RESUMO

This article examines the presence and influence of the work of Swiss psychiatrist Ludwig Binswanger and existential analysis (Daseinsanalyse) in Spanish psychiatry in the central decades of the 20th century. First, and drawing on various printed and archival sources, it reconstructs the important personal and professional ties that Binswanger maintained with numerous Spanish colleagues and describes the notable dissemination of his work in Spain through bibliographical reviews, scientific events, academic reports, university lectures and translations. Next, it reviews the incorporation of the postulates of existential analysis into the discourse of Spanish psychiatrists and assesses their most elaborate and original contributions to the foundations of 'anthropological-existential' psychiatry or the 'existential-analytical' interpretation of certain disorders or clinical conditions. And, finally, it tries to clarify the assessment according to which the (inevitable) instrumentalisation of existential analysis in the context of Franco's Spain first compromised the critical recognition of its true possibilities (and limits) and later contributed to the discrediting of psychopathological research among Spanish psychiatrists.

This article examines the milieu of Hermann Rorschach's Psychodiagnostics (1921/2021) under development between 1911 and his death in 1922 and explores new evidence about the direction Rorschach's test might have taken after publication of Psychodiagnostics. This includes direct and indirect influences from turn of the century continental philosophy and science and innovative colleagues in the Swiss psychiatric and psychoanalytic societies. The availability of newly translated scholarship, including the correspondence between Ludwig Binswanger and Hermann Rorschach following the 1921 publication of Psychodiagnostics, Binswanger's posthumous 1923 commentary in the International Journal of Psychoanalysis, and recent new translation of Psychodiagnostics, permits a fresh appraisal of the milieu and foundations of Rorschach's development. Understanding these sources and influences opens new vistas in reappraising the nature of Rorschach's "test theory" which Rorschach considered undeveloped at the time of his death. This paper presents new evidence that, under the influence of Rorschach's close colleague, Ludwig Binswanger, the Geisteswissenschaften and phenomenology might have figured prominently in future developments. The paper concludes that Rorschach, preoccupied with considerations of kinesthetic subjectivity in his innovative conceptualization of human movement responses, was a nascent phenomenologist whose untimely death cut short further developments in his theory of the test.

Background and Purpose: The clinical diagnosis of Binswanger's disease (BD), a chronic progressive form of subcortical vascular dementia, remains challenging. 3D pseudo-continuous arterial-spin-labeling (pcASL) and diffusion kurtosis imaging (DKI) can quantitatively reveal the microcirculation changes and heterogeneity of white matter (WM), respectively. We thus aimed to determine the diagnostic value of the combined 3D-pcASL and DKI in BD. Materials and Methods: A total of 35 patients with BD and 33 healthy controls underwent 3D-ASL and DKI experiments. The perfusion parameter of cerebral blood flow (CBF), diffusion parameters of fractional anisotropy (FA), mean/axial/radial diffusivity (MD/Da/Dr), and kurtosis parameters of anisotropy fraction of kurtosis (FAk) and mean/axial/radial kurtosis MK/Ka/Kr were obtained to quantitatively measure the parametric distributions of functional brain subregions. One-way analysis of variance and post hoc t-test were applied to explore the different distributions of DKI/ASL-derived parameters among brain subregions of BD. In addition, all region-specific DKI/ASL parameters were separately analyzed in Pearson correlation analysis to investigate the relationship with Mini-Mental State Examination (MMSE), a typical clinical scale for cognitive function assessment in patients with BD. Results: FA/FAk/MK/Ka/Kr was significantly declined in all WM hyperintensities (WMHs) of BD compared with healthy controls, while the corresponding MD/Da/Dr was significantly increased (all p < 0.005). In addition, significant changes, similar to the WMHs of patients with BD, were also observed in almost all DKI parameters in WM normal areas and genu/splenium of the corpus callosum (GCC/SCC) in BD (p < 0.005). Finally, CBF was significantly reduced in all of the above regions we measured in patients with BD (p < 0.005). For patients with BD, MMSE showed a negative correlation with MD/Da in thalamus (r = -0.42/-0.58; p < 0.05), and a positive correlation with CBF in PWM/TWM (r = 0.49/0.39; p < 0.05). Using receiver operating characteristic (ROC) analysis, FA/FAk/Kr in GCC, CBF/FA/Dr/FAk in SCC, MD/Da/Ka in thalamus, and the combined FA/MD/Dr/CBF in TWM showed high accuracy [area under the curves (AUCs) 0.957/0.946/0.942/0.986] in distinguishing patients with BD from healthy controls. Conclusion: We found that combined DKI and 3D-ASL are helpful in diagnosing patients with BD, especially with FA, MD, Dr, and CBF in the temporal WM region. Additionally, the kurtosis parameters of DKI can sensitively monitor the potentially damaged WM areas in patients with BD patients, adding complementary clinical value.

This article presents an introduction to Ludwig Binswanger's Comments on Hermann Rorschach's Psychodiagnostik, published in the International Journal of Psychoanalysis in 1923, after Rorschach's death in 1922. Binswanger, one of the most distinguished psychiatrists of the twentieth century and a close professional colleague and compatriot in the Swiss Psychiatric and Psychoanalytic Societies, was blazing new trails by incorporating turn-of-the-century phenomenology and experimental psychology into Swiss psychiatry. His comments, which have been noted for over 100 years but never before translated, are a critical review of Rorschach's monograph, highlighting the undeveloped status of the test theory and philosophical foundations. Binswanger's comments illuminate philosophical, conceptual and scientific pathways not taken in the development of the test following Rorschach's untimely demise.

A clínica fenomenológica de Ludwig Binswanger propõe uma nova compreensão sobre a psicopatologia ao realçar a importância na história de vida do paciente. Encontra-se dividida em três fases mostrando a fertilidade de seus estudos psicopatológicos após a leitura de Ser e tempo e através da influência husserliana, com o intuito de esclarecer a constituição da experiência delirante. Este artigo tem como objetivo apresentar dois casos clínicos, Lola Voss e Suzanne Urban, para ilustrar a contribuição oferecida por Ludwig Binswanger à psicopatologia fenomenológica em sua segunda e última fase.


Ludwig Binswanger's phenomenological clinic provides a new understanding of psychopathologies by highlighting the importance of patients' life stories. It is divided into three phenomenological stages and shows how fertile his psychopathological studies became after he had read Being and Time and due to Husserl's influence, which he undertook to investigate the formation of delusional experience. This article presents two clinical cases, Lola Voss and Suzanne Urban, to illustrate Ludwig Binswanger's contribution to phenomenological psychopathology in its second and final stage.


La clinique phénoménologique de Ludwig Binswanger propose une nouvelle compréhension de la psychopathologie soulignant l'importance de l'histoire de vie du patient. Les trois phases de sa pensée phénoménologique montrent la fécondité de ses études psychopathologiques, influencées par la lecture de l'Être et Temps et par Husserl, qu'il menait pour élucider la formation de l'expérience délirante. Cet article présente deux cas cliniques, Lola Voss et Suzanne Urban, qui illustrent la contribution apportée par Ludwig Binswanger à la psychopathologie phénoménologique dans sa deuxième et dernière phase.

O presente artigo tem como objetivo refletir, por meio de um estudo teórico, acerca do existencial cuidado (Sorge) no pensamento de Martin Heidegger e amor (Liebe) em Ludwig Binswanger. O texto retoma os principais aspectos da forma como ambos compreendem a noção de cuidado e amor, desdobrando criticamente o impasse gerado a partir daí. Demonstra-se que o cerne da querela se dá no modo como Binswanger compreende e interpreta o ser-no-mundo (In-der-Welt-sein) heideggeriano, ligando-o à noção de ser-para-além-do-mundo (Über-die-Welt-hinaus-sein). O psiquiatra suíço procede desse modo na medida em que julga ter identificado um limite e uma carência do elemento eternidade no modo como Heidegger desenvolve sua ontologia fundamental. Com tal apontamento, Binswanger elege a tonalidade do amor como fundamento de toda relação, que faz dessa experiência de infinito sua forma de ser. No bojo de tal apontamento se encontra o desenvolvimento da Daseinsanálise como o desdobramento ­ a princípio psiquiátrico ­ de uma relação clínica pautada pelo amor como meio do acesso fundamental ao outro e como elemento que promove a cura. O artigo conclui que tal indicação binswangeriana recai sutilmente em elementos instrumentais e produtivistas, uma vez que Binswanger reproduz elementos da técnica justamente lá onde tais elementos deveriam permanecer inoperantes. This article is a theoretical study to reflect on the existential care (Sorge) in Martin Heidegger and the love (Liebe) in Ludwig Binswanger. The text retakes the main aspects the way both understand care and love, unfolding in a critical way the impasse caused from there. It is demonstrated that the center of the discussion is the way Binswanger understands and interprets the Heideggerian being-in-the-world (In-der-Welt-sein), linking it to the notion of being beyond the world (Über-die-Welt-hinaus-sein). Binswanger identifies a limit and a flaw in the element of eternity in the way Heidegger develops his fundamental ontology. By this he points out to the experience of love as the basis of each relationship, making this experience of the infinite as the way of being of love and the basis of all relationships. In the center of such consideration is the development of Daseinsanalysis as the unfolding - at first psychiatric - of a clinical relationship guided by love as a means of fundamental access to the other and as an element that promotes healing. It is concluded that the Binswangerian indication subtly falls on the productivism and instrumental elements, since Binswanger reproduces elements of technology exactly where those elements must remain inoperative. El objetivo de este artículo es reflejar teóricamente sobre el existencial cuidado (Sorge) en los estudios de Martin Heidegger y amor (Liebe) en Ludwig Binswanger. El texto retoma los principales aspectos de cómo ambos entienden esas nociones, para desplegar críticamente el impasse generado a partir de ahí. El núcleo de la disputa tiene lugar en la forma en que Binswanger entiende e interpreta el ser-en-el mundo (In-der-Welt-sein) heideggeriano, vinculándolo a la noción de estar más-allá-del-mundo (Über-die-Welt-hinaus-sein). El psiquiatra lo hace cuando considera identificar un límite y una falta de eternidad en la forma en que Heidegger desarrolla su pensamiento. Con tal nota, Binswanger elige el tono del amor como la base de cada relación, que hace de esta experiencia del infinito su forma de ser. En el abultamiento de esta nota está el desarrollo de la Daseinsanalysis como un desdoblarse del inicio psiquiátrico ­ de una relación clínica guiada por el amor como el acceso fundamental del otro y aún como aquello que conduce a la cura. El artículo concluye que aquello lo que indica Binswanger se queda sutilmente en elementos instrumentales y productivistas, pues Binswanger reproduce elementos de la técnica donde tales elementos deberían mantenerse inoperantes. Binswanger disease (BD) involves injuries to the brain small vessels, resulting to gradually progressive subcortical ischemia. This disorder manifests with dementia, gait abnormalities, upper motor signs and parkinsonism, and presents as extensive, confluent, bilateral cerebral white matter hyperintensities in the MRI. Cases of BD typically manifests with vascular risk factors, such as hypertension and multiple strokes. We report a unique case of a Filipino patient whom we have diagnosed with BD presenting with no cardinal signs of parkinsonism, but with generalized choreiform movement disorder and without a history of hypertension and symptomatic strokes. To our knowledge, this is the first report presenting an adult patient with subcortical leukoaraiosis of Binswanger type associated with a hyperkinetic movement disorder.

Objective: This research is devoted to intracerebral transcatheter laser photobiomodulation therapy (PBMT) in the treatment of ischemic and neurodegenerative lesions of cerebral white matter in patients with Binswanger's disease (BD) and vascular parkinsonism (VP) in comparison with conservative treatment methods. Background: Recent studies have shown PBMT high potential in the treatment of various cerebral lesions. Materials and methods: Twenty-seven patients with BD, 58-81 years of age (mean age 78), 17 (62.96%) men, and 10 (27.04%) women. Of these, test group 1-14 (51.85%) patients-underwent intracerebral transcatheter laser PBMT, and control group 1-13 (48.15%) patients-had conservative treatment. Besides, 62 patients with VP, 52-80 years of age (mean age 77), 48 (77.42%) men, and 14 (22.58%) women. Of these, test group 2-37 (59.68%) patients-underwent intracerebral transcatheter laser PBMT, and control group 2-25 (40.32%) patients-had conservative treatment. Results: Good and satisfactory clinical results were obtained in Test group 1 and Test group 2 patients in 49 (92.45%) cases, with a persistent decrease of dementia and motor impairment, and recovery of cognitive functions and daily life activity. Control group 1 and Control group 2 patients showed a satisfactory clinical result in 6 (15.79%) cases. Persistent positive dynamics was not observed. Conclusions: Intracerebral transcatheter laser PBMT is a pathogenetically justified, effective treatment for BD and VP; it restores cerebral collateral and capillary blood supply, improves microcirculation, restores cellular and tissue metabolism, stimulates neurogenesis, and causes regenerative processes in the brain.

Binswanger's disease is a form of subcortical ischemic vascular disease (SIVD-BD) with extensive white matter changes. To test the hypothesis that biomarkers could improve classification of SIVD-BD, we recruited 62 vascular cognitive impairment and dementia (VCID) patients. Multimodal biomarkers were collected at entry into the study based on clinical and neuropsychological testing, multimodal magnetic resonance imaging (MRI), and cerebrospinal fluid (CSF) analysis. The patients' diagnoses were confirmed by long-term follow-up, and they formed a "training set" to test classification methods, including (1) subcortical ischemic vascular disease score (SIVDS), (2) exploratory factor analysis (EFA), (3) logistic regression (LR), and (4) random forest (RF). A subsequently recruited cohort of 43 VCID patients with provisional diagnoses were used as a "test" set to calculate the probability of SIVD-BD based on biomarkers obtained at entry. We found that N-acetylaspartate (NAA) on proton magnetic resonance spectroscopy (1H-MRS) was the best variable for classification, followed by matrix metalloproteinase-2 in CSF and blood-brain barrier permeability on MRI. Both LR and RF performed better in diagnosing SIVD-BD than either EFA or SIVDS. Two-year follow-up of provisional diagnosis patients confirmed the accuracy of statistically derived classifications. We propose that biomarker-based classification methods could diagnose SIVD-BD patients earlier, facilitating clinical trials.

Time-consciousness has long been a focus of research in phenomenology and phenomenological psychology. We advance and extend this tradition of research by focusing on the character of temporal experience under conditions of mania. Symptom scales and diagnostic criteria for mania are peppered with temporally inflected language: increased rate of speech, racing thoughts, flight-of-ideas, hyperactivity. But what is the underlying structure of temporal experience in manic episodes? We tackle this question using a strategically hybrid approach. We recover and reconstruct three hypotheses regarding manic temporality that were advanced and modeled by two pioneers of clinical phenomenology: Eugène Minkowski (1885-1972) and Ludwig Binswanger (1881-1966). We then test, critique, and refine these hypotheses using heterophenomenological methods in an interview-based study of persons with a history of bipolar and a current diagnosis of acute mania. Our conclusions support a central hypothesis due to Minkowski and Binswanger, namely, that disturbance in the formal structure of temporal experience is a core feature of mania. We argue that a suitably refined variant of Binswanger's model of disturbance in manic protention helps to explain a striking pattern of impaired insight and impaired reasoning in manic episodes.

White matter lesions (WMLs) are a common manifestation of small vessel disease (SVD) in the elderly population. They are associated with an enhanced risk of developing gait abnormalities, poor executive function, dementia, and stroke with high mortality. Hypoperfusion and the resulting endothelial damage are thought to contribute to the development of WMLs. The focus of the present study was the analysis of the microvascular bed in SVD patients with deep WMLs (DWMLs) by using double- and triple-label immunohistochemistry and immunofluorescence. Simultaneous visualization of collagen IV (COLL4)-positive membranes and the endothelial glycocalyx in thick sections allowed us to identify endothelial recession in different types of string vessels, and two new forms of small vessel/capillary pathology, which we called vascular bagging and ghost string vessels. Vascular bags were pouches and tubes that were attached to vessel walls and were formed by multiple layers of COLL4-positive membranes. Vascular bagging was most severe in the DWMLs of cases with pure SVD (no additional vascular brain injury, VBI). Quantification of vascular bagging, string vessels, and the density/size of CD68-positive cells further showed widespread pathological changes in the frontoparietal and/or temporal white matter in SVD, including pure SVD and SVD with VBI, as well as a significant effect of the covariate age. Plasma protein leakage into vascular bags and the white matter parenchyma pointed to endothelial damage and basement membrane permeability. Hypertrophic IBA1-positive microglial cells and CD68-positive macrophages were found in white matter areas covered with networks of ghost vessels in SVD, suggesting phagocytosis of remnants of string vessels. However, the overall vessel density was not altered in our SVD cohort, which might result from continuous replacement of vessels. Our findings support the view that SVD is a progressive and generalized disease process, in which endothelial damage and vascular bagging drive remodeling of the microvasculature.

RESUMO

We report a case of a 55-year-old known hypertensive female who presented with features suggestive of dementia, which, on further workup unveiled background dyslipidemia and type 2 diabetes mellitus. An magnetic resonance imaging of the brain revealed findings suggestive of Binswanger's disease. This is a discussion of this unusual disease and its presentation and the differentials of this presentation which may be encountered in general clinical practice.

Os nomes de Foucault e Binswanger figuram lado a lado com tão pouca frequência que, à primeira vista, a aproximação entre os autores pode apresentar-se como uma questão despropositada. No presente trabalho, com base na análise de algumas de suas obras e nas contribuições de seus comentadores, procuramos apresentar algumas indicações para uma breve incursão em certos aspectos da história desse acercamento, cujos desdobramentos não se reduzem a uma casual afinidade intelectual entre o filósofo francês e o psiquiatria suíço. Defendemos a existência de uma convergência entre ambos, centrada em aspectos como a recusa de modelos explicativos organicistas e psicodinâmicos; e a utilização metodológica da noção de a priori. Concluímos pela existência de uma importante aproximação entre as obras dos dois autores.


The names Foucault and Binswanger are so seldom seen together that the question of any convergence between the authors may at first sound preposterous. In the present study, based on the analysis of some of their works and on contributions from their commentators, we sought to present some indications for a brief incursion into certain aspects of the story behind this connection, whose implications cannot be dismissed as a mere casual intellectual affinity between the French philosopher and Swiss psychiatrist. We argue that there exists a convergence between the two, centered on aspects such as the rejection of organicist and psychodynamic explanatory models; and the methodological use of the notion of a priori. We conclude that there is an important convergence between the works of the two authors.


Los nombres de Foucault y Binswanger aparecen uno junto al otro tan infrecuentemente que, inicialmente, la aproximación entre los autores puede presentarse como una cuestión sin sentido. En este trabajo, basado en el análisis de algunas de sus obras y en las contribuciones de sus comentadores, hemos buscado presentar algunas indicaciones para una breve incursión en determinados aspectos de la historia de esta aproximación, cuyos desenvolvimientos no se reducen a una casual afinidad intelectual entre el filósofo francés y el psiquiatra suizo. This expert working group report proposes an updated approach to subtype definition of vascular parkinsonism (VaP) based on a review of the existing literature. The persistent lack of consensus on clear terminology and inconsistent conceptual definition of VaP formed the impetus for the current expert recommendation report. The updated diagnostic approach intends to provide a comprehensive tool for clinical practice. The preamble for this initiative is that VaP can be diagnosed in individual patients with possible prognostic and therapeutic consequences and therefore should be recognized as a clinical entity. The diagnosis of VaP is based on the presence of clinical parkinsonism, with variable motor and non-motor signs that are corroborated by clinical, anatomic or imaging findings of cerebrovascular disease. Three VaP subtypes are presented: (1) The acute or subacute post-stroke VaP subtype presents with acute or subacute onset of parkinsonism, which is typically asymmetric and responds to dopaminergic drugs; (2) The more frequent insidious onset VaP subtype presents with progressive parkinsonism with prominent postural instability, gait impairment, corticospinal, cerebellar, pseudobulbar, cognitive and urinary symptoms and poor responsiveness to dopaminergic drugs. A higher-level gait disorder occurs frequently as a dominant manifestation in the clinical spectrum of insidious onset VaP, and (3) With the emergence of molecular imaging biomarkers in clinical practice, our diagnostic approach also allows for the recognition of mixed or overlapping syndromes of VaP with Parkinson's disease or other neurodegenerative parkinsonisms. Directions for future research are also discussed.

Vascular cognitive impairment and dementia (VCID) is a major public health concern because of the increased incidence of vascular disease in the aging population and the impact of vascular disease on Alzheimer's disease. VCID is a heterogeneous group of diseases for which there are no proven treatments. Biomarkers can be used to select more homogeneous populations. Small vessel disease is the most prevalent form of VCID and is the optimal form for treatment trials because there is a progressive course with characteristic pathological changes. Subcortical ischemic vascular disease of the Binswanger type (SIVD-BD) has a characteristic set of features that can be used both to identify patients and to follow treatment. SIVD-BD patients have clinical, neuropsychological, cerebrospinal fluid (CSF) and imaging features that can be used as biomarkers. No one feature is diagnostic, but a multimodal approach defines the SIVD-BD spectrum disorder. The most important features are large white matter lesions with axonal damage, blood-brain barrier disruption as shown by magnetic resonance imaging and CSF, and neuropsychological evidence of executive dysfunction. We have used these features to create a Binswanger Disease Scale and a probability of SIVD-BD, using a machine-learning algorithm. The patients discussed in this review are derived from published studies. Biomarkers not only aid in early diagnosis before the disease process has progressed too far for treatment, but also can indicate response to treatment. Refining the use of biomarkers will allow dementia treatment to enter the era of precision medicine. This article is part of the Special Issue "Vascular Dementia".

Vascular cognitive impairment and dementia (VCID) include a wide spectrum of chronic manifestations of vascular disease related to large vessel strokes and small vessel disease (SVD). Lacunar strokes and white matter (WM) injury are consequences of SVD. The main vascular risk factor for SVD is brain hypoperfusion from cerebral blood vessel narrowing due to chronic hypertension. The hypoperfusion leads to activation and degeneration of astrocytes with the resulting fibrosis of the extracellular matrix (ECM). Elasticity is lost in fibrotic cerebral vessels, reducing the response of stiffened blood vessels in times of increased metabolic need. Intermittent hypoxia/ischaemia activates a molecular injury cascade, producing an incomplete infarction that is most damaging to the deep WM, which is a watershed region for cerebral blood flow. Neuroinflammation caused by hypoxia activates microglia/macrophages to release proteases and free radicals that perpetuate the damage over time to molecules in the ECM and the neurovascular unit (NVU). Matrix metalloproteinases (MMPs) secreted in an attempt to remodel the blood vessel wall have the undesired consequences of opening the blood-brain barrier (BBB) and attacking myelinated fibres. This dual effect of the MMPs causes vasogenic oedema in WM and vascular demyelination, which are the hallmarks of the subcortical ischaemic vascular disease (SIVD), which is the SVD form of VCID also called Binswanger's disease (BD). Unravelling the complex pathophysiology of the WM injury-related inflammation in the small vessel form of VCID could lead to novel therapeutic strategies to reduce damage to the ECM, preventing the progressive damage to the WM.

Objective To investigate the influence of the different degree of internal carotid stenosis on the white matter lesion and cognition of Binswanger disease.Methods A total of 108 elderly patients with Binswanger disease from Department of Geriatric Neurology of Qilu Hospital were recruited during December 2013 and June 2014.At the end of follow-up,6 cases showed acute eerebrovaseular disease,39 (< 10 %) had no internal carotid stenosis,31 (10 %-49 %) had mild internal carotid stenosis,32 (50%-70%)had moderate internal carotid stenosis through B ultrasound examination and MRI and MRA examination on internal carotid artery and brain.The B ultrasound examination of internal carotid artery included intima-media thickness (IMT),plaque index and the peak systolic velocity (PSV).Cognitive function of Binswagner disease was assessed by Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MOCA).The white matter lesion was assessed by reformed visual Scheltens scale.The relationship among IMT,plaque index,PSV,white matter lesion,and cognitive function was investigated.The variation of cognition was observed after 1 year.Results There were statistically significant differences in IMT,PSV,plaque index,reformed Scheltens scale scores between groups of non,mild,moderate internal carotid stenosis (all P <0.05).The IMT was thicker in moderate internal carotid stenosis group than in mild internal carotid stenosis group (P<0.05).The differences in PSV,plaque index,and reformed Scheltens scale scores between mild and moderate internal carotid stenosis groups were not significant (P>0.05).There were positive correlation between PSV and reformed Scheltens scale scores (r=0.630,P =0.020).There were negative correlation between PSV and MMSE scores (r=-0.970,P=0.040).The scores of MMSE and MOCA both were declined after 1 year in three groups (0.61 ± 0.60,0.68 ± 0.81),(0.70±0.60,0.93±0.69),(1.06±0.68,1.13±0.76).The declination of MMSE and MOCA of BD patients was higher in moderate internal carotid stenosis group than in non internal carotid stenosis group (P<0.05).The differences in the declination of MMSE and MOCA between moderate and mild internal carotid stenosis groups were not significant (P> 0.05).Conclusions Internal carotid stenosis is one of risk factors for the cognitive impairment of BD,the abnormal IMT and PSV are both correlated with white matter lesion and cognitive impairment in BD.Early standardized therapy can postpone the rate of cognitive impairment in BD.

August Strindberg (1849-1912), famoso escritor sueco, deixou como legado dezenas de obras que, além da qualidade literária, forneceram um retrato fiel de sua doença mental. Tais descrições chamaram atenção de diversos psiquiatras, principalmente no que tange à relação entre a psicose e a criação estética. Neste artigo, revisitamos a patografia de Strindberg, apresentada por Jaspers, e a análise fenomenológica, realizada por Binswanger, apresentando suas principais características e como esse caso ilustra as diferentes concepções desses autores. Jaspers se interessava pelos escritos autobiográficos de Strindberg por se tratar de um exemplo daquilo que apresenta acerca da inacessibilidade a uma compreensão psicológica da psicose e da compreensão processual do adoecimento. Binswanger, retornando à fenomenológica genética, realiza a análise da experiência delirante de Strindberg a partir de questões egológicas e corporais. Para Jaspers, o mundo pré-mórbido de Strindberg diferenciava-se de seu mundo delirante posterior. Já Binswanger identifica, primariamente, o anúncio do que viria a ser o delírio de Strinberg. São percursos distintos que, para além da distinção, refletem marcas da utilização inicial de uma lente fenomenológica por Jaspers que se efetivam posteriormente através de autores da psicopatologia fenomenológica, como o próprio Binswanger.


August Strindberg (1849-1912), famous Swedish writer, left a legacy of dozens of works, which, besides literary quality, produced a faithful portrait of his mental disease. Such descriptions have drawn the attention of diverse psychiatrists, especially when it comes to the relation between psychosis and esthetic creation. In this article, we have revisited Strindberg’s pathology, presented by Jaspers, and phenomenological analysis, realized by Binswanger, presenting its main characteristics and how this case illustrates the different conceptions by these authors. Jaspers was interested in the autobiographical writings of Strindberg because it consisted of an example of what is presented concerning the inaccessibility to a psychological understanding of psychosis and of the process involved in falling sick. Binswanger, returning to genetic phenomenology, realizes Strindberg’s delirious experience from egological and bodily issues. To Jaspers, Strindberg’s pre-morbid world was different from his posterior delirious world. Binswanger, on the other hand, primarily identifies the announcement of what came to be Strindberg’s delirium. These are distinct trajectories that, beyond distinction, reflect marks of the initial employment of a phenomenological lens by Jaspers which become effective later on by means of Phenomenological Psychopathology authors, such as Binswanger.

Jaspers se interesaba por los escritos autobiográficos de Strindberg por ser un ejemplo de lo que se presenta sobre la inaccesibilidad a una comprensión psicológica de la psicosis y de la comprensión del proceso de la enfermedad. Binswanger, de volta a la fenomenología genética, realiza el análisis de la experiencia de delirio de Strindberg empezadas en cuestiones egologicas y corporales. Para Jaspers, el mundo pre-mórbido de Strindberg se diferenciaba de su mundo delirante posterior. Ya Binswanger identificaba primariamente el prenuncio de lo que iba a ser el delirio de Strinberg. Son caminos distintos que, además de la distinción, reflejen marcas de la utilización inicial de una lente fenomenológica por Jaspers que se efectiva posteriormente por medio de la Psicopatología Fenomenológica, como el propio Binswanger.

This paper aims to present a different way of understanding psychic suffering, based on Heidegger’s considerations about Dasein and Sorge in Being and Time and on his thoughts on psychopathology in the Zollikon Seminars. From this point of view, the psychopathological phenomena are understood as a restriction of meaning and a feeling of strangeness, caused by the loss of familiarity with the world.(AU)


L’objectif de cet article est de présenter une autre façon de comprendre les souffrances psychiques au moyen des considérations de Heidegger sur les concepts Dasein et Sorge que l’on retrouve dans son œuvre Être et Temps, ainsi qu’au moyen de ses réflexions sur la psychopathologie recueillies dans les Séminaires Zollikon. Les phénomènes psychopathologiques sont alors compris comme une restriction de sens et une étrangeté, résultats de la perte de familiarité avec le monde.(AU)
