RESUMO

Objective: The aim of this study was to explore the seronegative conversion status of syphilis patients after centralized management and to analyze potential determinants. Materials and Methods: A retrospective population-based cohort study was conducted, and data for individuals who had been diagnosed with syphilis between 2011 and 2019 were retrieved from the Shenzhen Nanshan Center for Chronic Disease Control. Seroconversion statuses were summarized as percentages. Univariable and multiple Cox proportional hazard regression models were used to analyze the factors associated with seronegative conversion among syphilis patients. Results: During the study period, 1,545 patients with syphilis participated in the syphilis convergence case management program on a voluntary basis, of whom 290 were excluded due to missing follow-up data. A total of 27.6% (346/1255) of patients with syphilis showed seronegative conversion. Multivariable analysis revealed that the following significantly determined syphilis seroconversion from positive to negative: younger age (15-19 years vs. ≥30 years: HR = 2.18), male gender (HR = 1.45), lower baseline toluidine red unheated serum test (TRUST) titer of ≤ 1:8 (HR = 2.23), and different disease stages, including latent syphilis (HR = 1.98), primary syphilis (HR = 7.67), and secondary syphilis (HR = 4.83). Conclusions: Few patients with syphilis tested negative after treatment at the end of the study. Seronegative conversion in the patients was associated with age, sex, baseline TRUST titer, and syphilis stage.

Through the functional combination of relevant departments involved in hospital procurement, to simplify and unify the work process, we establish a standardized procurement system, to realize the pre-procurement budget and approval, power balance, strengthen the fairness and openness of procurement process. By introducing the closed-loop process of in-process supervision to ensure the impartiality of review and post-evaluation control, it comprehensively strengthens the internal control of procurement management, and finally realizes the purpose of strengthening procurement risk prevention and procurement quality management.

RESUMO

Anaerobic digestion (AD) reduces GHG emission and facilitates renewable energy generation. The slow rate of adoption of this technology is often attributed to economic and technical considerations. Collaboration of two or more dairy farms into a centralized AD system can improve the process economics through economies of scale. However, uncertainties related to the process parameters and the scope/scale of the collaborative implementation impede its adoption. This study presents techno-economic optimization model as a design aid to determine ideal location, capacity, and participation level (cluster size) that maximize economic return on a cooperative digester. This study employs a probabilistic approach to overcome uncertainty regarding project parameters such as manure biomethane potential (BMP), project capital, and electricity sale price. Two case studies based on dairy production regions in Wisconsin were developed to test the model and demonstrate its capabilities. Herd sizes and spatial distribution in a given region were found to be critical factors in determining the viability of digestion projects in general, and collaborative digestion systems in particular. The number of simulation runs needed to capture the probability of profitable AD facility establishment was less than 1000 for both case studies assessed. Electricity sale price and biomethane potential of feedstock utilized were found to be the most restrictive to the feasibility of AD adoption. Changing the optimization objective function, to adopting maximization, favored the formation of collaborative AD facilities for both case studies evaluated.

RESUMO

This paper introduced the performance appraisal scheme for digestive endoscopy centers featuring centralized management. A comparison of the operations before and after the use of such performance management, proves the efficiency improvement of such centers, and the supplementary effect on centralized management of digestive endoscopies. Thus the authors propose that performance management is conducive to centralized management of the equipment for reference in designing performance management of departments.

Objective To investigate the regular spot check implementation of central sterile supply department(CSSD) at hospitals of all grades across China and analyze its influencing factors to provide the basis for further improvement of regular cleaning quality inspection. Methods Cross-sectional study was conducted form February to July of 2017,and 132 hospitals all over the country were selected to investigate with the questionnaire the current status of CSSD management model, staffing and regular cleaning quality inspection. Results The ratio of the nursing staff and the number of beds was 1.15:100,the ratio of staff and the number of beds was 2. 00:100. 81. 82% (n =108) of the hospitals adopted the centralized management model. The qualification rate for regular spot check execution was 87.12% (n=115). The results of multiple logistic regression showed that the centralized management model was more conducive to the regular spot check than the non-centralized management model (OR=4.71,95% CI:1.05-21.08). Conclusions The centralized management model proves positive for the implementation of regular spot checks;and CSSDs should pay more attention to the regular spot check of the cleaning quality.

Objective:To investigate the centralized management mode of cleaning disinfection and for ventilator, monitor, injection pump and infusion pump in our hospital so as to achieve scientific management and further development for hospital.Methods: The qualified rates of ventilator, monitor, injection pump and infusion pump, and satisfied degree of various department for them were analyzed after the centralized management was implemented.Results:After the centralized management was implemented, the cleaning qualified rates of ventilator, monitor, injection pump and infusion pump were, 99.25%, 99.78%, 99.47% and 98.94%, respectively. And the satisfaction scores of various departments for the application of equipment, equipment allocation and equipment recovery were significantly higher than those in 2013 (t=4.18,t=4.62,t=3.86,P<0.01).Conclusion: After the centralized management for routine emergency equipment is carried out by the cleaning disinfection center, the cleaning qualified rates of equipment and satisfaction of various departments has been significantly increased. Therefore, it is worthy to be applied and promoted.

Objective:To establish the emergency medical equipment rental center, not only can reduce the repeated purchase for medical equipment, but also can achieve resource sharing, reduce equipment maintenance cost, and improve utilization efficiency.Methods: To establish emergency medical equipment rental center, which adopted lease mode and centralized management mode to use equipment. Department of medical engineering managed this center and this department is responsible to ensure every clinical department can use equipment by lease method.Results: The center has optimized the resource allocation of hospital, obviously reduced equipment input cost, improved utilization rate of medical facility since it were established. This mode provided reliable service for clinical departments.Conclusion: The established rental center of emergency medical equipment has changed the traditional management mode to a new feasible mode which can improve centralized management for medical devices, reduce waste for medical resource and achieve resource sharing.

Objective To solve the problems in medical equipment utilization,supervision,management,clinical safety and etc.Methods A centralized management and sharing system of medical equipment was developed with functional requirements analysis,C/S architecture and programming.Results The system eliminated the needs for manual accounting and allocation,enhanced medical equipment usage rate and improved equipment borrowing procedure.Conclusion The system contributes to dynamic management and efficacy evaluation of medical equipment,so as the working efficiency can be increased while the costs can be decreased for manpower and materials.

Objective: To introduce the centralized management method for the conventional medical equipment and its utilization benefits and economic benefits to hospital and clinical department through scientific management pattern. Methods: Through several parts such as utilization ratio, usage cost, daily maintenance, measure control and data analysis, they could explain the advantage and convenience after proceeding centralized management. Results:Reasonable and scientific centralized management pattern could reduce or wipeout the idle and waste phenomenon for conventional medical equipment, reduce the department's expenditure of equipment depreciation, enhance the economic efficiency to hospital and clinical department. Through timely preventive maintenance and measure control, not only could save the precious time for the service staff to share personal experience, enhance the equipment repair time and repair rate effectively, but also could guarantee the equipment operation status and operation stability, protect the safety of patient medical device's usage. Conclusion:By making full use of various valuable data, it could be the reference point for these device's analysis and evaluation. As a result, concluded from economic, safety and scientific research part, it is necessary to promote centralized management for the conventional medical equipment.

RESUMO

Objective To discuss the effect of skill training for staff working in washing rooms of sterilization and supply center after application of centralized management model.Methods The training objects included all the working staff in the washing rooms and the training procedure covered four steps:preparation,discussion,implementation and evaluation.The evaluation indices involved level of post skill,cleaning and sterilization effect of instruments and satisfaction degree of staff.Results After training the level of post skills were increased dramatically(P<0.01).The cleaning and sterilization effect of instruments was also improved(P<0.01).96% of staff considered their post ability elevated after training.Conclusion After skill training for staff working in washing rooms,the post skill and cleaning and sterilization quality of instruments were both enhanced.It proved valuable for guarantee of cleaning and sterilization quality of medical instruments and improvement of professional skills for staff working in washing rooms of sterilization and supply center.

OBJECTIVE To conduct the centralized management of the medical instrument in the supply room,and to assure the quality and validation of the clinical nursing and the patient safety. METHODS The cleaning and packing procedures of the instrument were standardized.The reusable instrument would be concentrated to clean,pack,and sterilize.To create a photograph management system,and use the photos to distinguish specialized instrument. RESULTS The instrument was reprocessed in the supply room instead of the clinical departments by the method of the centralized management of the instrument,which provided better instrument care and reprocessing. CONCLUSIONS The supply room conducts the centralized management of the medical instrument and enhances the quality of the instrument reprocessing.

RESUMO

OBJECTIVE:To establish a way to monitor drug-induced liver damage by using hospital centralized management system for monitoring ADR.METHODS:Using self-designed computer program,the data of inpatients with abnormal ALT,AST and TBIL,admitted in the period from Dec. 2001 to Feb. 2002,were extracted from hospital HIS system of databa_se,and ADR and irrational drug-use were retrospectively analysed.RESULTS:There were 50 ADR incidents concerned with 30 kinds of drug and 11 cases receiving irrational medication concerned 10 kinds of drug.CONCLUSION:By this way,we can timely get the information of drug-induced liver damage and set up a new way for developing centralized ADR monitoring in hospital.
