Introducción: Los síntomas de depresión y ansiedad en pacientes que reciben Asesoramiento Genético en Oncología (AGO), se encuentran dentro de las afectaciones psicológicas más prevalentes, por lo que su identificación de forma oportuna, valida y confiable es prioritaria. Objetivo: Determinar las propiedades psicométricas del Cuestionario sobre la Salud del Paciente, PHQ-9, y de la Escala de Ansiedad Generalizada, GAD-7, en pacientes con cáncer portadores de variantes patogénicas germinales en genes de alta susceptibilidad. Método: Se empleó un diseño instrumental, transversal con un muestreo no probabilístico. Se incluyeron 163 participantes con variantes patogénicas en BRCA1/BRCA2, asociadas a cáncer hereditario, entre 19 y 79 años, (48,2 ± 11,2 años). Resultados: La validez de constructo de cada instrumento se probó a través de un análisis factorial exploratorio y confirmatorio. La GAD-7 obtuvo un α = 0,899 y 62,3 % de la varianza explicada, por otro lado, el PHQ- 9 obtuvo un α = 0,896 y 54,8 % de la varianza explicada. El análisis factorial confirmatorio sugiere que los modelos teóricos de ambos instrumentos se ajustan a un solo factor, con una consistencia e índices de validez adecuados. Discusión y conclusión: El PHQ-9 y la GAD-7 son instrumentos parsimoniosos, breves, válidos y confiables para la detección de síntomas de depresión y ansiedad en pacientes que reciben AGO, en población mexicana. Se recomienda su uso en la atención clínica (al inicio, y durante el seguimiento), así como en investigaciones futuras (AU)


Introduction: The symptoms of depression and anxiety in patients receiving Genetic Counseling in Oncology (AGO) are among the most prevalent psychological affectations, so their timely, valid, and reliable identification is a priority. Objective: To determine the psychometric properties of the Patient Health Questionnaire, PHQ-9, and the Generalized Anxiety Scale, GAD-7, in cancer patients carrying germinal pathogenic variants in high susceptibility genes. Method: An instrumental, cross-sectional design was used with a non-probabilistic sampling. 163 participants with pathogenic variants in BRCA1/BRCA2, associated with hereditary cancer, between 19 and 79 years (48.2 ± 11.2 years) were included. Statistical analysis: The construct validity of each instrument was tested through an exploratory and confirmatory factor analysis. Results: The GAD-7 obtained α = 0.899 and 62.3% of the explained variance, on the other hand, the PHQ-9 obtained α = 0.896 and 54.8% of the explained variance. Confirmatory factor analysis suggests that the theoretical models of both instruments fit a single factor, with adequate consistency and validity indices. Discussion and conclusion: The PHQ-9 and the GAD-7 are parsimonious, brief, valid and reliable instruments for the detection of symptoms of depression and anxiety in patients receiving AGO, in the Mexican population. Its use is recommended in clinical care (at baseline, and during follow-up), as well as in future research (AU)

Objetivos: valorar el impacto emocional de recibir los resultados del estudio genético (EG) en pacientes con antecedente oncológico personal y sospecha de síndrome hereditario, aplicando el Cuestionario Multidimensional del Impacto de la evaluación de riesgo de cáncer (MICRA). Método: 219 pacientes con diagnóstico oncológico que concurrieron a la consulta de Asesoramiento Genético Oncológico en el Instituto Alexander Fleming entre 2014 y 2019, fueron evaluados aplicando el MICRA. Resultados: Edad promedio 49,84 (42,21; 62,02), 82,2% con diagnóstico de cáncer de mama. En un 16% se halló una variante patogénica (VP). La media de los puntajes obtenidos por cada subescala fue: 5,26 (DS=4,48, rango 0-22) para Malestar Emocional (ME); 12,31 (DS 7,42 rango 0-37) para Incertidumbre (I); 16,36 (DS 4.30 rango 2-20) para Experiencias Positivas (EP) y de 34,37 (DS 10,24 rango 8-62) para la puntuación global, lo que muestra un bajo nivel de ME e I y la presencia de EP entre los pacientes. Se hallaron diferencias significativas según tipo de resultado: los portadores de VP, mostraron una modesta elevación del nivel de ME y menor puntuación en EP, respecto de aquellos que tuvieron resultados no informativos o inciertos. Sin diferencias significativas según edad, pacientes con o sin hijos, o tiempo entre la realización del estudio y la aplicación del cuestionario. Conclusiones: Recibir resultados de estudio genético no produciría un impacto psicológico adverso. Las puntuaciones altas de esta escala podrían ser usadas para identificar a pacientes con malestar emocional y ofrecerles un seguimiento psicooncológico específico (AU)


Objective: The aim of our study is to assess the emotional impact of genetic test results disclosure to patients with a personal cancer history and suspected hereditary syndrome, applying the Multidimensional Impact of Cancer Risk Assessment (MICRA) Questionnaire. Methods: two hundred nineteen patients affected with cancer, referred to the Cancer Genetic Counseling department at the Alexander Fleming Institute, between 2014 and 2019, were evaluated using the MICRA questionnaire. Results: Average age 49.84 (42,21; 62,02), 82.2% presented breast cancer. In 16% a pathogenic variant (PV) was found. The mean of the scores obtained for each subscale in the questionnaire was 5.26 (0-22, SD 4.48) for Distress (D); 12.31 (0-37, DS 7.42) for Uncertainty (U); 16.36 (2-20 SD 4.30) for Positive Experiences (PE) and 34.37 (8-62 SD 10.24) for the global score, which shows a low level of D and U and the presence of PE among the patients. Significant differences were found according to the type of result: We found modestly increased distress in PV carriers compared to patients who received uninformative or negative test results. No significant differences according to age, patients with or without children, or time between the completion of the genetic test and the application of the questionnaire. Conclusion: Genetic test disclosure does not seem to produce an adverse psychological impact. High scores on this scale could be used to identify patients with emotional distress and offer them specific psycho-oncological follow-up (AU)

Abstract Background: Patients with familial erythrocytosis type 2 have no increased risk of von Hippel-Lindau-associated tumors, although mutations in the VHL gene cause both pathologies. Case report: We present a case of a compound heterozygote patient with von Hippel-Lindau disease and familial erythrocytosis type 2. One of the mutations found in our patient, c.416C>G (p.Ser139Cys) of the VHL gene, has not been previously reported. This case is the second one reported where von Hippel-Lindau disease and familial erythrocytosis type 2 coexist in the same individual. Conclusions: Despite the low frequency of familial erythrocytosis type 2 in patients with von Hippel-Lindau disease, the possibility of this diagnosis should be considered to avoid unnecessary invasive studies to explain the polyglobulia in these patients and guarantee an adequate follow-up and vigilance of both diseases.


Resumen Introducción: Los pacientes con eritrocitosis familiar tipo 2 no muestran un riesgo incrementado de desarrollar tumores asociados con la enfermedad de von Hippel-Lindau, a pesar de que ambas afecciones están causadas por variantes patogénicas en el gen VHL. Caso clínico: Se presenta el caso de un paciente heterocigoto compuesto con enfermedad de von Hippel-Lindau y eritrocitosis familiar tipo 2. Una de las variantes patogénicas en el paciente, VHL c.416C>G (p.Ser139Cys), no ha sido previamente reportada. Este es el segundo reporte de caso en que la enfermedad de von Hippel-Lindau y la eritrocitosis familiar tipo 2 coexisten en el mismo individuo. Conclusiones: A pesar de la baja frecuencia de la eritrocitosis familiar tipo 2 en pacientes con enfermedad de von Hippel-Lindau, la posibilidad del diagnóstico debe considerarse con el fin de evitar estudios invasivos innecesarios para explicar la presencia de poliglobulia en estos pacientes y para garantizar un adecuado seguimiento y una correcta vigilancia de ambas enfermedades.

INTRODUCTION AND OBJECTIVES: Peutz-Jeghers syndrome is a rare autosomal dominant inherited disease caused by a germline mutation of the STK11/LKB1 gene, located on chromosome 19p13.3. It is characterized by mucocutaneous hyperpigmentation, hamartomatous polyposis, and predisposition to cancer. The aim of the present study was to identify and register patients with Peutz-Jeghers syndrome, describe the disease, and estimate its prevalence in Valencia (Spain). MATERIALS AND METHODS: A print-out of the clinical histories from 10 hospitals was obtained utilizing the ICD-9 code 759.6 from the Minimum Basic Data Set of Hospital Admissions of the Spanish Ministry of Health and Consumer Affairs. RESULTS: From a total of 405 clinical histories found, 15 (9 males and 6 females) fit the diagnostic criteria of Peutz-Jeghers syndrome. Mean age at diagnosis was 13.8 years and mean age at death was 54.2 years. Four males died, all from cancer. The estimated disease prevalence was 0.4/100,000 inhabitants. All the patients presented with anemia and polyps in the small bowel (80% in the duodenum, 66.7% in the ileum, and 40% in the jejunum), 93.3% underwent urgent surgical intervention and presented with intestinal invagination, and 40% of the patients developed cancer at a mean age of 48.5 years. CONCLUSION: The present study is the first register of patients with Peutz-Jeghers syndrome in Valencia, Spain. The ICD-9 code is nonspecific for rare diseases. The duodenum was the most frequent location for polyps and the majority of cases presented with intestinal invagination, bowel obstruction, and urgent surgical intervention. A large percentage of patients presented with cancer. It would be of interest to review and evaluate the existing surveillance protocols in the Valencian Community.

Li-Fraumeni Syndrome (LFS) is a hereditary disorder that confers an approximately 90% lifetime risk of cancer and requires comprehensive lifetime cancer screening. We explored healthcare roles for managing LFS-related cancer risks and treatments that were assumed by parents, adolescents, and adult children. Semi-structured interviews were conducted with 23 families. Family groupings were comprised of 2-5 members, with the younger generation in each family ranging in age from 7 to 40 years. Using grounded theory methods, we conducted open and focused coding of interview transcript content. Family members described how the role of health leader was implemented in their family, as well as factors such as maturation of a child or death of a member that determined who assumed particular roles and how these roles shifted over time. They often expressed collective responsibility for helping relatives understand LFS and implement appropriate cancer risk management. Members demonstrated their health role by attending others' medical appointments for support or information gathering. The health leader role was intergenerational and provided the family necessary support in navigating complicated healthcare decisions. Our findings provide insight into healthcare providers regarding how LFS patients and their relatives develop unique medical decision-making and caring roles influenced by the hereditary nature of LFS, and how these roles change over time. Providers who are attuned to family role dynamics may be better able to meet relatives' psychosocial and medical needs by understanding how living with LFS influences the family system's functioning and facilitating members' support for each other.


El síndrome de Li-Fraumeni (LFS) es un trastorno hereditario que concede aproximadamente un 90 % de riesgo durante toda la vida de contraer cáncer y exige exámenes completos para la detección del cáncer de por vida. Analizamos los roles sanitarios a la hora de manejar los riesgos y los tratamientos de cáncer relacionados con el LFS que asumieron los padres, los adolescentes y los hijos adultos. Se realizaron entrevistas semiestructuradas con 23 familias. Los agrupamientos familiares estaban compuestos por entre 2 y 5 familiares, donde la edad de la generación más joven de cada familia oscilaba entre 7 y 40 años. Utilizando los métodos de la teoría fundamentada, realizamos una codificación abierta y centrada del contenido de la transcripción de la entrevista. Los miembros de la familia describieron cómo se implementó el rol de jefe de la salud en su familia, así como factores como la maduración de un niño o la muerte de un miembro que determinaron quiénes asumieron roles particulares y cómo estos roles cambiaron con el tiempo. Con frecuencia ellos expresaron la responsabilidad colectiva de ayudar a los familiares a comprender el LFS y a implementar el manejo adecuado del riesgo de contraer cáncer. Los familiares demostraron sus roles sanitarios asistiendo a citas médicas de los demás para recibir apoyo u obtener información. El rol de jefe sanitario fue intergeneracional y proporcionó a la familia el apoyo necesario para manejarse ante decisiones complicadas sobre la asistencia sanitaria. Nuestros resultados brindan información para los prestadores de servicios médicos con respecto a cómo los pacientes de LFS y sus familiares desarrollan roles únicos para la toma de decisiones médicas y el cuidado influenciados por la índole hereditaria del LFS, y cómo estos roles cambian con el tiempo. Es posible que los prestadores que estén acostumbrados a la dinámica de roles familiares sean más capaces de satisfacer las necesidades psicosociales y médicas de los familiares si comprenden cómo vivir con LFS influye en el funcionamiento del sistema familiar y si facilitan el apoyo mutuo de los familiares.

INTRODUCCIÓN: El cáncer de mama es la primera causa de muerte de la mujer en la Argentina, con una incidencia estimada de más de 19.000 casos nuevos por año. Dentro de estos, el tipo de cáncer hereditario más común es el de mama/ovario hereditario, provocado por mutaciones en los genes BRCA 1(Breast cáncer) y BRCA 2. A su vez el cáncer colorrectal es la segunda causa de muerte en Argentina, con una incidencia estimada de más de 11.000 casos nuevos por año. OBJETIVO: de la presente investigación es evaluar la utilidad de la realización de los estudios genéticos en personas con cáncer hereditario en el contexto del consejo genético, con un asesoramiento antes y después de realizarse la prueba genética POBLACIÓN Y MÉTODOS: Se estudiaron 34 mujeres con diagnóstico de cáncer de mama/ovario y 31 pacientes de ambos sexos con diagnóstico de cáncer colorrectal (CCR). En las mujeres se analizaron los genes BRCA 1 y 2 por secuenciación de próxima generación (NSG) y grandes rearreglos de los genes BRCA 1 y 2 por amplificación de sonda dependiente de la ligadura multiplex (MLPA). En las personas de ambos sexos se determinó la Inestabilidad de Microsatelites(IMS), el análisis de mismatch repair (MMR) por MLPA y la mutación del gen BRAF (Protooncogen B-Raf) RESULTADOS: Los resultados mostraron que las pacientes con cáncer de mama / ovario con antecedentes familiares tienen un alto porcentaje de BRCA negativo. En cuanto a los cambios fenotípicos, el más predominante en este estudio, fue el subtipo triple negativo y la paciente con BRCA 2 positivo presentó este fenotipo. Con respecto al estudio del cáncer de colon detectamos cuatro pacientes con IMS-alta y mutación del V600E del gen BRAF. Cuando se les realizó el análisis de MLPA en los genes MSH6, MLH1, MSH2 y PMS2 a los efectos de establecer la diferencia entre CCR y síndrome de Lynch, los resultados fueron negativos, por lo tanto, estos pacientes fueron diagnosticados como CCR esporádico. CONCLUSIONES: Como lo demuestra este trabajo, para el consejo genético, el estudio de vías moleculares en pacientes con cáncer hereditario es un instrumento de ayuda para la valoración del riesgo génico. INTRODUCTION: Breast cancer is the leading cause of death of women in Argentina, with a estimated incidence of more than 19,000 new cases per year. Among these, the most common type of inherited cancer is the breast /ovarian caused by mutations in the BRCA1 and BRCA 2 genes. At the same time, the cancer colorrectal is the second cause of death in Argentina, with an estimated incidence of more than 11,000 new cases per year. OBJECTIVE: of the present investigation is to evaluate the usefulness of the realization of the genetic studies in people with hereditary cancer in the context of genetic counseling, with a advice before and after the genetic test. POPULATIONS AND METHODS: We studied 34 women diagnosed with breast / ovarian cancer and 31 patients of both sexes with diagnosis of colorectal cancer (CRC). In women, the BRCA 1 and 2 genes by next generation sequencing (NSG) and large rearrangements of BRCA 1 and 2 genes by probe amplification dependent on multiplex ligation(MLPA). The instability of microsatellites (IMS) was determined in people of both sexes, the analysis of mismatchrepair (MMR) by MLPA and the mutation of the BRAF gene. RESULTS: There results showed that patients with breast / ovarian cancer with a history family members have a high percentage of negative BRCA. As for the changes phenotypic, the most predominant in this study, was the triple negative subtype and the patient with positive BRCA 2 presented this phenotype. With respect to the study of colon cancer detected four patients with IMS-high and mutation of the V600E of the BRAF gene. When the MLPA analysis was performed on the MSH6, MLH1, MSH2 genes and PMS2, in order to established difference between CRC and Lynch syndrome, the results were negative and therefore these patients were diagnosed as sporadic CRC. CONCLUSIONS: As this work demonstrates, for him genetic counseling, the study of pathways molecules in patients with hereditary cancer is a helpful instrument for risk assessment. (AU)

Cancer is a genetic disease caused by accumulation of sporadic genetic changes in the tumor tissue, generally after the age of 50. However, 10% to 30% of cases occur before the age of 50, some have a family history of cancer and may have inherited genetic changes that are transmitted from generation to generation and are present since conception. The number of genes identified is more than 80 related to approximately 200 genetic syndromes of predisposition to cancer. The importance of detecting these syndromes in patients and their families is because it allows an accurate diagnosis and prediction of risks and specific controls for each person and family, avoiding illness and death in many cases.


El cáncer es una enfermedad genética producto de la acumulación de cambios genéticos esporádicos en el tejido tumoral. Generalmente se presenta después de los 50 años. Sin embargo, un 10% a 30% de los casos ocurren antes de los 50 años, algunos tienen historia familiar de cáncer y podrían presentar cambios genéticos hereditarios que se transmiten de generación en generación y están presentes desde la concepción. El número de genes que se han identificado son más de 80 relacionados con aproximadamente 200 síndromes genéticos de predisposición al cáncer. La importancia de detectar estos síndromes en los pacientes y familias se debe a que nos permite hacer un diagnóstico preciso, predicción de los riesgos y controles específicos para cada persona y familia, evitando la enfermedad y muerte en muchos casos.

RESUMEN El cáncer epitelial de ovario representa uno de los tumores ginecológicos más letales ya que más del 75% de las pacientes son diagnosticadas en estadío avanzado. Aún no se ha demostrado que la realización de pruebas y exámenes pélvicos rutinarios haya reducido la mortalidad, no existiendo actualmente, un cribado eficaz para su diagnóstico precoz. Aunque la sintomatología metastásica extraperitoneal más común es el derrame pleural, las linfadenopatías neoplásicas a nivel supraclavicular aparecen hasta en el 4% de casos, generalmente asociándose a un mal pronóstico. La identificación de una adenopatía supraclavicular se relaciona hasta en un 58-83% de los casos, con el hallazgo de una tumoración maligna. Por otro lado, la dermatomiositis del adulto puede tener un origen paraneoplásico en un 15-25% de las ocasiones, siendo el cáncer de mama y de ovario la etiología más frecuente en la población femenina. Las pacientes portadoras de mutaciones en los genes BRCA 1 y 2 tienen un aumento del riesgo de padecer neoplasias de mama y ovario. En aquellas afectas de un cáncer de ovario y portadoras de una mutación en los genes BRCA, no se debería plantear una cirugía profiláctica de rutina sobre la mama, al menos en los primeros 5 años tras el diagnóstico de la neoplasia ovárica. Presentamos el caso de una paciente portadora de una mutación germinal del gen BRCA 1, que debuta con un cáncer de ovario, tras el estudio de una adenopatía neoplásica de cuello, biopsiada en el contexto de un síndrome paraneoplásico cutáneo.


ABSTRACT Epithelial ovarian cancer represents one of the most lethal gynecological tumors, since more than 75% of affected women are diagnosed at an advanced stage. However, studies have not demonstrated yet that performing routine pelvic exams and tests had reduced mortality in ovarian cancer, and currently there is no effective screening for early diagnosis. The most common extraperitoneal metastatic symptomatology of ovarian cancer is pleural effusion, but there are other, like neoplastic lymphadenopathies at supraclavicular level, described in up to 4% of cases and generally related to a poor prognosis. The identification of a supraclavicular adenopathy is associated with the finding of a malignant tumor in 58-83% of the cases. On the other hand, adult dermatomyositis can have a paraneoplastic origin in 15-25% of patients, being breast and ovarian cancer the most frequent etiology in the female population. Patients with BRCA 1 and 2 genes mutations have an increased risk of breast and ovarian malignancies. In those affected by an ovarian cancer and carriers of a mutation in the BRCA genes, routine prophylactic surgery should not be considered on the breast, at least in the first 5 years after the diagnosis of ovarian neoplasia. We present the case of a patient with a germinal mutation of the BRCA 1 gene, who debuts with an ovarian cancer, after the study of a neoplastic adenopathy of neck, biopsied in the context of a cutaneous paraneoplastic syndrome.

Genetic diagnosis of hereditary cancer syndromes offers the opportunity to establish more effective predictive and preventive measures for the patient and their families. The ultimate objective is to decrease cancer morbidity and mortality in high genetic risk families. Next Generation Sequencing (NGS) offers an important improvement in the efficiency of genetic diagnosis, allowing an increase in diagnostic yield with a substantial reduction in response times and economic costs. Consequently, the implementation of this new technology is a great opportunity for improvement in the clinical management of affected families. The aim of these guidelines is to establish a framework of useful recommendations for planned and controlled implementation of NGS in the context of hereditary cancer. These will help to consolidate the strengths and opportunities offered by this technology, and minimise the weaknesses and threats which may derive from its use. The recommendations of international societies have been adapted to our environment, taking the Spanish context into account at organisational and juridical levels. Forty-one statements are grouped under six headings: clinical and diagnostic utility, informed consent and genetic counselling pre-test and post-test, validation of analytical procedures, results report, management of information and distinction between research and clinical context. This guide has been developed by the Spanish Association of Human Genetics (AEGH), the Spanish Society of Laboratory Medicine (SEQC-ML) and the Spanish Society of Medical Oncology (SEOM).

Introdução: O câncer é um distúrbio genético no qual ocorre a perda do controle da proliferação celular. De maneira geral, podemos dividir os casos de câncer em esporádicos (mutações somáticas restritas ao tumor), que são a maioria, e hereditários (mutações germinativas presentes em todas as células do indivíduo), que em conjunto correspondem a aproximadamente a 10% de todos os casos. É importante compreender o papel da Oncogenética na identificação de pacientes com risco aumentado para desenvolvimento de câncer para possibilitar medidas de detecção precoce, de prevenção e de tratamento, diferenciadas das recomendadas para a população em geral. Métodos: Foi realizada revisão da literatura através dos sites de busca PubMed e Scielo, bem como através de literatura e Guidelines pertinentes à área da Oncogenética. Resultados: A indicação de investigação genética molecular deve ser baseada em uma suspeita de câncer hereditário, sugerida pela história de câncer do paciente e de sua família. Os critérios de indicação variam para as diversas síndromes hereditárias. Assim, torna-se importante o aconselhamento genético pré e pós-teste, a fim de direcionar a investigação mais indicada para cada caso e permitir que o paciente possa realizar escolhas informadas e adaptar-se ao risco e/ou à condição que esse diagnóstico traz à sua vida. Conclusão: A fim de tornar a oncogenética acessível à população em risco, é necessário capacitar mais profissionais no aconselhamento genético, buscar um maior acesso aos exames moleculares especialmente no serviço público de saúde, garantir a qualidade dos testes realizados por diferentes centros e a adequada interpretação de seus resultados.


Introduction: Cancer is a genetic disorder in which occurs a loss of control of cell proliferation. In general, we can divide cancer cases into sporadic (tumor-restricted somatic mutations), which are the majority, and hereditary (germ mutations present in all the cells of the individual), which together account for approximately 10% of all cases. It is important to understand the role of Oncogenetics in identifying patients at increased risk for cancer development in order to enable early detection, prevention and treatment measures, unlike those recommended for the general population. Methods: A review of the literature was performed through PubMed and Scielo databases, as well as through literature and guidelines considered relevant to the area of Oncogenetics. Results: The indication of molecular genetic research should be based on a suspected hereditary cancer, suggested by the patient's and his family's cancer history. The indication criteria vary for the various hereditary syndromes. Thus, pre and post-test genetic counseling becomes important in order to direct the most appropriate investigation for each case, allowing the patient to make informed decisions and to adapt to the risk and / or to the condition this diagnosis brings to his / her life. Conclusion: In order to make Oncogenetics accessible to the population at risk, it is necessary to train more professionals in genetic counseling, to seek greater access to molecular tests, especially in the public health service, to ensure the quality of the tests carried out by different centers and also to provide adequate interpretation of the results.

Introdução: O câncer de mama e ovário hereditário ("hereditary breast and ovarian cancer", HBOC) associa-se a mutações herdadas nos genes BRCA1/2. Nesses casos, o aconselhamento genético é mandatório, e a investigação do indivíduo tem como base características pessoais, a história familiar e as características da neoplasia. Uma vez que o diagnóstico HBOC é estabelecido através de testagem genética, existem protocolos bem estabelecidos para manejo do paciente e familiares em risco com objetivo de reduzir risco de câncer nos indivíduos não afetados e personalizar o tratamento nos pacientes já com diagnóstico de neoplasia. Métodos: Esse é um estudo observacional, transversal, de prevalência. Foi realizada uma revisão pelo banco de dados do "Projeto Amazona III" em todas pacientes incluídas do Hospital São Lucas de Porto Alegre. O objetivo do atual estudo foi avaliar as pacientes com critérios de indicação de testagem genética para HBOC através de 2 critérios. Para discussão, foram pesquisados artigos científicos dos últimos 12 anos. Resultados: Dentre os 112 participantes do estudo, 23% preenchiam critérios para testagem genética da Síndrome HBOC. Conclusão: Esse estudo revelou que um expressivo número (23%) de pacientes incluídas no Projeto Amazona III no Hospital São Lucas da PUCRS apresenta critérios para testagem da síndrome HBOC, considerando apenas 2 critérios indicadores de testagem. Além disso, 27% das pacientes apresentavam história familiar de até terceiro grau de câncer de mamãe/ ou ovário, necessitando de aconselhamento genético. A não identificação dessas pacientes significa em diagnósticos de câncer evitáveis.


Objectives: The primary objective is to examine the necessity of genetic testing for Hereditary Breast and Ovarian Cancer (HBOC) syndrome as a preventive measure. Basically, HBOC genetically inherited disorder arising out of mutations in the BRCA1 and BRCA2 genes. The HBOC diagnosis can be conducted through genetic testing. The investigation in these cases is carried out on the basis of individual characteristics, family history and features of the cancer. There are well-established protocols to manage patients and family members whom carry these mutations and to personalize the treatment for the patients already diagnosed with cancer. Methods: This is an observational, cross-sectional, and prevalence study. We conducted a review applying the database of the "Projeto Amazona III" on patients enrolled at the Hospital São Lucas de Porto Alegre (HSL-PUCRS). The purpose of the study was to evaluate patients that fall within the criteria for HBOC genetic testing. We considered only two criteria. We also analyzed the relevant scientific articles of the past 12 years to discuss the subject. Results: 23% of the 112 participants of the study met with the criteria for HBOC genetic testing. Conclusion: This study revealed that a significant number (23%) of the patients enrolled in the "Projeto Amazona III" at the HSL-PUCRS have the two criteria for HBOC genetic testing. Furthermore, 27% of the patients required genetic testing since they had a third-degree family history of breast cancer or ovarian cancer. The failure to identify these patients leads to the absence of the diagnosis of preventable cancer.

El objetivo de este trabajo fue caracterizar demográfica y molecularmente las familias con diagnóstico de síndrome de Lynch en base a estudios genéticos. Se utilizó la base prospectiva del Registro de Epidemiología Molecular de Cáncer Colorrectal (REM-CCR) del Hospital Italiano de Buenos Aires (Clinical trials.gov NCT02781337). El criterio de inclusión fue que tuvieran hecho un estudio genético entre 1996 y 2017 (secuenciación y/o determinación de grandes rearreglos de al menos un gen reparador de error de apareamiento). Se analizaron 50 familias con los criterios de Amsterdam. En 23 (46%) se identificaron variantes patogénicas (n=19) y probablemente patogénicas (n=2). El 28.6% de las variantes patogénicas fueron originalmente descritas en esta serie, entre ellas la variante c.1911del en el exón 12 de MSH2 identificada en una familia con agregación de cáncer de mama. Fue identificada una mutación fundadora de Piamonte, Italia (c.2252_2253del). Los genes afectados incluyeron MSH2 (13 variantes) MLH1 (9 variantes) y PMS2 (1 variante). La tasa de detección de mutaciones fue del 46%. Entre las familias con mutación identificada (n=23), se detectó una edad mediana de inicio del cáncer menor (46 vs. 50 años, p=0.02) y mayor incidencia de tumores extra-colorrectales (90.5% vs. 45.8%, p <0.01), que las 27 sin mutaciones. La implementación de estudios genéticos permitió caracterizar variables demográficas en base a la identificación de mutaciones germinales asociadas al síndrome de Lynch, identificándose dos grupos diferenciados por la edad de afectación y la incidencia de tumores extracolónicos (AU)


The aim of this study was to characterize demographically and molecularly families diagnosed with Lynch syndrome based on genetic studies. Families with a genetic study performed between 1996 and 2017 (sequencing and/or determination of large rearrangements of a mismatch repair gene at least) were selected from the prospective database REM-CCR of Hospital Italiano de Buenos Aires (Clinical trials. Gov NCT02781337). Fifty families fulfilled Amsterdam criteria were analyzed. Pathogenic variants were found in 23 out of 50 (46%) families, being 21 pathogenic and 2 likely pathogenic. The 28.6% of the pathogenic variants were originally described in this series. Among them, the variant c.1911del in MSH2 in a family with breast cancer aggregation and a founder MLH1 mutation from Piedmont, Italy (c.2252_2253del) were identified. Affected genes include MSH2 (13 variants), MLH1 (9 variants), PMS2 (1 variant). Mutations detection rates was 46%. Those families with an identified mutation (n=23) had a lower median age of cancer onset (46 vs. 50 years, p=0.02) and a higher incidence of extra-colorectal tumors (90.5% vs. 45.8%, p<0.01) than those without identified mutations (n=27). The implementation of genetic studies allowed characterizing demographic variables based on the identification of germline mutations associated with Lynch syndrome. INTRODUCTION: It is believed that 5% of all cancers are hereditary, on being caused by mutations in the germinal line in cancer susceptibility genes. The hereditary pattern in the majority of cases is autosomal dominant. Genetic tests are only recommended to individuals whose personal or family history is highly suggestive of a hereditary cancer. The appropriate assessment of these individuals and their families must be performed in Cancer Genetic Counselling Units (UCGC). MATERIAL AND METHODS: Representatives of the Spanish Medical Oncology Society (Sociedad Española de Oncología Médica [SEOM]) and the three primary care scientific societies: Spanish Society of Family and Community Medicine (Sociedad Española de Medicina de Familia y Comunitaria [SEMFyC]), Spanish Society of Primary Care Physicians (Sociedad Española de Médicos de Atención Primaria [SEMERGEN]) and the Spanish Society of General and Family Doctors (Sociedad Española de Médicos Generales y de Familia [SEMG]), met to prepare this consensus document on hereditary cancer. The consensus identified the three main aspects: how to identify subjects at risk of hereditary cancer; how to refer to a UCGC; and the usefulness of the assessment and genetic studies. RESULTS: A document, with the text fully agreed by all the participants, has been prepared. It contains a summary of the principal characteristics of the care for individuals with hereditary cancer. It shows how to; identify them, assess them, refer them to a UCGC. How to assess their genetic risk, perform genetic studies, as well as prevention measures and reduction of the risk is also presented. CONCLUSIONS: This consensus document is a landmark in the relationships with several Scientific Societies that represent the professionals who provide care to individuals with cancer and their families, and will help to improve care in hereditary cancer in Spain.

Se denomina pólipo intestinal a una lesión visible elevada o tumor que se proyecta desde la superficie epitelial al lumen visceral. En relación al número, presencia de antecedentes familiares, manifestaciones extraintestinales y estudios genéticos es que se constituyen diversas poliposis intestinales. Si bien, las poliposis intestinales se manifiestan en general en la edad adulta, existen manifestaciones que pueden hacer sospechar la presencia de un síndrome poliposico hereditario en la infancia. Además en una proporción considerable estas poliposis presentan manifestaciones extraintestinales, tanto benignas como tumores en otros órganos. Es por esto, que una alta tasa de sospecha, en particular frente a pacientes con antecedentes familiares, puede conducir a un diagnóstico y tratamiento oportuno, además de considerar a la familia como potenciales pacientes e ingresar al grupo familiar a un registro de tumores hereditarios. Diversas técnicas de biología molecular han permitido la identificación de las mutaciones que son heredadas en estas enfermedades, permitiendo realizar conductas preventivas al saber el riesgo de cada persona en una familia afectada. El objetivo de esta revisión, es caracterizar las distintas poliposis intestinales, en cuanto a sus manifestaciones clínicas, clasificaciones, estudio genético y enfrentamiento multidisciplinario.

Molecular biology has created different techniques to identify the presence of hereditary mutations that are specific for intestinal polyposis. Acknowledgment of these mutations establishes risks groups allowing adequate prevention strategies. The objective of this revision is to characterize and different types of intestinal polyposis, according to clinical manifestations, classification, genetic study and multidisciplinary approach.

Introdução: A síndrome de Lynch (SL) ou Câncer colorretal hereditário sem polipose (HNPCC) é uma doença autossômica dominante que predispõe ao câncer colorretal e risco aumentado para câncer de endométrio, estômago, intestino delgado, sistema hepatobiliar, rim, ureter, ovário, tumores de glândulas sebáceas, entre outros. Mutações germinativas nos genes de reparo de DNA (MMR) MLH1, MSH2, MSH6, PMS1 e PMS2, são conhecidos por causar síndrome de Lynch (SL). Além disso, mutações herdadas em CHEK2 e TP53 também influenciam o risco de câncer colorretal. Mutações, no primeiro, tem sido identificadas em pacientes com SL, incluindo 1100delC, I157T e IVS2 +1 G> A e, no segundo, tem sido associadas com o câncer gástrico e colorretal no contexto da síndrome de Li-Fraumeni. Objetivo- O objetivo deste estudo foi determinar a frequência de tumores extracolônicos em famílias com os critérios de Amsterdam e de mutações nos genes MSH6, PMS1, PMS2, TP53 e CHEK2 em pacientes suspeitos de SL. Material e Métodos- A fim de avaliar a frequência de tumores colorretais e extracolônicos na SL, foram incluídas 60 famílias não relacionadas que preenchiam, exclusivamente, os critérios de Amsterdam I e II. Para a análise da frequência de mutações nos genes MSH6, PMS1, PMS2, TP53 e CHEK2, 71 famílias não relacionadas, negativas para mutações em MLH1 e MSH2, foram incluídas. Resultados- Para a frequência de tumores avaliados, um total de 2095 indivíduos foram analisados e o câncer colorretal foi encontrado em 334 casos. Tumores extracolônicos foram encontrados em 200 indivíduos, dos quais o câncer de mama (32 casos) foi a manifestação extracolônica mais frequentemente encontrada. Objetivo: Presentar el caso de una paciente, quien acudió para evaluación ginecológica. La misma resultó clínicamente sana pero en su grupo familiar destaca la alta frecuencia de cáncer de mama y de ovario, lo cual indujo a sospechar alto riesgo para el cáncer de mama hereditario (CaMH); a propósito del mismo se revisa esta patología y se resalta la importancia de la historia familiar. Caso clínico: Mujer de 25 años de edad, que acude a la consulta para realizar control ginecológico. Diagnóstico: paciente clínicamente sana, pero con antecedentes familiares, de alta frecuencia de cáncer de mama (CaM) y cáncer de ovario, razón por la cual se procede a investigar al grupo familiar. Se revisan las historias clínicas del archivo del IAHULA y se entrevistan los familiares. Familia no consanguínea, padres sanos, mayores de 70 años sin cáncer, 3 hermanos y 7 hermanas. Cuatro hermanas fallecieron en un periodo de 9 años, en edades comprendidas entre 24 y 35 años: 3 por CaM y 1 por Ca de ovario, tía materna con CaM y abuela paterna con probable Ca de ovario (síndrome ascítico no ictérico) ambas fallecidas en el medio rural. Antecedentes personales de las 4 hermanas fallecidas: menarquia en promedio de 12.3 años, paridad y lactancia 2/4, nulípara 2/4, ninguna obesa, ni hábitos alcohólicos. Al momento del diagnóstico todas en estadios avanzados de cáncer. Tipo histopatológico del CaM: ductal infiltrante poco diferenciado (2/3), lobulillar y medular (2/3); bilateral (1/3); sin determinar receptores de estrógeno, progesterona o andrógenos. Metástasis a cerebro y pulmón (2/3) durante tratamiento. En ovario: cistoadenoma mucinoso, estadio IV, recidiva al año a pesar del tratamiento. Sobrevida de 3 meses a 3 años. A la paciente se le recomendó realizar el estudio genético para poder precisar el riesgo de padecer cáncer de Mama y ovario Hereditario (CaMH) y establecer las medidas de vigilancia. Conclusiones: Se recomienda que ante una paciente con antecedentes familiares de CaM, se investigue a fondo su historia familiar, la cual aún en ausencia de marcadores genéticos, permite sospechar la presencia de un CaMH el cual requiere de un protocolo de prevención y vigilancia diferente al CaM esporádico.


Objective: To present the case of a patient, who went for a gynecological assessment. She was healthy but in his family group stressed a high frequency of breast and ovarian cancer, which led to suspect high risk for hereditary breast cancer (HBCa). This condition is reviewed, highlighting the importance of family history. Case report: A 25 year old woman who consults for a gynecological examination. Healthy patient with a high frequency of breast cancer (BCa) in her family. Not consanguineous family. Healthy parents, 3 brothers and 6 sisters. Four sisters died within a period of 9 years, aged between 24 and 35 years old: 3 from BCa and 1 from ovarian cancer. Maternal aunt with BCa and paternal grandmother with ovarian cancer. Personal history of the 4 deceased sisters: average age of menarche 12.3 years, parity and lactation 2/4, nulliparous 2/4, no obesity, nonalcoholic habits. Initial diagnosis in advanced stages of cancer. Histopathological type of BCa: poorly differentiated (2/3), lobular and medullary (2/3); bilateral 1/3; lung and brain metastases during treatment (2/3). In ovarian: mucinous cystadenoma stage IV, recurrence in one year despite treatment. Survival between 3 months and 3 years. The patient is recommended to perform the genetic study in order to clarify the risk of suffering BCa and surveillance measures. Conclusions: It is recommended in a patient with a family history of BCa, to carry out a thorough investigation of family history, which, even in the absence of genetic markers, allows to suspect the presence of hereditary breast cancer.

Neste artigo, apresentam-se alguns dos dados obtidos em um estudo qualitativo sobre o quotidiano dos sujeitos em risco genético de câncer hereditário do cólon. A perspectiva analítica, que é possível esboçar a partir da análise dos relatos desses indivíduos remete para a importância das condições-limite de aceitação positiva do risco e que, actualmente, se exprime na tendência de diferentes projectos tecnológicos - de que a genética é um exemplo ? apelarem aos desejos individuais e colectivos de um futuro melhor. Outro dos traços estruturantes da gestão quotidiana do risco genético centra-se na experiência familiar do câncer, que permite accionar práticas preventivas indispensáveis ao controlo da doença. Esse saber acumulado e transmitido de geração em geração, por vezes oralmente, outras vezes através da experiência quotidiana com o sofrimento e a morte, ou na partilha das alegrias inerentes ao triunfo sobre o câncer configura-se como um dos pilares na gestão quotidiana da herança, indesejada.


This study presents some results obtained on a qualitative study about the everyday life of the individuals in genetic risk of hereditary cancer. The analytical perspective, which can be outlined on the basis of an analysis of the individuals' reports, points out the importance of the limiting conditions of risk acceptance. This is presently expressed in the tendency for the different technological projects -including genetics - to appeal to both individual and collective wishes for a better future. Another conclusion is that the daily management of genetic risk is centred on the family experience of cancer, which allows preventives practices, indispensable to control the disease, to be put into action. This knowledge, sometimes accumulated and transmitted orally from generation to generation, and other times obtained from experiencing pain and death on a daily basis, or even by sharing in the joy of triumphing over cancer represents one of the pillars of the daily management of an unwelcome inheritance.
