM samples with detection limits of 1.43 and 2.05 pg m-³ for Pd and Pt in MIL10 sample, respectively, and 76.6 pg m-³ for Pd and 110 pg m-³ for Pt, in samples GOB10 and GOB2.5, respectively.

BACKGROUND: This observational research study analyses the uptake of physical therapies treatments in the Polyclinic during the Rio 2016 Olympic Games. OBJECTIVE: To describe the usage of physical therapies services - physical therapy, osteopath, chiropractic, and sports massage - by athletes and non-athletes and across different sports. METHODS: The multidisciplinary team of physical therapies recorded treatment modalities, information on provider discipline and reason for attendance, in an Electronic Medical Record system throughout the 32 days of operation of the Olympic Polyclinic. Cold-therapy total immersion ice baths (TIIB) were provided as part of the services, but were reported and analysed separately. RESULTS: There were 4993 encounters (4038 athletes, 955 non-athlete encounters). 1395 athletes (12.4% of all athletes) and 393 non-athletes sought treatment. For all four provider disciplines, in addition to TIIB, the primary reason for athlete attendance was for recovery (52% of all encounters), followed by injury treatment (30%), and maintenance (16%). Athletes reported "injury" as the main reason for physical therapy (92% of all encounters, 2.8 encounters per athlete), chiropractic (94%, 1.9) and osteopathy (91%, 1.8) visits. Almost all TIIB visits were used for recovery (98% of all TIIB encounters; 2.1 encounters per athlete). Athletes from handball (37% of all handball athletes), followed by judo (22%), and athletics (21%), presented the largest user groups. CONCLUSION: This Olympic Polyclinic study evaluates the physical therapies' activity, and athlete's reason for use of the multidisciplinary physical therapies team, including total immersion ice bath provision. These results emphasise the importance of a multidisciplinary approach.

Introducción: El p-clorofenol alcanforado es un derivado clorofenólico de uso común como medicación intraconducto en Endodoncia. Son escasos los reportes científicos sobre sus efectos en la musculatura lisa vascular arterial y la regulación del flujo sanguíneo local. Objetivo: Determinar el efecto del p-clorofenol alcanforado sobre la dinámica contráctil del músculo liso vascular arterial en el tiempo. Método: Se realizó una investigación experimental preclínica utilizando 14 anillos de aorta obtenidos de ratas Wistar. Los anillos se colocaron en baño de órganos y se preactivaron con noradrenalina, registrándose luego la tensión desarrollada por el músculo liso vascular tras la adición de p-clorofenol alcanforado durante diferentes intervalos de tiempo. Resultados: El 51,4 porciento de la musculatura lisa vascular se relajó por la acción del p-clorofenol alcanforado. El mayor descenso del tono vascular se produjo entre el tercer y quinto minuto de añadido el medicamento. Las pruebas de Wilcoxon de los rangos con signos evidenciaron diferencias significativas entre la tensión base inicial y la registrada en los diferentes intervalos de tiempo estudiados. Conclusiones: el p-clorofenol alcanforado, induce in vitro, relajación del músculo liso arterial a través de un acoplamiento excitación-contracción de tipo farmacomecánico, la cual se incrementa en función del tiempo(AU).
Introduction: Camphorated p-chlorophenol is a chlorophenolic derivative commonly used as an intra-oral medication in endodontics. Scientific reports on its effects in arterial vascular smooth muscle and local blood flow regulation are scarce. Objective: To determine the effect of camphorated p-chlorophenol on the contractile dynamics of arterial vascular smooth muscle. Method: An experimental and preclinical research was conducted with the use of 14 aortic rings of Wistar rats. The rings were placed in an organ bath and preactivated with noradrenaline, and the tension developed by the vascular smooth muscle at different time intervals was recorded after induction of camphorated p-chlorophenol. Results: Most of the vascular smooth muscle (51.4 percent) relaxed with the use of camphorated p-chlorophenol. The greatest decrease in vascular tone occurred between the third and fifth minute after use the drug. Wilcoxon rank tests showed significant differences between tension observed at baseline and those recorded at the different time intervals studied. Conclusions: Camphorated p-chlorophenol, induces in vitro, relax the arterial smooth muscle through a pharmacomechanical excitation-contraction link, which increases according to the time(AU).
Introdução: O cânfora-clorofenol é um derivado clorofenólico comumente utilizado como medicamento intracanal em Endodontia. Relatórios científicos sobre seus efeitos no músculo liso vascular arterial e na regulação do fluxo sanguíneo local são escassos. Objetivo: Determinar o efeito da cânfora-clorofenol na dinâmica contrátil do músculo liso vascular arterial ao longo do tempo. Método: Foi realizada investigação experimental pré-clínica com 14 anéis aórticos obtidos de ratos Wistar. Os anéis foram colocados em banho de órgãos e pré-ativados com norepinefrina, em seguida, a tensão desenvolvida pela musculatura lisa vascular foi registrada após a adição de cânfora-clorofenol em diferentes intervalos de tempo. Resultados: 51,4 porcento dos músculos lisos vasculares estavam relaxados pela ação do cânfora-clorofenol. A maior diminuição do tônus vascular ocorreu entre o terceiro e o quinto minuto após a adição do medicamento. Os testes de Wilcoxon das faixas com sinais mostraram diferenças significativas entre a tensão base inicial e a registrada nos diferentes intervalos de tempo estudados. Conclusões: O cânfora-clorofenol induz, in vitro, relaxamento da musculatura lisa arterial por meio de um acoplamento excitação-contração do tipo farmacomecânico, que aumenta em função do tempo(AU).

We construct a microscopic model to study discrete randomness in bistable systems coupled to an environment comprising many degrees of freedom. A quartic double well is bilinearly coupled to a finite number N of harmonic oscillators. Solving the time-reversal invariant Hamiltonian equations of motion numerically, we show that for N=1, the system exhibits a transition with increasing coupling strength from integrable to chaotic motion, following the Kolmogorov-Arnol'd-Moser (KAM) scenario. Raising N to values of the order of 10 and higher, the dynamics crosses over to a quasi-relaxation, approaching either one of the stable equilibria at the two minima of the potential. We corroborate the irreversibility of this relaxation on other characteristic timescales of the system by recording the time dependences of autocorrelation, partial entropy, and the frequency of jumps between the wells as functions of N and other parameters. Preparing the central system in the unstable equilibrium at the top of the barrier and the bath in a random initial state drawn from a Gaussian distribution, symmetric under spatial reflection, we demonstrate that the decision whether to relax into the left or the right well is determined reproducibly by residual asymmetries in the initial positions and momenta of the bath oscillators. This result reconciles the randomness and spontaneous symmetry breaking of the asymptotic state with the conservation of entropy under canonical transformations and the manifest symmetry of potential and initial condition of the bistable system.

Caffeic acid is a phenolic compound widely found in commonly consumed foods such as pears, apples and coffee, and is pharmacologically known for its antioxidant, anti-inflammatory and anti-asthmatic properties. However, its relaxant activity in the aorta, uterus and ileum smooth muscle has not been investigated. This study aimed to comparatively evaluate the effect of caffeic acid on smooth muscle from different organs (aorta, uterus and ileum), and the contractions of this different organ were induced by different agonists. The organ bath technique was used, where the organs were placed in different cuvettes with 10 mL of Tyrode solution for 1 h to stabilize, then, myometrial, intestinal strip and aortic ring contractions were evoked using different contractile agonists (KCl 60 mM, PHE 0.1 µM, OT 10-2 IU/mL, CCh 10-6 M and BaCl2 0.1-30 mM); increasing concentrations of caffeic acid (0.03-7 mM) were administered in the experimental preparations. In the presence of KCl (60 mM), caffeic acid caused relaxations with the following EC50 values: 2.7 ± 0.26 mM/mL (aorta), 5.7 ± 0.71 mM/mL (uterus) and 2.1 ± 0.39 mM/mL (ileum). When in the presence of different agonists, PHE (0.1 µM) for the aorta, OT (10-2 IU/mL) for the uterus and CCh (10-6 M) for the ileum, caffeic acid caused relaxations with EC50 values of: 2.7 ± 0.31 mM/mL; 2.2 ± 0.34 mM/mL and 2.0 ± 0.28 mM/mL, respectively. The inhibitory effect of caffeic acid on serotonergic (aorta and uterus) and muscarinic receptors (uterus and ileum), as well as its possible involvement with L-type Ca2+ channels, was also observed. This study reports the pharmacological characterization of caffeic acid on smooth muscle from different organs, for which caffeic acid was more potent in the ileum. A diverse understanding of its performance as a possible therapeutic product is attributed to its relaxant effect.

Proper advice regarding home-based care of normal infants is no less important than managing critical diseases. Otherwise parents tend to follow traditional practices running in the family which are often deleterious. Clinicians must also be aware of the faulty traditional practices so that they can actively enquire them in the child's history.The author enlisted various such faulty traditional practices as experienced by him during his OPD practice in various public and corporate hospitals over a period of five years. Topics relating mainly to the infant age group have been chosen for this article. Relevant search was done on these topics in standard paediatric textbooks, journals, and websites (like WHO, NICE, AAP) to find the validity of these practices as well as the standard recommendations on these topics.It was found that the standard recommendations often differ from the traditional practices. Detailed text was included from various authentic sources regarding few dubious topics related to home-based infant care.Clinicians should be aware of the correct recommended methods regarding the day to day home-based infant care. They should counsel the parents accordingly so as to avoid any harm caused by faulty traditional practices and social beliefs.
El asesoramiento adecuado sobre la atención domiciliaria de los bebés normales no es menos importante que el manejo de enfermedades críticas. De lo contrario, los padres tienden a seguir las prácticas tradicionales de la familia que a menudo son perjudiciales. Los médicos también deben ser conscientes de las prácticas tradicionales defectuosas para poder investigarlas activamente en la historia del niño.El autor reclutó varias prácticas tradicionales defectuosas que experimentó durante su práctica en OPD en varios hospitales públicos y corporativos durante un período de cinco años. Para este artículo se han elegido temas relacionados principalmente con el grupo de edad infantil. Se realizaron búsquedas relevantes sobre estos temas en libros de texto, revistas y sitios web estándar de pediatría (como OMS, NICE, AAP) para encontrar la validez de estas prácticas, así como las recomendaciones estándar sobre estos temas.Se encontró que las recomendaciones estándar a menudo difieren de las prácticas tradicionales. Se incluyó texto detallado de varias fuentes auténticas con respecto a algunos temas dudosos relacionados con el cuidado infantil en el hogar.Los médicos deben conocer los métodos correctos recomendados con respecto al cuidado infantil diario en el hogar. Deben asesorar a los padres en consecuencia para evitar cualquier daño causado por prácticas tradicionales y creencias sociales defectuosas.
