Periosteal pedicle graft for the treatment of gingival recession defects current status and future prospects: What the evidence suggests?

BACKGROUND: Gingival Recession defects are one of the most common defects for which patients seek periodontal treatment. Many treatment options are available for the management of gingival recession. Most of the treatments offered aim to treat the cause, cover the denuded root surface and produce a long term aesthetic result. The use of periosteal pedicle graft (PPG) is a recent innovation for the treatment of gingival recession defects and has gained much attention in a short span of time. Although studies have been done utilizing PPG successfully for the treatment of gingival recession defects (GRD) but it is still not clear, whether PPG technique should be included in the established list of techniques used to treat GRD? An effort has been made to arrive at a decision on the current utility of PPG in the treatment of GRD based on the scientific evidence available in literature. MATERIALS AND METHODS: A review of current literature was done to critically evaluate the evidence related to the Periosteal pedicle graft technique. RESULTS AND CONCLUSION: Periosteal Pedicle Graft has come up as a viable treatment option for the treatment of GRD although it's still too early to predict the long-term results associated with PPG.

A substituição e restauração de dentes ausentes com implantes dentais têm se tornado um procedimento rotineiro. Os autores descrevem uma situação clínica onde um implante (interface do tipo hexágono interno-Conexão Sistemas de Prótese, Brasil) foi colocado imediatamente após a exodontia do dente 62 e realizada a provisionalização imediata. Após 8 meses, foi associado um enxerto de tecido mole devido à presença de um tecido extremamente fino na região vestibular. Substituição unitária e posterior associação a enxerto de tecido conjuntivo demonstraram ser um procedimento seguro. Além disso, este tratamento pode ser considerado com o objetivo de se obter uma restauração com excelência funcional e estética

The replacement and restoration of missing single teeth with dental implants are routine procedures. The authors describe a clinical situation where an implant (internal hex connection type implant – Conexão Sistemas de Prótese, Brasil) was placed and immediately provisionalized after the removal of teeth 62. After 8 months, is was associated to a soft tissue graft due to an extremely thin tissue in the buccal region. Single-tooth replacement by immediate implant and posterior association to a connective tissue autograft was demonstrated to be a predictable procedure. Moreover, this treatment can be considered in order to reach an excellent functional and harmonious aesthetic restoration
