La obstrucción de la luz del intestino delgado por un cálculo biliar ubicado de forma anómala, denominada incorrectamente íleo biliar, representa del 1 al 3 % de los síndromes obstructivos intestinales. Se define como una entidad inusual, caracterizada por la obstrucción intestinal mecánica, causada por el impacto de uno o varios cálculos biliares en la luz intestinal, por lo general en el ilion terminal. El objetivo es describir el presente caso por lo infrecuente de esta entidad, describir la sistemática diagnóstica y conducta tomada, así como realizar el análisis de la literatura relacionada. Se trata de una paciente femenina de 87 años de edad, que acude por cuadro de vómitos y distensión abdominal de 20 días de evolución, con muy mal estado general. Se realiza laparotomía exploradora, donde se observa en el ilion proximal gran cálculo de aproximadamente 4 cm, que obstruye la luz en su totalidad. Se realiza enterolitotomía y resección intestinal sin intervención en la vesícula biliar o la vía biliar. Fallece al tercer día del postoperatorio mediato por descompensación de su enfermedad cardiovascular. Se concluye que el íleo biliar es una entidad infrecuente sin estudios que estandaricen un tratamiento específico, y que requiere de intervención quirúrgica. La enterotomía para extracción de cálculos es una técnica segura y eficaz, sin necesidad de intervención en la vía biliar en un primer tiempo.


Obstruction of the lumen of the small intestine by an abnormally located gallstone, incorrectly called gallstone ileus, accounts for 1% to 3% of all obstructive bowel syndromes. It is defined as an unusual entity characterized by mechanical intestinal obstruction, caused by the impact of one or several gallstones in the intestinal lumen, usually in the terminal ilium. The objective is to describe the present case due to the infrequency of this entity, to describe the diagnostic system and conduct taken, as well as to carry out the analysis of the related literature. An 87-year-old female patient presented with symptoms of vomiting and abdominal distension of 20 days of evolution, with an unusual, very poor general condition, characterized by mechanical intestinal obstruction caused by the impact of one or several gallstones in the intestinal lumen, usually in the terminal ilium. An exploratory laparotomy was performed, where a large stone of approximately 4 cm was observed in the proximal ileum which completely obstructed the lumen. Enterolithotomy and bowel resection were performed without intervention in the gallbladder or bile duct. She died on the third day of the immediate postoperative period due to decompensation of her cardiovascular disease. It is concluded that gallstone ileus is a rare entity without studies that standardize a specific treatment and that it requires surgical intervention. Enterotomy for stone removal is a safe and effective technique without the need for intervention in the bile duct in the first time.

Gallstones causing bowel obstruction, known as gallstone ileus, are rare and account for less than 0.5% of small bowel obstruction cases. Additionally, it is a rare complication affecting only 0.3% of patients who have gallstones. Fistula formation between the biliary system, most commonly between the gallbladder and duodenum because of their proximity, facilitates the migration of gallstones into the enteric system with subsequent impaction in the small intestine, usually in the distal ileum close to the ileocecal valve, promoting the development of mechanical small bowel obstruction. Computerized tomography of the abdomen and pelvis is a confirmatory and widely used imaging study when there are two signs of Rigler's triad, which includes pneumobilia, evidence of small bowel obstruction and the presence of radiopaque stones. We report a case of a 75-year-old Caucasian man who presented with abdominal distention with signs of severe dehydration secondary to intractable nausea and vomiting complicated with severe acute kidney injury and was found to have a 4.7-centimeter gallstone-induced small intestinal obstruction.

El íleo biliar es una rara complicación de la litiasis vesicular, que se comporta como un cuadro de obstrucción intestinal mecánica, cuya causa es la impactación de uno o más cálculos biliares en su luz. Es evidente que, por lo infrecuente de esta entidad en los servicios quirúrgicos, el diagnóstico de la misma es demorado o no se realiza, incidiendo en la evolución del paciente de manera desfavorable. El objetivo de este trabajo es considerar que un diagnóstico precoz del íleo biliar -basado en el análisis de los antecedentes del enfermo, las manifestaciones clínicas y la interpretación correcta en busca de los signos de la tríada de Rigler- permite una mejor evolución de los pacientes. Los estudios imagenológicos y su interpretación son fundamentales en el diagnóstico de esta entidad.


Biliary ileus is a rare complication of gallbladder lithiasis, which behaves like a mechanic intestinal obstruction, the cause of which is the impaction of one or more gallstones in its lumen. It is evident that because of the infrequency of this entity in the surgical services, its diagnosis is delayed or not carried out, affecting the patient's evolution in an unfavorable way. The aim of this paper is to consider that a precocious diagnosis of biliary ileus-based on the analysis of the patient's history, the clinical manifestations and correct interpretation in search of the signs of Rigler's triad-allows a better evolution of patients. Imaging studies and their interpretation are fundamental in the diagnosis of this entity.

El síndrome de Bouveret es una causa infrecuente de íleo biliar, posee una elevada morbimortalidad. Se produce por la migración de un cálculo, generalmente de gran tamaño, hacia el tracto gastrointestinal a través de una fistula colecistoduodenal. El objetivo de esta carta científica es presentar un paciente con un estadio previo de la enfermedad donde la fistula se encuentra constituida, pero sin migración del cálculo, por lo que el diagnóstico oportuno tendría un impacto favorable en el tratamiento y pronóstico debido a que no se encuentra instaurado el cuadro obstructivo intestinal.


Bouveret's syndrome is a rare cause of biliary ileus, with a high morbimortality. It is caused by the migration of a stone, usually large, into the gastrointestinal tract through a cholecystoduodenal fistula. The objective of this scientific letter is to present a patient with a previous stage of the disease where the fistula is constituted, but without migration of the calculus, so that the timely diagnosis would have a favorable impact on the treatment and prognosis because the obstructive intestinal condition is not established.


A síndrome de Bouveret é uma causa rara de íleo biliar com uma morbilidade e mortalidade elevadas. É causada pela migração de uma pedra, geralmente grande, para o tracto gastrointestinal através de uma fístula colecystoduodenal. O objectivo desta carta científica é apresentar um doente com uma fase anterior da doença em que a fístula está presente, mas sem migração da pedra, para que o diagnóstico atempado tenha um impacto favorável no tratamento e prognóstico, uma vez que a condição obstrutiva instestinal não está estabelecida.

Biliary ileus is a consequence of the migration of a gallstone from the gallbladder to the digestive tract, most often via a biliodigestive fistula that causes a bowel obstruction. The clinic is atypical and capricious, including bowel obstruction and signs of cholecystitis, causing a delay in diagnosis. The therapeutic objective is to remove the intestinal obstacle by an enterotomy with stone extraction, with or without treatment of the biliary pathology (cholecystectomy and biliary fistula cure). The surgery remains the treatment of choice; laparoscopy and endoscopy present a less invasive alternative and are beginning to prove their effectiveness. The morbi-mortality remains high for biliary ileus, this is principally caused by the delay in diagnosis.

El íleo biliar es una complicación poco frecuente de la colelitiasis o colecistitis litiásica, aparece en menos del 1-3% de los pacientes con estas enfermedades, se define como una obstrucción intestinal mecánica debido a la impactación de uno o más cálculos biliares dentro del tracto gastrointestinal. Ubicaciones menos comunes incluyen estómago y duodeno (síndrome de Bouveret) y el colon. El diagnóstico del síndrome de Bouveret (SB) suele iniciar con una radiografía simple de abdomen; la ecografía abdominal confirma el síndrome de Bouveret si revela neumobilia y ubicación ectópica del cálculo biliar, sin embargo, en la mayoría de los casos se necesita una tomografía computarizada que permite una evaluación completa.Caso Clínico : Se presenta caso de masculino de 57 años de edad quien 8 días previo a su ingreso presenta dolor abdominal en epigastrio de moderada intensidad acompañado de pirosis, halitosis, náuseas, vómitos, intolerancia a la vía oral y ausencia de evacuaciones; acude inicialmente a gastroenterólogo que realiza endoscopia digestiva superior encontrando una imagen de obstrucción a nivel de primera porción de duodeno, motivo por el cual refiere a servicio de cirugía general donde es intervenido quirúrgicamente, se diagnostica el SB realizando gastrotomía, extracción del lito y piloroplastia, presentando adecuada evolución postoperatoria.Conclusión : El SB es un diagnóstico raro que afecta a <0,5% de los pacientes con cálculos biliares por lo que requiere un grado extremadamente alto de sospecha para el diagnóstico y puede observarse de manera incidental en estudios de imágenes en pacientes que presentan síntomas de obstrucción del vaciamiento gástrico(AU)


Biliary ileus is a rare complication of cholelithiasis or lithiasic cholecystitis, appearing in less than 1 to 3% of patients with these diseases, it is a mechanical intestinal obstruction due to the impaction of one or more gallstones within the gastrointestinal tract. Less common locations include the stomach and duodenum (Bouveret syndrome) and the colon. For the diagnosis of Bouveret's syndrome (BS) the initial step is usually a simple abdominal X-ray, while an abdominal ultrasound confirms Bouveret's syndrome if it reveals pneumobilia and ectopic location of the gallstone, in most cases a computed tomography (CT) scan is needed for diagnosis and full evaluation.Clinical Case: We present the clinical case of a 57-year-old male who 8 days prior to his admission presented with moderate intensity abdominal pain in the epigastrium accompanied by heartburn, halitosis, nausea, vomiting, intolerance to oral intake, and absence of evacuations; initially went to a gastroenterologist who performed upper digestive endoscopy, finding an image of obstruction at the level of the first portion of the duodenum, which is why he refers to the general surgery department where he underwent surgery, diagnosing BS by performing gastrotomy, stone extraction, and pyloroplasty, presenting adequate postoperative evolution.Conclusion : BS is a rare diagnosis that affects <0.5% of patients with gallstones and therefore requires an extremely high degree of suspicion for the diagnosis and can be seen incidentally on imaging studies in patients with symptoms of obstruction of gastric emptying(AU)

Resumen Introducción: El íleo biliar es una causa poco frecuente de obstrucción intestinal mecánica, siendo más frecuente en pacientes mayores de 65 años y sexo femenino. Casos Clínicos: Se exponen cuatro casos clínicos diagnosticados de íleo biliar, intervenidos en nuestro centro. Discusión: El íleo biliar es una enfermedad que se produce a consecuencia del paso de un cálculo al lumen intestinal. Esto se produce por la presencia de una fístula entre la vesícula y el intestino, localizada mayormente a nivel duodenal. Se manifiesta con una clínica muy inespecífica, lo que condiciona un retraso en su diagnóstico. La tomografía computarizada es actualmente el examen de elección. El tratamiento consiste en la enterotomía con extracción del cálculo, realizándose la reparación de la fístula en un primer o segundo tiempo. Tiene un mal pronóstico debido a comorbilidad de los pacientes y la demora diagnóstica y terapéutica.


Introduction: Gallstone ileus is a rare cause of mechanical intestinal obstruction, being more frequent in patients over 65 years and females. Clinical Cases: Four clinical cases diagnosed with gallstone ileus, operated on in our center, are reported. Discussion: Gallstone ileus frequently occurs as a result of a fistula that communicates the digestive tract with the gallbladder. Their clinic is very unspecific, which determines a delay in its diagnosis. Computed tomography is currently the gold standard. The treatment is enterolithotomy and fistula repair in a first or second stage. Prognosis is poor due to patient comorbidities and diagnostic and therapeutic delay.

Resumen El síndrome de Bouveret (SB) es una patología poco frecuente, la cual se caracteriza por la presencia de obstrucción gástrica o duodenal secundaria a un cálculo enclavado en la luz duodenal, el cual migra a través de una fístula colecistoduodenal. Su incidencia es de alrededor 1% al 3% de todos los casos de íleo biliar. Los principales síntomas consisten en vómito, dolor abdominal, hematemesis, pérdida de peso y anorexia. En el 91% de los casos se requiere manejo quirúrgico. En este artículo se presenta un caso de un paciente de 50 años, con cuadro clínico de 2 meses de evolución de dolor abdominal en epigastrio y mesogastrio, distensión abdominal y múltiples episodios de emesis. Al examen físico con clínica de obstrucción intestinal se realizó tomografía abdominal, donde se evidenció obstrucción intestinal por imagen intraluminal en primera porción duodenal asociada con tríada de Rigler, con diagnóstico de síndrome de Bouveret.


Abstract Bouveret syndrome is a rare pathology which is characterized by gastric or duodenal obstruction secondary to a gallstone embedded in the lumen after migrating through a cholecystoduodenal fistula. Its incidence is approximately 1% to 3% of all cases of biliary ileus. The main symptoms consist of vomiting, abdominal pain, hematemesis, weight loss and anorexia. Surgery is required in 91% of cases. This article presents the case of a 50-year-old patient who had suffered from abdominal pain in the epigastrium and mesogastrium, abdominal distension and multiple episodes of emesis for two months. Physical examination indicated obstruction of the intestine. An abdominal CT scan showed that the obstruction was in the first duodenal portion and that Rigler's triad was present. It was diagnosed as Bouveret Syndrome.

Background: Gallstone or biliary ileus is a late complication of gallstone disease. It accounts for 1%-4% of all bowel obstructions and is more common in elderly patients. The preferred treatment option is to mechanically remove the impacted stones. It is done surgically using open or laparoscopic approach and rarely, when stones are impacted in the colon, endoscopically. In this paper we present five consecutive cases of gallstone ileus and describe possible diagnostic and minimally invasive treatment options. Case presentation: During a five-month period a total of five patients were treated for gallstone ileus. All patients were female and from 48 to 87 years of age. Symptoms were not specific and common for all small bowel obstructions. Upon admission the patients also had unspecific laboratory findings-neutrophilic leukocytosis and various C-reactive protein concentrations, ranging from 8 to 347 mg/L. According to the hospital protocol, all patients initially underwent an abdominal ultrasound, which was inconclusive, and therefore every patient additionally had a CT scan with intravenous contrast. After these two diagnostic modalities one patient still did not have the definitive gallstone ileus diagnosis, as the ectopic stone was not visible. Four patients in our case series were treated using minimally invasive methods: in one case the stone was removed endoscopically, and laparoscopically in the other three. Treatment outcomes were good in four cases as the patients fully recovered, however one patient suffered a massive cerebral infarction after the operation and passed away. Conclusions: Gallstone ileus is a rare and difficult-to-diagnose condition. Management of these patients in every case should be individualized, as there are many options, each with their own advantages and disadvantages. We show that minimally invasive treatment such as colonoscopy or laparoscopy is possible in these cases.

Biliary ileus, first described byThomas Bartholin in the year1654, is a rare cause of mechanical ileus (small bowel obstruction) (1-3% in patients younger than 65 years), increasing significantly from that age (25%). The necessarycondition forthis pathologyis the presence ofa fistula between the gallbladderandthe gastrointestinaltract. Simple abdominal X-ray and ultrasonography are widely available and of relatively low cost, together presenting a sensitivity of 74% when they show the classic signs of Rigler’s triad (pneumobilia, ectopic gallstone and dilated loops of small intestine), but computed tomography of the abdomen is considered the gold standard, with a sensitivity and specificity higher than 90%. The aim of this article is to present a case of radiological diagnosis of biliary ileus in a patient with vesicular lithiasis + cholecystoduodenal fistula, associated with inguinal hernia on the left.


El íleo biliar, descrito por primera vez por Thomas Bartholin en el año 1654, constituye una causa poco frecuente de íleo mecánico (1-3% en menores de 65 años) aumentando significativamente a partir de esa edad (25%). La condición necesaria para esta patología es la presencia de una fístula entre la vesícula biliar y el tracto gastrointestinal. La radiografía simple de abdomen y la ecografía son de amplia disponibilidad y coste relativamente bajo, presentando en conjunto una sensibilidad del 74% cuando manifiestan los signos clásicos de la tríada de Rigler (neumobilia, lito biliar ectópico y dilatación de asas de intestino delgado), pero se considera que la tomografía computada de abdomen es el gold standard, con una sensibilidad y especificidad superiores al 90%. El objetivo de este artículo es presentar un caso de diagnóstico radiológico de íleo biliar en un paciente con litiasis vesicular + fístula colecistoduodenal asociadas a hernia inguinal izquierda.

O íleo biliar (IB) é complicação rara da colecistite decorrente de uma comunicação anormal entre a via biliar e o tubo digestivo, a qual permite a migração de um cálculo biliar volumoso para a luz entérica. É responsável por menos de 3% de todos os casos de obstrução intestinal mecânica e apresenta maior incidência em pacientes com mais de 65 anos de idade, representando até 25% dos casos de obstrução intestinal mecânica nestes. Frequentemente cursa com um quadro de obstrução intestinal que, se não tratada a tempo, pode apresentar elevados índices de morbidade e mortalidade. O conhecimento prévio dessa patologia é de grande importância principalmente em atendimentos de urgência e emergência, pois o tratamento é eminentemente cirúrgico e a incidência de diagnóstico pré-operatório é relativamente baixa. O artigo abaixo apresenta o caso de uma paciente de 74 anos, com íleo biliar, que foi cirurgicamente tratada por uma laparotomia exploradora, na qual foi realizada a retirada de cálculo em jejuno (enterotomia). Não foram realizados os procedimentos de colecistectomia e correção de fístula colecistoduodenal em um primeiro momento.


Biliary ileus (IB) is a rare complication of cholecystitis due to an abnormal communication between the biliary tract and the digestive tract which allows the migration of a large gallstone to the enteric light. It accounts for less than 3% of all cases of intestinal obstruction and it has a higher incidence in patients over 65 years of age, representing up to 25% of the cases in this age group. IB if not treated in time, can have high rates of morbidity and mortality. Prior knowledge of this pathology is of great importance, especially in urgent and emergency care because the treatment is mainly surgical and the incidence of preoperative diagnosis is relatively low. The following article presents the case of a patient with 74 years of age with biliary ileus, which was surgically treated by performing a laparotomy, in which removal of calculus was performed in the jejunum through an enterotomy. Nor laparoscopic procedures neither correction of the colecistoduodenal fistula were performed.

La litiasis vesicular es una entidad nosológica de frecuente ingreso y complicaciones, por lo cual el tratamiento médico-quirúrgico resulta frecuente en todos los servicios quirúrgicos. Una grave complicación de la litiasis vesicular la constituye el íleo biliar, entidad relativamente poco frecuente, caracterizada por un cuadro de obstrucción mecánica del intestino delgado o del colon, como consecuencia del paso de uno o más cálculos a través de una fístula biliodigestiva. Es una enfermedad caracteristica de la edad avanzada, con una edad promedio de 64 años y poco habitual antes de los 50 años. Se presenta el caso de una paciente del sexo femenino de 50 años de edad con un cuadro de obstrucción intestinal mecánica por cálculo biliar que ocluye la luz del íleon terminal, a nivel de los 40 cm proximales a la válvula de Bahuin, a quien no se le efectuó preoperatorio. Se realizó enterolitotomía y colecistectomía con fistulectomía y duodenorrafia. La paciente evolucionó favorablemente. Se concluye que es un caso característico de esta infrecuente afección y que el enfoque diagnóstico y terapéutico utilizado garantizó una evolución satisfactoria de la paciente


Vesicular lithiasis is a disease entity leads to frequent admissions and complications, thus, the medical-surgical treatment is usual in all surgical services. A severe complication of vesicular lithiasis is the infrequent biliary ileus characterized by a picture of mechanical obstruction of small bowel or the colon due to the passing of one or more calculi through a biliary-digestive fistula. It is disease characteristic of old age, with a mean age of 64 years and unusual before 50 years. This is the case of a female patient aged 50 with a picture of mechanical intestinal obstruction due to biliary calculus occluding the lumen of terminal ileum at the proximal 40 cm to Bahuin valve without preoperative diagnosis. An enterolithotomy and cholecystectomy were carried out with fistulectomy and duodenorrhaphy. Patient's course was favorable. We conclude that it a case characteristic of this infrequent affection and that the diagnostic and therapeutic approach used guaranteed a satisfactory course of the patient

Una grave complicación de la litiasis vesicular la constituye el íleo biliar, entidad relativamente poco frecuente, caracterizada por un cuadro de obstrucción mecánica del intestino delgado o del colon, como consecuencia del paso de uno o más cálculos a través de una fístula biliodigestiva. Es una enfermedad caracteristica de la edad avanzada, con una edad promedio de 64 años y poco habitual antes de los 50 años. Se presenta el caso de una paciente del sexo femenino de 50 años de edad con un cuadro de obstrucción intestinal mecánica por cálculo biliar que ocluye la luz del íleon terminal, a nivel de los 40 cm proximales a la válvula de Bahuin, a quien no se le efectuó preoperatorio. Se realizó enterolitotomía y colecistectomía con fistulectomía y duodenorrafia. La paciente evolucionó favorablemente. Se concluye que es un caso característico de esta infrecuente afección y que el enfoque diagnóstico y terapéutico utilizado garantizó una evolución satisfactoria de la paciente(AU)


Vesicular lithiasis is a disease entity leads to frequent admissions and complications, thus, the medical-surgical treatment is usual in all surgical services. A severe complication of vesicular lithiasis is the infrequent biliary ileus characterized by a picture of mechanical obstruction of small bowel or the colon due to the passing of one or more calculi through a biliary-digestive fistula. It is disease characteristic of old age, with a mean age of 64 years and unusual before 50 years. This is the case of a female patient aged 50 with a picture of mechanical intestinal obstruction due to biliary calculus occluding the lumen of terminal ileum at the proximal 40 cm to Bahuin valve without preoperative diagnosis. An enterolithotomy and cholecystectomy were carried out with fistulectomy and duodenorrhaphy. Patient's course was favorable. We conclude that it a case characteristic of this infrequent affection and that the diagnostic and therapeutic approach used guaranteed a satisfactory course of the patient(AU)
